Corneal collagen crosslinking (CXL) is, at present, the only treatment that can slow or even stop the progression of keratoconus. It uses riboflavin and ultraviolet A (UVA) to create covalent bonds ("crosslinks") between collagen fibrils thus increasing corneal rigidity. Although to date there has been no direct evidence of intrastromal corneal crosslinking, several studies have reported the safety and efficacy of the conventional CXL protocol. This protocol is indicated for progressive keratoconus with a minimal corneal thickness (without the epithelium) of at least 400 μm. It should be perfo

While efforts have been made over the years, the exact cause of keratoconus (KC) remains unknown. The aim of this study was to identify alterations in endogenous metabolites in the tears of KC patients compared with age-matched healthy subjects. Three groups were tested: 1) Age-matched controls with no eye disease (N = 15), 2) KC - patients wearing Rigid Gas permeable lenses (N = 16), and 3) KC - No Correction (N = 14). All samples were processed for metabolomics analysis using LC-MS/MS. We identified a total of 296 different metabolites of which >40 were significantly regulated between groups

Keratoconus (KC) is a bilateral, asymmetric, corneal disorder that is characterized by progressive thinning, steepening, and potential scarring. The prevalence of KC is stated to be 1 in 2000 persons worldwide; however, numbers vary depending on size of the study and regions. KC appears more often in South Asian, Eastern Mediterranean, and North African populations. The cause remains unknown, although a variety of factors have been considered. Genetics, cellular, and mechanical changes have all been reported; however, most of these studies have proven inconclusive. Clearly, the major problem h

Acute corneal hydrops is a condition characterized by stromal edema due to leakage of aqueous through a tear in descemet membrane. The patient presents with sudden onset decrease in vision, photophobia, and pain. Corneal thinning and ectasias combined with trivial trauma to the eye mostly by eye rubbing is considered as the underlying cause. With conservative approach self-resolution takes around 2 to 3 months. Surgical intervention is required in cases of non-resolution of corneal edema to avoid complications and for early visual rehabilitation. Intracameral injection of air or gas such as pe

INTRODUCTION: Keratoconus (KC) is an ectatic disorder characterized by the progressive thinning and scarring of central cornea as a consequence of structural and/or compositional anomalies, although the exact etiology is largely unknown. The resultant conical protrusion of the cornea causes significant irregular astigmatism, myopia and visual impairment.
AREAS COVERED: This paper will review the biochemical factors involved in the multifactorial pathogenesis of KC and the possible emerging role of inflammation in this process. Additionally, the authors discuss the development of new corneal c

We describe the case of a 37-year-old caucasian man with a history of keratoconus and long-term hard contact lens wear. Histological analysis of the corneal button after penetrating keratoplasty revealed staining with Congo red and apple green birefringence detected using polarized light. In the absence of other systemic or ocular pathology, the authors infer this amyloid deposition to be secondary to keratoconus and endeavor to discuss the possible etiopathogenesis of the disease.

Keratoconus is an ectatic corneal dystrophy and is a leading indication for corneal transplantation surgery worldwide. The disease was first described in detail more than 150 years ago by Dr John Nottingham, but the understanding of the disease and its management have undergone significant changes over the last few decades. Corneal specialists have adopted new techniques and technologies for the effective management of keratoconus, while adhering to the age-old concepts of contact lens fitting and penetrating keratoplasty. Lamellar keratoplasty has been revived with improved outcomes and devic

The observational case describes bilateral recurrent keratoconus in corneal transplants performed in a patient with self-induced keratoconus secondary to compulsive eye rubbing. Slitlamp findings demonstrated corneal stromal thinning and scarring in the patient's right eye and temporal corneal hydrops in his left eye. Videokeratography of the right eye confirmed the presence of corneal steepening and irregular astigmatism, consistent with the diagnosis of keratoconus involving each transplant. Together with the history of ongoing compulsive eye rubbing, these findings support the concept that

To determine if differences in topographic progression between unaffected keratoconus relatives and normal controls can predict factors associated with the development of keratoconus in a longitudinal study. We recruited 369 unaffected keratoconus relatives and 119 normal controls in Los Angeles. Both eyes of subjects were examined at baseline clinically and by quantitative videokeratography and at a period ranging from 1 year to 8 years. Progression to keratoconus was evaluated by quantitative videokeratography variables. Unaffected relatives had higher Central K (CK), I-S and KISA values and

PURPOSE: To quantify the complexity involved in fitting contact lenses on the eyes of patients with keratoconus.
METHODS: The contact lens care of one randomly selected eye each of 38 keratoconus patients was retrospectively analyzed and compared to that of 38 gender and age matched controls. We evaluated the number of diagnostic contact lenses used to establish the initial contact lens order, number of ordered rigid gas permeable (RGP) contact lenses needed to complete the fit, number of office visits during the initial 4 months of care, best spectacle and RGP contact lenses corrected Log MA

Evidence increasingly supports keratoconus as an inherited, genetic disorder. If this is the case, it would be expected to express itself as a binocular condition even though variable expression of the gene may show differing levels in the two eyes. It is a common occurrence in specialist contact lens practices to see pseudo-monocular keratoconus, that is, an overt clinical level of the condition in one eye and forme fruste keratoconus in the other. The advent of videokeratoscopy has shown almost every case of apparent monocular keratoconus to be different expressions of the condition in the t

PURPOSE: To report baseline differences between eyes on key variables in the Collaborative Longitudinal Evaluation of Keratoconus (CLEK) Study cohort compared with a retrospectively assembled group of myopic contact lens wearers without ocular disease.
METHODS: A total of 1,079 keratoconus patients who had not undergone a penetrating keratoplasty in either eye before their baseline visit were enrolled and examined at baseline. Records from 330 contact lens-wearing myopes were reviewed. Corneal curvature (keratometry), visual acuity, refractive error (manifest refraction), and corneal scarring

OBJECTIVE: The authors evaluated the efficacy of the modified technique of fluid deep lamellar keratoplasty in keratoconus.
DESIGN: Noncomparative case series.
PARTICIPANTS: Twenty-six eyes of 25 patients with moderate to advanced keratoconus were included. Fourteen males and 11 females with age range of 16 to 37 years.
INTERVENTION: Deep lamellar keratoplasty aided by intrastromal injection of balanced salt solution to facilitate corneal dissection down to Descemet's membrane. Full-thickness graft devoid of endothelium and Descemet's membrane was then sutured in place.
MAIN OUTCOME MEASUR

The diagnosis of keratoconus is based on slip-lamp and keratometric findings, and its classification includes 4 different stages. In this study, we compare the ultrasound biomicroscopy (UBM) findings and the keratoconus index (KI) determined using UBM with the severity of the disease. By means of UBM (Humphrey Instruments system model 840) using a 50-MHz probe, we studied 49 eyes affected with different forms of keratoconus. Using a method previously described, we calculated the KI and compared these data with the findings obtained using videokeratography. The mean thickness at the corneal ape
