Development of a broad-range 16S rDNA real-time PCR for the diagnosis of septic arthritis in children.

The broad-range PCR has been successfully developed to search for fastidious, slow-growing or uncultured bacteria, and is mostly used when an empirical antibiotic treatment has already been initiated. The technique generally involves standard PCR targeting the gene coding for 16S ribosomal RNA, and includes a post-PCR visualisation step on agarose gel which is a potential source of cross-over contamination. In addition, interpretation of the presence of amplified products on gels can be difficult. We then developed a new SYBR Green-based, universal real-time PCR assay targeting the gene coding

Multicenter Retrospective Analysis of 702 Pediatric Cases of Bone and Joint Infections: Definition of Clinical and Biological Features to Discriminate K. kingae, S. aureus, and Other Bacterial Infections.

This study aimed to identify key differences between K. kingae infections and those caused by S. aureus or other pathogens. Differentiating these infections is crucial due to their nonspecific clinical presentations and overlapping laboratory and radiological findings, particularly when isolates are unavailable. We retrospectively analyzed data from 702 pediatric patients with bone and joint infections from 2010 to 2023 across two hospitals. The most common diagnoses were osteomyelitis (35.3%) and arthritis (29.6%), with fever present in 46.0% of cases. Pathogen identification showed K. kingae

[Septic arthritis in children with normal initial C-reactive protein: clinical and biological features].

Septic arthritis has to be suspected in children with joint effusion and fever so as to perform joint aspiration, which will confirm the diagnosis by bacteriological methods, and to perform surgical treatment by joint lavage. Since development of current molecular methods, such as real-time PCR, Kingella kingae has become the first microbial agent of osteoarticular infections in young children, whereas Staphylococcus aureus is second. C-reactive protein (CRP) is an aid used to diagnose septic arthritis, but its elevation could be moderate. In a previous study, conducted at our hospital, 10% of

A Swiss Retrospective Case Series of Pediatric Primary Subacute Hematogenous Osteomyelitis.

This study aimed to characterize the clinical spectrum and the bacteriological and microbiological etiology of pediatric primary subacute hematogenous osteomyelitis (PSAHO) and to evaluate a modern diagnostic approach for these infections. A single-center, 25-year retrospective review (2000-2025) of 107 consecutive cases of PSAHO was performed. Clinical presentation, traditional inflammatory markers, conventional cultures and nucleic acid amplification tests (NAATs) on blood, and bone and oropharyngeal samples were assessed. Most patients (73.8%) were <4 years. Fever was uncommon (15.9%), and

The Microbiology and Clinical Presentation of Acute Bacterial Arthritis in Houston Area Children <5 Years Old in the Era of Molecular Diagnostics.

BACKGROUND: While Staphylococcus aureus is the most common pathogen causing acute bacterial arthritis (ABA), the microbiology is diverse, particularly in young children. Kingella kingae is a well-known pathogen of ABA and can be particularly difficult to identify. We examined the impact of molecular diagnostics on ABA in a historically methicillin-resistant S. aureus (MRSA) endemic region. METHODS: Cases of ABA in children ≤5 years old between 2015 and 2022 were reviewed. The clinical features of cases were compared by causative pathogen. Trends in utilization of molecular diagnostics and rat

Mycobacterium leprae is identified in the oral mucosa from paucibacillary and multibacillary leprosy patients.

The epidemiology of acute paediatric osteoarticular infections (OAI) has recently evolved, mainly due to the improvement of microbiological diagnosis. We conducted a prospective study to analyse the recent epidemiology and the clinical evolution of paediatric OAI in order to validate the adequacy of our probabilistic first-line antibiotic treatment (intraveinous cefamandole + gentamicin). All children suspected of community acquired OAI were included and followed-up for 3 years. The etiologic diagnosis was based on blood cultures, joint aspirations and bone punctures. All osteoarticular (OA) s

Short-course antibiotic treatment of bone and joint infections in children: a retrospective study at Montpellier University Hospital from 2009 to 2013.

BACKGROUND: Acute haematogenous bone and joint infections (AHBJI) represent a diagnostic and therapeutic emergency in children, with significant potential sequelae in the case of delayed treatment. Although historically the recommendations for treatment have been based on surgery and prolonged antibiotic therapy, recent studies have demonstrated that short-course antibiotic therapy is also effective. OBJECTIVES: We evaluated a short-term antibiotic protocol for both osteomyelitis and septic arthritis in a 6 year retrospective study at the University Hospital of Montpellier. METHODS: This pro

Discriminating Osteoarticular Infection Bacterial Aetiology Using Immune-Inflammatory Biomarkers.

Identifying the causative pathogen of paediatric osteoarticular infections (OAIs) is essential for guiding management. Kingella kingae infections are generally milder than pyogenic infections, which may have more severe and lasting consequences. Because standard inflammatory markers have limited discriminatory value, we assessed whether complete blood count-derived immune-inflammatory biomarkers (IIBs) could provide a simple, low-cost diagnostic aid. We retrospectively reviewed children younger than 5 years admitted to a tertiary hospital with confirmed OAIs. K. kingae cases were included from

Identification of integrative and conjugative elements in pathogenic and commensal Neisseriaceae species via genomic distributions of DNA uptake sequence dialects.

Mobile genetic elements (MGEs) are key factors responsible for dissemination of virulence determinants and antimicrobial-resistance genes amongst pathogenic bacteria. Conjugative MGEs are notable for their high gene loads donated per transfer event, broad host ranges and phylogenetic ubiquity amongst prokaryotes, with the subclass of chromosomally inserted integrative and conjugative elements (ICEs) being particularly abundant. The focus on a small number of model systems has biased the study of ICEs towards those conferring readily selectable phenotypes to host cells, whereas the identificati

Urgent Surgical Treatment of Aortic Endocarditis in Infants and Children.

Infective endocarditis (IE) in the pediatric population can present as a life-threatening condition. Optimal timing for surgical intervention should consider surgical risks versus the risk of neurologic complications. We herein report our experience with this group of critically ill children. Retrospective analysis of patient charts of all patients who underwent urgent surgical treatment of aortic IE from 1994 to 2014 was performed. Nine patients with acute storming IE of the aortic valve or the ascending aorta were urgently operated (eight normal heart, one congenital aortic stenosis), age ra

How Should Bacteriological Sampling Be Stratified in Paediatric Septic Arthritis? A Narrative Review with a Proposed Risk-Stratified Framework.

Joint aspiration remains the gold standard and an urgent step in the diagnosis of paediatric septic arthritis (SA). Unlike in acute haematogenous osteomyelitis, it has both diagnostic and therapeutic value. No previous review has specifically addressed when bacteriological sampling is essential and when it may reasonably be omitted. The bacteriological profile in children is highly age-dependent: Kingella kingae predominates before 4 years of age, whereas Staphylococcus aureus-including Panton-Valentine leukocidin (PVL)-producing and methicillin-resistant (MRSA) strains-predominates thereafter
