Surveillance of Lassa fever in missionaries stationed in West Africa.

To determine the distribution of Lassa virus in West Africa, a serological survey was undertaken. A number of mission hospital supplied sera from patients admitted with a history of fever and specimens were also collected in New York from missionaries who had experienced an unusual febrile illness while working in Africa. More cases of Lassa fever were detected among missionaries than among Africans, possibly because many African patients had left hospital before the complement fixation tests had become positive. Although most adults had fairly high fever and some were prostrated, fever was le

Detection of Lassa virus RNA in specimens from patients with Lassa fever by using the polymerase chain reaction.

Suitable oligonucleotide primers and probes were synthesized to amplify Lassa virus (Josiah strain)-specific nucleoprotein and glycoprotein gene fragments by using reverse transcription combined with the polymerase chain reaction (PCR). Our primers did not amplify the related lymphocytic choriomeningitis virus. By using PCR, about 50 50% tissue culture infective doses could be detected in the supernatant of infected cells. Furthermore, in all five serum specimens and four of five urine specimens of patients with acute Lassa fever, viral RNA could be demonstrated. Negative results were obtained

Expansion of myeloid suppressor cells and suppression of Lassa virus-specific T cells during fatal Lassa fever.

Lassa fever is a highly lethal hemorrhagic fever endemic to West Africa. In the absence of efficient prophylactic or therapeutic countermeasures, it poses a substantial threat to public health in this region. The pathophysiological mechanisms underlying the severity of the disease are poorly known because Lassa virus (LASV), its causative agent, has to be handled in BSL-4 laboratories and access to clinical samples is difficult. The control of Lassa fever is associated with a rapid and well-balanced immune response and viral clearance. However, severe disease is characterized by uncontrolled i

Lassa virus-like particles displaying all major immunological determinants as a vaccine candidate for Lassa hemorrhagic fever.

BACKGROUND: Lassa fever is a neglected tropical disease with significant impact on the health care system, society, and economy of Western and Central African nations where it is endemic. Treatment of acute Lassa fever infections has successfully utilized intravenous administration of ribavirin, a nucleotide analogue drug, but this is not an approved use; efficacy of oral administration has not been demonstrated. To date, several potential new vaccine platforms have been explored, but none have progressed toward clinical trials and commercialization. Therefore, the development of a robust vacc

Passive antibody therapy of Lassa fever in cynomolgus monkeys: importance of neutralizing antibody and Lassa virus strain.

Lassa virus-infected cynomolgus monkeys were passively immunized with immune plasma of primate or human origin to gain insight into criteria for plasma selection and administration to human Lassa fever patients. Protective efficacy was correlated with neutralizing antibody concentrations, expressed as a log10 neutralization index (LNI). Convalescent Lassa-immune monkey plasma was titrated for protective efficacy in monkeys by intravenous inoculation with dilutions of plasma on the day of subcutaneous Lassa virus inoculation (day 0) and again on days 3 and 6. Monkeys that received undiluted pla

Identifying immunodominant multi-epitopes from the envelope glycoprotein of the Lassa mammarenavirus as vaccine candidate for Lassa fever.

PURPOSE: Lassa fever is a zoonotic acute viral hemorrhagic disease caused by Lassa virus (LASV). There is currently no licensed vaccine for the prevention of the disease. This study is aimed at discovering immunodominant epitopes from the envelope glycoprotein of the Lassa mammarenavirus and designing of a multi-epitope vaccine candidate (VC). MATERIALS AND METHODS: The amino acid sequences of the envelope glycoprotein of 26 strains of LASV from five countries were selected. After evaluation for antigenicity, immunogenicity, allergenicity, and toxicity, immunodominant CD8, CD4, and linear B l

The sensitivity and specificity of Lassa virus IgM by ELISA as screening tool at early phase of Lassa fever infection.

BACKGROUND: Early diagnosis, prompt treatment, and disease containment are vital measures in the management of Lassa fever (LF), a lethal and contagious arenaviral hemorrhagic disease prevalent in West Africa. Lassa Virus (LAV)-specific Reverse Transcriptase Polymerase Chain Reaction (RT-PCR) test, the gold standard for diagnosis, is unavailable in most centers. Serologic detection of LAV IgM is a more accessible tool and this work was to investigate its adequacy as an early marker for LF. PATIENTS AND METHODS: A prospective case-control study conducted July 2007-March 2011 in a tertiary refe

Knowledge of Lassa fever and use of infection prevention and control facilities among health care workers during Lassa fever outbreak in Ondo State, Nigeria.

INTRODUCTION: Hospital-acquired infections of Lassa fever (LF) have been described in many West African countries. We assessed the availability of Infection Prevention and Control (IPC) measures and their use in the health centres (HCs) at the affected Local Government Areas (LGAs) during an ongoing LF outbreak in Ondo State, Nigeria. METHODS: We included all primary and secondary HCs and their healthcare workers (HCWs) in the affected Ose and Owo LGAs. We collected data from respondents using self-administered questionnaires and used a checklist to assess the IPC measures at the HCs. We gene

Lassa fever prediction using data from Nigeria's national surveillance registry.

Lassa fever is a dangerous viral illness that is endemic in Nigeria and clinically difficult to separate from malaria and other febrile conditions in its early phase, which delays diagnosis and treatment. Machine learning has been proposed as a triage aid, yet several reported successes rest on small samples. Using the openly available national surveillance dataset of 20,027 suspected cases, of which 15.8% were laboratory-confirmed, we predicted laboratory confirmation from pre-laboratory bedside features alone. Synthetic oversampling was confined to training folds. Nine classifiers, including

Neural network modeling of Lassa fever spread and disability effects.

Lassa fever is one of the viral hemorrhagic illnesses produced through the Lassa virus and may be transmitted to individuals via contact with diseased rodents or their feces and urine. The current study presents the solutions to the nonlinear zoonotic spread of Lassa virus fever leading to disabilities model by designing a supervised neural network. The model is divided into the human (susceptible, severely infected, mildly infected, and recovered) and rodent (susceptible, infected) populations. The dynamic structure of one class to the other performs the zoonotic spread of Lassa virus fever m

Lassa Fever: Viral Replication, Disease Pathogenesis, and Host Immune Modulations.

Despite major discoveries made in the last few decades about Lassa fever, there are still many unresolved key issues that hamper the development of effective vaccines and therapies against this deadly disease that is endemic in several West African countries. Some of these issues include the lack of a detailed understanding of the viral and participating host factors in completing the virus life cycle, in mediating disease pathogenesis or protection from disease, and in activating or suppressing host innate and cellular immunity against virus infection, as well as of the animal models required

Data set on Lassa fever in post-conflict Sierra Leone.

Lassa fever is a rodent-borne illness that is endemic to parts of sub-Saharan Africa, including Sierra Leone, Nigeria, and Guinea. The disease is named after the town of Lassa, Nigeria where it was discovered in 1969. This data article focuses on the epidemiology of Lassa fever in Sierra Leone following a decade-long civil war that ended in 2002. The data were collected at Kenema Government Hospital (KGH) in Kenema, Sierra Leone, which maintains the country׳s only Lassa fever treatment facility and a biosafety level 3 (BSL-3) laboratory. The key data set variables include Lassa fever serostatu

Management of a Lassa fever outbreak, Rhineland-Palatinate, Germany, 2016.

Due to rapid diagnosis and isolation of imported cases, community outbreaks of viral haemorrhagic fevers (VHF) are considered unlikely in industrialised countries. In March 2016, the first documented locally acquired case of Lassa fever (LF) outside Africa occurred, demonstrating the disease's potential as a cross-border health threat. We describe the management surrounding this case of LF in Rhineland-Palatinate - the German federal state where secondary transmission occurred. Twelve days after having been exposed to the corpse of a LF case imported from Togo, a symptomatic undertaker tested

Lassa fever - full recovery without ribavarin treatment: a case report.

BACKGROUND: Lassa fever is a rodent-borne zoonosis that clinically manifests as an acute hemorrhagic fever. It is treated using ribavarin. Surviving Lassa fever without receiving the antiviral drug ribavarin is rare. Only few cases have been documented to date. CASE PRESENTATION: We report a case of a 59-year old female with fever who was initially thought to have acute pyelonephritis and sepsis syndrome with background malaria. Further changes in her clinical state and laboratory tests led to a suspicion of Lassa fever. However at the time her laboratory confirmatory test for Lassa fever ret

A recombinant Yellow Fever 17D vaccine expressing Lassa virus glycoproteins.

The Yellow Fever Vaccine 17D (YFV17D) has been used as a vector for the Lassa virus glycoprotein precursor (LASV-GPC) resulting in construction of YFV17D/LASV-GPC recombinant virus. The virus was replication-competent and processed the LASV-GPC in cell cultures. The recombinant replicated poorly in guinea pigs but still elicited specific antibodies against LASV and YFV17D antigens. A single subcutaneous injection of the recombinant vaccine protected strain 13 guinea pigs against fatal Lassa Fever. This study demonstrates the potential to develop an YFV17D-based bivalent vaccine against two vir

Lassa fever: implications of T-cell immunity for vaccine development.

Lassa fever is a re-emerging viral hemorrhagic fever, which causes significant human morbidity in endemic regions of West Africa. Attempts to vaccinate against this virus in animal models including non-human primates have revealed that eliciting a strong cellular immune response protects from clinical disease, but not infection, in the absence of measurable neutralizing antibodies. As there is renewed interest in developing a vaccine against Lassa fever for use in humans, several questions should be addressed in view of the scarce knowledge of the mechanisms of natural immunity against this di

Lassa fever in Onitsha, East Central State, Nigeria in 1974.

Three cases of Lassa fever occurred in Onitsha, East Central State, Nigeria, in January and February 1974. The first case was a 19-year-old Nigerian; the other 2 cases were German missionary physicians at St Charles Borromeo Hospital, Onitsha, one of whom cared for the patient who was the first case. Thus, 2 of the 3 cases were hospital acquired. Investigations failed to discover a village outbreak or the source of virus for the first case. A serosurvey of 258 hospital staff members and contacts of the 3 cases showed no other persons with antibody to Lassa virus. The absence of Lassa virus ant
