Risk factors for domestic physical violence: national cross-sectional household surveys in eight southern African countries.

BACKGROUND: The baseline to assess impact of a mass education-entertainment programme offered an opportunity to identify risk factors for domestic physical violence. METHODS: In 2002, cross-sectional household surveys in a stratified urban/rural last-stage random sample of enumeration areas, based on latest national census in Botswana, Lesotho, Malawi, Mozambique, Namibia, Swaziland, Zambia and Zimbabwe. Working door to door, interviewers contacted all adults aged 16-60 years present on the day of the visit, without sub-sampling. 20,639 adults were interviewed. The questionnaire in 29 languag

Outcomes of comprehensive care for children empirically treated for multidrug-resistant tuberculosis in a setting of high HIV prevalence.

BACKGROUND: Few studies have examined outcomes for children treated for multidrug-resistant tuberculosis (MDR-TB), including those receiving concomitant treatment for MDR-TB and HIV co-infection. In Lesotho, where the adult HIV seroprevalence is estimated to be 24%, we sought to measure outcomes and adverse events in a cohort of children treated for MDR-TB using a community-based treatment delivery model. METHODS: We reviewed retrospectively the clinical charts of children ≤15 years of age treated for culture-confirmed or suspected MDR-TB between July 2007 and January 2011. RESULTS: Nineteen

Predictors of self-reported sexually transmitted infections (STIs) among men in 54 low and middle income countries (LMICs): a comparison of deep learning and classical machine learning algorithms.

INTRODUCTION: Sexually transmitted infections (STIs) remain a global health challenge, with 374 million new cases annually. Among men, STIs cause serious complications, increase the risk of HIV infection, and contribute to stigma and economic burdens, especially in low and middle income countries. Traditional models often miss complex risk interactions, while machine learning and deep learning offer stronger predictive tools. This study applied deep learning and traditional machine learning algorithms to predict self-reported STIs and to model predictors of self-reported STIs among men in 54 L

"I Feel Like I Am a Winner:" High Levels of Autonomous Decision-Making Among Women Receiving HIV Pre-Exposure Prophylaxis (PrEP) Choice Counselling in Sub-Saharan Africa.

As pre-exposure prophylaxis (PrEP) options expand, providers play a critical role in supporting clients to select the method that best meets their needs. We examined how client-provider interactions during PrEP choice counselling influenced women's autonomous decision-making and early continuation on PrEP. CATALYST was a mixed-methods study that delivered an informed choice of oral PrEP, the dapivirine vaginal ring, and injectable cabotegravir among women attending 28 public-sector facilities in Kenya, Lesotho, South Africa, Uganda, and Zimbabwe. We analysed cohort data collected between June

Tolerability of lopinavir versus dolutegravir in children and adolescents with HIV.

BACKGROUND: Children and adolescents with HIV previously taking ritonavir-boosted lopinavir (LPV/r)-based antiretroviral therapy (ART) were recently programmatically transitioned to dolutegravir (DTG)-based ART in Lesotho, southern Africa. We investigated associated changes in treatment satisfaction and potential side effects. METHODS: This single-center prospective cohort study enrolled participants younger than 18 years transitioned from LPV/r-based to DTG-based ART during the national programmatic DTG rollout in 2022-2023. Virally suppressed participants who were at least 6 years old and a

Accuracy of chest x-ray screening of silica-exposed miners for tuberculosis.

BACKGROUND: The World Health Organization recommends systematic chest X-ray (CXR) screening for tuberculosis (TB) in silica-exposed workers. However, evidence on the accuracy of CXR screening in such populations is lacking. AIMS: To measure the accuracy of CXR screening for active TB in silica-exposed miners, in a population with a high prevalence of silicosis, post-TB lung disease and HIV. METHODS: A secondary analysis of data from a miner screening programme in Lesotho was undertaken. We measured the performance of CXR (in participants with and without cough) for 'abnormalities suggestive

Numeracy skills learning of children in Africa:-Are disabled children lagging behind?

Significant progress has been achieved in universal basic education in African countries since the late 1990s. This study provides empirical evidence on the within- and across-country variation in numeracy skills performance among children based on nationally representative data from eight African countries (DR Congo, The Gambia, Ghana, Lesotho, Sierra Leone, Togo, Tunisia, and Zimbabwe). We assess whether and to what extent children with disabilities lag in numeracy skills and how much it depends on their type of disabilities. More specifically, we explore whether disabled children benefit eq

Optimal Timing of Antiretroviral Therapy Initiation in Children and Adolescents With Human Immunodeficiency Virus-Associated Pulmonary Tuberculosis.

BACKGROUND: There is insufficient evidence in children and adolescents with human immunodeficiency virus (CAHIV) to guide the timing of antiretroviral treatment (ART) initiation after starting treatment for pulmonary tuberculosis (pTB). To address this knowledge gap, we evaluated the risk of mortality associated with timing of ART initiation in ART-naive CAHIV treated for pTB. METHODS: Data were extracted from electronic medical records of ART-naive patients, aged 0-19 years, who were treated for HIV-associated pTB at Baylor Centers of Excellence in Botswana, Eswatini, Malawi, Lesotho, Tanzan

5th International AIDS Society Conference on HIV Pathogenesis, Treatment and Prevention: summary of key research and implications for policy and practice - operations research.

Operations research was added as a fourth scientific track to the pathogenesis conference series at the 5th IAS Conference on HIV Pathogenesis, Treatment and Prevention (IAS 2009) in recognition of the importance of this growing research field and the need for applied research to inform and evaluate the scale up of some key interventions in HIV treatment, care and prevention.Several studies demonstrated how task shifting and the decentralization of health services can leverage scarce health care resources to support scale-up efforts. For example, a Ugandan study comparing home-based and facili

Promethazine use and QT prolongation risk during treatment with a shortened all-oral regimen for MDR/RR-TB.

BACKGROUND: Shortened all-oral regimens for multidrug- or rifampicin-resistant tuberculosis (MDR/RR-TB) combine multiple QTc prolonging agents, including bedaquiline and delamanid, which act partly through hERG potassium channel binding. Promethazine, a first-generation antihistamine used for nausea during TB therapy, shares this mechanism. Whether adjunctive promethazine further raises the risk of clinically relevant QTc prolongation in this setting is unclear. METHODS: We conducted secondary analyses from a prospective observational cohort study of patients ≥15 years in Lesotho who received
