Tracheoesophageal fistula (TEF) is a rare but devastating complication of mechanical ventilation and tracheostomy, classically described as a late sequela of prolonged airway instrumentation. Patients with severe burn injury and inhalation injury represent a uniquely vulnerable population due to airway mucosal ischemia, thermal injury, and impaired tissue healing. We report a case of fulminant posterior tracheal wall failure with early TEF formation occurring within days of tracheostomy in a 75-year-old patient with extensive full-thickness facial burns following thermal inhalation injury from

At birth the transition by the fetal situation to neonatal life occurs. The real mechanism of lung liquid reabsorption and lung aereation at birth, in the past attributed only to the epithelial sodium channel function, has recently been linked to the first important breaths too and to the following changes in transpulmonary pressure. If this quite easily happens in the term baby, the preterm infants and all the babies with poor respiratory effort may have a delayed achievement of an adequate functional residual capacity (FRC). Premature delivery is always associated to the failure of respirato

There is no precise definition for severe community-acquired pneumonia (SEHP), but there are a number of factors which are associated with a greater severity, and which therefore recommended the admission of these patients in an intensive care unit (ICU). In the present article, we mainly refer to SEHP in the immune competent population. SEHP makes up 8-10% of the total number of admissions of an ICU although this depends greatly on the type of unit concerned. The majority of patients admitted, do so because they need mechanical ventilation, because they present a shock situation, or because t

Although much has been learned about BPD in the 25 years since its initial description, BPD remains a significant complication of prematurity. Substantial advances into the understanding of its pathophysiology and pathogenesis have been made and are reflected in new therapeutic interventions. Much current research is directed towards the role of prevention, exploring new approaches for accelerating lung maturation with combined maternal steroid and thyrotropin releasing hormone (TRH) therapy, surfactant replacement therapy, high frequency oscillatory ventilation, antioxidant administration, ma

The indoor use of household insecticides introduces complex mixtures of volatile organic compounds (VOCs) into residential environments, raising potential health concerns. This study characterized the emission profiles of total VOCs (TVOCs) and targeted aromatic compounds (BTEXS) from 19 household insecticides, comprising aerosol insect sprays (IS), liquid vaporizers (EV), mat-type fumigators (MA), and mosquito coils (MC), using environmental test chambers and TD-GC-MS analysis. Emission rates (ERs) and emission factors (EFs) were calculated, and health risks were assessed using non-carcinogen

The presence of meconium in the respiratory tract causes atelectasis, hypoxemia, hypercapnia, persistent pulmonary hypertension, inflammatory changes, and surfactant inactivation. Prevention is the most important factor in the management of meconium aspiration syndrome and it includes both prenatal and postnatal care. Prenatal procedures include constant fetal heart rate monitoring during labor, examination of acid-base equilibrium in the capillary blood from the fetal scalp, and eventually amnioinfusion. Amnioinfusion is not a widely accepted care and further studies to confirm its benefits a

Sulbactam-durlobactam is a combination β-lactam/β-lactamase inhibitor antibiotic approved in the United States for the treatment of adult patients with hospital-acquired and ventilator-associated bacterial pneumonia due to susceptible isolates of Acinetobacter baumannii-calcoaceticus complex. A liquid chromatography tandem mass spectrometry method for the quantification of sulbactam and durlobactam in human plasma and aqueous matrices has been developed and validated. The standard curves for each drug were linear over the range 0.5 to 50 μg/mL and use the isotopic analogs sulbactam-d5 and [13C

During strenuous exercise in sheep, lung lymph flow increases within seconds and rises to levels 7- to 10-fold over baseline. Concomitant with the flow increase, the lymph protein content rapidly decreases to levels consistent with severe capillary hypertension. This pattern of clearance of filtered fluid is quite different than is seen with the passive capillary hypertension that results from mechanical obstruction of the mitral valve. In passive capillary hypertension, the increase in lymph flow and reduction in lymph protein content develop over several hours. The purpose of this study was

Calfactant is an animal-derived surfactant, FDA-approved for endotracheal tube administration. Complications during surfactant administration may be related to the volume being delivered. We conducted a controlled comparison of calfactant against a more concentrated, lower volume calfactant. We compared the efficacy and complications of delivery of the two calfactant formulations to each other, and to no surfactant treatment, in the well-established preterm newborn lamb model of surfactant-deficient respiratory distress syndrome. After initial stabilization, a single dose of liquid calfactant

BACKGROUND: Mercury is a complex toxin with clinical manifestations determined by the chemical form, route, dose, and acuity of the exposure. Parenteral injection of elemental mercury remains uncommon.
CASE REPORT: A 40-year-old male injected 3 mL of elemental mercury intravenously and ingested 3 mL as a suicide attempt. Within 24 hours, he became dyspneic, febrile, tachycardic, and voiced mild gastrointestinal complaints. Chest X-ray revealed scattered pulmonary infiltrates and embolized mercury bilaterally. A ventilation/perfusion scan demonstrated ventilation/ perfusion deficits. Additiona

Most pediatric surgeons are wary of gastric transposition as a means for esophageal replacement in children, especially during infancy. We present our initial experience of this technique in five children followed up for an average 1.7 years postoperatively. Four of them were infants (age range 5 m to 10 m at the time of transposition) with wide-gap esophageal atresia, while one was operated upon for an extensive corrosive esophageal stricture at 4 years age. The stomach was placed transhiatally in 3 patients and through the retrosternal route in 2 patients. A gastric outlet drainage procedure

A 24-year old student died from asphyxiation while filling flasks with liquid nitrogen. The upper and lower extremities and the back were frozen. The face, ears and neck were livid with a horizontal line of demarcation. A few wide ventricular complexes were seen in the initial ECG, followed by asystole. The patient's trachea was intubated, his lungs ventilated with 100% oxygen and CPR initiated. The site of larynx and pharynx were without pathological findings. 250 ml of a crystalloid solution were infused into the external jugular vein. It was not possible to measure the body temperature. Exogenous lipoid pneumonia (ELP) is a rare pulmonary disorder resulting from the aspiration or inhalation of exogenous lipid substances. While most documented cases follow an indolent, chronic course and are associated with favorable outcomes, fulminant postoperative ELP remains exceedingly uncommon-often presenting with rapid respiratory decompensation that may clinically and radiologically mimic severe community-acquired pneumonia or acute respiratory distress syndrome (ARDS). We report a 61-year-old man with occult fulminant postoperative ELP after repeated oral liquid paraffin use for bowe

Fluorocarbons are characterized by exceptional chemical and biological inertness, extreme hydrophobicity, lipophobicity, high gas-dissolving capacities, low surface tensions, high fluidity and spreading coefficients, high density, absence of protons, and magnetic susceptibilities comparable to that of water. These unique properties are the foundation for a range of biomedical applications. An injectable fluorocarbon-in-water emulsion is in advanced clinical trials as a temporary oxygen carrier (blood substitute) to prevent tissue hypoxia or ischemia in the surgical and critical care patient. OBJECTIVES: To investigate factors associated with indoor formaldehyde levels in office settings within an academic medical institute.
METHODS: This cross-sectional study was conducted in 25 offices (261 workers) at a medical university in Thailand. Questionnaires gathered data on demographics, work patterns, and office equipment usage (printers, photocopiers, air fresheners, liquid paper, glue, cleaning agents, and marker pens). The building environment was assessed by a multidisciplinary team. Formaldehyde levels and relevant parameters (temperature and relative humidity) were measured in e

Although supplemental oxygen is necessary to support preterm infants at birth, it is unclear what fraction of inspired oxygen ([Formula: see text]) should be used. We hypothesized that the presence of liquid in the distal gas exchange regions of the lung will impede oxygen exchange and so the [Formula: see text] levels required during lung aeration will rapidly decrease. Using standardized ventilator settings, we examined the effects of high, mid, and low [Formula: see text] on oxygenation during and after lung aeration. Preterm lambs (126/147 days) were intubated and instrumented with cathete

BACKGROUND: The role of extracorporeal membrane oxygenation (ECMO) in patients with congenital diaphragmatic hernia is still evolving. The use of ECMO is invasive with potential complications during instrumentation for cannulation and heparinization. There are no reliable predictors of outcome in patients requiring ECMO. We aimed to identify (a) the factors that could predict outcome and (b) the incidence and relation of complications during ECMO to outcome.
METHODS: "Pre" ECMO (age, sex, birth weight, blood gasses, and ventilator settings) and "on" ECMO variables (mode of ECMO, use of nitric

The first description diaphragmatic hernia appeared in 1575. In 1848, Bochdalek described congenital diaphragmatic hernia (CDH) occurring through a posterolateral defect. Successful surgical treatment of CDH in an infant was first performed in 1902, whereas the first neonate operated within 24 hours of life was reported in 1946. However, early surgery did not improve survival rates and the mortality was in the region of 50%. One reason for this was that more neonates underwent surgery who previously would have died without the repair of the CDH. Pulmonary hypoplasia and pulmonary hypertension

BACKGROUND: Transient tachypnea of the newborn (TTN) is characterized by tachypnea and signs of respiratory distress. Transient tachypnea typically appears within the first two hours of life in term and late preterm newborns. Supportive management might be sufficient. Non-invasive (i.e. without endotracheal intubation) respiratory support may, however, be administered to reduce respiratory distress during TTN. In addition, non-invasive respiratory support might improve clearance of lung liquid thus reducing the effort required to breathe, improving respiratory distress and potentially reducing
