Liver support systems for adults with acute liver failure.

RATIONALE: Acute liver failure (ALF) is a life-threatening condition, characterised by serious damage to the liver cells and function. The aetiology of ALF determines the prognosis, and without immediate care or liver transplantation, people with ALF will develop life-threatening complications and die. Liver support systems are used as a bridge therapy to provide detoxification and restore liver function until either functional recovery occurs or a liver becomes available for transplantation. Liver support systems have continued to be used despite conflicting results of randomised clinical tri

Living Donor Liver Transplantation for Colorectal Cancer Liver Metastasis.

Liver transplantation (LT) for unresectable colorectal liver metastases has evolved from early feasibility studies to a biologically selected, multidisciplinary strategy for patients with liver-confined disease. Evidence from the SECA program and randomized data from TransMet support meaningful survival in carefully chosen candidates. Contemporary selection emphasizes sustained chemotherapy response, time-based observation, PET-derived metabolic burden, CEA kinetics, and extrahepatic disease. Living donor LT has expanded LT access within these established frameworks, offering elective timing a

Nanotherapy targeting liver sinusoidal endothelial cells in liver fibrosis.

Liver fibrosis represents a common pathological process in the progression of most chronic liver diseases. While hepatic stellate cells (HSCs) have long been recognized as central drivers of fibrogenesis, growing evidence highlights the critical role of liver sinusoidal endothelial cells (LSECs) in maintaining hepatic homeostasis, regulating vascular tone, and facilitating bidirectional molecular exchange between the bloodstream and hepatocytes through their unique fenestrated architecture. During liver fibrosis, however, LSECs undergo capillarization, namely the loss of fenestrae, the formati

Neurologic Consequences of Liver Disease: The Liver-Brain Axis.

The liver serves as a key metabolic and inflammatory organ involved in maintaining homeostasis of different organs and systems. This includes ensuring proper functioning of the central nervous system through a liver-brain axis that has bidirectional communication via signaling mediators and direct neural pathways. Damage to, or disease of, the liver from various etiologies leads to a loss of hepatic function, which then impacts cognitive, motor, and other neurologic functions. Severity, type, and duration of liver dysfunction can impact the brain in different ways via different metabolic and i

Liver TET1 promotes metabolic dysfunction-associated steatotic liver disease.

Global hepatic DNA methylation change has been linked to human patients with metabolic dysfunction-associated steatotic liver disease (MASLD). DNA demethylation is regulated by the TET family proteins, whose enzymatic activities require 2-oxoglutarate (2-OG) and iron that both are elevated in human MASLD patients. We aimed to investigate liver TET1 in MASLD progression. Depleting TET1 using two different strategies substantially alleviated MASLD progression. Knockout (KO) of TET1 slightly improved diet induced obesity and glucose homeostasis. Intriguingly, hepatic cholesterols, triglycerides,

Recurrent and De Novo Liver Disease After Liver Transplantation.

Disease recurrence after liver transplantation (LT) is common. Certain liver diseases such as autoimmune hepatitis and steatotic liver disease may appear de novo after LT. This review discusses post LT alcohol-associated liver disease, metabolic dysfunction-associated liver disease, autoimmune hepatitis, primary biliary cholangitis, and primary sclerosing cholangitis. Both recurrent and de novo diseases are important causes of allograft failure.

Protocol liver biopsy predicts graft survival after liver transplantation.

BACKGROUND: The use of protocol liver biopsy to monitor liver allograft status remains controversial. There is limited data from modern transplantation populations that includes protocol biopsies to evaluate its value in predicting clinical outcomes. METHODS: All protocol liver biopsies were identified from 875 patients who underwent liver transplantation at Helsinki University Hospital between 2000 and 2019. Each histologic component was analyzed for its ability to predict long-term outcomes, especially graft survival. We determined the frequency of significant biopsy findings based on the B

Instrumentless liver suspension for liver retraction in robotic pancreatoduodenectomy.

The popularity of robotic pancreatoduodenectomy (RPD) is increasing, yet it remains a complex procedure. Outcomes are influenced by various factors, including patient-specific variables, disease characteristics, and surgical technique. Numerous and intricate details contribute to the technical success of RPD. In this study, our focus is on achieving effective and "gentle" liver retraction. The use of liver retractors has been associated with the risk of retractor-related liver injury (RRLI), which can have serious consequences. Here, we introduce a refined technique for instrumentless liver re

Hydrogen Sulfide and Liver Health: Insights into Liver Diseases.

Significance: Hydrogen sulfide (H2S) is a recently recognized gasotransmitter involved in physiological and pathological conditions in mammals. It protects organs from oxidative stress, inflammation, hypertension, and cell death. With abundant expression of H2S-production enzymes, the liver is closely linked to H2S signaling. Recent Advances: Hepatic H2S comes from various sources, including gut microbiota, exogenous sulfur salts, and endogenous production. Recent studies highlight the importance of hepatic H2S in liver diseases such as nonalcoholic fatty liver disease (NAFLD), liver injury, a

Lipid alterations in chronic liver disease and liver cancer.

Lipids are a complex and diverse group of molecules with crucial roles in many physiological processes, as well as in the onset, progression, and maintenance of cancers. Fatty acids and cholesterol are the building blocks of lipids, orchestrating these crucial metabolic processes. In the liver, lipid alterations are prevalent as a cause and consequence of chronic hepatitis B and C virus infections, alcoholic hepatitis, and non-alcoholic fatty liver disease and steatohepatitis. Recent developments in lipidomics have also revealed that dynamic changes in triacylglycerols, phospholipids, sphingol

Increasing Living Donor Liver Transplantation Using Liver Paired Exchange.

BACKGROUND: Living donor liver transplantation (LDLT) continues to be the primary modality of liver transplantation in Asia, but it accounts for about 5% of all liver transplantations in the US. ABO incompatibility is the primary reason motivated donors are declined. Although kidney paired exchanges are common, liver paired exchange (LPE) is still evolving in the US. STUDY DESIGN: This is a retrospective review (between January 1, 2019, and July 31, 2021) of our initial experience with LPE. RESULTS: A total of 10 LPEs (20 LDLTs) were performed during the study period. Seven LPEs were initiat

Gut Microbiota and Liver Injury (II): Chronic Liver Injury.

Chronic liver injury mainly comprises viral hepatitis, fatty liver disease, autoimmune hepatitis, cirrhosis and liver cancer. It is well established that gut microbiota serves as the key upstream modulator for chronic liver injury progression. Indeed, the term "gut-liver axis" was mostly applied for chronic liver injury. In the current chapter, we will summarize the relationship between gut microbiota and chronic liver injury, including the interaction between them based on latest clinic and basic research.

Liver function tests in identifying patients with liver disease.

BACKGROUND AND AIMS: Many patients with liver disease come to medical attention once they have advanced cirrhosis or acute decompensation. Most often, patients are screened for liver disease via liver function tests (LFTs). There is very limited published data evaluating laboratory values with biopsy-proven stages of hepatic fibrosis. We set out to evaluate whether any correlation exists between routine LFTs and stages of hepatic fibrosis. METHODS: A large retrospective observational study on 771 liver biopsies was conducted for evaluating the stage of fibrosis with AST, ALT, INR, BUN, creati
