This is a retrospective study of clinical data from 106 patients with pyogenic liver abscess (PLA) treated in a traditional Chinese hospital during the eight years preceding this publication. We aimed to provide evidence to improve the diagnosis accuracy and the treatment strategies for PLAs. We collected records of patients treated at the Guangxing Hospital, which is affiliated to the Zhejiang Traditional Chinese University in Hangzhou, and we collected their general background information, laboratory and imaging features, and clinical manifestations and outcomes to perform a retrospective an

BACKGROUND: Juxta-papillary duodenal diverticulum (JPDD) has been associated with obstructive jaundice and ascending cholangitis. Potential mechanisms include periampullary colonization of pathogenic bacteria and mechanical obstruction. However, the relation of JPDD with pyogenic liver abscess (PLA) has not been reported. Moreover, approximately one third of patients with PLA have no identifiable risk factors and are labelled as "cryptogenic". We hypothesized that JPDD is an unidentified risk factor for cryptogenic PLA and the aim of this study was to examine this association.

BACKGROUND: Proton pump inhibitors (PPI) are often used in patients with gastro-esophageal reflux and peptic ulcer disease. A higher risk for infectious diseases and for pyogenic liver abscess has been reported in patients with prolonged PPI intake. Although many patients have ongoing PPI treatment after diagnosis of liver abscess, there are no data available that focus on the prognostic impact of PPI treatment in these patients.
AIM: To analyse the effect of PPI treatment on mortality in patients with pyogenic liver abscesses.
METHODS: Between January 2005 and March 2017, one hundred and ei

BACKGROUND: Transdiaphragmatic extension of pyogenic liver abscess is the rarest cause of pericarditis and pleural empyema. It is a rapidly progressive and highly lethal infection with mortality rates reaching 100% if left untreated. However, the transmission route, treatment methods and prognosis have not been well studied.
CASE PRESENTATION: A 65-year-old male patient presented with a fever, dyspnea, and right upper quadrant abdominal pain. Computed tomography of the chest and abdomen showed huge liver abscess without full liquefaction in the left lobe, large amount of left pleural effusion

A 66-year-old male patient presented with symptoms and signs of L4/5 radiculopathy. He was found to be anaemic with elevated inflammatory markers and deranged hepatic enzymes. Imaging revealed lumbar canal stenosis and the presence of pyogenic liver abscesses from which Fusobacterium nucleatum and Streptococcus vestibularis were isolated. The hepatic abscesses were attributed to asymptomatic diverticular perforation. Multiple coexisting incidental infections were discovered, including oesophageal candidiasis, Helicobacter pylori, stool cultures positive for Strongyloides stercoralis, and sputu

Papillon-Lefevre syndrome (PLS) is an autosomal recessive disease that is characterized by symmetric palmoplantar keratodermatitis and severe periodontal destruction. Mutations in the cathepsin C gene (CTSC) have recently been detected in PLS. Immune dysregulation, due to a mutation in CTSC, increases the risk of pyogenic infections in PLS patients. A child with PLS is presented here with liver abscesses and peritonitis caused by Rhizopus oryzae. His liver abscess and peritonitis were cured with amphotericin B without surgical care. This is the first case in the literature liver abscess due to

OBJECTIVES: Pyogenic liver abscess (PLA) is a life-threatening infection with high mortality in Asia. Although Klebsiella pneumoniae is commonly implicated, emerging data suggest a more diverse microbial spectrum. This study investigated pathogen detection using conventional culture and next-generation sequencing (NGS) and characterized gut microbiome alterations in PLA patients compared to healthy controls.
METHOD: This was a prospective, multicenter cohort study conducted across eight tertiary hospitals. We enrolled 100 PLA patients who underwent percutaneous aspiration. Abscess aspirates u

INTRODUCTION: Pyogenic liver abscess (PLA) is a common intra-abdominal infection with substantial morbidity and mortality, among which gas-forming pyogenic liver abscess (GFPLA) represents a more severe clinical subtype. Basically, GFPLA is usually associated with more severe clinical outcomes compared to non-gas-forming PLA (non-GFPLA). The underlying mechanisms driving gas formation remain unclear. This study aimed to explore clinical, microbial, and metabolic characteristics of GFPLA.
MATERIALS AND METHODS: A total of 176 PLA patients (39 GFPLA, 137 non-GFPLA) were retrospectively analyzed

Hepatic actinomycosis is an uncommon diagnostic mimic of pyogenic or amoebic liver abscess. A 34-year-old healthy man presented with fever, vomiting, and right upper quadrant pain after recent travel. Computed tomography showed a 7 x 7 x 5 cm subcapsular segment VIII hepatic abscess. Empirical piperacillin-tazobactam and metronidazole were started, followed by ultrasound-guided drainage with catheter placement. Blood cultures grew Actinomyces odontolyticus, and the abscess aspirate grew Streptococcus intermedius, supporting a mixed actinomycotic-pyogenic abscess rather than incidental bacterem

BACKGROUND: In recent years, there has been a significant increase in pyogenic liver abscesses (PLAs) caused by multidrug-resistant (MDR) Gram-negative bacteria (GNB), predominantly Klebsiella pneumoniae and Escherichia coli.
AIM: To clarify the clinical characteristics and risk factors associated with MDR-GNB-related PLAs, develop a predictive nomogram for personalized risk assessment, and enhance the timeliness of empirical antibiotic selection.
METHODS: Based on the antibiotic susceptibility profiles, enrolled patients were divided into two groups: A MDR group comprising 105 individuals a

PURPOSE: The clinical manifestations of pyogenic liver abscess (PLA) vary between patients with and without diabetes mellitus (DM). However, the relationship between PLA and the gut microbiome remains unknown. This study analyzed the composition of gut microbiota in PLA patients with and without DM and healthy controls (HCs) with the goal of identifying potential reasons for the observed variations in clinical manifestations.
PATIENTS AND METHODS: Using 16S ribosomal RNA(16S rRNA) gene sequencing, we analyzed the compositions of gut microbiota in 32 PLA patients with DM, 32 PLA patients witho

RATIONALE: Spontaneous rupture of PLA (pyogenic liver abscess) is an extremely rare and life-threatening event. Ruptured PLA is very difficult to distinguish from malignant HCC (hepatocellular cancer) rupture or cholangiocarcinoma rupture on CT (computed tomography) scan.
PATIENT CONCERNS: We describe the case of a 71-year-old man with fever, right upper abdominal pain, nausea with intermittent vomiting, and general fatigue. He had no medical or surgical history.
DIAGNOSIS: CT scan showed a hypodense mass in right hepatic lobe and MRI (magnetic resonance imaging) revealed a heterogenous mass

OBJECTIVE: This study aimed to investigate the correlation between the transient segmental enhancement (TSE) of liver abscesses on contrast-enhanced ultrasound (CEUS) imaging and contrast-enhanced computed tomography (CT) scans.
METHODS: In total, 42 abscesses in 38 patients were evaluated with real-time CEUS and contrast-enhanced CT imaging. The CT imaging and CEUS examinations were performed within one to 2 days of each other in all cases. The initial reports of the observations of TSE on CEUS scans were correlated later with the findings of TSE on contrast-enhanced CT images. Contrast-enha

BACKGROUND: Intracranial extension of infection via the optic nerve is a rare but serious complication of bacterial endophthalmitis.
CASE: A 79-year-old women was hospitalized complaining of right eyelid swelling, severe hyperemia and purulent conjunctival discharge in the right eye. Although the fundus was invisible due to cataract, right endophthalmitis of unknown origin was suggested by pus in the anterior chamber and brain computed tomography (CT) findings showing nasal scleral rupture and orbital cellulitis. BACKGROUND & AIMS: The relationship between pyogenic liver abscess (PLA) and gastrointestinal (GI) cancer was first reported more than 20 years ago, yet little is known about this connection. We evaluated this association in a population-based, retrospective, cohort study.
METHODS: Using Taiwan National Health Insurance claims data, we collected data on a cohort of 14,690 patients with PLA diagnosed from 2000 to 2007. A reference cohort of 58,760 persons without PLA (controls) was selected from the same database, frequency matched by age, sex, and index year. Both cohorts were followed up unt

We describe an 83-year-old woman who developed a pyogenic liver abscess complicated with early bile duct carcinoma. After percutaneous abscess drainage, endoscopic retrograde cholangiography revealed a filling defect in the extrahepatic bile duct suggestive of a bile duct tumor. Resection of the extrahepatic bile duct with regional lymph node dissection was carried out. The resected specimen showed a polypoid tumor in the middle bile duct and histologic examination revealed well-differentiated tubular adenocarcinoma limited to the mucosal layer. These findings suggest that careful investigatio

This letter systematically evaluated a retrospective comparative study by Qiu et al investigating ultrasound-guided percutaneous catheter drainage (PCD) for pyogenic liver abscess management during the non-liquefaction phase. The results demonstrated that early ultrasound-guided PCD significantly shortened hospitalization duration, accelerated fever resolution, and promoted > 50% abscess volume reduction compared to delayed intervention after liquefaction, without increasing procedural risks. While challenging the conventional "wait-for-liquefaction" paradigm, the evidence quality limitations

Pyogenic liver abscess secondary to ingestion and migration of a fishbone is extremely rare and poses diagnostic and therapeutic challenges. Early recognition and definitive removal of the foreign body are crucial for successful treatment. An 80-year-old Thai man with diabetes mellitus, hypertension, and chronic kidney disease presented with fever, chills, and dyspnea. Imaging revealed a large multiloculated abscess in the left hepatic lobe containing a linear calcified foreign body consistent with a fishbone. Despite antibiotic therapy and percutaneous drainage, clinical improvement was incom

Pyogenic liver abscess is a potentially life-threatening condition primarily caused by bacterial spread from the gastrointestinal or biliary tract to the liver. As endoscopic retrograde cholangiopancreatography (ERCP) with biliary sphincterotomy becomes more common in the treatment of choledocholithiasis, the risk of developing pyogenic liver abscess increases, with Klebsiella pneumoniae continuing to emerge as a more commonly identified pathogen in North America. Our case describes this unique presentation of a 54-year-old female diagnosed with Klebsiella liver abscess (KLA) following multipl
