QT interval disturbances in elderly residents of long-term care facilities.

BACKGROUND: The QT interval reflects the total duration of ventricular myocardial repolarization. Disturbed QT--either prolonged or shortened--is associated with arrhythmia and is life-threatening. OBJECTIVES: To investigate an elderly population for disturbed QT interval. METHODS: We conducted a cross-sectional study on residents of long-term care wards in a geriatric hospital. Excluded were those with pacemaker, atrial fibrillation or bundle branch block. The standard 12 lead and lead 2 electrocardiograms in the patients' files were used for the evaluation of QT interval. RESULTS: We scre

Delivering care to long-term adult survivors of hematopoietic cell transplantation.

This review highlights long-term and late consequences of hematopoietic cell transplantation (HCT) as well as strategies to manage or prevent complications that are more prevalent after HCT than most other cancer treatments. Chronic graft-versus-host disease stands out as a unique late effect of allogeneic HCT that is not seen after other types of cancer treatment. However, many other complications seen after solid tumor treatments are also common after HCT, including infections, second cancers, bone loss, and cardiovascular, pulmonary, renal, and endocrine dysfunction. Symptoms and syndromes

Continuous assessment of low back loads in long-term care nurses.

Considerable effort has been spent evaluating aspects of low back injury risk in nursing yet comprehensive evaluation of all work tasks has been limited. The purpose of this study was to evaluate peak and cumulative lumbar spine loads experienced by personal support workers. A total of 20 female long-term care workers were observed and had trunk posture monitored via an inclinometer throughout their shift. When adjusted for an 8-h workday, workers experienced cumulative loads of 21.3 +/- 4.6 MNs, 1.8 +/- 0.6 MNs and 2.9 +/- 1.4 MNs for compression, lateral and anterior shear, respectively. Pat

Legionnaires' disease in long-term care facilities: overview and proposed solutions.

Pneumonia is a leading cause of morbidity and mortality in nursing home patients. In acute care hospitals, there is considerable evidence to indicate that Legionnaires' disease is a significant cause of nosocomial pneumonia, the source of which is the potable water system. A relatively limited amount of data exists as to the role of Legionnaires' disease as a cause of pneumonia acquired in long-term care residents. Several lines of evidence suggest that Legionnaires' disease may be an important but underrecognized cause of pneumonia in long-term care residents. These include reports of outbrea

Formal long-term care networks. Forming a partnership with gerontological nurses.

Faced with overwhelming demands on their time, physical reserves, and emotional energy, contemporary families are depending more and more on formal networks to help them with their elder care responsibilities. In the past, these interactions worked in a somewhat complimentary fashion with each caregiver having identifiable roles and responsibilities. Now, reforms in health and human services are pushing and pulling families into complex caregiving partnerships. These situations have the very real potential for creating stress, strain, tension and conflict among caregivers, as well as opportuni

A nursing challenge: methicillin-resistant Staphylococcus aureus in long-term care.

1. Methicillin-resistant Staphylococcus aureus (MRSA) are gram-positive cocci, differing from methicillin-resistant species of S epidermidis because they produce coagulase. MRSA is distinguished from methicillin-sensitive S aureus by its resistance to methicillin and other antibiotics. MRSA has not been found to be an organism of greater virulence than methicillin-sensitive strains of S aureus, but infections caused by MRSA are sometimes of greater concern because of the limited antibiotic treatment options. 2. In the past 25 years there has been an increase in the number of reported infection

[Propofol versus midazolam. Long-term sedation in the intensive care unit].

Sedative-analgesic treatment of patients on long-term artificial ventilation aims at protection from stress related to their disease or therapy. By stabilising both the patient's vital functions and psychological state this treatment may contribute to therapeutic success. The choice of drugs depends primarily on the nature and course of the underlying disease. Midazolam and propofol are available as hypnotics for short-term sedation during the post-operative period. The purpose of this study was to evaluate the effects of both agents on cardiovascular function, cortisol production, lipometabol

Long-Term Intensive Care Unit (ICU) Stays Can Lead to Long-Term Cognitive Impairment (LTCI): Neurosurgery Nursing Strategies to Minimize Risk.

Long-term cognitive impairment (LTCI) is a phenomenon predominantly seen in patients within intensive care units (ICU) that causes chronic dysfunction, defined as new or worsening deficits in memory, attention, mental processing speed, executive function, intellectual function, and visual-spatial abilities for over 12 months, inhibiting the necessary return to baseline function without appropriate intervention. Our objective is to provide a guideline of nursing strategies to reduce LTCI through different studies that evaluate pharmacological and non-pharmacological methods. Current literature
