Senescent Kidney Cells in Hypertensive Patients Release Urinary Extracellular Vesicles.

Background Hypertension may be associated with renal cellular injury. Cells in distress release extracellular vesicles (EVs), and their numbers in urine may reflect renal injury. Cellular senescence, an irreversible growth arrest in response to a noxious milieu, is characterized by release of proinflammatory cytokines. We hypothesized that EVs released by senescent nephron cells can be identified in urine of patients with hypertension. Methods and Results We recruited patients with essential hypertension (EH) or renovascular hypertension and healthy volunteers (n=14 each). Renal oxygenation wa

Heterogeneity of osteopontin expression among nephrons in mouse kidneys and enhanced expression in sclerotic glomeruli.

BACKGROUND: Osteopontin (OPN) is a secreted Ca(2+)-binding phosphoprotein able to mediate cell attachment to bone via an RGD sequence and the alpha v beta 3 integrin. OPN mRNA is found at high levels in the kidney, and the protein is found in the urine. Because published reports of where the protein is produced conflict, we undertook a comprehensive study to localize OPN expression. EXPERIMENTAL DESIGN: In situ hybridization with a mouse cDNA probe and immunohistochemical staining with three different antisera to mouse OPN were used to identify those cells that contained significant levels of

Discovery of Small Molecule Renal Outer Medullary Potassium (ROMK) Channel Inhibitors: A Brief History of Medicinal Chemistry Approaches To Develop Novel Diuretic Therapeutics.

The renal outer medullary potassium (ROMK) channel is a member of the inwardly rectifying family of potassium (Kir, Kir1.1) channels. It is primarily expressed in two regions of the kidney, the cortical collecting duct (CCD) and the thick ascending loop of Henle (TALH). At the CCD it tightly regulates potassium secretion while controlling potassium recycling in TALH. As loss-of-function mutations lead to salt wasting and low blood pressure, it has been surmised that inhibitors of ROMK would represent a target for new and improved diuretics for the treatment of hypertension and heart failure. Does magnesium play a role in the hypokalemia of Bartter's syndrome?

A patient with Bartter's syndrome manifested hypomagnesemia in addition to hypokalemia. Under conditions of maximal free water production, he had a fractional distal solute reabsorption of 0.65, a value consistent with a renal defect in sodium chloride reabsorption in the thick ascending limb of the loop of Henle. This is also the site of 65% to 70% of urinary magnesium reabsorption. With magnesium supplementation and amiloride, the urinary potassium decreased and the serum potassium increased. Atrial natriuretic peptide concentrations in the plasma were low. Evaluation of family members showe

Enhanced fractional sodium reabsorption in the ischaemic kidney revisited with lithium as a probe.

Extraction of lithium and 51Cr-EDTA across each of the two kidneys was determined during renal vein catheterization in 14 hypertensive patients with unilateral or bilateral renovascular disease before and after i.v. injection of furosemide. Before the administration of furosemide an increased fractional lithium reabsorption was demonstrated across the affected, or more affected kidney. This difference was abolished by furosemide. Using lithium as a probe for sodium, our data suggest an increased fractional tubular sodium reabsorption in the ischaemic human kidney probably located to the proxim

A novel mutation of the claudin 16 gene in familial hypomagnesemia with hypercalciuria and nephrocalcinosis mimicking rickets.

Familial hypomagnesemia with hypercalciuria and nephrocalcinosis (FHHNC) is caused by a mutation in the gene CLDN16, which encodes paracellin 1 (claudin-16), atight junction protein mediating paracellular transport which is expressed in the thick ascending loop of Henle and in the distal convoluted tubule, where reabsorption of magnesium occurs. We present a 4 years old Turkish female child with a chief complaint of hypocalcemic tetany. A diagnosis of FHHNC was confirmed by genetic testing for a mutation in claudin 16 gene. Claudin 16 gene revealed homozygosity for the p.K183E(AAA>GAA) C. 547A

[Michaelis-Manz syndrome. A case report].

Michaelis-Manz syndrome is an autosomal recessive hereditary tubulopathy associated with mutations in the tight-junction proteins claudin-16 and claudin-19, which are present in the distal convoluted tubule and the loop of Henle in the kidney. Claudin-19 is also expressed in the retinal pigmentary epithelium. The clinical picture causes hypomagnesemia, hyper-calciuria and nephrocalcinosis that can lead to renal failure, which is the condition that marks the prognosis of the disease. Ophthalmologically patients can present macular coloboma, myopic staphyloma and nystagmus. We present the case r

[Pharmacologic bases of cardiovascular therapy with diuretics].

Diuretics may have high, medium or low diuretic potency and their main site of diuretic action at the ascending limb of Henle's loop or at different biochemical sites of the distal tubulus. Loop diuretics have high diuretic potency, act abruptly, are not antihypertensives when prescribed as a monotherapy and are potentially ototoxic. The diuretics acting at the distal tubulus have a gentle time-course of diuretic activity and variable potencies and other properties. The mose frequent side-effect of diuretics is hypokalemia, whose development may be partially prevented by the prescription of a

Diuretic effect of sodium artesunate in patients with malaria.

Previously, we described a direct inhibitory effect of sodium artesunate on sodium chloride transport in the thick ascending limb of Henle's loop, indicating that artesunate acts as a diuretic agent. Here we present 2 cases of falciparum malaria treated with 4 intravenous 60-mg doses of sodium artesunate. Neither diuretics nor vasoactive drugs were administered. A rise in diuresis (6 L/24 hours) was accompanied by an increase in natriuresis, and both declined at the end of the treatment. This diuretic effect has not been reported previously in patients and may modify the course of renal failur

Nephrogenic diabetes insipidus in a dog with renal medullary lesions.

Antidiuretic hormone (ADH) administration to a polyuric Miniature Poodle did not alter diuresis. Plasma ADH concentrations were high, and urine osmolality remained low during water deprivation. From these findings, it was concluded that the polyuria was of renal origin. In addition, the glomerular filtration rate was found to be high. Electron microscopic examination of the renal medulla revealed vacuoles containing myelinic figures and fingerprint structures in the cells of the Henle loops, blood vessels, and interstitium, similar to those in lysosomal lipid storage disease. Their absence in

Furosemide stimulates macula densa cyclooxygenase-2 expression in rats.

BACKGROUND: During a low salt intake, maintenance of renal blood flow and renin secretion depends on intact formation of prostaglandins. In the juxtaglomerular apparatus, the inducible isoform of cyclooxygenase, cyclooxygenase-2 (COX-2), is restricted to the macula densa and the cortical thick ascending limb of Henle (cTALH) cells, and is inversely regulated by dietary salt intake. This study aimed to elucidate whether the effect of NaCl on macula densa COX-2 expression is mediated by transepithelial transport of NaCl. METHODS: To this end, male Sprague-Dawley rats received subcutaneous infus

Genome-wide Association Study for AKI.

KEY POINTS: Two genetic variants in the DISP1-TLR5 gene locus were associated with risk of AKI. DISP1 and TLR5 were differentially regulated in kidney biopsy tissue from patients with AKI compared with no AKI. BACKGROUND: Although common genetic risks for CKD are well established, genetic factors influencing risk for AKI in hospitalized patients are poorly understood. METHODS: We conducted a genome-wide association study in 1369 participants in the Assessment, Serial Evaluation, and Subsequent Sequelae of AKI Study; a multiethnic population of hospitalized participants with and without AKI m

Interaction of thiazide and loop diuretics with the basolateral para-aminohippurate transport system in isolated S2 segments of rabbit kidney proximal tubules.

The effect of thiazide and loop diuretics on the tubular uptake of [3H]para-aminohippurate ([3H]PAH), a marker of the organic anion transport system, was tested in vitro on single S2 segments of the proximal tubule of rabbit kidneys. Because the tubules were not perfused, and hence were collapsed, the tubular [3H] PAH uptake reflects [3H]PAH transport across the contraluminal membrane. The diuretics were added to the bath solution in at least six different concentrations. The concentration of [3H]PAH in the bath solution amounted to 1.3 mumol/l, and was 12 times lower than the PAH concentratio

Inherited disorders of renal hypomagnesaemia.

The kidney plays a key role in the maintenance of normal magnesium balance. The distal tubule of the kidney, namely the thick ascending limb of the loop of Henle and the distal convoluted tubule, is crucial for the regulation of serum magnesium levels and body magnesium content. The identification of molecular defects related to rare inherited magnesium losing disorders has contributed greatly to a better understanding of the process of renal magnesium handling. Since the number of genetic defects related to magnesium metabolism is still increasing, it might be expected that our knowledge on m

Neo-Mull-Soy metabolic alkalosis: a model of Bartter's syndrome?

A case of Neo-Mull-Soy-induced metabolic alkalosis occurred in an 8-month-old child. This child had hypochloremic hypokalemic alkalosis as well as hyperreninemia. Initially, a diagnosis of Bartter's syndrome was made and treatment consisted of KCl replacement, indomethacin, and aspirin. In retrospect, the diagnosis of Neo-Mull-Soy induced metabolic alkalosis could have been suspected on the basis of the low chloride concentration in his urine. Proposed mechanisms for the etiology of Bartter's syndrome are reviewed. Neo-Mull-Soy induced metabolic alkalosis simulates Bartter's syndrome and suppo

KLOSSIELLA DULCIS N. SP. (APICOMPLEXA: KLOSSIELLIDAE) IN THE KIDNEYS OF PETAURUS BREVICEPS (MARSUPIALIA: PETAURIDAE).

Two cases of renal klossiellosis were diagnosed by histopathology in pet sugar gliders (Petaurus breviceps). In both cases, parasites were associated with tubular dilation and mild interstitial nephritis. Rare schizonts were seen in the proximal convoluted renal tubular epithelium, whereas all other life cycle stages were found within distal convoluted tubule cells or the urinary space of the structures distal to the loop of Henle. Conventional optical and transmission electron microscopies were used to assess the life stages of the parasite. The morphologic characteristics and measurements ob

Vascular responsiveness to angiotensin II and phenylephrine, the tubular function and the prostaglandin, renin-angiotensin system in a patient with Bartter's syndrome.

In a 39-yr-old female patient with Bartter's syndrome, vascular responsiveness to angiotensin II and phenylephrine was studied. Pressor responses to angiotensin II and to phenylephrine were markedly decreased prior to treatment and were improved by administration of indomethacin, dextran, KCl, captopril, propranolol or pindolol. Moreover, the responses of total peripheral vascular resistance to angiotensin II and phenylephrine were markedly decreased before treatment and were improved by the treatment. A relatively low fractional distal chloride reabsorption was found, and that was not changed

Acute renal failure due to urinary tract obstruction.

Urinary tract obstruction is a frequent cause of acute renal failure that is potentially life threatening but reversible, if it is promptly recognized and corrected. The level of urinary tract obstruction is variable, dependent on the underlying disease, and may range from the loop of Henle to the urethral meatus. Clinical manifestations are most commonly due to renal failure, but the history and physical examination can aid in determining the localization and cause of the obstruction. Laboratory findings may suggest urinary tract obstruction as the etiology for acute renal failure. Radiologic

Peritoneal dialysis-associated peritonitis in two patients with Bartter syndrome and end-stage kidney disease: a case report.

Bartter syndrome is a rare inherited salt-wasting disorder affecting the thick ascending limb of the loop of Henle. It is characterized by hypokalemic metabolic alkalosis, polyuria, and secondary normotensive hyperaldosteronism. A minority of patients may progress to end-stage kidney disease while maintaining polyuria and normokalemia. Peritoneal dialysis may represent a suitable option for these patients when kidney transplantation is not available. However, due to its rarity, there are no data on PD-related outcomes. Here we report two cases of PD treatment with BS from two Italian referral

Characterization of a Na : K : 2C1 cotransport system in the apical membrane of a renal epithelial cell line (LLC-PK1).

86Rb uptake into LLC-PK1 cells (an established renal epithelial cell line) was found to be comprised of an active ouabain-sensitive component, a loop diuretic-sensitive component which was passive and strictly dependent upon the presence of extracellular Na+ and Cl- for activity, and a "leak" component. The diuretic-sensitive component of influx was investigated further in apical membrane vesicles derived from these cells. A large fraction of 86Rb, 22Na and 36Cl flux into these vesicles was sensitive to inhibition by furosemide and dependent upon the presence of the other two co-ions, in keepi
