The Contributions of Pitch, Loudness, and Rate Control to Speech Naturalness in Cerebellar Ataxia.

PURPOSE: The goal of this study was to determine the relationship between the perceptual measure of speech naturalness and objective measures of pitch, loudness, and rate control as a potential tool for assessment of ataxic dysarthria. METHOD: Twenty-seven participants with ataxia and 29 age- and sex-matched control participants completed the pitch glide and loudness step tasks drawn from the Frenchay Dysarthria Assessment-Second Edition (FDA-2) in addition to speech diadochokinetic (DDK) tasks. First, group differences were compared for pitch variability in the pitch glide task, loudness var

Perceptual consequences of disrupted auditory nerve activity.

Perceptual consequences of disrupted auditory nerve activity were systematically studied in 21 subjects who had been clinically diagnosed with auditory neuropathy (AN), a recently defined disorder characterized by normal outer hair cell function but disrupted auditory nerve function. Neurological and electrophysical evidence suggests that disrupted auditory nerve activity is due to desynchronized or reduced neural activity or both. Psychophysical measures showed that the disrupted neural activity has minimal effects on intensity-related perception, such as loudness discrimination, pitch discri

Loudness and Sound Category: Their Distinct Roles in Shaping Perceptual and Physiological Responses to Soundscapes.

When compared with nature sounds, exposure to mechanical sounds evokes higher levels of perceptual and physiological arousal, prompting the recruitment of attentional and physiological resources to elicit adaptive responses. However, it is unclear whether these attributes are solely related to the sound intensity of mechanical sounds, since in most real-world scenarios, mechanical sounds are present at high intensities or if other acoustic or semantic factors are also at play. We measured the skin conductance response (SCR), reflecting sympathetic nervous system activity as well as the pleasan

Quantifying dysphonia severity using a spectral/cepstral-based acoustic index: Comparisons with auditory-perceptual judgements from the CAPE-V.

This study investigated the relationship between acoustic spectral/cepstral measures and listener severity ratings in normal and disordered voice samples. CAPE-V sentence samples and the vowel /a/were elicited from eight normal speakers and 24 patients with varying degrees of dysphonia severity. Samples were analysed for measures of the cepstral peak prominence (CPP), the ratio of low-to-high spectral energy, and their respective standard deviations. Perceptual ratings of overall severity were also obtained for all samples. Results showed that all acoustic variables combined in a four-factor m

A portable visual-feedback device for reducing excessive vocal loudness in persons with mental retardation.

A simple portable device was employed to reduce excessive vocal loudness in two adults who functioned within the moderate mental retardation range (one in the lower and one in the upper half). The device provided these adults visual feedback when the voice exceeded a preset level of loudness. Data showed that the device was useful in helping both adults reduce excessive vocal loudness across different daily situations. Characteristics and applicability of the device are discussed.

Temporal analysis in normal and impaired hearing.

The ear contains an array of filters that separate the components of a complex signal into "channels" tuned to different center frequencies. Temporal analysis can be considered as two processes: analysis of the time pattern occurring within each channel, and comparison of the time patterns across channels. Within-channel acuity can be characterized by tasks such as gap detection, or by the ability to detect amplitude modulation as a function of modulation rate. The smallest detectable gap duration for a white noise stimulus is 2-3 ms. The results can be modeled by an array of filters, with eac

From snoring to sleep apnea in a Singapore population.

We have no information on snoring and obstructive sleep apnea (OSA) in our population, which is predominantly Chinese. Our perception is that sleep apnea syndrome is more common than the 2-4% prevalence (Young et al., 1993) often quoted, judging from the experience in our sleep disorder unit. We studied the snorers in an adult population in Singapore and then went on to see how many snorers suffer pathological apnea and sleep apnea syndrome. Room partners, 220 of them aged 30-60 years, were interviewed for their observation of snoring among each other. 106 consecutive habitual loud snorers of

Amplitude-mapping effects on speech intelligibility with unilateral and bilateral cochlear implants.

OBJECTIVE: A study was conducted examining the effects of amplitude-mapping adjustments on speech intelligibility with bilateral cochlear implant users, for both unilateral and bilateral device use. The main motivation for this study was the consideration that bilateral loudness summation may result in sounds becoming too loud if no adjustments are made to standard monaural amplitude-mapping functions (maps) when used bilaterally. In that case, reductions of the stimulation levels may be needed. The objective was to assess the impact of such adjustments on speech intelligibility in noise and i

Bone-anchored hearing aids in unilateral inner ear deafness.

In nine patients with unilateral deafness and normal hearing in the contralateral ear, measurements of sound localization and speech perception were obtained before intervention, with a conventional contralateral routing of sound (CROS) hearing aid and later with a bone-anchored hearing aid (BAHA) implanted in the deaf ear. Sound localization did not show any differences between the three conditions. Speech perception using short, everyday sentences showed a reduction in the head-shadow effect of 2 dB for both the conventional CROS hearing aid and the BAHA in comparison to the unaided conditio

Long latency event-related potentials in mice: effects of stimulus characteristics and strain.

The P3, or P300 component of the event-related potentials (ERPs) is a positive going waveform that can be averaged from the EEG approximately 250-500 ms following the presentation of task or context 'relevant' stimuli. This potential has been demonstrated to be a sensitive measure of both normal and abnormal cognitive functioning. P300 models have been developed in monkeys, cats and rats. The aim of the present study was to develop an auditory ERP paradigm suitable for use in mice that resembled those used in humans and other animal models. The results of the studies showed that late positive

Effect of exceeding compliance voltage on speech perception in cochlear implants.

OBJECTIVES: In cochlear implants, the maximum current I (Amperes) that can be delivered on a cochlear implant electrode is determined by V = I * R, where V = compliance voltage (Volts) and R = electrode impedance (Ohms). Generally, electrode impedances are measured during each clinical visit and are used to set electrical stimulation parameters in cochlear implants. However, electrode impedances can rise during the course of cochlear implant use and lead to electrical stimulation voltage requirements exceeding the maximum compliance voltage of the medical device. Electric stimulation requireme

Selective Impairment in Frequency Discrimination in a Mouse Model of Tinnitus.

Tinnitus is an auditory disorder, which affects millions of Americans, including active duty service members and veterans. It is manifested by a phantom sound that is commonly restricted to a specific frequency range. Because tinnitus is associated with hearing deficits, understanding how tinnitus affects hearing perception is important for guiding therapies to improve the quality of life in this vast group of patients. In a rodent model of tinnitus, prolonged exposure to a tone leads to a selective decrease in gap detection in specific frequency bands. However, whether and how hearing acuity

The effect of impulse intensity and the number of impulses on hearing and cochlear pathology in the chinchilla.

Forty-one chinchillas, divided into seven groups, were exposed to 1, 10, or 100 noise impulses (one every 3s) having peak intensities of 131, 135, 139, or 147 dB. Hearing thresholds were measured in each animal before and after exposure using an avoidance conditioning procedure; a surface preparation of the cochlear sensory epithelia was performed approximately 90 days after exposure. There was generally an orderly relation between the amount of permanent threshold shift and the severity of exposure, and a general agreement between averaged histological data and the audiometric data. For the i

Intensity coding in the auditory periphery of the cat: responses of cochlear nerve and cochlear nucleus neurons to signals in the presence of bandstop masking noise.

The dynamic range over which fine intensity discrimination is possible has been reported to be largely unaffected by limitation of the spread of neuronal activity to neighbouring frequency regions by bandstop noise masking. We have therefore examined the responses of cochlear nerve and nucleus neurons to tone and noise signals in the presence of a bandstop masking noise designed to be comparable to that employed in the psychophysical experiments. Under these conditions, the vast majority of cochlear nerve fibres were saturated by sound levels at which some 50% of our sample of cochlear nucleus

Binaural benefits for adults who use hearing aids and cochlear implants in opposite ears.

OBJECTIVE: This study aimed to investigate 1) how a hearing aid needs to be adjusted for an adult who uses a cochlear implant in the contralateral ear; 2) whether the use of a hearing aid with a cochlear implant leads to interference; and 3) whether adults derive binaural benefits from using a hearing aid with a cochlear implant for speech perception, localization, and functional performance in everyday life. DESIGN: Twenty-one adults (11 female and 10 male) who used either a Nucleus CI-22 (N = 3) or a Nucleus CI-24 (N = 18) cochlear implant system in one ear participated in this study. Twelv

Transcranial direct current stimulation (tDCS), a treatment option for tinnitus in profound hearing loss.

A 45-year-old man presented with a history of sudden sensory neural hearing loss and severe tinnitus in his left ear. Audiological investigations revealed a profound hearing loss on his left ear and mild conductive hearing loss on his right. Tinnitus pitch and loudness were matched to a 4 kHz narrow-band noise at 50dBHL and subjective tinnitus questionnaires revealed that he had a catastrophic handicap (grade IV). Traditional audiological treatment approaches (tinnitus maskers, hearing aid and sound therapy) that stimulate the cochlea to induce cortical reorganisation were futile. Hence, a top

Progressive Loss of Sensitivity to Electrical Stimulation After Cochlear Implantation in X-Linked Incomplete Partition Type III Deafness.

OBJECTIVES: Patients with X-linked incomplete partition type III (IP3) deafness treated with cochlear implants exhibit higher "Most Comfortable Loudness" (MCL) levels of stimulation and more electrode deactivation than patients with normal morphology. We endeavored to analyze the progression of the MCL levels and electrode deactivation over time and assess those factors that could have led to deactivation. Furthermore, we aimed to assess whether speech perception was affected by a progressive loss of neural contact. DESIGN: All 13 patients with the IP3 malformation in our clinical database we

Amplification of the poorer ear by StereoBiCROS in case of asymmetric sensorineural hearing loss: effect on tinnitus.

Tinnitus is prevalent among patients suffering from Single-Sided Deafness (SSD) and Asymmetrical Hearing Loss (AHL). In addition to bothersome tinnitus in the poorer ear, these patients also report issues with understanding speech in noise and sound localization. The conventional treatment options offered to these patients to improve auditory abilities are cochlear implantation, bone conduction devices or Contralateral Routing Of Signal (CROS) hearing aids. It was recently found that the benefit of cochlear implantation for tinnitus associated with AHL/SSD was greater than the other two approa
