Nutritional supplementation is associated with improved wound reduction during negative pressure wound therapy in complex wounds: a retrospective cohort study.

BACKGROUND: Negative pressure wound therapy (NPWT) is central to the management of complex wounds, but wound contraction and granulation require adequate metabolic substrate. We evaluated the association between nutritional supplementation and wound healing outcomes in patients treated with NPWT. METHODS: This retrospective cohort study included 48 adults who underwent negative pressure wound therapy for complex wounds. Patients were grouped according to receipt of nutritional supplementation. The primary outcome was absence of wound reduction during follow-up. RESULTS: Nutritional supplemen

Mechanical countermeasures for spaceflight-associated neuro-ocular syndrome during 30-days of head down tilt bed rest: design, implementation, and tolerability.

After longer duration space missions, some astronauts experience structural and functional changes in the eye and structural changes in the brain, termed Spaceflight-Associated Neuro-Ocular Syndrome (SANS). Countermeasures against SANS are required to minimize potential operation impacts and negative long-term health consequences. Headward fluid shifts, which appear to promote SANS, provide a target for countermeasures. The SANS countermeasures study, a 30 days strict head down tilt bed rest (HDTBR) study, tested two mechanical countermeasures aimed at reversing cephalad fluid overload. This w

Inhibition of CYP450 pathways reduces functional sympatholysis in healthy young adults.

Functional sympatholysis, the blunting of sympathetic vasoconstriction during exercise, is critical for regulating exercise hyperemia. The role of cytochrome P450-2C9 (CYP450) pathways in functional sympatholysis remains unclear. A total of 21 participants (11 females) completed three study visits (2 experimental). Participants ingested a placebo (PLA) or CYP450 inhibitor fluconazole (FLZ) 120 min before testing in a double-blind, randomized, crossover design. Forearm blood flow (FBF) and mean arterial pressure (MAP) were continuously measured to calculate forearm vascular conductance (FVC) du

Anesthetic Management of Thoracoscopic Pulmonary Cystectomy in a Patient With Fontan Circulation and Disturbed Lung Inflation During Leakage Testing: A Case Report.

Congenital heart disease may require multiple cardiac surgeries during childhood. Subsequent non-cardiac surgeries increase the perioperative bleeding and hypoxia because of changes in circulation. An 18-year-old male patient with a history of multiple cardiac interventions, including Fontan surgery, underwent a thoracoscopic right lung suture and coverage for recurrent right spontaneous pneumothorax under general anesthesia with one lung ventilation (OLV). The superior dorsal and inferior lobes, which were inflatable before surgery, failed to expand during leakage testing. The trachea's condi

Cerebral blood flow autoregulation assessment by correlation analysis between mean arterial blood pressure and transcranial doppler sonography or near infrared spectroscopy is different: A pilot study.

PURPOSE: Recently, cerebral autoregulation indices based on moving correlation indices between mean arterial pressure (MAP) and cerebral oximetry (NIRS, ORx) or transcranial Doppler (TCD)-derived middle cerebral artery flow velocity (Mx) have been introduced to clinical practice. In a pilot study, we aimed to evaluate the validity of these indices using incremental lower body negative pressure (LBNP) until presyncope representing beginning cerebral hypoperfusion as well as lower body positive pressure (LBPP) with added mild hypoxia to induce cerebral hyperperfusion in healthy subjects. Insulin resistance is related to albuminuria in patients with type II (non-insulin-dependent) diabetes mellitus.

To determine whether albuminuria is associated with insulin resistance in patients with type II (non-insulin-dependent) diabetes mellitus, we performed hyperinsulinemic (40 mU/m2/min) euglycemic clamp studies in patients with a urinary albumin excretion (UAE) rate greater than 30 mg/24 h and in patients with a UAE less than 30 mg/24 h. The UAE-positive group (n = 22) did not differ significantly from the UAE-negative group (n = 18) with respect to age, sex, treatment of diabetes, body mass index, fasting or postload blood glucose or plasma insulin levels, blood pressure, or known duration of d

Ventilation-perfusion effects of negative-pressure ventilation: insights from an experimental rat model.

BACKGROUND: Mechanical ventilation typically utilizes positive-pressure ventilation (PPV), which fundamentally differs from physiological pressure conditions. In contrast, negative-pressure ventilation (NPV) more closely mimics physiological pressure conditions; however, its impact on ventilation-perfusion matching remains unclear. Therefore, we compared PPV and NPV in terms of their effects on ventilation-perfusion matching and determined the consequences of increasing end-expiratory pressure (EEP). METHODS: Anesthetized rats (n = 9) were ventilated using PPV at a positive EEP of 0, 3, 6, an

Space motion sickness: phenomenology, countermeasures, and mechanisms.

A summary of the incidence of Space Motion Sickness (SMS) in 27 Soviet cosmonauts who flew on missions varying from 2-185 d in the Salyut-6/Soyuz vehicle complex is presented. A questionnaire indicated that 88% (24) of the cosmonauts developed some type of "illusionary sensations" while 44% (12) presented some degree of SMS. The SMS countermeasures used in flight included an antihistaminic drug, pneumatic cuffs applied to the thigh region, application of lower body negative pressure, a head cap that restricted head movement while simultaneously providing force stimulus to the cervical antigrav

Aortic root dilatation at sinuses of valsalva and aortic regurgitation in hypertensive and normotensive subjects: The Hypertension Genetic Epidemiology Network Study .

The association of sinuses of Valsalva dilatation and aortic regurgitation with hypertension is disputed, and few data are available in population-based samples. We explored the relations of sinuses of Valsalva dilatation and aortic regurgitation to hypertension and additional clinical and echocardiographic data in 2096 hypertensive and 361 normotensive participants in the Hypertension Genetic Epidemiology Network study. Age and body surface area were used to predict aortic root diameter using published equations developed from a separated reference population. Aortic dilatation was defined as

Severely damaged lower limb salvage in a critically ill burned patient. Lessons learned.

Lower limb salvage in severely injured burned patients with bone or tendon exposure may be a reconstructive challenge. In this cases, local or regional flaps and other more conservative therapies such as dermal substitutes and negative-pressure wound therapy are usually not available or are not good enough to solve the problem. In such situations, microsurgical reconstruction with distant flaps seems to be the best option, even though the particularities of the severe burn patient may decrease free flaps' success rate. We report the case of a patient with severe electrical injuries affecting 7

Facilitating residual wound closure after partial graft loss with vacuum assisted closure therapy.

Third degree burns require skin grafting. In most instances, if the graft becomes infected, it requires debridement of the site and re-grafting. The purpose of this report is to illustrate the successful healing of a skin graft using negative pressure wound therapy with silver impregnated foam and soft silicone wound contact layer in a 4% total body surface area burn of a lower extremity skin graft infected with Pseudomonas aerugenosa without regrafting. A 27-year-old Hispanic male sustained a gasoline flame burn and presented 72 hours postincident with right lower extremity cellulitis. After

Acute intermittent hypercapnic hypoxia and sympathetic neurovascular transduction in men.

KEY POINTS: Intermittent hypoxia leads to long-lasting increases in muscle sympathetic nerve activity and blood pressure, contributing to increased risk for hypertension in obstructive sleep apnoea patients. We determined whether augmented vascular responses to increasing sympathetic vasomotor outflow, termed sympathetic neurovascular transduction (sNVT), accompanied changes in blood pressure following acute intermittent hypercapnic hypoxia in men. Lower body negative pressure was utilized to induce a range of sympathetic vasoconstrictor firing while measuring beat-by-beat blood pressure and f

Impact of transient hypotension on regional cerebral blood flow in humans.

We examined the impact of progressive hypotension with and without hypocapnia on regional extracranial cerebral blood flow (CBF) and intracranial velocities. Participants underwent progressive lower-body negative pressure (LBNP) until pre-syncope to inflict hypotension. End-tidal carbon dioxide was clamped at baseline levels (isocapnic trial) or uncontrolled (poikilocapnic trial). Middle cerebral artery (MCA) and posterior cerebral artery (PCA) blood velocities (transcranial Doppler; TCD), heart rate, blood pressure and end-tidal carbon dioxide were obtained continuously. Preconditioning with sodium deficits to improve orthostatic tolerance in rats.

We hypothesized that prior deficits in total body sodium would improve the subsequent ability of non-hypovolemic rats to maintain arterial pressure when subjected to an orthostatic challenge. This hypothesis was based on similarities in the response of neurohumoral cardiovascular control systems to lower-body negative pressure and negative sodium balance. Sodium deficits were induced in male Sprague-Dawley rats for 7-8 d by feeding sodium restricted diets, or by administering furosemide daily. After this, rats were allowed to regain a positive sodium balance for 1 d by increasing dietary intak
