The spectrum of sebaceous differentiation in tumors of thymic origin.

Sebaceous differentiation is an unexpected and unusual phenomenon in the normal thymic gland. Among thymic tumors, sebaceous glands are a relatively common component of mediastinal teratomas but are exceedingly rare in other neoplasms of thymic origin. This review explores the spectrum of non-teratomatous thymic tumors in which sebaceous differentiation has been identified, discusses the potential histogenesis and evaluates the effects of this occurrence on diagnosis, treatment and prognosis.

Surgical management of mediastinal paraganglioma: All hands on deck!

Mediastinal paragangliomas are very uncommon neuroendocrine neoplasms. Due to their tissue of origin (sympathetic ganglia of the great vessels), they tend to arise deep within pericardial space and, more importantly, intimately attached to great vessels, which makes surgical resection, even with cardiopulmonary bypass, very challenging. This commentary accompanies the case report describing complex surgical management of a paraganglioma located in the anterior mediastinum that was initially thought to be a thymoma.

Multimodal Imaging of Mediastinal Epithelioid Hemangioendothelioma: Two Case Reports.

INTRODUCTION: Epithelioid Hemangioendothelioma (EHE) is a rare vascular neoplasm that typically occurs in the bone, soft tissue, liver, and lung but rarely in the mediastinum. Multimodal imaging of EHE is poorly understood, often leading to misdiagnosis as other mediastinal tumors. CASE PRESENTATION: Two female cases with incidental mediastinal masses were retrospectively analysed, focusing on multimodal presentations. For both cases, CT studies showed well-defined, low-density oval masses in the right anterior superior mediastinum with the Superior Vena Cava (SVC) invasion. Intralesional pun

Myxofibrosarcoma with metastasis to the lungs, pleura, and mediastinum: a case report and review of literature.

Myxofibrosarcoma is a common connective tissue neoplasm of malignant fibrocytes in a myxoid matrix favoring the extremities. Like many other tumors of connective tissue, soft tissue sarcoma exhibits high recurrence rates but is rarely known to metastasize. We present a patient with myxofibrosarcoma of the hand with metastases to the lungs, pleura, and mediastinum. The mediastinal metastasis presented clinically with gastrointestinal symptoms due to compression of the gastroesophageal junction. To our knowledge, this is the first report of metastatic myxofibrosarcoma of the hand and also the fi

Ectopic pancreas in the anterior mediastinum: A report of two cases and review of the literature.

Ectopia of the pancreatic tissue is a developmental anomaly found in ~2% of all autopsies, and 70~90% of these anomalies are located in the gastrointestinal tract. Mediastinal localization of an ectopic pancreas is extremely rare. Herein, we report two cases with mediastinal ectopic pancreas clarified by pathology and shown by thoracic contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI). In addition, a brief review of the relevant literatures is presented. Although CT and MRI manifestations of this lesion are nonspecific, certain notable findings need to be focused

[Diagnostic difficulties in diagnosis of Hodgkin's disease in a young man--a case report].

UNLABELLED: Lymphomas are neoplasms of lymphocytes and their precursor cells. This disease develops from lymph nodes or extranodal lymphoid tissue. A common site for such a tumour is the chest. The authors describe the case of a young man, who was admitted do the thoracic surgery department with a mediastinal tumour and an induration of the base of the neck. Initial diagnostic tests gave the wrong diagnosis of actinomycosis. A lack of progress in the therapy and the occurrence of a life-threatening condition led to the implementation of more aggressive diagnostic methods. In biopsies taken dur

Hodgkin Lymphoma As the Underlying Cause of a Chronic Cough.

Hodgkin lymphoma (HL) is a monoclonal lymphoid neoplasm derived from B cells and is one of the most common lymphomas among young adults in developed countries. It typically presents insidiously, often as a painless cervical lymphadenopathy or an asymptomatic mediastinal mass. B symptoms (fever, night sweats, and weight loss), fatigue, pruritus, or alcohol-induced pain may be present along with respiratory symptoms in cases of mediastinal involvement. A definitive diagnosis requires lymph node biopsy for histological and immunophenotypic evaluation. We report the case of a 24-year-old female sm

The role of fine needle aspiration cytology in the diagnosis of mediastinal lesions: A 9-year experience from coastal Karnataka.

Fine needle aspiration cytology has emerged as a minimally invasive, cost-effective diagnostic tool with significant utility in evaluating mediastinal lesions. However, comprehensive data on its diagnostic accuracy, efficacy, and clinical impact on managing these lesions remain limited. This study aims to evaluate the diagnostic utility of fine needle aspiration cytology in mediastinal lesions, focusing on its accuracy, safety, and role in guiding therapeutic approaches. A cross-sectional retrospective analysis was conducted on patients who underwent fine needle aspiration for mediastinal lesi

The role of molecular pathology in mediastinal sarcomas.

Mediastinal sarcomas represent rare neoplasms of mesenchymal origin. Most published data on mediastinal sarcomas is primarily derived from small series and case reports. Although rare, primary mediastinal sarcomas have a clinically aggressive course with worse 10-year survival rates than other types of mediastinal tumors, highlighting the importance of adequate diagnosis of these lesions. The diagnosis of mediastinal sarcomas is complicated by the varied histologic subtypes of tumors that can occur and which can sometimes display overlapping clinical, morphological, imaging, and immunohistoche

[Mediastinal tumors in children].

Forty patients with primary mediastinal masses, treated at the Children's Hospital "La Fe" between 1971-1983, were review. Malignant neoplasms were the most common pathology in this series (23 cases), followed by benign lesions (10 cases), congenital malformations (five cases) and inflammatory diseases (two cases). Complementary explorations for the preoperative evaluation of theses masses is discussed. Surgical removal was carried out on all tumors. Mortality caused by performing biopsy on lymphomas was high. Two of the five congenital malformations were removed. The other three did not recei

Precise diagnosis and effective management of highly vascular anterior mediastinal paragangliomas: A case report.

INTRODUCTION: Mediastinal paragangliomas are rare neoplasms arising from extra-adrenal neural crest cells, presenting as either functional or nonfunctional tumors. Clinical manifestations range from catecholamine-related symptoms to physical compression effects. Accurate recognition of these tumors is crucial for diagnosis and management due to their rarity and association with vital mediastinal structures. PRESENTATION OF CASE: We report the case of a 53-year-old Syrian male presenting with progressive dyspnea, fatigue, and weakness. Imaging revealed a highly vascular anterior mediastinal ma

Primary mediastinal tumors in children.

Mediastinal masses in children constitute a heterogeneous group of malignant and benign neoplasms. The majority of malignant tumors in our series of 188 children with a mediastinal mass were Hodgkin's and non-Hodgkin's lymphoma (87 patients) arising in the anterior and middle mediastinum. Ganglioneuroma (17) arising in the posterior mediastinum made up the bulk of benign tumors (52). Infants less than 2 yr old presented with symptoms of tracheal compression, whereas older children had fewer symptoms but a far greater likelihood of having a malignancy. In the assessment of patients with mediast

Spectrum of thymic uptake at 18F-FDG PET.

Fluorine-18 fluorodeoxyglucose (FDG) positron emission tomography (PET) has emerged as a strong diagnostic tool for the diagnosis and staging of neoplasms. Uptake in the thymus at (18)F-FDG PET complicates the assessment of mediastinal involvement by tumor in children and young adults. Increased thymic (18)F-FDG uptake may represent normal physiologic uptake but may also indicate the presence of thymic hyperplasia, lymphomatous infiltration, primary thymic neoplasm, or metastatic disease. Familiarity with the patterns of (18)F-FDG uptake that characterize these pathologic conditions is crucial

Endobronchial ultrasound-guided transbronchial needle aspiration in the diagnosis of mediastinal metastases of clear cell renal cell carcinoma.

Evaluation of mediastinal lymphadenopathy in patients with a previous diagnosis of renal cell carcinoma (RCC) is critical for the determination of further treatment. A minimally invasive method of cytology sampling of mediastinal lymph nodes using endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) has emerged as a useful tool in diagnosis. Between January 2010 and April 2018, we performed 1744 EBUS-TBNA studies of mediastinal and hilar lymph nodes for a variety of clinical indications including mediastinal malignancy. Sixteen patients (93.7% males, mean age 59.1 years

Eosinophilic vacuolated tumor of the kidney can have distant metastasis: a histo-molecular case study.

Eosinophilic vacuolated tumor (EVT) is an emerging renal entity and further studies are required to characterize this neoplasm. EVT is considered generally indolent because metastasis or death from the disease has never been reported. Herein, we report the first case of EVT with mediastinal lymph node metastasis to validate its at least borderline behavior. Based on the morphological features, we additionally used a large panel of immunohistochemical antibodies and next-generation sequencing (NGS) to confirm the diagnosis of EVT and exclude other renal entities. This case report highlights the

Endobronchial Metastasis with Bloody Sputum 20 Years after Complete Resection of type A Non-Invasive Thymoma.

Masaoka stage I type A thymomas rarely recur. We report the case of an 82-year-old man who developed endobronchial metastasis after thymothymectomy for Masaoka stage I type A thymoma. Twenty years after surgery, the patient developed bloody sputum, and chest computed tomography revealed a neoplasm obstructing the right upper lobe bronchus of the lung with enlarged mediastinal lymph nodes. He underwent right upper lobectomy and mediastinal lymph node dissection. Although preoperative pathological diagnosis was squamous cell carcinoma of the lung, postoperative histopathology revealed endobronch

Carcinoid tumor of the thymus.

Primary carcinoid tumor of the thymus is a rare, slow-growing neoplasm occurring most commonly in middle-aged men. The usual clinical manifestations are those of a mediastinal mass, although in asymptomatic patients, the diagnosis may be suggested by a routine chest roentgenogram. Excision is the most common treatment. Because the disease may recur up to ten years after surgery, follow-up is important. The case reported here is of a 49-year-old man who was hospitalized for evaluation of a mediastinal mass first detected six years previously. When laboratory and other tests proved nondiagnostic

Endobronchial ultrasound-guided transbronchial needle aspiration diagnosis of mediastinal lymph node metastasis of mucinous adenocarcinoma: arborizing stromal meshwork fragments as a diagnostic clue.

Endobronchial Ultrasound-Guided Transbronchial Needle Aspiration (EBUS-TBNA) is a reliable and accurate method for the diagnosis of mediastinal metastases in patients with pulmonary and extrathoracic neoplasms. We report the cytopathologic findings of a case of metastatic signet-ring cell carcinoma with abundant extracellular mucin production in the mediastinal lymph nodes of a 41-year-old woman, who presented with nausea, abdominal pain, and weight loss. Imaging studies showed a renal mass, numerous lung nodules, and mediastinal and retroperitoneal lymphadenopathy. EBUS-TBNA of level 4R and 7
