Automated electronic medical record abstraction algorithm for radical prostatectomy outcomes.

OBJECTIVE: Variation in outcomes following radical prostatectomy and inclusion of prostate cancer surgery metrics in hospital ratings signal need for procedure-specific quality improvement (QI) efforts. We hypothesized that a novel electronic medical record (EMR)-based, automated algorithm-driven algorithm for surgical outcomes and quality metrics following robot-assisted laparoscopic radical prostatectomy (RALP) would demonstrate >90% sensitivity and specificity and significant inter-rater reliability (IRR) with National Surgical Quality Improvement Program (NSQIP) abstraction. STUDY DESIGN:

Implementation and Usability of an Electronic Medical Record-based Ureteral Stent Tracker.

OBJECTIVE: To design and implement a simple electronic medical record-based ureteral stent tracker. To assess its impact on stent dwell time and stent-related complications. METHODS: Patients with stents placed 12 months before and 6 months after stent tracker implementation were identified at 3 urban hospitals. Those with stents-on-strings and intentional chronic indwelling stents (greater than 90 days) were excluded. Patient demographics, stent characteristics (eg, indication, string, dwell time), and clinical outcomes (eg, positive urine cultures, complications) were reviewed and compared

An artificially intelligent (or algorithm-enhanced) electronic medical record in orofacial pain.

This review examines how a highly structured data collection system could be used to create data-driven diagnostic classification algorithms. Some preliminary data using this process is provided. The data collection system described is applicable to any clinical domain where the diagnoses being explored are based predominately on clinical history (subjective) and physical examination (objective) information. The system has been piloted and refined using patient encounters collected in a clinic specializing in Orofacial Pain treatment. In summary, whether you believe a branching hybrid check-bo

An objective study of the impact of the electronic medical record on outcomes in trauma patients.

It is commonly believed that the electronic medical record (EMR) will improve patient outcomes. However, there is scant published literature to support this claim and no studies in any surgical population. Our hypothesis was that the EMR would not improve objective outcome measures in patients with traumatic injury. Prospectively collected data from our university-based Level I trauma center was retrospectively reviewed. Demographic, injury severity as well as outcomes and complications data were compared for all patients admitted over a 20-month period before introduction of the EMR and a 20-

Epidemiology of angina pectoris: role of natural language processing of the medical record.

BACKGROUND: The diagnosis of angina is challenging because it relies on symptom descriptions. Natural language processing (NLP) of the electronic medical record (EMR) can provide access to such information contained in free text that may not be fully captured by conventional diagnostic coding. OBJECTIVE: To test the hypothesis that NLP of the EMR improves angina pectoris ascertainment over diagnostic codes. METHODS: Billing records of inpatients and outpatients were searched for International Classification of Diseases, Ninth Revision (ICD-9) codes for angina pectoris, chronic ischemic heart

How accurately does the medical record capture maternal history of cancer?

We sought to assess the reliability of information regarding the maternal history of cancer by comparing the medical records of 214 women with breast cancer, ages 26-59 years and diagnosed in 1974-1995, and of their controls with the medical records of their mothers. Subjects were members of Kaiser Permanente, Northern California, selected for a study of early-life predictors of breast cancer. For any type of cancer identified in the mother's medical record, the proportion noted in the daughter's medical record at least 6 months before the daughter's diagnosis or reference date was 56% among c

Feasibility of Using Electronic Medical Records to Identify Postdischarge Mental Health Screening Documentation in Trauma Patients.

BACKGROUND: Injury leads to long-term physical and mental health challenges, yet inpatient screening for post-traumatic stress disorder and depression remains inconsistent, and postdischarge mental health is poorly understood, particularly in less resourced trauma centers where emerging symptoms may go unrecognized. OBJECTIVE: To evaluate the feasibility of using the electronic medical record to identify postdischarge mental health screening documentation in adult trauma patients within 6 months after discharge. METHODS: This retrospective descriptive feasibility study was conducted at a mid

Electronic health records reveal variations in the use of blood units by hour and medical specialty.

BACKGROUND AND OBJECTIVES: Efficient blood supply chain requires accurate demand estimates. Blood demand is created by clinicians making transfusion decisions based on patient status. To better understand the use of blood units, we tracked their use hourly and across a large hospital organization. MATERIALS AND METHODS: We analysed blood use in adult patients over 2021-2022 at HUS Helsinki University Hospital, serving a population of 1.7 million and consuming a third of blood units used in Finland. We utilized electronic health records (EHR) to map transfusions to patient demographics, diagno
