Meniscus Profiling of Fly Fishing Lures by Optical Diffraction.

Fly fishing is a method of fishing using hand-tied lures called "flies" that mimic local prey like insects or crayfish. In this study, we focus on fly fishing in calm waters using "dry" flies, which are absent of any gel coating, and float on the water surface by virtue of their construction of low-density materials. Despite an 1800-year history of fly fishing, little is known how these flies look like to fish. We introduce an optical method using the diffraction of transmitted underwater riverbed patterns to quantify the meniscus shape around floating objects. We use this method to describe t

Corneal aberrations are associated with low-energy meniscus injuries.

BACKGROUND: Visual impairment can cause balance problems. Therefore, visual impairment caused by an increase in corneal deviations can lead to sudden and unstable loads in the lower extremities. We aimed to investigate the possible relationship between low-energy meniscal injuries and corneal structural measures. METHODS: This prospective, observational study included individuals aged between 18-40 years with a normal body-mass index. The study group consisted of 54 patients with grade 2 or 3 meniscus injuries after low-energy activity. The control group consisted of 54 healthy individuals wi

Technique for Medial Meniscus Ramp Repair: An Anatomic Approach.

Medial meniscus ramp lesions are commonly associated with anterior cruciate ligament injury. They are one of the causes of positive pivot shift and can lead to failure to obtain rotary stability if missed during the surgical procedure. Several repair methods use a suture lasso from 2 posteromedial portals to attach the posteromedial capsule to the meniscus. Because it was concluded at the consensus meeting of the European Society for Sports Traumatology, Knee Surgery and Arthroscopy in 2019 that ramp lesions are not considered a meniscal tear and the lesion is essentially in either the articul

Arthroscopic Repair of a Lateral Meniscus Oblique Radial Tear.

BACKGROUND: Lateral meniscal oblique radial tears (LMORTs) are the most common type of lateral meniscal tears and occur concomitantly with anterior cruciate ligament (ACL) tears in up to 12% of cases. Four types of LMORTs have been classified in the literature based on proximity to the meniscal root and extent of the tear. Type 1 and 2 LMORTs represent partial and complete posterior root tears less than 10 mm from the root insertion. Type 3 and 4 LMORTs are partial and complete posterior radial meniscus tears greater than 10 mm from the root insertion. INDICATIONS: Type 3 and 4 LMORTs can and

Horizontal Cleavage Meniscus Tear Repaired Using a Cerclage Technique.

BACKGROUND: Horizontal meniscal cleavage tears often occur in older individuals and have a degenerative component. Commonly, these are managed with partial meniscectomy. However, this results in significantly decreased contact area and increased peek contact forces. Arthroscopic repair has shown comparable results with repairs of other tear types. INDICATIONS: Cerclage repair is performed for a horizontal cleavage tear in individuals without significant arthritis and in whom partial meniscectomy would remove an unacceptably large portion of meniscal tissue. TECHNIQUE DESCRIPTION: Using stand

Lateral Meniscus Allograft Transplantation in a Skeletally Immature Patient.

BACKGROUND: Lateral meniscus deficiency in young patients typically results from irreparable lateral meniscus tears, often associated with discoid meniscus. Such cases present a challenging clinical scenario given the known natural history of meniscus deficiency leading to degenerative lateral knee compartment changes. Managing this problem in a skeletally immature patient presents additional clinical and surgical considerations. INDICATIONS: Lateral meniscus allograft transplantation (LMAT) is indicated in patients with prior total or subtotal lateral meniscectomy resulting from irreparable

Treatment, Return to Play, and Performance Following Meniscus Surgery.

PURPOSE OF REVIEW: The standard of care in meniscal tear management is constantly evolving, especially for athletes and high-demand patients. Meniscus repairs, meniscus transplants, and partial meniscectomies are commonly performed, and rehabilitation methods following these operations are becoming more sophisticated. The ultimate goal of these procedures is returning patients to full activity with minimal risks. Return to play should be systematic, pathology dependent, and individualized to an athlete's needs, expectations, and level of play. This article provides a review of the current trea

Degenerative Meniscus in Knee Osteoarthritis: From Pathology to Treatment.

Knee osteoarthritis is a common degenerative joint disease characterized by chronic knee pain and disability in daily living. The lesion can involve the cartilage as well as the synovium, bone, ligaments, and meniscus, indicating a complicated pathology for knee osteoarthritis. The association with the meniscus has recently attracted much attention. Meniscal tears can initiate and progress knee osteoarthritis, with deleterious effects on the important roles of the meniscus in load distribution, shock absorption, and stability of the knee joint. Degenerative meniscus lesions are commonly observ

Etiology of posterior meniscus root tears: medial vs. lateral.

PURPOSE: The aim of this study was to generate more information on the etiology and pathogenesis of medial (MM) and lateral (LM) meniscus root tears. Our hypothesis was that root tears of the MM predominantly result from degenerative damage, whereas root injuries of the LM are mainly of traumatic origin. METHODS: Consecutively, 53 patients with a root tear of the medial meniscus (MMRT) and 51 patients with a root tear of the lateral meniscus (LMRT) were included in this study. The diagnosis was confirmed radiologically by MRI as well as arthroscopically. In addition to patient-specific data s

Arthroscopic Medial Meniscus Root Repair With Transtibial Ripstop Technique.

BACKGROUND: Meniscal root tears are radial tears occurring at or within 1 cm of the meniscal root attachment. These injuries have been shown to be the biomechanical equivalent of a total meniscectomy and are thought to be responsible for a rapid progression of osteoarthritis if left untreated. INDICATIONS: Meniscal root repair is indicated if possible, with the exception of patients who have diffuse Outerbridge 3-4 osteoarthritis of the ipsilateral compartment, those who are poor surgical candidates due to age or medical comorbidity, or in whom nonsymptomatic tears are found incidentally. Medial Meniscus Radial Tear: A Transtibial 2-Tunnel Technique.

Complete radial tears of the medial meniscus significantly decrease the meniscal tissue's ability to dissipate tibiofemoral loads and have been described as functionally similar to a total meniscectomy, predisposing patients to early osteoarthritis. At present, no consensus exists regarding the optimal surgical treatment of a radial meniscal tear. Current repair techniques have led to a reportedly high rate of incomplete healing or healing of the meniscus in a nonanatomic, gapped position, which compromises its ability to withstand hoop stresses. Improvement regarding the ability to repair and

[S2k-Guideline on Meniscus Diseases: from Aetiology to Scoring].

The number of patients with meniscal injuries is increasing constantly, but the treatment algorithms are undergoing continuous change. The effects of meniscal surgery, as well as the indications for the procedure, are currently a matter of heated debate. Various German speaking associations addressing topics related to the knee have joined forces to develop guidelines for the diagnosis, evaluation and therapy of meniscal lesions. The hope is that this first of two publications will shed light on some of the ongoing issues and offer guidance to health care professionals treating these patients.

Lateral meniscus allograft transplantation using a single-incision technique.

PURPOSE: The aim of this work is to introduce and evaluate a novel arthroscopically assisted meniscus allograft transplantation (MAT) technique that requires only one small anterior incision for an arthrotomy. METHODS: In the novel technique, the posterior horn of the allograft meniscus was repaired using the modified all-inside suture technique instead of the conventional inside-out technique. By doing this, an additional posterior incision could be avoided. Thirteen consecutive patients with total or subtotal meniscectomy of the lateral meniscus underwent lateral MAT using the novel techniq

[Hypermobile snapping medial meniscus: features on flexion-extension MRI].

We report 3 cases of snapping medial meniscus due to hypermobility confirmed on extension and flexion MR imaging. The anterior horn of the medial meniscus was anteriorly subluxed during extension with reduction during flexion. Analysis of our imaging data and a review of the literature confirm that this relates to hypermobile medial meniscus, an anatomical variant.

Arthroscopic all-inside lateral meniscus suture using posterolateral portal.

A new arthroscopic all-inside suture for lateral meniscus posterior horn tear using a single posterolateral portal is described. A lateral meniscus posterior horn peripheral longitudinal tear is often seen with anterior cruciate ligament injury or discoid lateral meniscus tear. Conventional repair methods, such as arthroscopic inside-out, outside-in, or all-inside sutures, can be cumbersome and technically demanding. Our all-inside suture using the posterolateral portal allows thorough visualization of the posterolateral structures, excellent coaptation of torn meniscus, strong knot tightening

Mechanical stimulation toward tissue engineering of the knee meniscus.

Current clinical practices do not adequately regenerate the meniscus of the knee secondary to a tear. Complete or partial meniscus removal leads to degenerative changes within the joint. Tissue engineering of the meniscus promises a potent solution. Before embarking on tissue engineering of the meniscus, it is crucial to have a thorough comprehension of the biomechanical role that this tissue fulfills and how the structure of meniscus is uniquely suited to that purpose. To better understand this, we have examined the meniscus, as well as associated tissues, within the body. For the first time,
