Erythrocyte glucose-6-phosphate dehydrogenase and pyruvate kinase activities in hemoglobin H disease.

Erythrocyte glucose-6-phosphate dehydrogenase (G6PD) and pyruvate kinase (PK) activities were studied in hemoglobin H (HbH) patients by spectrophotometric method, cytochemical method and the methemoglobin reduction (MR) test for the detection of heterozygous G6PD deficiency. G6PD deficiency was found in 7 of 64 cases (10.9%), including 3 cases of genotype alpha 1/alpha 2 and 4 cases of genotype alpha 1/CS. None of the HbH patients was found to be PK-deficient. Spectrophotometrically determined G6PD and PK activities were significantly higher in HbH patients than in normals (p less than 0.001),

Drug-induced methaemoglobinaemia. Treatment issues.

In normal erythrocytes, small quantities of methaemoglobin are formed constantly and are continuously reduced, almost entirely by the reduced nicotine adenine dinucleotide (NADH) diaphorase system, rather than the reduced nicotine adenine dinucleotide phosphate (NADPH) diaphorase system. Methaemoglobinaemias are usually the result of xenobiotics, either those that may directly oxidise haemoglobin or those that require metabolic activation to an oxidising species. The most clinically relevant direct methaemoglobin formers include local anaesthetics (such as benzocaine and, to a much lesser exte

When methylene blue fails: a case of methemoglobinemia induced by azoxystrobin and propiconazole managed by exchange transfusion.

BACKGROUND: Methemoglobinemia is a potentially life-threatening condition resulting from the oxidation of hemoglobin's iron moiety from ferrous (Fe²⁺) to ferric (Fe³⁺). Clinical features include cyanosis unresponsive to oxygen therapy, chocolate-colored blood, and a marked saturation gap between pulse oximetry and arterial blood gas. Methylene blue remains the cornerstone of treatment; however, in rare instances, patients may exhibit refractory responses necessitating advanced interventions. Literature documenting methemoglobinemia due to fungicides like azoxystrobin and propiconazole remains

Congenital Methemoglobinemia-Induced Cyanosis in Assault Victim.

Methemoglobinemia is a blood disorder in which there is an elevated level of methemoglobin. In contrast to normal hemoglobin, methemoglobin does not bind to oxygen, which leads to functional anemia. The signs of methemoglobinemia often overlap with other cardiovascular and pulmonary diseases, with cyanosis being the key sign of methemoglobinemia. Emergency physicians may find it challenging to diagnose cyanosis as a result of methemoglobinemia. Our patient is a healthy 28-year-old male, a heavy smoker, who presented to the emergency department with multiple minimum bruises on his body, claimin

Severe Methemoglobinemia due to Sodium Nitrite Poisoning.

Case. We report a case of severe methemoglobinemia due to sodium nitrite poisoning. A 28-year-old man was brought to our emergency department because of transient loss of consciousness and cyanosis. He was immediately intubated and ventilated with 100% oxygen. A blood test revealed a methemoglobin level of 92.5%. Outcome. We treated the patient with gastric lavage, activated charcoal, and methylene blue (2 mg/kg) administered intravenously. Soon after receiving methylene blue, his cyanosis resolved and the methemoglobin level began to decrease. After relocation to the intensive care unit, his

Tolerance of anemia: understanding the adaptive physiological mechanisms which promote survival.

Acute anemia and hemodilution are associated with increased organ injury (brain, kidney) and mortality in patients undergoing surgery. A more complete understanding of the adaptive physiological mechanisms which maintain tissue oxygen delivery, and the hemoglobin concentration at which these mechanisms are overwhelmed, would help to direct treatments to improve event free survival in patients who experience acute anemia. Adaptive cardiovascular and cellular responses are key to maintaining survival during acute anemia. We have identified neuronal nitric oxide synthase (nNOS) and hypoxia induci

Methemoglobinemia presenting in a circumcised baby following application of prilocaine: a case report.

INTRODUCTION: Local anesthesia with prilocaine has become a routine part of ambulatory circumcision procedures. Methemoglobinemia is a rare but potentially lethal complication of local anesthetics. CASE PRESENTATION: We report the case of a 40-day-old Turkish boy who presented with cyanosis after receiving local anesthesia with prilocaine. His methemoglobin level revealed severe methemoglobinemia (methemoglobin = 44%). His cyanosis resolved after intravenous administration of methylene blue. CONCLUSION: Although the association between prilocaine use and methemoglobinemia has generally restr

Methemoglobinemia due to application of prilocaine during circumcision and the effect of ascorbic acid.

Local anesthesia with prilocaine has become a routine part of ambulatory circumcision procedures. Methemoglobinemia is a rare but potentially lethal complication of local anesthetics. We report the case of a 42-day-old boy who presented with cyanosis after receiving local anesthesia with prilocaine. Methemoglobin level revealed severe methemoglobinemia (methemoglobin=44.5%). His cyanosis promptly resolved after intravenous administration of ascorbic acid. Cases of local anesthetic-induced methemoglobinemia in urology are under recognized. Methemoglobinemia induced by refrigerated vegetable puree in conjunction with supraventricular tachycardia.

UNLABELLED: We report on a case of methemoglobinemia in conjunction with supraventricular tachycardia. A 6-mo-old infant was admitted to hospital with perioral cyanosis three hours after eating a refrigerated mixed-vegetable puree. The patient developed supraventricular tachycardia (SVT) and had a heart rate of 230 beats/min. The arrhythmia resolved spontaneously. A chemical analysis showed oxygen saturation of 85% and a methemoglobin level of 25%. The infant was treated with oxygen and referred to the Pediatric Intensive Care Unit. The patient had two additional short episodes of SVT, which w

Kinetic analysis in single, intact cells by microspectrophotometry: evidence for two populations of erythrocytes in an individual heterozygous for glucose-6-phosphate dehydrogenase deficiency.

A microspectrophotometric technique was used to measure the kinetics of methemoglobin reduction in intact, unaltered human erythrocytes. Reduction was catalyzed by endogenous NADPH-methemoglobin reductase in the presence of Nile Blue. The technique was applied to the study of erythrocytes from a female donor with decreased glucose-6-phosphate dehydrogenase (G-6-PD) activity. The individual was shown to be heterozygous for deficiency of G-6-PD. The kinetic study revealed two distinct populations of erythrocytes that were nearly equal in number. One cell population showed reduction rates between

Nitrite poisoning in herring gulls (Larus argentatus) and ring-billed gulls (Larus delawarensis).

Landfill disposal of a fertilizer manufacturing waste product was associated with a die-off of gulls in New Hanover County, North Carolina. An estimated 250 herring and ring-billed gulls were found dead at the site following the initial disposal of this material. Chemical analyses revealed that the fertilizer waste contained predominately calcium (12.0 to 22.2%) and nitrite (3.0 to 15.2%). Contents of the proventriculi and gizzards of dead gulls also contained calcium (3.0 to 10.9%) and nitrite (1,730 ppm). Fertilizer waste administered orally to 16-day-old domestic turkeys resulted in acute,

Pro-oxidative effects of tea and polyphenols, epigallocatechin-3-gallate and epigallocatechin, on G6PD-deficient erythrocytes in vitro.

Glucose-6-phosphate dehydrogenase (G6PD)-deficient subjects are vulnerable to chemical-induced hemolysis if exposed to oxidative agents. Recent studies reported that green tea and its constituents might act as pro-oxidants. Our objective was to investigate effects of tea and its polyphenolic components on the oxidative status of human G6PD-deficient erythrocytes. Erythrocytes of G6PD-deficient (n = 8) and normal (n = 8) subjects were incubated with water extracts of 3 types of tea samples (black tea, green tea and decaffeinated green tea extract) and 6 polyphenols. The resulting levels of redu

Protein thiol modifications of human red blood cells treated with t-butyl hydroperoxide.

Oxidative stress causes modification of cellular macromolecules and leads to cell damage. The objective of this study was to identify protein modifications that relate to thiol groups in human red blood cells under oxidative stress. With t-butyl hydroperoxide (t-BH) treatment, results of isoelectric focusing (IEF) analysis showed that two dithiothreitol-reversible modifications are observed, one toward the cathode and the other to the anode. Protein change toward the cathode was demonstrated to be hemoglobin oxidation, which gains a net positive charge, based on the same focus on IEF gels as h

Nitroxyl accelerates the oxidation of oxyhemoglobin by nitrite.

Angeli's salt (Na₂N₂O₃) decomposes into nitroxyl (HNO) and nitrite (NO₂(-)), compounds of physiological and therapeutic interest for their impact on biological signaling both through nitric oxide and nitric oxide independent pathways. Both nitrite and HNO oxidize oxygenated hemoglobin to methemoglobin. Earlier work has shown that HNO catalyzes the reduction of nitrite by deoxygenated hemoglobin. In this work, we have shown that HNO accelerates the oxidation of oxygenated hemoglobin by NO₂(-). We have demonstrated this HNO mediated acceleration of the nitrite/oxygenated hemoglobin reaction with

Acute methemoglobinemia after a blast injury: a case report.

Methemoglobin is a structurally modified form of hemoglobin incapable of binding oxygen, and elevated levels of methemoglobin cause tissue hypoxia. Occupational exposure to 2,4,6-trinitrotoluene, commonly called trinitrotoluene, causes methemoglobinemia. This case report describes a 27-year-old male sergeant who developed methemoglobinemia upon exposure to trinitrotoluene after a blast injury while welding the walls of tank shells. This is the first case of its kind in Korea. The patient had multiple burns in his abdomen and open fractures in his right leg. While his body temperature, heart ra

Antidotes for acute cyanide poisoning.

Cyanide poisoning can present in multiple ways, given its widespread industrial use, presence in combustion products, multiple physical forms, and chemical structures. The primary target of toxicity is mitochondrial cytochrome oxidase. The onset and severity of poisoning depend on the route, dose, physicochemical structure and other variables. Common poisoning features include dyspnea, altered respiratory patterns, abnormal vital signs, altered mental status, seizures, and lactic acidosis. Our present knowledge supports cyanide poisoning treatment based on excellent supportive care with adjunc

Congenital methemoglobinemia misdiagnosed as polycythemia vera: Case report and review of literature.

Methemoglobinemia is a rare overlooked differential diagnosis in patients presented with cyanosis and dyspnea unrelated to cardiopulmonary causes. Our patient is 29 year old Indian non-smoker male, his story started 6 months prior to presentation to our center when he had generalized fatigue and discoloration of hands. He presented with persistent polycythemia with elevated hemoglobin level. The patient was misdiagnosed in another center as polycythemia and treated with Imatinib. The diagnosis of PV was revisited and ruled out in view of negative JAK2, normal erythropoietin level and absence o

[A Case of Methemoglobinemia Caused by Toluidine Revelation with Dyspnea and Cyanosis].

Toluidine is a known cause of bladder cancer, but it is less widely recognized as a cause of methemoglobinemia because methemoglobinemia is rare. We herein report a case of methemoglobinemia caused by toluidine in a 50-year-old man. A solution of toluidine overflowed from its container during transportation and adhered to the man's clothes, but he drove to his workplace 100 km away without changing his clothes or undergoing decontamination. Before arriving at his workplace, he developed dyspnea and called emergency services, and he was then transported to a local hospital. He had significant c
