Efficacy of continuous high-dose metoclopramide in patients receiving daily cisplatin infusions.

Continuous high-dose metoclopramide (MCP), 2 mg/kg/24 hours, was administered for a period of 84-90 hours to 30 consecutive patients with various advanced solid malignancies receiving concomitantly a 3-day infusion of cisplatin. The latter was given at a dose of 40 mg/m2/day over 6 hours every day, in combination with doxorubicin and cyclophosphamide. Diphenhydramine and dexamethasone were also administered. This prolonged antiemetic therapy proved to be safe with only a 16% incidence (five of 30 patients) of extrapyramidal side effects not necessitating discontinuation of MCP. Also, antiemeti

Bolus and infusions of metoclopramide: insights on benefits and risks from pharmacokinetic modelling.

Metoclopramide is used for prevention or treatment of emesis and gastroesophageal reflux in dogs. Historically, it has been administered three to four times a day. However, reports of adverse reactions associated with intravenous boluses raise concerns. A more modern trend is to administer the total daily dose as a constant rate infusion of 1 to 2 mg/kg/day. A loading dose prior to the infusion is often omitted, which can delay the onset of action. Adequate loading doses for these constant rate infusions are not known. We have reanalysed pharmacokinetic data from dogs receiving metoclopramide

Oral metoclopramide for pediatric migraine management in the ED: health records review.

OBJECTIVES: Migraine is a common presentation in pediatric emergency departments (ED), and non-invasive treatment options are desirable. This study evaluated what proportion of children treated with oral metoclopramide would not need a rescue medication for the treatment of migraine in the ED. METHODS: We conducted a retrospective cohort study at a tertiary pediatric ED in Montréal, Canada, over a one-year period. Patients aged 8-18 years diagnosed with migraine and treated with either oral or intravenous metoclopramide were included. The primary outcome was headache relief, defined as no nee

Akathisia Induced by Metoclopramide in a Pregnant Woman With Nausea and Vomiting: A Case Report.

BACKGROUND: Nausea and vomiting are common during pregnancy and can significantly impair maternal well-being. Metoclopramide is widely used to treat these symptoms but may cause extrapyramidal side effects, including akathisia. Akathisia is characterized by inner-restlessness and psychomotor agitation, which can be difficult to distinguish from perinatal anxiety disorders due to symptom overlap. Reports of metoclopramide-induced akathisia during pregnancy are limited. CASE PRESENTATION: We report a case of metoclopramide-induced akathisia in a pregnant woman at 33 weeks of gestation with naus

Metoclopramide nasal spray in vitro evaluation and in vivo pharmacokinetic studies in dogs.

Metoclopramide (MCP) can effectively alleviate motion sickness-caused nausea and vomiting. Nasal administration offers the greatest patient compliance. It is suitable for self-administration and offers rapid and complete absorption, no first-pass effects and high bioavailability. In the present study, a MCP nasal spray was prepared and evaluated in vitro and in vivo. Nasal cilia toxicity of Bufo toads was used to screen the preservative types and concentrations. Rabbit nasal mucosa was used to evaluate the mucosa permeability of different MCP nasal sprays with different penetration enhancers a

Effects of metoclopramide and ranitidine on preoperative gastric contents in day-case surgery.

This prospective, randomized, double-blinded study was performed to evaluate the effects of intravenous metoclopramide and ranitidine on preoperative gastric contents in outpatients receiving intravenous anesthesia for laparoscopic gynecologic surgery. Fifteen minutes before the induction of anesthesia, the Z-M group (n=20) received 50 mg ranitidine and 10 mg metoclopramide intravenously and the control group (n=20) received the same volume of normal saline. Before the surgery, a 14-F multiorifice nasogastric tube was inserted to aspirate the gastric contents of patients under sedation with pr

[Metoclopramide and dexamethasone in prevention of postoperative nausea and vomiting after inhalational anaesthesia].

OBJECTIVE: Because of its complex profile of action (binding to dopamine, serotonin and histamine receptors), low rate of adverse effects and low cost as a medicinal preparation, metoclopramide is an interesting substance for the prophylaxis of post-operative nausea and vomiting (PONV). As a single substance its antiemetic effects are slight at the usual dose, so the aim was to test the efficacy of a combination of metoclopramide and dexamethasone for the prevention of PONV on a group of patients with the same operative trauma. METHOD: All patients (n = 204) were recruited prospectively (Janu

The cardiovascular effects of metoclopramide in multiple system atrophy and pure autonomic failure.

Metoclopramide (MCP), a central and peripheral dopaminergic blocker with cholinergic activity, has been proposed to treat orthostatic hypotension (OH) on the basis that it could antagonize the vasodilator and natriuretic effects of dopamine. The authors evaluated cardiovascular responses to MCP in 11 subjects with OH: 6 with multiple system atrophy (MSA) and 5 with pure autonomic failure (PAF), along with 6 healthy control subjects. Supine blood pressure (BP), heart rate (HR), and breathing were continuously monitored before, during, and after MCP infusion. The pre-MCP head-up tilt test was to

Pain on injection of propofol: the mitigating influence of metoclopramide using different techniques.

BACKGROUND: Metoclopramide administered intravenously (i.v.) immediately before injection of propofol, after mixing with propofol, or after a rubber tourniquet for 1 min before propofol injection will reduce pain induced by propofol injection. In this study, these three different techniques in reducing propofol injection pain with metoclopramide were compared with lidocaine or saline to evaluate the most effective method in reducing propofol injection pain. METHODS: In a randomized, semi-double-blind treatment, 175 patients were included into this study. Patients in group A were pretreated wi

An adolescent case of sulfhemoglobinemia associated with high-dose metoclopramide and N-acetylcysteine.

We describe the case of an adolescent girl who received high-dose metoclopramide in combination with oral N-acetylcysteine therapy for acute acetaminophen toxicity. Whole blood-sample analysis for abnormal hemoglobin pigments established the diagnosis of sulfhemoglobinemia. Metoclopramide has been shown to cause sulfhemoglobinemia, particularly when used in repeated high doses. Although N-acetylcysteine alone has not been associated as the cause, we suggest that sulfhemoglobine-mia is a potential complication in patients treated with metoclopramide for the nausea that often accompanies oral N-

Comparison of dihydroergotamine with metoclopramide versus meperidine with promethazine in the treatment of acute migraine.

Migraineurs often seek office-based treatment for acute headache. To compare the efficacy and side effect profile, we entered 27 migraineurs into a prospective, randomized, double-blind study where each patient received either 75 mg meperidine with 25 mg promethazine IM or .5 mg dihydroergotamine with 10 mg metoclopramide IV. After 1 hour, pain relief was similar in the two groups, but side effects were significantly greater in the meperidine with promethazine regimen group. The dihydroergotamine with metoclopramide regimen is effective, and has minimal side effects, making it an attractive me

Metoclopramide in the treatment of reflux oesophagitis: a comparison of normal and controlled-release formulations.

Twenty patients with reflux oesophagitis were recruited into a randomized, single (investigator)-blind, two-part crossover study of controlled-release metoclopramide ('Gastrobid Continus' tablets) 15 mg given twice daily, and normal-release metoclopramide ('Maxolon'), 10 mg given 4-times daily. Both treatment regimens were efficacious in reducing the severity of heartburn, the incidence of waking at night due to pain from heartburn and the severity of gastro-intestinal symptoms. The incidence of volunteered and sought side-effects/symptoms was similar on both treatments. More patients preferre
