Nuclear magnetic resonance (NMR) imaging of Arnold-Chiari type I malformation with hydromyelia.

Saturation recovery nuclear magnetic resonance (NMR) images and metrizamide computed tomography (CT) scans were obtained in an adult patient with a clinical history suggestive of syringomyelia. Both NMR and CT studies showed low lying cerebellar tonsils. The CT study demonstrated central cavitation of the spinal cord from the midthoracic to midcervical levels but could not exclude an intramedullary soft tissue mass at the cervico-medullary junction. The NMR images in transverse, coronal, and sagittal planes demonstrated extension of an enlarged central spinal cord cerebrospinal fluid space to

[Primary empty sella syndrome treated by transsphenoidal extradural balloon expansion--a case report].

A 34-year-old woman was admitted for the chief complaints of headache and blurred vision. She had bilateral papilledema and slight increase in CSF pressure (175mmH2O) with normal visual acuity and field. Neurological and hormonal examination were normal except for over response of PRL and TSH to TRH test. The sella was enlarged and MRI and metrizamide CT demonstrated intrasellar CSF filling with remodeling of the pituitary gland. The patient was diagnosed as primary empty sella syndrome associated with benign intracranial hypertension. The complaints did not subside for six months. The patient

[A case of non-neoplastic pineal cyst presenting Parinaud's syndrome].

A case of non-neoplastic pineal cyst with Parinaud's syndrome is presented. A 54-year-old woman was admitted to the department of neurosurgery. Fukui Medical School on October, 1985, complaining of the paralysis of upward gaze. A computed tomography (CT) demonstrated a low density mass in the pineal region and the mass was not enhanced after the injection of contrast agent. Metrizamide CT cisternogram delineated the mass clearly. Bilateral carotid and vertebral angiograms showed no significant findings. On November 5, an operation was performed by means of an occipital transtentorial approach.

Myelography in achondroplasia: value of a lateral C1-2 puncture and non-ionic, water-soluble contrast medium.

Because of technical difficulties and diagnostic limitations encountered with other myelographic techniques in patients with achondroplasia, the authors employed a lateral C1-2 puncture and non-ionic, water-soluble contrast medium in 18 achondroplastic patients with spinal compression (21 procedures). This technique proved most appropriate for identifying the upper limit of degenerative osteophytes causing exacerbation of congenital spinal stenosis, which is crucial in planning decompressive surgery. A potentially important additional finding was the presence of degenerative lower cervical spi

Contributory aspects of MRI in the evaluation of basal encephaloceles.

Basal encephaloceles are rare pathologies which belong to the now well-defined complex called median cleft face syndrome. This entity includes median craniofacial dysraphism associated to a varying degree with optic tract pathology, dysfunctioning of the hypothalamus-hypophysis axis, agenesis of the corpus callosum and basal encephalocele. The latter is that virtually constant element for which surgery is frequently requested. Owing to the large number of surgical failures (especially in the transsphenoidal type) medical imaging for the evaluation of malformative damages must be very accurate.

Sonographic evaluation of spinal cord birth trauma with pathologic correlation.

Birth trauma to the spinal cord is a serious potential complication of delivery. Determining the presence, severity, and extent of injury poses a difficult problem because of the often confusing clinical setting. Myelography has been recommended for assessing spinal cord birth trauma but is invasive and may not be helpful. The role of sonography in evaluating spinal cord birth trauma has not been previously described. We assessed the value of sonography in four patients, three of whom also had CT metrizamide myelography. Autopsy correlation was available in three patients. Sonography was able

Chymopapain for the treatment of intervertebral disc herniation. The final report of a double-blind study.

Sixty patients with unilateral sciatica resulting from lumbar intervertebral disc herniation were entered into a double-blind study. All patients had failed to respond to conservative management, and the diagnosis had been confirmed by metrizamide myelography. Following a randomized schedule, 30 patients had intradiscal injections of chymopapain, and 30 patients had intradiscal injections of an equivalent volume of normal saline solution. Two years after the injection, 77% of patients treated with chymopapain were assessed as moderately improved, compared with 47% of saline-injected patients.

The tethered spinal cord. Etiology and clinical manifestations.

Tethering of the spinal cord is frequently associated with myelomeningocele. However, orthopaedic surgeons must be familiar with the primary tethered cord, which is associated not with an overt myelomeningocele but with more subtle clinical findings, particularly in the pediatric patient. The charts of patients with tethered spinal cords from the Honolulu Unit of the Shriners Hospital for Crippled Children are reviewed to highlight potential diagnostic and treatment problems. Tethered spinal cord should be considered in the differential diagnosis of patients who present with isolated lower ext

MR imaging of the craniovertebral junction, cranium, and brain in children with achondroplasia.

MR imaging of the craniovertebral junction, cranium, and brain was performed in 10 patients (aged 3 months to 16 years) with achondroplasia. All patients had narrowing of the subarachnoid space at the level of the foramen magnum and five had compressive deformities of the cervicomedullary junction. Apparent upward displacement of the brainstem and a relatively vertical course of the optic nerve were seen in all patients. Dilated lateral and third ventricles were seen in five patients and bifrontal widening of the subarachnoid space was evident in four. Skull asymmetry was seen in two patients

Syringomyelic syndrome: clinical features in 31 cases confirmed by CT myelography or magnetic resonance imaging.

The clinical features of 31 cases of syringomyelic syndrome, confirmed by metrizamide CT myelography (CTM) or magnetic resonance imaging (MRI), were analysed. Bilateral suspended sensory disturbance, previously regarded as a characteristic feature, was encountered in only 2 cases (6.5%). Thirteen patients (41.9%) showed no motor deficits of the upper limbs and 7 patients (22.6%) did not have weakness or atrophy in any of the four extremities. Cervical segmental signs were absent in 7 cases (22.6%) and in 2 (6.5%) signs were only present in the lower limbs. Scoliosis and foot deformity, such as

[Etiologic factors of cervical spondylotic myelopathy in aged patients--clinical and radiological studies].

Thirty-seven aged patients with cervical spondylotic myelopathy were investigated clinically and radiologically, and their findings were compared with those of younger patients. Dynamic canal stenosis in addition to developmental canal stenosis had a significant role in the occurrence of myelopathy in the aged patients. This dynamic canal stenosis was commonly encountered in the upper disc levels of C3-4 and C4-5. Myelography and CT-metrizamide myelography represented multisegmental lesions which resulted in variable clinical features. Spinal cord compression was correlated with severity of sy

Percutaneous retrogasserian glycerol rhizotomy for trigeminal neuralgia. A prospective study of 100 cases.

A prospective study of percutaneous retrogasserian glycerol rhizotomy (PRGR) with and without metrizamide trigeminal cisternography is reported in the treatment of the trigeminal neuralgia. A series of 100 patients with typical trigeminal neuralgia were allocated randomly to two treatment groups: Group I patients received PRGR with trigeminal cisternography (50 cases) and Group II patients received PRGR without trigeminal cisternography (50 cases). The results indicate that PRGR without trigeminal cisternography is a valid alternative to the original technique. Factors that assured the accurat

Symptomatic spinal cord deformity secondary to a redundant intramedullary shunt catheter. Clinical and radiographic features.

Right arm pain, motor and sensory loss in the right arm and right facial numbness recurred in a 27 year old quadraplegic shortly after a posttraumatic spinal cord cyst (PTSCC) was shunted via a catheter into the adjacent subarachnoid space. Although shunt malfunction was clinically suspected, metrizamide computed tomography (MCT) suggested that redundancy of the catheter had caused deformity of the spinal cord. This hypothesis was confirmed at surgery when intraoperative spinal sonography (IOSS) showed that the spinal cord deformity at C1-C2 disappeared when the catheter was withdrawn. This ca

Subarachnoid space: middle ear pathways and recurrent meningitis.

Congenital bony abnormalities of the inner ear may result in a communication between the middle ear and the subarachnoid space. Patients with this anomaly often present with recurrent meningitis associated with acute otitis media or with middle ear fluid. This article presents three cases of recurrent meningitis with open middle ear--subarachnoid space connections. The first two cases involve a cerebrospinal fluid leak into the middle ear via the oval window, both patients having a Mondini-type of inner ear deformity. The pathway in the third case opened into the middle ear along the horizonta

[Radiodiagnosis of the spinal canal using a nonionic contrast medium].

In 320 cases contrast investigations of the vertebral column were carried out injecting by lumbar tap Metrizamide in 174 cases and Amipaque in 126 cases. In 259 cases the investigation was limited to the lumbar part, in 25 cases to the thoracic part, and in 19 cases to the cervical part of the spine. In 12 cases the whole vertebral canal was explored in this way. In 5 cases only epidurography was done. Very good or good contrast filling of the dural sac was obtained in the thoracolumbar segment using contrast medium 200 mg I/ml. In some cases this degree of concentration was inadequate for goo

Identification of subpopulations of rat granulosa cells: sedimentation properties and hormonal responsiveness.

Ovarian granulosa cells from small follicles have generally been considered to comprise a homogeneous cell population; however, stratified arrangements of hormone receptors have been found in antral and mural granulosa cells of Graafian follicles. Using cultured granulosa cells derived from immature, hypophysectomized, estrogen-treated rats, we have previously shown that vasoactive intestinal peptide (VIP) as well as FSH stimulates steroid production by these cells. Dose-response analysis indicated that the actions of these hormones were additive, suggesting the presence of subpopulations of g

Abdominal circumference growth velocity as a predictor of adverse perinatal outcomes in small-for-gestational-age fetuses.

OBJECTIVE: To assess the predictive value of abdominal circumference growth velocity (ACGV) between the second and third trimesters to predict adverse perinatal outcomes in a cohort of small-for-gestational-age fetuses without evidence of placental insufficiency (i.e. fetal growth restriction). MATERIAL AND METHODS: This is a single-center retrospective cohort study of all singleton pregnancies with small-for-gestational-age fetuses diagnosed and delivered at a quaternary institution.
