[Microbiology of acute exacerbation chronic sinusitis in adults].

The bacterial flora taken by sinus puncture from 115 patients with uni and bilateral acute exacerbation of chronic maxillary sinusitis was analyzed. Germ-free culture was obtained in 49 cases (40%) and in 69 cases (60%) bacteria were isolated from all sinus aspirates. Microbiological evaluation shows, that in majority of cases (90%) of acute exacerbation of chronic sinusitis there were pathogenic bacteria namely aerobic and anaerobic. The most frequent pathogens isolated from the aspirates were; Staphylococcus aureus, Haemophilus influenzae, Escherichia coli, Streptococcus pneumoniae, Staphylo

Microbiology and management of endodontic infections in children.

The first step in the origination of caries is the formation of a dental plaque. Dental caries can lead to destruction of enamel and dentin resulting in bacterial invasion of the pulp. Invasion of the pulp and the periapical areas can promote the development of dento-alveolar abscess and spread of the infection to other anatomical areas. Several oral acid producing aerobic and anaerobic bacteria, including Streptococcus mutans, Lactobacillus acidophilus, and Actinomyces viscosus, are capable of initiating the carious lesion. The organisms that predominate in pulpitis and dento-alveolar abscess

Microbiology of chronic suppurative otitis media in Singapore.

The objective of the study was to study the microflora and the antibiograms of patients with chronic suppurative otitis media (CSOM) in Singapore. Ninety patients with CSOM were prospectively studied. They had chronic ear discharge and had not received antibiotics for the previous five days. Swabs were taken, and cultured for bacteria. Antibiotic testing was done using modified Kirby Bauer disk diffusion method. In addition to the usual antibiotics, the three most common topically available antibiotics (chloramphenicol, gentamicin and neomycin) were tested. There were 135 positive cultures for

The microbiology laboratory's role in response to bioterrorism.

CONTEXT: Bioterrorism has existed since before the 14th century; however, the specter of such an attack is much greater today than ever before. Technical expertise in microbiology and molecular testing, combined with the rapidity of worldwide air travel, has ensured that no geographic area would be untouched in a widespread attack. Clinical microbiology laboratories will play a pivotal role in the detection of attacks involving weapons of mass destruction. OBJECTIVE: To identify and discuss the microorganisms most likely to be used as agents of bioterrorism. DATA SOURCES: Data were obtained

Microbiology. Turning up the heat on Histoplasma capsulatum.

Many fungal pathogens are opportunistic, that is, they infect individuals who have a compromised immune system. Histoplasma capsulatum is a common pathogenic fungus that lives happily inside the phagosomes of macrophages. As Klein explains in his Perspective, an important H. capsulatum virulence factor, CBP1, has been found, which mops up free calcium ions within the phagosome, enabling the yeast to live under calcium-poor conditions (Sebhgati et al.). Chelating calcium ions may also have the added benefit that when the phagosome fuses with the lysosome, destructive lysosomal enzymes that requ

Microbiology. Unlocking the secrets of the grim reaper.

On page 1594, researchers unveil the workings of YopJ, one of a group of nefarious proteins that Yersinia bacteria--including Y. pestis, the bacterium that is infamous for causing the bubonic plague--deploy. Early in the disease, Yersinia bacteria inject YopJ into macrophage cells of the immune system, where, the researchers have now found, YopJ acts like scissors, cleaving the macrophage proteins that would normally send an SOS to other immune cells for help. Leaving the macrophage stranded clears the way for other Yersinia proteins to destroy the cell.

Aerobic and anaerobic microbiology of infection after trauma.

Clinical and laboratory data from 1973 to 1988 were retrospectively reviewed to study the microbiology of infection following trauma. A total of 368 specimens obtained from 340 trauma patients showed bacterial growth. The traumas included lacerations (163), blunt trauma (76), penetrating trauma (65), bites (20), and open fractures (10). Anaerobic bacteria only were isolated in 119 (32%) specimens, aerobic bacteria only in 58 (16%), and mixed aerobic-anaerobic flora in 191 (52%). A total of 444 anaerobic (1.2 isolates per specimen) and 267 aerobic or facultative (0.7 per specimen) were recovere

[Bacteremia with granulocytopenia--microbiology and empiric antibiotic treatment].

This article sums up a retrospective analysis of 84 episodes of bacteraemia in acute leukaemia patients with severe neutropenia in a Norwegian teaching hospital during the period 1990-95. Gram negative bacteria represented 54% of the blood culture isolates, all of which were susceptible to aminoglycosides, and nearly all to ceftazidime and imipenem. Penicillin/aminoglycoside was used as initial therapy in 43% of the episodes. Initial empiric therapy was modified in 52% of the events. Only 15% of patients receiving the penicillin/aminoglycoside combination actually had infections with organisms

The microbiology and antibiotic treatment of peritonsillar abscesses.

Pus from 53 peritonsillar abscesses was cultured and associations between the microbiological results and clinical data were investigated with the aim of developing a clinical protocol for treatment. A positive culture grew in 85% of quinsies and of these 16% produced aerobes and 84% anaerobes. Penicillin-resistant organisms were grown from 32% of patients and all but one of these organisms (Haemophilus influenzae) was sensitive to metronidazole. There was no association between clinical presentation and cultured organism which could guide treatment, hence we recommend penicillin and metronida

[Application of polymerase chain reaction in medical microbiology].

An overview is presented of different nucleic acid amplification techniques, particularly the polymerase chain reaction (PCR). The possible applications of the PCR in medical microbiology and the different phases of the PCR are discussed. Four PCR applications, as elaborated and used in the authors' laboratory are presented: the diagnoses of Mycoplasma pneumoniae infections, herpes simplex virus encephalitis and Toxoplasma gondii encephalitis, and the detection of Mycobacterium leprae. For the first and the last of these, original internal controls to detect the presence of inhibitors were ela

Helicobacter pylori: microbiology of a 'slow' bacterial infection.

The bacterium Helicobacter pylori lives in the gastric mucus layer of humans and induces a chronic inflammatory response that can result in both peptic ulceration and gastric neoplasms. Helicobacter pylori infection can be considered as a 'slow', adaptive and autoregulating process. The mechanisms by which this slow bacterial pathogen survives and interacts with the host immune system may provide a model for other persistent mucosal pathogens.

Microbiology in the management of destructive periodontal disease.

This paper summarizes the rationale for the application of microbiology in the management of destructive periodontal diseases. The subgingival microbiota in patients with severe periodontitis is complex and contains high numbers of obligate anaerobic bacteria as well as facultative micro-organisms. It has become clear that major differences exist in the composition of the subgingival microflora. These differences are not only quantitative but also qualitative. Difference in plaque composition is the basis for the application of clinical microbiology in the management of periodontal disease. Se

Longitudinal study of the microbiology of endemic trachoma.

A longitudinal study of trachoma in 100 members of nine Tanzanian families was conducted to assess the sources of variation in the laboratory identification of trachoma and the changes that might occur over time. Multiple conjunctival swabs were collected every 3 months for 1 year and examined by direct fluorescent-antibody cytology (DFA), enzyme immunoassay, or microimmunofluorescence serology for tear antichlamydial antibodies. DFA specimens collected 5 min apart had a discordance rate of 10% and this is attributable to sampling variation. DFA specimens collected 2 days or more apart show a

Microbiology of chronic suppurative otitis media in children.

The aerobic microorganisms present in 51 middle ears of 36 children with chronic suppurative otitis media without cholesteatoma were evaluated. Specimens were taken directly from the middle ear through a patent perforation or tympanostomy tube and cultured using standard techniques. Twenty-three microbiologic species were identified, with Pseudomonas aeruginosa being the most prevalent (67% of ears) and the only organism in 31% of ears. The initial treatment of all patients was intravenous administration of an antimicrobial selected on the basis of culture and susceptibility reports; this trea

Rapid methods in microbiology for in-office testing.

There are now a variety of rapid test methods available to assist in the diagnosis of the three most common infectious diseases seen in ambulatory medicine: pharyngitis, urinary tract infection, and venereal disease. Before choosing to implement any of these methods, several aspects should be carefully considered. The objectives met by onsite testing should be clearly identified and the appropriate test chosen to meet those objectives. For example, if the goal is to establish a diagnosis while the patient is still available in the office, a test method that requires more than 20 to 30 minutes
