Morphogenesis of isotretinoin-induced microcephaly and micrognathia studied by scanning electron microscopy.

Isotretinoin ingestion during the first trimester of human pregnancy can induce malformations of the skull, ears, face, central nervous system, eyes, palate, lungs, circulatory system, limbs, and digits. A single oral dose of isotretinoin on day 8 of gestation in hamsters induces a similar syndrome of congenital malformation. The present study concerned scanning electron microscopic (SEM) observation of embryonic and fetal hamster craniofacial structures at 4, 8, 12, 24, 48, and 72 hr after administration of an oral dose of 50 mg/kg isotretinoin or an equivalent volume of the vehicle. The vari

New syndrome? Prominent, constricted ears with malformed condyle of the mandible.

We report on several relatives in 5 generations of one family with prominence of the ears, with a marked constriction at the junction between the lower and middle thirds of the pinna. Computerized tomography and radiographs in the propositus and his affected father showed abnormalities of the condyle of the mandible. The propositus had more severe changes in the condyle with microstomia and reduced range of motion of the mandible in the temporomandibular joint. There was no hearing loss or abnormalities of the bones of the middle ear.

Spontaneous pneumothorax in a patient with systemic sclerosis.

A 28-year-old woman developed spontaneously a right- sided pneumothorax, the leading clinical symptom of an as yet undiagnosed systemic sclerosis. The diagnosis was confirmed by Raynaud's phenomenon, microstomia, arthralgia, distal oesophageal dysfunction and antinuclear antibodies. Initial treatment with pleural suction was followed by thoracoscopy and segmental pulmonary resection. Spontaneous pneumothorax is a rare complication in patients with systemic sclerosis, most likely caused by the rupture of subpleural cysts.

The prosthetic management of oral commissure burns.

The complications of electrical burn to the oral cavity include microstomia and cosmetic deformity. Surgical burn excision and repair by primary closure, skin grafts, or tongue flaps have recognized disadvantages. The use of intraoral fixed or removable appliances, or the extraoral facemask prosthesis, serves to maintain adequate commissure spacing and allows normal skin and mucosal healing to occur. Secondary commissuroplasties may be required after prosthetic management of the burn.

Situs Inversus in an Infant With Hypomandibular Faciocranial Syndrome: Clinical Overlap With the Agnathia-Otocephaly Complex.

There have been three previously reported cases of hypomandibular faciocranial syndrome (HFS), which is characterized by dysgnathia (an absent or hypoplastic mandible), a protruding lower face, microstomia, normally positioned ears, and craniosynostosis. The dysgnathia and oro-pharyngo-laryngeal abnormalities in HFS are virtually identical to those found in the agnathia-otocephaly complex (AOC), of which severe forms can include holoprosencephaly (HPE), synotia-melotia, and situs abnormalities, but not craniosynostosis. We report an infant with HFS who also presented with situs inversus totali

Freeman-Sheldon syndrome: case management from age 6 to 16 years.

OBJECTIVE: This is the first description of long-term follow-up of a case of Freeman-Sheldon syndrome. Microstomia was treated with a mouth expander for 2 to 3 hours per day before active orthodontic treatment. Separate impressions were necessary in each quadrant of both upper and lower jaws because of limited mouth opening. CONCLUSION: Orthodontic treatment improved the patient's Class II malocclusion, which was accompanied by crowding and a deep bite.

Technique for making flexible impression trays for the microstomic patient.

This impression technique can be used for patients in whom routine use of stock impression trays is hindered by microstomia. Putty wash material can be manipulated with minimal effort and time. Placing the completed preliminary impression in a free-flowing mix of dental stone stabilizes the impression material and facilitates boxing and pouring of the impression. The resultant preliminary casts can then be used for diagnostic purposes and for making rigid sectional trays for final impressions.

Comprehensive Orthodontic Therapy and Retention Protocol in Freeman-Sheldon Syndrome: A Case Report.

Freeman-Sheldon syndrome (FSS) is a rare congenital disorder with craniofacial and musculoskeletal abnormalities, presenting unique orthodontic challenges due to microstomia and restricted oral access. This case report describes the successful non-surgical orthodontic management of a 15-year-old girl with severe crowding using pre-adjusted edgewise appliances. Favorable alignment and functional occlusion were achieved, and a customized retention protocol combining Essix and fixed lingual retainers ensured stability.

Expanding oral plastic splint for burn patients.

An expanding oral plastic splint for preventing microstomia from deep facial burns is described. This splint adjusts to suit the affected oral angles without the need for taking any impression before fabrication. It is especially suitable for severely injured patients starting from the early period shortly after injury, even if the patient has been intubated. Furthermore the expansion forces can be controlled easily according to the degree of contracture.

Scleroderma and dentistry: every dentist is a scleroderma specialist.

Scleroderma is an autoimmune, rheumatoid factor-positive disease that may be localized or systemic, affecting the skin, lungs, kidneys, and cardiovascular system. Dental effects include xerostomia, microstomia, idiopathic resorption of tooth and bone, oral effects of medications, erosion and decay caused by gastroesophageal reflux disease (GERD), and poor oral hygiene due to physical and emotional effects of the disease. All dentists have the knowledge and ability to treat those suffering with scleroderma.

Full Thickness, Near-Total to Total Lower Lip Reconstruction Using Superficial Circumflex Iliac Artery Perforator Free Flap: A Case Report.

Near-total to total lower lip defects present significant challenges for reconstructive surgeons, requiring restoration of oral competency, maximization of oral aperture, and cosmetic appearance. This report explores a novel reconstructive option addressing all three needs. Traditionally, local flap options restore cosmesis and oral competency by recruiting local tissue of similar thickness, pliability, and appearance, but often result in microstomia. Conversely, free flaps prevent microstomia but can lead to bulky and cosmetically unacceptable reconstructions. We present the first case of usi

Commissuroplasty Using Split Dry Lips: A Case Report.

Commissuroplasty is a procedure that is performed to correct deformities at the corner of the mouth or oral commissure. Herein, we report a case of postoperative microstomia treated with commissuroplasty using split dry lips. In a surgical procedure, the dry lip was divided into orbicularis oris muscle cutaneous flaps and transpositioned into the cleft formed. The deformation of the corners of the mouth improved, and mouth opening improved enough to wear dentures. We believe that this method enables commissuroplasty that combines aesthetics with function.

Hallermann-Streiff syndrome: airway problems during anaesthesia.

In the patients with Hallermann-Streiff Syndrome, presence of mandibular hypoplasia and microstomia results in difficult intubation. The anaesthetic management of a three month old child with this syndrome for lensectomy is presented in this report. There was difficulty in maintaining the airway patency during induction which was solved with the help of a modified, improvised airway. Intubation was done after induction with a combination of inhalational and intravenous anaesthetic agents and muscle relaxant. Recognition of this syndrome should alert the physician to the possibility of difficul

Survival up to age 10 years in a patient with partial duplication 6q: case report and review of the literature.

Partial duplication of chromosome 6q has been recognized as a distinct dysmorphic syndrome with severe psychomotor and growth retardation, typical craniofacial features including microcephaly and microstomia, neck webbing, congenital contractures, and variable internal malformations. Most patients have died in the first year of life. We describe the clinical features and disease course in a boy with a duplication of 6q23.3-qter who lived up to the age of 10 years and discuss similarities with other patients.

[Variants of plastic surgery using trapezoid adipose-cutaneous flaps in post-burn marginal deformities and contractures].

Most optimal methods of plasty with the maximum use of intact skin of a healthy leaf and adjacent area as a trapezoid cellulo-cutaneous flap are proposed. Different variants of the operation are proposed as one-, two-, three- and many-flaps plasty with local tissues and one- and two-flaps combined plasty. The choice is dependent on the localization and degree of contracture. Plasty by the methods proposed allows to adequately eliminate marginal contractures of large vessels, syndactylia, microstomia and narrowing of the lid slit.
