Exercise with induced pluripotent stem cells enhances Wnt1-Lmx1a signaling and dopaminergic neurogenesis to alleviate Parkinsonian symptoms.

This article focused on the recent contribution by Jiang et al, who demonstrated that voluntary exercise can significantly potentiate the effects of induced pluripotent stem cell transplantation in a Parkinson's disease (PD) model through activation of the Wnt1-Lmx1a signaling cascade. Jiang et al's findings highlight the role of exercise as a molecular modulator of neurogenesis and support the development of integrated strategies combining physical activity, stem cell transplantation, and biomaterials to improve outcomes in PD. We highlight exercise as a molecular modulator that fosters a neu

Extracellular Vesicles in Renal Inflammatory Diseases: Revealing Mechanisms of Extracellular Vesicle-Mediated Macrophage Regulation.

Renal inflammatory diseases are a group of severe conditions marked by significant morbidity and mortality. Extracellular vesicles (EVs), as facilitators of intercellular communication, have been recognized as pivotal regulators of renal inflammatory diseases, significantly contributing to these conditions by modulating immune responses among other mechanisms. This review highlights the intricate mechanisms through which EVs modulate macrophage-kidney cell interactions by regulating macrophages, the principal immune cells within the renal milieu. This regulation subsequently influences the pat

A multivariate model of patients' satisfaction with treatment for posttraumatic stress disorder.

Prior studies have concluded that patients' pretreatment characteristics contribute more to their satisfaction with mental health treatment than any other domain. We expand the representation of treatment characteristics in an examination of satisfaction across both inpatient and outpatient settings. Data were drawn from an inpatient (n = 831) and an outpatient (n = 554) study of the treatment of PTSD. We used structural equation modeling to specify and evaluate a model of satisfaction with comparable elements for inpatient and both short and long-term outpatient treatment. Results indicate th

Bioengineered Mesenchymal Stem/Stromal Cells in Anti-Cancer Therapy: Current Trends and Future Prospects.

Mesenchymal stem/stromal cells (MSCs) are one of the most widely used cell types in advanced therapies due to their therapeutic potential in the regulation of tissue repair and homeostasis, and immune modulation. However, their use in cancer therapy is controversial: they can inhibit cancer cell proliferation, but also potentially promote tumour growth by supporting angiogenesis, modulation of the immune milieu and increasing cancer stem cell invasiveness. This opposite behaviour highlights the need for careful and nuanced use of MSCs in cancer treatment. To optimize their anti-cancer effects,

Combinational CAR T-cell therapy for solid tumors: Requisites, rationales, and trials.

Chimeric antigen receptor (CAR) T-cell therapy has achieved potent antitumor efficacy in hematological malignancies; however, because of limitations in CAR T-cell recruitment, infiltration, activation, and functional persistence in the tumor, its efficacy in solid tumors has been suboptimal. To overcome these challenges, combinational strategies that include chemotherapy, radiation therapy, or immune checkpoint inhibitor agent therapy with CAR T-cell therapy are being investigated. The established functional characteristics of the abovementioned therapies provide a rationale for the use of a c

Oncogene inactivation-induced senescence facilitates tumor relapse.

Oncogene-directed therapies can induce profound tumor regression in oncogene-addicted cancers, but their long-term benefit is often limited by resistance and relapse. Here we show that oncogene inactivation rapidly induces senescence and a pro-inflammatory senescence-associated secretory phenotype (SASP). In vivo, oncogene inactivation-induced senescence (OIIS) predisposes tumors to relapse, accompanied by polyploidy, chromosomal instability, acquisition of alternative oncogenic pathways including mouse double minute 2 homolog (Mdm2) upregulation, and tumor microenvironmental remodeling toward

Blood Vessels and Peripheral Nerves as Key Players in Cancer Progression and Therapy Resistance.

Cancer cells continuously interact with the tumor microenvironment (TME), a heterogeneous milieu that surrounds the tumor mass and impinges on its phenotype. Among the components of the TME, blood vessels and peripheral nerves have been extensively studied in recent years for their prominent role in tumor development from tumor initiation. Cancer cells were shown to actively promote their own vascularization and innervation through the processes of angiogenesis and axonogenesis. Indeed, sprouting vessels and axons deliver several factors needed by cancer cells to survive and proliferate, inclu

Metronomic therapy for refractory/relapsed lymphoma: the PEP-C low-dose oral combination chemotherapy regimen.

BACKGROUND: Metronomic therapy is the application of continuous, low dose chemotherapy. The doses of chemotherapy are usually not sufficient to destroy neoplastic cells, but impact the milieu, particularly angiogenesis. OBJECTIVE: To determine if the oral PEP-C regimen, consisting of prednisone 20 mgm, etoposide 50 mgm, procarbazine 50 mgm, and cyclophosphamide 50 mgm given in either a daily, alternate day, or fractionated basis, is effective in a variety of lymphomas. METHODS: One hundred twenty two patients were studied although the majority had low grade or mantle cell lymphoma. All had r

Harnessing Silicene-to-Silicic Acid Conversion for Organelle-Specific Silica Deposition in Tumor Therapy.

Inducing localized mineralized lesions offers a promising drug-free strategy for tumor suppression, yet calcium-phosphate systems are limited by slow crystallization, high ion requirements, and poor organelle specificity. Here, we develop a silicene-derived nanoplatform that serves as an "inorganic silicic acid reservoir", enabling controlled, organelle-specific biosilicification for cancer therapy. Silicene nanosheets are sequentially engineered with tannic acid and PEI-anchored triphenylphosphonium (TPTS), conferring high colloidal stability, efficient endosomal escape, and selective mitocho

Chimeric Antigen Receptor T Cell Therapy for Hepatocellular Carcinoma: Where Do We Stand?

Hepatocellular carcinoma (HCC) remains a global health challenge that urgently calls for innovative therapeutic strategies. Chimeric antigen receptor T cell (CAR T) therapy has emerged as a promising avenue for HCC treatment. However, the therapeutic efficacy of CAR T immunotherapy in HCC patients is significantly compromised by some major issues including the immunosuppressive environment within the tumor, antigen heterogeneity, CAR T cell exhaustion, and the advanced risk for on-target/off-tumor toxicity. To overcome these challenges, many ongoing preclinical and clinical trials are underway

Chronic lymphocytic leukemia (CLL) with Reed-Sternberg-like cells vs Classic Hodgkin lymphoma transformation of CLL: does this distinction matter?

The distinction between chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) with isolated Hodgkin/Reed-Sternberg cells (CLL-HRS; background milieu with a paucity of inflammatory cells) and overt transformation to classic Hodgkin lymphoma (CLL-HL; mixed inflammatory background) is incompletely understood. This retrospective study examined the clinicopathologic features of CLL-HRS (n = 15) and CLL-HL (n = 31) patients seen over the past three decades from a single institution. The phenotypic features of Reed-Sternberg cells in both groups were similar, including expression of CD30,

Endoscopic GERD therapy: a primer for the transoral incisionless fundoplication procedure.

Patients with medically refractory GERD have the option of surgery but may opt for effective minimally invasive interventions, when available. However, the primary GERD pharmacologic therapy, proton pump inhibitors, does not satisfactorily address the pathophysiology of the disease. Moreover, a therapeutic gap exists in those severely symptomatic patients who fail medical management and who are poor candidates for surgical fundoplication. Recently, a revival of minimally invasive endoscopic interventions aiming to correct the antireflux barrier has followed existing device modifications, enhan

Posttranslational remodeling micelle reverses cell-surface and exosomal PD-L1 immunosuppression in tumors resistant to PD-L1 antibody therapy.

Sustained replenishment of cell-surface and exosomal PD-L1 has been believed as the most important factor in the progression of PD-L1 antibody therapy resistance. Given that direct genetic blockade of PD-L1 may lead to unintended consequences and that posttranslational modifications (PTMs) are often used as pharmacological targets of cancer therapy, approaches targeting PD-L1 PTMs show great potential as new therapeutic paradigms. Palmitoylation has emerged as a critical PTM for modulating PD-L1 stability and distribution. In this work, it is found that disruption of palmitoylation by 2-bromop

Early Anti-Inflammatory Effect of Bupivacaine-Meloxicam Therapy in a Rabbit Model of Arthrofibrosis.

Arthrofibrosis is a common complication following total knee arthroplasty and is the result of a dysregulated immuno-inflammatory cascade, which culminates in excessive scar deposition by myofibroblasts within the knee joint. Pharmacotherapies for arthrofibrosis are limited, with treatment reliant on manipulation under anesthesia or lysis of adhesions with or without component revision. Bupivacaine (a local anesthetic) and meloxicam (a nonsteroidal anti-inflammatory drug) combination therapy may exert antifibrotic properties by limiting inflammatory cell recruitment and the subsequent release

Liposomal prednisolone phosphate potentiates the antitumor activity of liposomal 5-fluorouracil in C26 murine colon carcinoma in vivo.

The antitumor efficacy of 5-fluorouracil (5-FU) in advanced colorectal cancer (CRC) is hindered not only by the low therapeutic index, but also by tumor cell resistance to this cytotoxic drug. Therefore, to enhance the 5-FU antitumor activity, the present research used a novel tumor-targeted therapy based on the co-administration of 5-FU encapsulated in long-circulating liposomes (LCL-5-FU) together with liposomal prednisolone phosphate (LCL-PLP), a formulation with known anti-angiogenic actions on C26 murine colon carcinoma cells. Thus, we assessed the in vivo effects of the combined liposoma

Has the use of antithyroid drugs for Graves' disease become obsolete?

In spite of an experience of almost 50 years of use of antithyroid drugs and radioiodine for the treatment of Graves' disease, the rationale for choice is often obscure. Early reports of high remission rates during thiourea therapy were followed by less optimistic ones, which along with other factors may have fueled the current major shift toward use of radioiodine. This review examines whether or not the use of antithyroid drugs indeed may have become obsolete. The intrathyroidal and extrathyroidal mechanisms of action of the drugs are reviewed with emphasis on their potential immunosuppressi

Emerging roles of ferroptosis in modulating the immune landscape of glial tumours.

The brain offers a unique environment for cancer, with limited access to nutrients and highly regulated immune surveillance. Here we explore the role of ferroptosis, a form of metabolically regulated cell death driven by iron-mediated lipid peroxidation, in shaping immune-cell composition and function in glial tumours. We review the complex metabolic crosstalk between cell populations regulating ferroptosis in the glioma milieu. Ferroptosis induces polarization of resident microglia and controls the cytotoxic roles of CD8+ T cells and the immunosuppressive effects of regulatory T cells. We dis

Lipids: a Potential Molecular Pathway Towards Diastolic Dysfunction in Youth-Onset Type 2 Diabetes.

PURPOSE OF THE REVIEW: Obesity and type 2 diabetes (T2D) with onset in youth are emerging public health concerns. Youth with obesity and T2D are at risk for the development of heart failure with preserved ejection fraction (HFpEF) due to diabetes-related cardiomyopathy with evidence of precursor stages, namely diastolic dysfunction, present in youth. We review the literature regarding diastolic dysfunction in youth with obesity and T2D; discuss the potential mechanisms including the role of lipids, contractile proteins and their post-translational modifications, and conclude with studies to gu

Role of AMPK in Diabetic Cardiovascular Complications: An Overview.

Macrovascular complications of diabetes like cardiovascular diseases appear to be one of the leading causes of mortality. Current therapies aimed at counteracting the adverse effects of diabetes on cardiovascular system are found to be inadequate. Hence, there is a growing need in search of novel targets. Adenosine Monophosphate Activated Protein Kinase (AMPK) is one such promising target, as a plethora of evidences pointing to its cardioprotective role in pathological milieu like cardiac hypertrophy, atherosclerosis and heart failure. AMPK is a serine-threonine kinase, which gets activated in

Bidirectional Relationship Between Cancer and Heart Failure: Old and New Issues in Cardio-oncology.

The main focus of cardio-oncology has been the prevention and treatment of the cardiac toxicity of chemotherapy and radiotherapy. Furthermore, several targeted therapies have been associated with unexpected cardiotoxic side-effects. Recently, epidemiological studies reported a higher incidence of cancer in patients with heart failure (HF) compared with individuals without HF. On this basis, it has been proposed that HF might represent an oncogenic condition. This hypothesis is supported by preclinical studies demonstrating that hyperactivation of the sympathetic nervous system and renin-angiot
