The Interaction of Genomics, Molecular Imaging, and Therapy in Gastrointestinal Tumors.

In the past 20 years there have been great advances in our understanding of the molecular genetics of gastrointestinal tumors. In general, cancers develop and proliferate due to: driver mutations or related alterations in the genome resulting in overactivity of oncogenes, underactivity of tumor suppressor genes allowing tumors to grow, and impaired DNA repair mechanisms either from sequential mutations with or without germline mutations predisposing patients to cancer. Infections such as hepatitis B and C, and Human Papilloma Virus (HPV) can lead to hepatocellular cancers and anal cancers, res

Molecular imaging in the diagnosis of Alzheimer's disease and related disorders.

INTRODUCTION: The diagnosis of Alzheimer's disease (AD) and its related disorders rely on clinical criteria. There is, however, a large clinical overlap between the different neurodegenerative diseases affecting cognition and, frequently, there are diagnostic uncertainties with atypical clinical presentations. Current clinical practices can now regularly use positron emission tomography (PET) and single-photon emission computed tomography (SPECT) molecular imaging to help resolve such uncertainties. Image-Guided Biopsy for the Diagnosis and Molecular Profiling of Hepatoblastoma.

INTRODUCTION: Studies on the use of image-guided percutaneous biopsy for hepatoblastoma (HB), and recommendations put forth by the pediatric hepatic international tumor trial (PHITT), are limited. It is unknown if sufficient tissue can be obtained for trial enrollment as well as molecular profiling, which will likely play a key role in informing future treatment strategies. METHODS AND MATERIALS: Patients with HB who underwent percutaneous biopsy at initial diagnosis in interventional radiology (IR) over a 12-year period at a single center were included. Patient demographics, pretreatment ext

Approach to the Patient With Primary Aldosteronism: Role of Molecular Imaging.

A common yet underdiagnosed cause of secondary hypertension, primary aldosteronism (PA) is characterized by excess aldosterone production, causing hypertension with increased risk of cardio-renal-metabolic complications. Accurate and timely localization of the source of aldosterone excess is crucial for management, in the form of curative adrenalectomy for unilateral aldosterone-producing adenoma or medical management for bilateral adrenal hyperplasia. The current diagnostic algorithm involves adrenal vein sampling (AVS) as the current "gold standard" in determining lateralization of aldostero

Chemical structure-aware molecular image representation learning.

Current methods of molecular image-based drug discovery face two major challenges: (1) work effectively in absence of labels, and (2) capture chemical structure from implicitly encoded images. Given that chemical structures are explicitly encoded by molecular graphs (such as nitrogen, benzene rings and double bonds), we leverage self-supervised contrastive learning to transfer chemical knowledge from graphs to images. Specifically, we propose a novel Contrastive Graph-Image Pre-training (CGIP) framework for molecular representation learning, which learns explicit information in graphs and impl

A role for artificial intelligence in molecular imaging of infection and inflammation.

The detection of occult infections and low-grade inflammation in clinical practice remains challenging and much depending on readers' expertise. Although molecular imaging, like [18F]FDG PET or radiolabeled leukocyte scintigraphy, offers quantitative and reproducible whole body data on inflammatory responses its interpretation is limited to visual analysis. This often leads to delayed diagnosis and treatment, as well as untapped areas of potential application. Artificial intelligence (AI) offers innovative approaches to mine the wealth of imaging data and has led to disruptive breakthroughs in

Targeting of endothelin receptors for molecular imaging of atherosclerosis in rabbits.

UNLABELLED: We wanted to determine whether a previously described in vivo accumulation of a (99m)Tc-labeled endothelin derivative in atherosclerotic lesions is mediated by binding to endothelin receptors. Furthermore, the expression of endothelin receptors in atherosclerotic lesions of 2 different rabbit animal models for atherosclerosis was to be evaluated to determine whether endothelin receptors generally are a suitable target for atherosclerosis imaging. METHODS: Normal vessels from untreated New Zealand White rabbits (NZW), balloon-denuded aortas from cholesterol-fed NZW, and atheroscler

Navigating iNOS-targeted molecular imaging for inflammation: from comparative analysis of the current state of molecular imaging and translational challenges to future strategies and prospects.

Inducible nitric oxide synthase (iNOS) plays a pivotal role in mediating inflammatory responses through the production of excessive nitric oxide (NO). Aberrant iNOS expression is closely associated with various pathological states, establishing it as a crucial biomarker and therapeutic target for inflammation-related disorders. In this study, we described the mechanisms of iNOS in inflammation, strategies for targeting iNOS, and approaches for improving iNOS tracers. Furthermore, in view of the immense diagnostic role of iNOS, we offered an outline of the various imaging technologies, includin

High-plex imaging of RNA and proteins at subcellular resolution in fixed tissue by spatial molecular imaging.

Resolving the spatial distribution of RNA and protein in tissues at subcellular resolution is a challenge in the field of spatial biology. We describe spatial molecular imaging, a system that measures RNAs and proteins in intact biological samples at subcellular resolution by performing multiple cycles of nucleic acid hybridization of fluorescent molecular barcodes. We demonstrate that spatial molecular imaging has high sensitivity (one or two copies per cell) and very low error rate (0.0092 false calls per cell) and background (~0.04 counts per cell). The imaging system generates three-dimens

Nectin4-targeted molecular imaging for precise endoscopic diagnosis and image-guided surgery in bladder cancer.

Bladder cancer (BC) is a common malignancy, yet conventional white light cystoscopy (WLC) has suboptimal diagnostic accuracy. Nectin4, a membrane protein highly expressed in BC, is an attractive molecular imaging target. By conjugating indocyanine green (ICG) to enfortumab vedotin, we develop NECTIN4-ICG, a BC-specific fluorescence imaging probe. In BC mouse models, NECTIN4-ICG enables targeted detection and doubles the complete tumor resection rate. In an ex vivo study of 18 fresh human bladders yielding 82 specimens, NECTIN4-ICG-guided biopsy achieves 96.3% diagnostic accuracy. In a clinical

Intraoperative Molecular Imaging in Thoracic Oncology: Expanding the Observable Disease Space.

BACKGROUND/OBJECTIVES: Intraoperative molecular imaging (IMI) enables real-time visualization of tumor biology during surgery using fluorescent probes and near-infrared imaging systems. As lung cancer screening increases detection of small and nonpalpable pulmonary nodules, conventional localization and margin assessment techniques remain limited, particularly during minimally invasive surgery. This review summarizes the technical foundations, imaging agents, clinical applications, and future directions of IMI in thoracic oncology. METHODS: We performed a narrative review to synthesize curren

Organic Activatable Afterglow Imaging Probes: From Molecular Design to Biomedical Translation.

Organic activatable afterglow probes have developed into a distinct class of optical imaging tools. They allow for a temporal separation between signal emission and external irradiation, thereby enabling high-contrast imaging. We have summarized the latest advances in the design of organic activated afterglow probes, with a particular focus on their excitation mechanisms, performance modulation, and activation strategies, including energy transfer, intramolecular regulation, and singlet oxygen(1O2)-mediated processes. The roles of various external energy sources, including light, ultrasound, a

Differential Optical Imaging of Antigen Presentation Machinery Using Molecular Optical Reporters.

Detection of antigen presentation is central to understanding immunological processes and developing therapeutics for cancer, infectious diseases, and allergies. However, methods with the ability to dynamically and noninvasively distinguish between major histocompatibility complex class I (MHC-I) and MHC-II antigen presentations remain lacking. Herein, we develop activatable molecular optical reporters (MORs) for real-time differential imaging of antigen presentations in lymph nodes (LNs). These MORs are engineered to passively target LNs and activated through proteolytic cleavage by key enzym

Fluorescence saturation imaging microscopy: molecular fingerprinting with a standard confocal microscope.

Molecular specificity in fluorescence imaging of cells and tissues can be increased by measuring parameters other than intensity. For instance, fluorescence lifetime imaging became a widespread modality for biomedical optics. Previously, we suggested using the fluorescence saturation effect at pulsed laser excitation to map the absorption cross-section as an additional molecular contrast in two-photon microscopy [Opt. Lett.47(17), 4455 (2022).10.1364/OL.465605]. Here, it is shown that, somewhat counterintuitive, fluorescence saturation can be observed under cw excitation in a standard confocal

Molecular Imaging Reveals Antineuroinflammatory Effects of HDAC6 Inhibition in Stroke Models.

Ischemic stroke is a devastating disease that causes neuronal death, neuroinflammation, and other cerebral damage. However, effective therapeutic strategies for ischemic stroke are still lacking. Histone deacetylase 6 (HDAC6) has been implicated in the pathogenesis of ischemic stroke, and the pharmacological inhibition of HDAC6 has shown promising neuroprotective effects. In this study, we utilized positron emission tomography (PET) imaging with the HDAC6-specific radioligand [18F]PB118 to investigate the dynamic changes of HDAC6 expression in the brain after ischemic injury. The results revea

Molecular Imaging of Neuroinflammation in Alzheimer's Disease and Mild Cognitive Impairment.

Alzheimer's disease (AD) is the most prevalent neurocognitive disorder. Due to the ineffectiveness of treatments targeting the amyloid cascade, molecular biomarkers for neuroinflammation are attracting attention with increasing knowledge about the role of neuroinflammation in the pathogenesis of AD. This chapter will explore the results of studies using molecular imaging for diagnosing AD and mild cognitive impairment (MCI). Because it is critical to interpreting the data to understand which substances are targeted in molecular imaging, this chapter will discuss the two most significant target
