The efficacy of somatosensory evoked potentials (SSEPs) to detect acute peripheral nerve injury during external-fixator application in the lower extremities was evaluated in 40 children with 42 Ilizarov surgical procedures. The study included patients who were either clinically normal or who had preexisting neuropathy but consistent and reliable SSEP responses preoperatively. SSEPs were recorded from the popliteal fossa and lumbar regions after alternating stimulation of the peroneal and posterior tibial nerves at the ankle. SSEP changes due to anesthesia, Ilizarov apparatus application, and o

BACKGROUND: Intraoperative cranial nerve monitoring has been applied for the preservation of nerve function during skull base surgery. However, its usefulness is controversial because clinical results reported are limited.
METHODS: We evaluated the usefulness of intraoperative electrophysiologic monitoring of the third to seventh cranial motor nerves in 15 patients undergoing skull base surgery. Intracranial bipolar electrical stimulation of each nerve was performed, and compound muscle action potentials (CMAP) were recorded from the innervated muscles.
RESULTS: CMAPs were successfully recor

Continuous intraoperative somatosensory-evoked potential monitoring during scoliosis surgery, along with improved instrumentation techniques, has contributed to the reduction of neurologic injury from 4-6.9% to 0-0.7%. To assess whether somatosensory-evoked potential monitoring might play a similar role in cervical surgery, the authors compared the morbidity and mortality rates associated with 218 patients who were not monitored and were operated on between 1985-1989 with those found in 100 consecutive somatosensory-evoked potential monitored procedures done from 1989-1991. The cervical proced

The likelihood of successful preservation of facial and cochlear nerve function during acoustic neuroma surgery has been improved by the advent of intraoperative monitoring techniques. The facial nerve is monitored by recording EMG from facial muscles, with no muscle relaxants used; mechanical irritation of the nerve during surgery causes increased EMG activity, which can be detected in real time using a loudspeaker. Brief episodes of activity associated with specific surgical maneuvers aid the surgeon in avoiding damage to the nerve, whereas prolonged tonic EMG activity may reflect significan

While identification of the intratemporal portion of the facial nerve is mandatory in most otologic surgical procedures, inadvertent instrumentation, traction, or thermal injury may still result from inaccurate delineation, purposeful avoidance, or false protection of this critical structure. Improved functional preservation of the facial nerve has been achieved in acoustic neuroma surgery through the monitoring of evoked facial electromyographic activity. This technique may also be used during otologic procedures in which facial nerve manipulation is anticipated in the management of recurrent

Injury to the facial nerve is of concern in surgery of cerebellopontine angle tumors. The crossed acoustic reflex provides a way to monitor the ipsilateral facial nerve with the auditory stimuli delivered to the contralateral side. Using the method of optimum digital filtering, it is possible to monitor the resulting brain stem facial evoked response (BFER) in real time. This paper presents preliminary experiences in more than 18 such operations monitored using this method. This preliminary study demonstrates a trend for a high (88.8%) correlation between BFER and postoperative facial nerve fu

OBJECTIVES: Recurrent laryngeal nerve (RLN) injury is a well-known complication of parathyroid surgery. Despite ample data, there is still uncertainty about the role of intraoperative monitoring (IONM) in mitigating RLN damage.
STUDY DESIGN: A retrospective review.
METHODS: We included all patients presenting for total, subtotal, or completion parathyroidectomy at a tertiary referral hospital from 2013 to 2018. Information about demographics, previous neck surgery, perioperative data, pathology, and possible RLN injury was collected. Two groups were formed for analysis: IONM vs. nonmonitored

BACKGROUND: There are reports of spinal cord injury (SCI) occurring after lower extremity (LE) surgery in children with mucopolysaccharidoses (MPS). Intraoperative neurological monitoring (IONM) has been adopted in some centers to assess real-time spinal cord function during these procedures. The aim of this investigation was to review 3 specialty centers' experiences with MPS patients undergoing LE surgery. We report how IONM affected care and the details of spinal cord injuries in these patients.
METHODS: All pediatric MPS patients who underwent LE surgery between 2001 and 2018 were reviewe

OBJECTIVE: In this study, we present the first 12 cases of the use of intraoperative neurophysiological monitoring (IONM) during therapeutic epiduroscopy in patients with clinical canal stenosis.
METHODS: IESS was performed using two working instruments: an epidural balloon to dilate the epidural space without damaging the nerve structures (Resaloon®) and an element to perform flavotomy of the ligamentum flavum (Resaflex®). The procedure was performed at levels of the greatest stenosis, as detected using preoperative magnetic resonance imaging.
RESULTS: Of the 12 cases that used IONM, 2 pati

AIMS: The use of intraoperative PTH monitoring (IOPTH) in combination with preoperative imaging has been useful to surgeons performing minimally invasive parathyroidectomy principally for adequacy of excision. However, its role within patients with equivocal imaging remains less clear particularly regarding the reduction of bilateral neck explorations. This study investigated the influence of IOPTH monitoring on the type of surgical approach adopted for patients with primary hyperparathyroidism (PHPT). Specifically, determining its impact amongst patients with equivocal imaging results.

OBJECTIVES: During surgeries that put the visual pathway at risk of injury, continuous monitoring of the visual function is desirable. However, the intraoperative monitoring of the visual evoked potential (VEP) is not yet widely used. We evaluate here the clinical utility of intraoperative VEP monitoring.
METHODS: We analyzed retrospectively 46 consecutive surgeries in 2011-2013. High luminance stimulating devices delivered flash stimuli on the closed eyelid during intravenous anesthesia. We monitored VEP features N75 and P100 and took patients' preoperative and postoperative visual function

OBJECTIVE: To assess the efficacy of intraoperative parathyroid hormone (PTH) monitoring in evaluating the outcome of parathyroidectomy in pediatric patients.
METHODS: Intraoperative PTH monitoring during parathyroidectomy was performed in five children (3M, 2F); three had parathyroid adenomas (single gland disease) and two had primary hyperplasia. One patient had undergone two previous surgical interventions to remove the parathyroid glands, but the PTH levels had remained high with persistence of symptoms. Immunoradiometric analysis was used for PTH measurements. Preoperative PTH values wer

The objective of this study was to improve the ability to detect cerebrovascular complications in patients undergoing complicated neurosurgical procedures using on-line monitoring of cerebral pH with in vivo microdialysis. We employed on-line pH monitoring in patients with a variety of neurosurgical procedures including high-flow bypass surgery, aneurysm clipping, and temporal resection in epilepsy treatment. The pH was monitored with a microdialysis probe, usually inserted into the frontal cortex and pH of the dialysate was measured on-line with a pH electrode. We monitored 17 cases: 12 high-

Of 135 carotid artery reconstructions performed under general anesthesia in 127 patients (mean age 68 years), 119 were performed with continuous intraoperative Doppler recording of the middle cerebral artery. This investigation was impossible in nine (6.7%) cases because of the absence of a visible temporal window, and results were deemed uninterpretable in six (4.5%) additional cases. The goals of this study were to test the feasibility and reliability of transcranial Doppler monitoring in the evaluation of intracranial perfusion and to determine the risk of cerebral ischemia during carotid a

Although bispectral index (BIS) monitoring provides an objective measure of hypnotic depth, its routine use to prevent awareness remains debated due to conflicting evidence and cost considerations. Therefore, it is of interest to evaluate BIS-guided versus clinically guided sevoflurane anaesthesia in 200 adults undergoing elective surgery over 60 minutes. No definite intraoperative awareness occurred in either group, confirmed by modified Brice interviews up to 30 days postoperatively. BIS monitoring reduced sevoflurane consumption by 24% (1.7 ± 0.4 vs 2.2 ± 0.5 MAC-hours; p < 0.001) and accel

Transcervical inflatable mediastinoscopic esophagectomy (TIME), with fewer incisions and without going through the thoracic cavity, may achieve the same results and lymph node dissection compared with Thoracoscopic assisted minimally invasive esophagectomy (TAMIE). However, its small visual field increases recurrent laryngeal nerve (RLN) injury risk. Intraoperative nerve monitoring (IONM) reduces RLN paralysis in TAMIE, but few reports have used it in TIME. This paper introduces continuous IONM in TIME to protect RLN. A patient underwent TIME with continuous left RLN monitoring using an EMG en

BACKGROUND: Lymphadenectomy around the recurrent laryngeal nerve (RLN) is an essential part of curative surgery for esophageal cancer. Although several single-center studies have shown that intraoperative nerve monitoring (IONM) can avoid RLN palsy, this has not been confirmed in a large-scale multicenter study. This study used a national database to evaluate whether IONM can reduce postoperative RLN palsy during minimally invasive esophagectomy (MIE) for esophageal cancer.
METHODS: We retrieved data of patients with esophageal cancer who underwent 3-field thoracoscopic or robotic MIE with ce
