Kinetic therapy in multiple trauma patients with severe thoracic trauma: a treatment option to reduce ventilator time and improve outcome.

PURPOSE: Continuous lateral rotational therapy (CLRT) has been described as a promising approach for prophylaxis and treatment of respiratory complications in critically ill patients over two decades ago. However, meta-analyses failed to demonstrate any significant benefit on outcome by CLRT, possibly due to the heterogeneity and low overall quality of available studies. METHODS: Observational trial over a 3-year period on outcome in trauma patients (Injury Severity Score, ISS ≥16) with severe thoracic injury (Abbreviated Injury Scale, AISThorax ≥3) initially treated with CLRT as standard of

Successful Acute-Phase Rehabilitation Leading to Activities of Daily Living Recovery in a Patient With Hypoxic Encephalopathy Following Prolonged Cardiopulmonary Arrest: A Case Report.

Hypoxic encephalopathy due to prolonged cardiopulmonary arrest is associated with poor neurological prognosis, often leading to significant impairment in activities of daily living (ADL) and social reintegration. Rehabilitation for patients experiencing cardiac arrest lasting over 15 minutes is rarely successful. This case report describes a 29-year-old male patient with hypoxic encephalopathy following a 19-minute cardiopulmonary arrest while in a sauna, who achieved substantial ADL recovery through intensive acute-phase rehabilitation. Cardiopulmonary resuscitation (CPR) was initiated 13 min

Nerve graft versus nerve transfer for neonatal brachial plexus: shoulder outcomes.

OBJECTIVE: The decision-making in neonatal brachial plexus palsy (NBPP) treatment continues to have many areas in need of clarification. Graft repair was the gold standard until the introduction of nerve transfer strategies. Currently, there is conflicting evidence regarding outcomes in patients with nerve grafts versus nerve transfers in relation to shoulder function. The objective of this study was to further define the outcomes for reconstruction strategies in NBPP with a specific focus on the shoulder. METHODS: A cohort of patients with NBPP and surgical repairs from a single center were

Technologies to investigate the mechanisms of force-based manipulations: a narrative methods review.

Force-based manipulations (FBM), as defined by the National Center for Complementary and Integrative Health (NCCIH), involve the passive application of mechanical force to the body with therapeutic intent. These approaches are commonly used in pain management, rehabilitation, wellness, and disease prevention, and include spinal and extremity manipulation, mobilization, soft tissue manipulation, massage, myofascial release, osteopathic manipulative treatment, and related manual or instrument-assisted therapies. FBM may involve light touch, pressure, mobilization, thrust, adjustment, needling, o

Nonoperative treatment of acute knee ligament injuries. A review with special reference to indications and methods.

Nonoperative treatment has received little attention in the numerous scientific reports on knee ligament injuries. Great controversy still exists concerning the proper treatment of a knee with a ruptured ligament, especially the anterior cruciate ligament. However, according to the studies of the authors and an extensive review of the literature the indications for conservative management can be established to be all grade I and II sprains (partial tears) of knee ligaments as well as an isolated grade III sprain (complete tear) of the posterior cruciate ligament. In addition, an isolated compl

Responsiveness of a patient in a persistent vegetative state after a coma to weekly injections of autologous activated immune cells: a case report.

INTRODUCTION: An 82-year-old Caucasian woman had remained in a persistent vegetative state after a coma of seven months duration, which occurred after a stroke with hemiplegia, nine years previously. The persistent vegetative state could be reversed in part by weekly injections with activated immune cells. After therapy, our patient responded to commands in addition to regaining spontaneous movements of both arms and the ability to swallow. This is the first report on the treatment with activated immune cells of a patient in a persistent vegetative state after a coma. CASE PRESENTATION: An 82

Passive exercise using whole-body periodic acceleration enhances blood supply to ischemic hindlimb.

OBJECTIVE: Whole-body periodic acceleration (WBPA) has been developed as a passive exercise technique to improve endothelial function by increasing shear stress through repetitive movements in spinal axis direction. We investigated the effects of WBPA on blood flow recovery in a mouse model of hindlimb ischemia and in patients with peripheral arterial disease. METHODS AND RESULTS: After unilateral femoral artery excision, mice were assigned to either the WBPA (n=15) or the control (n=13) group. WBPA was applied at 150 cpm for 45 minutes under anesthesia once a day. WBPA significantly increase

[Arthroscopic stabilization of a primary traumatic dislocation of the glenohumeral joint].

PURPOSE OF THE STUDY: The authors present their experience with arthroscopic stabilization of a primary traumatic dislocation of the glenohumeral joint in young patients. MATERIAL: In up to 80% of patients younger than 25 years, traumatic dislocation of the glenohumeral joint is associated with its recurrence within one year of treatment even in well performed conservative therapy. Repeated dislocations gradually damage the joint and eventually result in the development of arthritis. The articular capsule becomes loose, glenoid surface is reduced and cartilage of the humeral head is affected.

Combined MPFL Reconstruction with Tibial Tubercle Osteotomy and Repair of Patellar Cartilage Defect with Particulated Juvenile Articular Cartilage.

UNLABELLED: Medial patellofemoral ligament (MPFL) reconstruction with tibial tubercle osteotomy (TTO) and particulated juvenile articular cartilage (PJAC) grafting can be performed in combination for the treatment of recurrent patellar instability with associated patellar cartilaginous defects. DESCRIPTION: Preoperative planning is an essential component for this procedure. Measurement of the tibial tubercle to trochlear groove (TT-TG) distance and the Caton-Deschamps index (CDI) allows for determination of the degree of medial and anterior translation and helps to identify whether distalizat

Hyperselective Neurectomy for Spastic Flexion-Pronation Deformity of the Forearm and Wrist Using Local Anesthesia without a Tourniquet.

BACKGROUND: Spasticity in the upper limb following stroke or traumatic brain injury substantially impairs function and quality of life by affecting the flexor and adductor muscles. Spasticity commonly presents with deformities such as shoulder adduction and internal rotation, elbow flexion, forearm pronation, wrist flexion, finger flexion, and thumb adduction-commonly referred to as "thumb-in-palm" deformity. As the spasticity is incurable, surgical interventions aim to improve function and aesthetics, and facilitate daily care. Procedures such as tendon transfers and fractional lengthening ca

Outcomes of Conservative Versus Surgical Treatment of Dropped Head Syndrome in a Single Institution: A Case Series and Review of the Literature.

Dropped head syndrome (DHS), also known as camptocephalia, is characterized by a mobile chin-on-chest deformity from hypotonia of the cervical extensor muscle or hypertonia of the anterior neck muscles. There remains a paucity of quality published literature on this topic, particularly its management. The objective of this study is to identify radiographic and clinical outcomes of surgery as compared to nonsurgical treatment, to summarize the literature, and to create a decision-making paradigm for managing patients with DHS. As such, we report outcomes of our retrospective chart review series
