Risk of herpes zoster in the Japanese population with immunocompromising and chronic disease conditions: Results from a claims database cohort study, from 2005 to 2014.

Older adults, women and patients with immunocompromised (IC) or chronic medical conditions have a higher incidence of herpes zoster (HZ) and are at higher risk of developing HZ-associated complications such as postherpetic neuralgia. The incidence rates of HZ in various IC and chronic conditions have been previously reported in a retrospective cohort study using claims data from Japanese adults. Here, we report further analyses from this cohort using univariate and multivariable Cox regression to estimate crude and adjusted hazard ratios (HRs) associated with different IC and chronic condition

Chronic Cellulitis in Elephantiasis: A Rare Debilitating Phenomenon.

Chronic edema, which has multiple etiologies, is predicted to be a significant underlying cause of lymphedema, potentially leading to serious complications. Elephantiasis, characterized by massive swelling of any body part, is a rare but debilitating condition often associated with lymphatic obstruction or anomalies in the lymphatic system. Lymphedema can predispose a patient to cellulitis, an infectious condition with multiple risk factors. This case study presents a 45-year-old male with a history of chronic lymphatic obstruction due to elephantiasis and recurrent cellulitis in his lower lim

Oral bisphosphonate compliance and persistence: a matter of choice?

Compliance to oral bisphosphonates is suboptimal, with negative consequences of increased healthcare utilization and less effective fracture risk reduction. Extending dose interval increased adherence only moderately. We used literature derived from multiple chronic conditions to examine the problem of noncompliance with osteoporosis medication. We reviewed the literature on adherence to osteoporosis medication as well as that across multiple chronic conditions to understand what is known about the cause of the poor adherence. Poor compliance to oral medications is due mostly, not to forgetful

Parkinsonism in a Patient with Human T-lymphotropic Virus 1 Myelopathy.

Human T-lymphotropic virus 1 (HTLV-1) infection is commonly associated with neurological conditions like chronic progressive myelopathy and tropical spastic paraparesis (TSP) but rarely also reported with polymyositis, multiple sclerosis, and parkinsonism. It is important to recognize that HTLV-1 infection increases the risk of these neurological conditions. We present a case of 71-year-old female with HTLV-1 associated chronic progressive myelopathy with parkinsonism which signifies that it is under-recognized and not frequently reported due to lack of expert neurological assessment in these

Leukemia, lymphoma, and multiple myeloma following selected medical conditions.

The role of selected prior medical conditions in the etiology of hematopoietic malignancies was examined in a case-control study of members of two regional branches of the Kaiser Permanente Medical Care Program (USA). Past history of chronic infectious, autoimmune, allergic, and musculoskeletal disorders was abstracted from medical records for leukemia (n = 299), non-Hodgkin's lymphoma (NHL, n = 100), and multiple myeloma (n = 175) cases and matched controls (n = 787). Little difference was found between cases and controls for most of the chronic conditions evaluated, including sinusitis, carb

Residential movement patterns of families of young children with chronic conditions in Ontario, Canada: a population-based cohort study.

INTRODUCTION: Care giving for children with chronic diseases can lead to financial strain and compromised family well being. Little is known about whether these stresses lead to changes in residential movement patterns as they relate to income adequacy and proximity to care. METHODS: We compared the residential movement patterns and associated changes in neighbourhood income of children with mild to severe chronic diseases compared with those that are healthy. A cohort of infants born from 2002-2007 in Ontario, Canada was followed for 5 years and divided into those with single- or multiple- b

Chronic infections can generate SARS-CoV-2-like bursts of viral evolution without epistasis.

Multiple SARS-CoV-2 variants have arisen during the first years of the pandemic, often bearing many new mutations. Several explanations have been offered for the surprisingly sudden emergence of multiple mutations that enhance viral fitness, including cryptic transmission, spillover from animal reservoirs, epistasis between mutations, and chronic infections. Here, we simulated pathogen evolution combining within-host replication and between-host transmission. We found that, under certain conditions, chronic infections can lead to SARS-CoV-2-like bursts of mutations even without epistasis. Chro

Targeted Molecular Therapies in Allergy and Rhinology.

Biologic agents, monoclonal antibodies that target highly-specific molecular pathways of inflammation, are becoming integrated into care pathways for multiple disorders that are relevant in otolaryngology and allergy. These conditions share common inflammatory mechanisms of so-called Type 2 inflammation with dysregulation of immunoglobulin E production and eosinophil and mast cell degranulation leading to tissue damage. Biologic agents are now available for the treatment of chronic rhinosinusitis with nasal polyps (CRSwNP), asthma, eosinophilic granulomatosis with polyangiitis (EGPA), atopic d

Beyond comorbidity: expanding the definition and measurement of complexity among older adults using administrative claims data.

BACKGROUND: Studies of patients with multiple chronic conditions using claims data are often missing important determinants of treatments and outcomes, such as function status and disease severity. We sought to identify and evaluate a class of function-related indicators (FRIs) from administrative claims data. POPULATION: The study cohort comprised US Medicare beneficiaries aged 65 years or older with Parts A and B fee-for-service and Part D coverage, with a hospitalization for acute myocardial infarction during 2007. METHODS: Measures during the year before admission included the FRIs, demo

Medication-related burden variation across chronic conditions: a population-based cross-sectional survey.

BACKGROUND: Medication therapy is an important healthcare intervention for patients with chronic conditions. Managing their own medication can place a burden known as medication-related burden (MRB) on the patients. The burden can vary among chronic conditions due to diverse medication and management needs. This study aimed to examine the variation in MRB across different chronic conditions in the adult general population. METHODS: This study was an online population-based cross-sectional survey conducted in 2021 representing Finnish adults aged 18-79 years. MRB was measured using a 13-item M

Phytotherapy for chronic prostatitis.

Chronic prostatitis is a poorly defined condition that is difficult to treat; there are therefore multiple therapies. Although there is a paucity of trials using phytotherapeutic agents, plant extracts have been postulated to have anti-inflammatory effects that might be useful in the treatment of chronic prostatitis. More placebo-controlled trials of longer duration in this condition are needed to ascertain whether there is a significant benefit to the use of phytotherapeutic agents for chronic prostatitis.

The Clinical Potential of Senolytic Drugs.

Senolytic drugs are agents that selectively induce apoptosis of senescent cells. These cells accumulate in many tissues with aging and at sites of pathology in multiple chronic diseases. In studies in animals, targeting senescent cells using genetic or pharmacological approaches delays, prevents, or alleviates multiple age-related phenotypes, chronic diseases, geriatric syndromes, and loss of physiological resilience. Among the chronic conditions successfully treated by depleting senescent cells in preclinical studies are frailty, cardiac dysfunction, vascular hyporeactivity and calcification,

The association between chronic diseases and depressive symptomatology in older Mexican Americans.

BACKGROUND: Among the elderly population, the risk for psychological distress increases with the number of chronic diseases and accompanying functional disability. The prevalence of chronic medical conditions and functional disability varies substantially across ethnocultural groups, however. Using data from the Hispanic EPESE, we previously reported that among older Mexican Americans, the total number of chronic medical conditions and the presence of functional impairment are strong predictors of depressive symptoms. METHODS: Using multiple regression, we examined the association between spe

Front-line decision-making: a thematic analysis of interviews with hospital staff on referrals, admissions, and care for people with multiple long-term conditions.

BACKGROUND: Multiple long-term conditions (MLTC) are increasingly common and present challenges for hospital healthcare professionals (HCPs) making decisions about admissions, referrals, and care management in the context of a single-organ oriented health system. This qualitative study examined how HCPs in emergency and acute hospital settings make such decisions for people living with MLTC, exploring factors that influence clinical judgement and how HCPs respond to these challenges. METHODS: HCPs involved in making decisions about care for people living with MLTC presenting to hospital were
