Primary pulmonary malignant melanoma with multiple metastases: a case report and literature review.

Primary malignant melanoma of lung (PMML) are relatively rare, accounting for about 0.4 % of all malignant melanomas and only 0.01 % of all lung tumors, and PMML with distant metastases found at diagnosis is extremely rare. A case of PMML presented as multiple solid and ground glass pulmonary nodules with brain, liver, intrapulmonary and lymph nodes metastasis was reported in this study. A 42-year-old man had a headache for 10 days after symptomatic treatment in the department of neurology in our hospital, but the above symptoms did not improved. Multiple nodules were considered by head MRI. Ch

Sarcoid-like hilar and mediastinal lymphadenopathy in a patient with metastatic testicular cancer.

Previous reports indicate that enlarged hilar and mediastinal lymph nodes due to sarcoid-like reactions may develop after curative resection of testicular cancer, and their presence does not necessarily denote neoplastic recurrence. Reports further suggest that coexisting pulmonary nodules in this setting may be related to nodular sarcoidosis. A patient developed progressive hilar and mediastinal adenopathy associated with multiple pulmonary nodules after apparent curative resection of a testicular embryonal cell cancer. Biopsy specimens from the mediastinal lymph nodes demonstrated granulomas

Pulmonary nodules: classification, biomarkers, signaling pathways, diseases, early diagnosis and management.

Pulmonary nodules are increasingly detected by thoracic imaging and low-dose computed tomography-based lung cancer screening, but most are benign, transient, or indolent. Clinically, the key task is to distinguish nodules that require timely cancer-directed evaluation from those that can be safely observed. This review summarizes the classification, disease spectrum, risk assessment, early diagnosis, biomarkers, signaling mechanisms, translational models, and management of pulmonary nodules. We first distinguish imaging phenotypes and disease entities, then differentiate the individual-level m

Navigating pulmonary nodules: Guidelines on assessing risk and determining next steps in primary care.

Pulmonary nodules are often encountered in primary care during radiographic evaluation of respiratory symptoms, incidental imaging, and lung cancer screening. While most nodules are low risk, accurately identifying nodules that require surveillance, determining appropriate follow-up intervals, and deciding when specialist evaluation and intervention is indicated are challenging.

Aggressiveness-guided nodule management for lung cancer screening in Europe-justification for follow-up intervals and definition of growth.

The European Society of Thoracic Imaging (ESTI) nodule management recommendation for lung cancer screening with low-dose CT builds on existing nodule management guidelines but puts a stronger focus on lesion aggressiveness and measurement error. Key objectives included finding a compromise between the overall number of follow-up examinations, avoiding a major stage shift, and reducing the risk for overtreatment. Nodule management categories at baseline are chosen depending on the size of a solid nodule or the solid component of a subsolid or cystic nodule, with suspicious morphology upgrading

CT-Guided Anchored Needle Versus Hook-Wire Localization for Pulmonary Nodules: A Meta-Analysis.

Preoperative computed tomography (CT)-guided localization is widely used to facilitate the surgical resection of pulmonary nodules (PNs) through video-assisted thoracoscopic surgery (VATS). This meta-analysis aims to compare the clinical performance and safety profile of 2 commonly used localization techniques: the anchored needle (AN) and the hook-wire (HW) method. A systematic search was conducted using the PubMed, Wanfang, and Cochrane Library databases to identify relevant comparative studies. Key outcome measures were extracted and analyzed using Stata v12.0 and RevMan v5.3. Seven retrosp

How I Do It: Management of Pleural-attached Pulmonary Nodules in Low-Dose CT Screening for Lung Cancer.

Lung cancer is the leading cause of cancer deaths globally. In various trials, the ability of low-dose CT screening to diagnose early lung cancers leads to high cure rates. It is widely accepted that the potential benefits of low-dose CT screening for lung cancer outweigh the harms. The ability to reliably predict the benignity of nodules, especially at the baseline round, further reduces the potential for harm. Pleural-attached nodules are an important subgroup that represents nodules attached (distance from any pleural surface, 0 mm) to any pleural surfaces (fissural, costal, mediastinal, an

Optimal approach for diagnosing peripheral lung nodules by combining electromagnetic navigation bronchoscopy and radial probe endobronchial ultrasound.

INTRODUCTION: Electromagnetic navigation bronchoscopy (ENB) and radial probe endobronchial ultrasound (RP-EBUS) are essential bronchoscopic procedures for diagnosing peripheral lung lesions. Despite their individual advantages, the optimal circumstances for their combination remain uncertain. METHODS: This single-center retrospective study enrolled 473 patients with 529 pulmonary nodules who underwent ENB and/or RP-EBUS biopsies between December 2021 and December 2022. Diagnostic yield was calculated using strict, intermediate, and liberal definitions. In the strict definition, only malignant

Robust explanation supervision for false positive reduction in pulmonary nodule detection.

BACKGROUND: Lung cancer is the deadliest and second most common cancer in the United States due to the lack of symptoms for early diagnosis. Pulmonary nodules are small abnormal regions that can be potentially correlated to the occurrence of lung cancer. Early detection of these nodules is critical because it can significantly improve the patient's survival rates. Thoracic thin-sliced computed tomography (CT) scanning has emerged as a widely used method for diagnosing and prognosis lung abnormalities. PURPOSE: The standard clinical workflow of detecting pulmonary nodules relies on radiologist

[Pulmonary Nodule Localization by Pleural Marking Using Virtual Thoracoscopic Imaging].

Percutaneous or transbronchial markings are performed to localize pulmonary nodules preoperatively. We present a novel intraoperative procedure that utilizes virtual thoracoscopic imaging-assisted pleural marking. In this procedure, a virtual thoracoscopic image is created preoperatively, and the coordinates of the pleural point above the tumor are determined. The pleural marker is intraoperatively placed on the coordinates, and dye is transferred to the visceral pleura with two lung ventilations. We present the specific procedures and countermeasures for cases when nodules are not palpable. A

Incidental Pulmonary Nodules - What Do We Know in 2022.

Lung cancer (LC) is the leading cause of cancer-related mortality worldwide, and early LC diagnosis can significantly improve outcomes and survival rates in affected patients. Implementation of LC screening programs using low-dose computed tomography CT in high-risk subjects aims to detect LC as early as possible, but so far, adoption of screening programs into routine clinical care has been very slow. In recent years, the use of CT has significantly increased the rate of incidentally detected pulmonary nodules. Although most of those incidental pulmonary nodules (IPNs) are benign, some of the

Preoperative radio-guided localization of lung nodules with I-125 seeds: experience with 32 patients at a single institution.

OBJECTIVES: Videothoracoscopic visualization and/or palpation of pulmonary nodules may be difficult due to their location, small size or limited solid component. The purpose of this study is to present our experience with computed tomography (CT)-guided preoperative localization of pulmonary nodules by percutaneous marking with radio-labelled iodine-125 seeds. METHODS: A total of 34 pulmonary nodules were marked under CT with the placement of 33 radio-labelled iodine-125 seeds in 32 consecutive patients. RESULTS: All patients underwent biportal video-assisted thoracic surgery (VATS) and in n

Quantitative Imaging features Improve Discrimination of Malignancy in Pulmonary nodules.

Pulmonary nodules are frequently detected radiological abnormalities in lung cancer screening. Nodules of the highest- and lowest-risk for cancer are often easily diagnosed by a trained radiologist there is still a high rate of indeterminate pulmonary nodules (IPN) of unknown risk. Here, we test the hypothesis that computer extracted quantitative features ("radiomics") can provide improved risk-assessment in the diagnostic setting. Nodules were segmented in 3D and 219 quantitative features are extracted from these volumes. Using these features novel malignancy risk predictors are formed with v

[Bronchiolo-alveolar carcinoma. The computed tomographic picture and histological correlations].

Twenty-one CT examinations of 18 patients with a known bronchioloalveolar carcinoma in the lung were retrospectively reviewed to describe the CT features of bronchioloalveolar carcinomas. Surgical specimens were available in 13 cases in which CT-histologic correlation was also obtained. In 5 patients the diagnosis was made with cytology and confirmed with radiologic-clinical follow-up. Three patients were reexamined for relapses 6-20 months after the resection of a localized carcinoma. Carcinomas exhibited 3 radiologic patterns: a) solitary pulmonary nodule (11 cases), b) mass or pulmonary con

[Hydatidosis simulating a cardiac tumour with pulmonary metastases].

BACKGROUND: The presence of multiple symptomatic pulmonary nodules and one cardiac tumour in a child requires urgent diagnosis and treatment. Until a few decades ago, the diagnosis of a cardiac tumour was difficult and was based on a high index of suspicion from indirect signs, and required angiocardiography for confirmation. Echocardiography and other imaging techniques have also helped in the detection of cardiac neoplasms. However, it is not always easy to make the correct diagnosis. CLINICAL CASE: The case is presented of a 12 year-old boy with pulmonary symptoms, and diagnosed with a car

Simple Pulmonary Eosinophilia Mimicking Lung Metastases on 18 F-FDG PET/CT.

A 72-year-old man with pancreatic tail cancer underwent distal pancreatectomy and splenectomy 1 year ago. Routine postoperative follow-up CT detected multiple pulmonary nodules. 18 F-FDG PET/CT showed multiple FDG-avid nodules in the bilateral lungs, which highly suggested the possibility of lung metastases of pancreatic cancer. Finally, the bronchoalveolar lavage fluid and pathology confirmed the diagnosis of simple pulmonary eosinophilia.

[A clinicopathological study of pulmonary cryptococcosis--chest CT and pathologic correlation].

We reviewed the clinicopathological features in 12 patients (7 males and 5 females; mean age 54 yr) with pulmonary cryptococcosis. Eleven of the patients were asymptomatic and the disease was detected by chest radiograph abnormalities. The underlying systemic disease had been diagnosed as diabetes mellitus in two. Chest CT scans showed a solitary nodule in 9 of the 12 patients, multiple nodules in 2, and infiltration in 1. The nodular diameter was less than 2 cm in 10 of the 12. All nodules were located in the subpleural region. On the chest CT, cavitary nodules, scattered nodules, or both, an

Rheumatoid Nodules.

Rheumatoid nodules are a common manifestation of rheumatoid arthritis. These lesions are often easily identified based on typical diagnostic features and characteristic locations. When biopsied, nodules have a characteristic histologic appearance. Uncommonly, rheumatoid nodules can occur in systemic locations. There is no evidence that systemic therapy treats underlying rheumatoid nodules. Paradoxically, methotrexate and possibly tumor necrosis factor inhibitors can increase nodule development. Treatment of rheumatoid nodules is often not necessary, unless patients are experiencing pain or the
