Assisted Partner Notification Services in Namibia: Comparison of Case-Finding in Persons With New and Previously Diagnosed Human Immunodeficiency Virus, and Success as a Platform for PrEP Referral.

BACKGROUND: Assisted partner notification services (APS) are widely implemented throughout sub-Saharan Africa. The effectiveness of APS among persons with previously diagnosed human immunodeficiency virus (HIV) infection is uncertain, and there are few published data on the success of integrating referrals for HIV preexposure prophylaxis (PrEP) into APS. METHODS: Staff in 22 Namibian Ministry of Health and Social Service clinics offered APS to patients newly and previously diagnosed with HIV (index cases [ICs]) between October 2019 and June 2021. Counselors used a structured interview guide t

Effect of changing from first- to second-line antiretroviral therapy on renal function: a retrospective study based on data from a single health facility in Namibia.

BACKGROUND: Tenofovir disoproxil fumarate (TDF) and lopinavir/ritonavir (LPV/r) can cause renal impairment with this combination co-administered during second-line combination antiretroviral therapy (cART) potentially associated with greater risk of nephrotoxicity. As a result, the aim of this study is to assess effects of second-line cART on renal function. METHODS: Retrospective longitudinal study in patients receiving cART. RESULTS: 71 patients received TDF, zidovudine or stavudine, each combined with 3TC/NVP or 3TC/EFV. Before second-line cART, 46.5% had abnormal kidney function. First-l

Results on Listeria monocytogenes Strains From Ready-to-Eat Meat Products and Food Production Environments in Namibia, Sudan, and Zambia: A Focus on Genomic Characterization of Strains.

Although one of the biggest listeriosis outbreaks was reported in South Africa, due to a ready-to-eat (RTE) meat product, to date, very few data on L. monocytogenes in foods and food environments are available from other African countries. The aims of this study were to document L. monocytogenes presence in RTE products and food environments from Namibia, Sudan, and Zambia, provide isolates' genomic characterization, and evaluate genomic similarity using data available at the Italian National Reference Laboratory for L. monocytogenes database. A total of 768 samples, including RTE meat product

Correlates of Substance Misuse, Transactional Sex, and Depressive Symptomatology Among Partnered Gay, Bisexual and Other Men Who Have Sex with Men in South Africa and Namibia.

Despite having some of the world's highest rates of HIV, there is a lack of knowledge on correlates of transmission risk among gay, bisexual and other men who have sex with men in Southern Africa. There is even less known about the factors that shape HIV risk in male-male couples. Using data from Together Tomorrow, a study of partnered GBMSM in South Africa and Namibia, this study assessed the individual and dyadic correlates of three major HIV risk factors in this population: substance misuse, transactional sex, and depressive symptomatology. Data were collected during November 2016-March 201

Post-treatment duration of positivity for standard and ultra-sensitive Plasmodium falciparum antigen-based rapid diagnostic tests, a cohort study from a low-endemic setting in Namibia.

BACKGROUND: The standard malaria rapid diagnostic test (RDT) and newer ultra-sensitive RDT (uRDT) target Plasmodium falciparum histidine rich protein-2 (HRP2), which persists post-treatment. The duration of test positivity has not previously been studied in a low transmission setting. METHODS: We conducted a longitudinal cohort study in a low transmission setting in Namibia. RDT-positive individuals identified through passive and active case detection were treated and followed weekly for testing by RDT and uRDT, HRP2 quantification, quantitative PCR (qPCR) of parasitemia, and quantitative rev

Subpatent malaria in a low transmission African setting: a cross-sectional study using rapid diagnostic testing (RDT) and loop-mediated isothermal amplification (LAMP) from Zambezi region, Namibia.

BACKGROUND: Subpatent malaria infections, or low-density infections below the detection threshold of microscopy or standard rapid diagnostic testing (RDT), can perpetuate persistent transmission and, therefore, may be a barrier for countries like Namibia that are pursuing malaria elimination. This potential burden in Namibia has not been well characterized. METHODS: Using a two-stage cluster sampling, cross-sectional design, subjects of all age were enrolled during the end of the 2015 malaria transmission season in Zambezi region, located in northeast Namibia. Malaria RDTs were performed with

Accelerators to reduce violence, HIV risk, and early pregnancy among adolescents and young people in Namibia: A cross-sectional analysis of the Violence Against Children & Youth Survey.

Our study applied the INSPIRE Framework - the WHO's 2016 technical package of evidence-based interventions for addressing violence against children - to identify accelerators for youth in Namibia. Accelerators are protective factors that contribute toward achieving multiple SDG targets. Using nationally representative data from the 2019 Namibia Violence Against Children & Youth Survey (n = 5167), three hypothesised accelerators (food security, parental support, and gender-equitable attitudes) were investigated for their impact on 12 adolescent outcomes. Associations between the hypothesised ac

Targeting malaria in high-risk populations in low endemic regions in northern Namibia: a quasi-experimental controlled trial to reduce malaria in seasonal agricultural workers and cattle herders.

BACKGROUND: Agricultural worksites are rarely targeted by malaria control programmes, yet may play a role in maintaining local transmission due to workers' high mobility, low intervention coverage and occupational exposures. METHODS: A quasi-experimental controlled intervention study was carried out in farming and cattle herding populations in northern Namibia to evaluate the impact of a targeted malaria intervention package. Eight health facility catchment areas in Zambezi and Ohangwena Regions were randomised to an intervention arm and eligible individuals within worksites in intervention a

Improving the benefits of HIV testing and referrals in large household surveys through active linkages to care: lessons and recommendations from the Namibia population-based HIV impact assessment (NAMPHIA), 2017.

In household-based surveys that include rapid HIV testing services (HTS), passive referral systems that give HIV-positive participants information about how and where to access ART but minimal follow-up support from survey staff may result in suboptimal linkage. In the 2017 Namibia Population-based HIV Impact Assessment (NAMPHIA), we piloted a system of active linkage to care and ART (ALCART) that utilized the infrastructure of existing community-based partner organizations (CBPOs). All HIV-positive participants age 15-64 years not on ART were given standard passive referrals to ART plus the o

Two-phase study to optimise diagnosis and treatment for children and adolescents with tuberculosis in Namibia: knowledge, attitudes and practices of healthcare workers before and after the rollout of updated child and adolescent tuberculosis clinical guidelines.

INTRODUCTION: Child and adolescent tuberculosis (CATB) poses a significant global burden with an estimated 1.25 million diagnoses and 191 000 deaths annually, disproportionately higher than adult rates. Over half of CATB cases remain unreported, and burden increases are expected following the USA's tuberculosis (TB) aid withdrawal, particularly affecting high-burden countries including Namibia. Namibian children under the age of 15 years account for 10% of notified TB cases, with under-reporting suggesting higher actual morbidity and mortality. Through a multistakeholder process in 2024, Namib

Serological evaluation of the effectiveness of reactive focal mass drug administration and reactive vector control to reduce malaria transmission in Zambezi Region, Namibia: Results from a secondary analysis of a cluster randomised trial.

BACKGROUND: Due to challenges in measuring changes in malaria at low transmission, serology is increasingly being used to complement clinical and parasitological surveillance. Longitudinal studies have shown that serological markers, such as Etramp5.Ag1, can reflect spatio-temporal differences in malaria transmission. However, these markers have yet to be used as endpoints in intervention trials. METHODS: Based on data from a 2017 cluster randomised trial conducted in Zambezi Region, Namibia, evaluating the effectiveness of reactive focal mass drug administration (rfMDA) and reactive vector c

Negotiating ethnolinguistic identity in a multilingual society: social meaning and linguistic choice in Namibian German.

INTRODUCTION: Linguistic choices rapidly evoke social meanings, that is, information related to social identities, group membership, and social evaluations of the speaker, the hearer, and their relationship, but this activation is influenced by the linguistic domain. Moreover, social meaning and language attitudes are shaped by the societal context, but most previous studies focused on monolingually oriented societies. METHOD: We report findings from the German-speaking minority in Namibia, a multilingually oriented society. We conducted an open-guise study (Soukup, 2013) that examined how li

Study protocol for a cluster randomised controlled factorial design trial to assess the effectiveness and feasibility of reactive focal mass drug administration and vector control to reduce malaria transmission in the low endemic setting of Namibia.

INTRODUCTION: To interrupt malaria transmission, strategies must target the parasite reservoir in both humans and mosquitos. Testing of community members linked to an index case, termed reactive case detection (RACD), is commonly implemented in low transmission areas, though its impact may be limited by the sensitivity of current diagnostics. Indoor residual spraying (IRS) before malaria season is a cornerstone of vector control efforts. Despite their implementation in Namibia, a country approaching elimination, these methods have been met with recent plateaus in transmission reduction. This s

The measurement of condom use in four countries in East and southern Africa.

Measurement of condom use is important to assess progress in increasing use. Since 2003, the Demographic and Health Surveys (DHS) and AIDS Indicator Surveys (AIS) have included new measures of self-reported condom use. We use data from Namibia, Swaziland, Tanzania, and Zambia to compare measures of condom use accounting for type of sexual partner. Condom use at last sex ranged from 20% in Tanzania to 57% in Namibia for men, and from 12% in Tanzania to 41% in Namibia for women. Reported condom use was lower in response to questions about condom use every time with last partner (from 13 to 47% f
