Middle-aged man with left neck pain and swelling.

BACKGROUND: Disorders of the salivary glands can be evaluated by bedside ultrasonography and should be considered in patients presenting with undifferentiated neck swelling. OBJECTIVE: Our aim was to describe the sonographic findings present in sialolithiasis and sialadenitis. CASE REPORT: A 61-year-old man presented to the emergency department with 2 days of neck swelling. Initial evaluation included a bedside ultrasound that demonstrated sialolithiasis, which was later confirmed by computed tomography. CONCLUSIONS: Bedside ultrasound can be a useful imaging modality in the evaluation of t

Experimental masseter muscle pain alters jaw-neck motor strategy.

BACKGROUND: A functional integration between the jaw and neck regions has been demonstrated during normal jaw function. The effect of masseter muscle pain on this integrated motor behaviour in man is unknown. The aim of this study was to investigate the effect of induced masseter muscle pain on jaw-neck movements during a continuous jaw opening-closing task. METHODS: Sixteen healthy men performed continuous jaw opening-closing movements to a target position, defined as 75% of the maximum jaw opening. Each subject performed two trials without pain (controls) and two trials with masseter muscle

Prevalence, practice patterns, and evidence for chronic neck pain.

ObjectiveTo estimate the prevalence of chronic neck pain in North Carolina, to describe health care use (providers, treatments, and diagnostic testing) for chronic neck pain, and to correlate health care use with the current best evidence.MethodsWe used data from a cross-sectional telephone survey of a representative sample of North Carolina households in 2006. A total of 5,357 households were contacted in 2006 to identify 141 noninstitutionalized adults ages ≥21 years with chronic neck pain and no chronic low back pain. The subjects were interviewed about their health and health care use (i.e

The association between cervical spine curvature and neck pain.

Degenerative changes of the cervical spine are commonly accompanied by a reduction or loss of the segmental or global lordosis, and are often considered to be a cause of neck pain. Nonetheless, such changes may also remain clinically silent. The aim of this study was to examine the correlation between the presence of neck pain and alterations of the normal cervical lordosis in people aged over 45 years. One hundred and seven volunteers, who were otherwise undergoing treatment for lower extremity problems in our hospital, took part. Sagittal radiographs of the cervical spine were taken and a qu

Residual neck pain after traumatic spondylolisthesis of the axis.

OBJECTIVE: Most patients with traumatic spondylolisthesis of the axis are treated nonsurgically. Some patients do not develop symptoms, but others experience strong and persistent neck pain. To clarify the factor that plays a major role in residual neck pain after this fracture, we reviewed nine patients who underwent conservative treatment. METHODS: Patients were assessed via telephone interviews an average of 62 months after injury and were divided into two groups: those with and those without neck pain. Angulation and anterior translation between the second and third cervical vertebrae wer

[Two patients with pseudogout manifested by severe neck pain].

We reported 2 patients with pseudogout manifested by severe posterior neck pain. Patient 1 was a 78-year-old woman. She had experienced attacks of posterior neck pain several times for 4 years. On July 3, 2001, she developed severe pain in the posterior neck and left acromioclavicular joint, and was admitted to our hospital. On examination, her body temperature was 38.1 degrees C, ESR 140 mm/hr and CRP 14.7 mg/dl. Linear calcifications in meniscus of the right knee and left acromioclavicular joint were observed in roentgenograms. The CT of the cervical spines revealed multiple nodular calcific

Specific and shared mechanisms mediating pain intensity and functional outcomes in chronic neck pain.

BACKGROUND: To date, very few studies have used mediation analysis to explore the reasons why people with chronic neck pain improve after physical therapy intervention involving manual therapy or exercise. OBJECTIVES: To evaluate which specific mechanisms (mobility, pain modulation and endurance) and shared mechanisms (self-efficacy, working alliance, engagement, fear, and positive coping) mediated the effects of treatment on clinical outcomes at 4-weeks and 6-months in individuals with chronic neck pain. METHODS: The study incorporated planned indirect‑only mediation analyses to evaluate bo

Back and neck pain: A comparison between acute and chronic pain-related Temporomandibular Disorders.

BACKGROUND: Temporomandibular disorders (TMDs) are common and cause persistent pain. Comorbidities are associated with TMDs and can affect the effectiveness of their treatments. The literature is lacking enough evidence on the difference between acute and chronic pain, particularly in TMDs. Investigating this difference could highlight potential risk factors for the transition from acute to chronic pain-related TMDs. AIM: To compare the likelihood of back and neck pain (BP, NP) between acute and chronic pain-related TMDs (AP-TMD, CP-TMD) as defined by pain duration and pain-related disability

Occupational cold exposure is associated with neck pain, low back pain, and lumbar radiculopathy.

Ambient cold exposure can pose health risks, and this study was aimed at investigating associations with musculoskeletal disorders. A postal survey was performed on 12,627 men and women, ages 18-70 years, living in northern Sweden. Statistical associations were determined using multiple logistic regression. The study sample consisted of 6,886 women (54.5%), and 5,741 men. Reporting high occupational ambient cold exposure was statistically significantly associated with neck pain (OR 1.36; 95% CI 1.16-1.59), low back pain (OR 1.38; 95% CI 1.17-1.63), and lumbar radiculopathy (OR 1.36; 95% CI 1.0

Low back pain and neck pain as predictors of sickness absence among municipal employees.

OBJECTIVE: To study whether having ever had local low back pain (LBP), sciatica, neck pain (NP), or some combination of LBP and NP, predicts sickness absence among municipal employees. METHODS: The study sample (n=6911, 80% women, response rate 67%) included employees of the City of Helsinki who reached the age of 40, 45, 50, 55, or 60 years between 2000-02. Survey data on pain, working conditions, and health behaviours were linked to register data on sickness absence for three subsequent years. Sickness absence was categorised as self-certified (lasting for 1-3 days) and medically certified

Neck pain and headache: Pathophysiology, treatments and future directions.

INTRODUCTION: Neck pain is a prevalent neurologic and musculoskeletal complaint in the general population and is often associated with primary headache disorders such as migraine and tension-type headache (TTH). A considerable proportion, ranging from 73% to 90%, of people with migraine or TTH also experience neck pain, and there is a positive correlation between headache frequency and neck pain. Furthermore, neck pain has been identified as a risk factor for migraine and TTH. Although the exact underlying mechanisms linking neck pain to migraine and TTH remain uncertain, pain sensitivity appe

Sleep quality and chronic neck pain: a cotwin study.

STUDY OBJECTIVES: Sleep quality and chronic neck pain (NP) are associated. However, the genetic influences on this association have not been explored. This study investigated the genetic and environmental influences on the association between sleep quality and chronic NP. METHODS: The sample comprised 2,328 individual twins from the Murcia Twin Registry (Spain). A bidirectional cotwin logistic regression analysis was performed (sleep quality assessed as the exposure and chronic NP as the outcome and vice versa). Analysis included 2 sequential stages: total sample analysis and within-pair twin

Acute hip pain: mimics of a femoral neck fracture.

Acute hip pain is a common presenting complaint among patients attending Emergency Departments particularly following a traumatic event. A proximal femoral fracture is the main consideration, which requires urgent confirmation and treatment; however, there are a number of conditions, which could mimic neck of femur fracture and the causes may not be directly related to the hip, so systematic and careful observation of pelvic MRI could shed light on the true culprit of acute hip pain.
