Cervical abscess and mediastinal adenopathy: an unusual presentation of childhood histoplasmosis.

Histoplasmosis is the most common endemic respiratory mycosis in the United States. We report the clinical and imaging findings in a case of a child with the rare presentation of a neck abscess and mediastinal lymphadenopathy secondary to acute, non-disseminated histoplasmosis. Imaging findings often mimic other granulomatous infections such as tuberculosis or neoplastic processes such as lymphoma. Histoplasmosis should be considered in the differential diagnosis of a child who presents with enlarged mediastinal and cervical lymphadenopathy.

Imaging of the olfactory system.

The olfactory system consists of the primary olfactory nerves in the nasal cavity, the olfactory bulbs and tracts, and numerous intracranial connections and pathways. Diseases affecting the sense of smell can be located both extracranially and intracranially. Many sinonasal inflammatory and neoplastic processes may affect olfaction. Intracranially congenital, traumatic, and neurodegenerative disorders are usually to blame for olfactory dysfunction. The breadth of diseases that affect the sense of smell is astounding, yet the imaging ramifications have barely been explored.

Radiographic herald lesion of the urinary bladder: pictorial essay.

At cystoscopy a focal mucosal abnormality may be indicative of a pathological process extrinsic to the urinary bladder and has been termed 'the herald lesion'. The aim of the present pictorial essay was to describe the radiographic counterpart to this cystoscopic finding. Radiographic herald lesions are shown in patients with extravesical inflammatory (Crohn's disease, colonic diverticulitis and pelvic inflammatory disease) and neoplastic (colon carcinoma) processes, and urinary tract complications of these conditions are described and illustrated.

Primary care of women infected with the human immunodeficiency virus.

AIDS is the leading cause of death among women aged 25 to 34 years in New York City. Gynecologic care should be directed toward early identification and aggressive treatment of infectious and neoplastic processes that may take a more fulminant course in immunosuppressed women. Dealing with women's contraceptive needs highlights the problem of preventing sexually transmitted diseases through contraceptive technology. Public health policies and recommendations will be of no avail without an educated and supportive medical community.

Selected Giant Cell Rich Lesions of the Temporal Bone.

Giant cell rich lesions of the temporal bone encompass a wide spectrum of disease that includes infectious, reactive, and neoplastic processes. When dealing with any lesion that can potentially involve bone, it is important to understand both the clinical presentation and to correlate the histologic findings with the radiologic imaging. This review discusses the clinical, the pathologic features including the differential diagnosis, and the treatment of some of the more commonly encountered giant cell rich entities in this region.

[The transcription factor NF-kappaB in human diseases].

The transcription nuclear factor kappa B (NF-kappaB) was first described in 1986. Currently, the NF-kappaB has been linked to several metabolic and inflammatory diseases as well as to several types of cancer. NF-kappaB will continue to be of interest in many disease processes such as inflammatory, infectious and neoplastic. The discovery of this factor has increased our knowledge about the regulation of inflammation and chronic diseases, as well as the medications used to treat those diseases.

Computed tomography findings mimicking appendicitis as a manifestation of colorectal cancer.

The primary computed tomography (CT) signs of appendicitis can also be seen with other inflammatory or neoplastic processes. We report on two cases in which appendiceal dilatation and peri-appendiceal fluid or stranding were the dominant imaging manifestations of colorectal carcinoma in the ascending colon. This study highlights the need to closely examine the ascending colon in patients with a suspected CT diagnosis of acute appendicitis, since these findings may be secondary to an inconspicuous colorectal carcinoma.

Infection Associated with Hemophagocytic Lymphohisticytosis triggered by nosocomial Infection.

Hemophagocytic lymphohistiocytosis (HLH) can occur as primary idiopathic syndrome or secondary to neoplastic, infection or autoimmune processes. It is characterized by the proliferation of histiocytes with phagocytosis of formed elements of blood. Clinical manifestations include signs and symptoms of immune activation and pancytopenia. This report presents a child with infection associated with HLH trigged by Acinetobacter baumannii sepsis. Multidrug-resistant Acinetobacter, an emergent nosocomial pathogen but so far in the literature, it has not been reported to cause HLH.

Metastatic carotid body paraganglioma in von Hippel-Lindau disease. An electron microscopic study.

Pulmonary metastases from a carotid body paraganglioma developed in a patient with von Hippel-Lindau disease. On electron microscopic examination, these metastases consisted solely of chief cells that contained membrane-bound, dense-core granules and formed numerous cytoplasmic processes. These data support the hypothesis that the chief cell is the sole neoplastic cell in paragangliomas. Finally, to our knowledge, this clinical association of a malignant carotid body paraganglioma and pheochromocytoma in von Hippel-Lindau disease is unique.

[Reccurrent lingual hemiatrophy].

The author reported a 45-year-old woman with numerous developmental anomalies in whom lingual hemiatrophy preceded by strong occipital headaches followed tonsillectomy. After treatment considerable regeneration of tongue nuscle took place. The course of the disease was recurrent. In the differential diagnosis the author considered inflammatory processes, neoplastic diseases, developmental anomalies and toxic effect of procaine used in tonsillectomy as well as the possibility of deeply situated adhaesions in the vicinity of the hypoglossal nerve.

Intestinal acinar adenocarcinoma in a blue-fronted Amazon parrot (Amazona aestiva).

A 54-year-old captive male blue-fronted Amazon parrot (Amazona aestiva) was presented with progressive weight loss, diarrhoea and dyspnoea. Contrast radiographs suggested a neoplastic process by revealing an opaque soft tissue mass occupying most of the coelomic cavity. Post-mortem findings included approximately 80 ml of serous ascitic fluid. The intestines had a diffuse firm consistency, and the serosal surface was covered by multifocal to coalescent, irregularly shaped, white to off-white elevated plaques and nodules, which obscured visualization of the pancreas. Histologically, there was a

MR imaging of synovial disorders of the knee.

Synovial membranes line the diarthrodial (movable) joints, bursae, and tendon sheaths of the body. The primary function of this specialized, vascular tissue is to serve as a filter system that lubricates and nourishes the articular structures as well as serving as a shock-absorber. The synovium is affected by a variety of disorders that can be localized to a specific articulation or can be systemic in nature. These include inflammatory, infectious, degenerative, traumatic, or neoplastic categories of disease. Further, MR imaging provides an excellent non-invasive tool for the evaluation of the

Pathology of acoustic nerve Schwannoma in vivo and in vitro.

The ultrastructural pattern of the acoustic nerve Schwannoma was studied on surgically excised tumours and on explants cultured in vitro. The typical neoplastic cell is elongated, spindle-shaped, coated with basement membrane and numerous protruding cellular processes. Cells demonstrating numerous microfilaments are also seen, sometimes intermixed with the typical spindle-shaped cells. Fibroblasts are not uncommon; nor are cells with a round nucleus and rich in organelle cytoplasm, resembling endothelial cells. Numerous collagen fibres and "Luse bodies" are seen in the extracellular space, esp

Epigenetic Changes in Neoplastic Mast Cells and Potential Impact in Mastocytosis.

Systemic mastocytosis (SM) is a hematologic neoplasm with abnormal accumulation of mast cells in various organ systems such as the bone marrow, other visceral organs and skin. So far, only little is known about epigenetic changes contributing to the pathogenesis of SM. In the current article, we provide an overview of epigenetic changes that may occur and be relevant to mastocytosis, including mutations in genes involved in epigenetic processes, such as TET2, DNMT3A and ASXL1, and global and gene-specific methylation patterns in neoplastic cells. Moreover, we discuss methylation-specific pathw

Composite lymphoma of follicular B-cell and peripheral T-cell types with distinct zone distribution in a 75-year-old male patient: a case study.

Composite lymphoma of T-/B-cell type is rare, and follicular lymphoma composite with peripheral T-cell lymphoma (PTCL) has not previously been reported. We report such a case with both neoplastic components displaying a unique zone of distribution. A 75-year-old male patient presented with generalized lymphadenopathy. Sections of axillary lymph node demonstrated potentially 2 clonal processes, PTCL with aberrant CD20 expression and follicular lymphoma. Interestingly, the 2 neoplastic components were confined to their respective classic distribution zones, with PTCL occupying the interfollicula

Histiocytic sarcoma with interdigitating dendritic cell differentiation: a case report with fine needle aspiration cytology and review of literature.

A hybrid histiocytic sarcoma-interdigitating dendritic cell sarcoma was found in a small perinephric lymph node of an asymptomatic 80-year-old man, who presented a year ago with two small foci of lung metastasis found during routine chest X-ray. Fine needle aspiration cytology demonstrated interconnecting long and thin cell processes radiating from dendrite-like neoplastic cells with oval, enlongated, reniform, and irregular nuclei with vesicular chromatin and distinct nucleoli. Histology showed spindled epitheliod and histiocytic cells with abundant, slightly eosinophilic cytoplasm with indis

Orbital inflammation.

Orbital inflammation is a common problem in adults and children, accounting for the majority of all orbital processes. The presentation may be acute, subacute, or insidious. When the onset is acute, the process can be mistaken for orbital cellulitis. In insidious cases, such as the sclerosing subtype of inflammation, the chronic painless course may prompt concerns about a neoplastic infiltration such as lymphoma. Orbital inflammation can be divided into nonspecific, idiopathic, and other specific diagnoses. The differential diagnosis includes allergic, infectious (fungal, mycobacterial, and pa

Small-bowel disease: categorization by CT examination.

Fifty patients with no small-bowel disease were evaluated by CT to determine the normal appearance of the small bowel and mesentery. Subsequently, the CT findings in 95 patients with proven small-bowel disease were analyzed to determine which CT observations correlated with neoplastic, inflammatory, or edema-producing processes. Thirty-three (83%) of 40 patients with wall thickening or mesenteric masses greater than 1.5 cm had a neoplastic process. Twenty-eight (82%) of 34 patients with normal mesenteric fat attenuation, wall thickening less than 1.5 cm, or mesenteric masses less than 1.5 cm h
