Studies on nephrolithiasis in patients with autosomal dominant polycystic kidney disease.

Eighty two patients with autosomal dominant polycystic kidney disease were examined clinically. Nephrolithiasis was diagnosed in 23 patients (28.1%). In 17 (73.5%) the lithiasis was bilateral. There were no patients with nephrolithiasis under the age of 20 years. The disease frequency increased in the patients between 21 and 40 years (38.5%) reaching 58.1% in those above 41 years. Nephrolithiasis in autosomal dominant polycystic kidney disease is not induced by the chronic renal failure. The opposite is more likely-nephrolithiasis precipitates the onset of renal failure. The autosomal dominant

Treatment Deferral and Coping Mechanisms Associated With Nephrolithiasis-related Financial Toxicity.

OBJECTIVE: To assess how financial toxicity may impact the risk of deferral of recommended therapy in nephrolithiasis patients. METHODS: We performed a nationwide cross-sectional survey of adult volunteers with kidney stone disease registered with ResearchMatch. Comprehensive Score for Financial Toxicity (COST-FACIT) score <21 was used to identify patients with nephrolithiasis-related financial toxicity. Coping mechanisms, dietary, and medical interventions for stone disease were compared between high/low financial toxicity groups. Multivariable logistic regression assessed characteristics as

Silicone injection-induced granuloma formation, hypercalcemia and nephrolithiasis: a case report.

Hypercalcemia and nephrolithiasis have been associated with various etiologies, including dysregulation of the parathyroid glands, malignancies, or sarcoidosis. Other causes of hypercalcemia, such as granulomatous disease resulting from silicone-based cosmetic injections, have been reported but without specific emphasis on nephrolithiasis. Herein, we report an unusual case of simultaneous bilateral obstructing ureteral calculi (SBUC) triggered by recalcitrant hypercalcemia and granulomatous disease due to silicone-based cosmetic injections. A careful surgical history, physical exam, and imagin

Activation of liver X receptor suppresses osteopontin expression and ameliorates nephrolithiasis.

Nephrolithiasis is a common disease of the urinary system, of which idiopathic calcium oxalate (CaOx) kidney stones, in particular, are one of the special types. In the initial stages of CaOx kidney stone formation, Randall's plaques (RPs) develop. Liver X receptors (LXRs) inhibit oxidative stress and inflammatory in other diseases; nevertheless, the role of LXRs in nephrolithiasis has yet to be elucidated. In this study, the role of LXRs in the progression of RP formation was investigated. Microarray analysis revealed that LXR/RXR levels were significantly greater in low-plaque tissues (<5%) t

Nephrolithiasis and pyelonephritis in two West Indian manatees (Trichechus manatus spp.).

Two West Indian manatees (Trichechus manatus spp.) were reported with severe emaciation. One animal was a Florida manatee from the Everglades; the other was an Antillean manatee from Cuba. On necropsy, both animals had nephrolithiasis, pyelonephritis, and moderate to severe renomegaly. Histopathology revealed multifocal to diffuse pyelonephritis, interstitial nephritis, and nephrocalcinosis. The stones were analyzed and consisted primarily of calcium carbonate. Serum chemistry values for the Florida animal revealed no renal abnormalities. The mechanism of calculus formation remains unclear in

Persistent Unilateral Nephrolithiasis: Do 24-h Urine Parameters Account for It?

Background: Bilateral nephrolithiasis is typically attributed to systemic metabolic abnormalities reflected in 24 h urine studies. Yet, some individuals form stones exclusively in one kidney over long-term follow-up, while the contralateral kidney remains unaffected. This study aimed to determine whether differences in urinary metabolites can explain persistent unilateral stone formation. Methods: We retrospectively reviewed our 2011-2024 nephrolithiasis database and analyzed 24 h urine compositions obtained under regular diet and daily activity conditions. Patients were classified into three

Preauricular sinus, nephrolithiasis, infantine eczema and natal tooth: a new association.

Preauricular sinuses (ear pits) are common congenital abnormalities. The incidence of preauricular sinus is widely varied. Usually asymptomatic, they manifest as small hollows adjacent to the external ear near the anterior margin of the ascending limb of the helix, most frequently on the right side. Preauricular sinuses can be either inherited or sporadic. They may be bilateral, increasing the likelihood of being inherited, in 25-50% of cases. Preauricular sinuses are features of other conditions or syndromes in 3-10% of cases, primarily in association with deafness and branchio-oto-renal (BOR

Rare variant of Lesch-Nyhan syndrome without self-mutilation or nephrolithiasis.

Lesch-Nyhan syndrome is a very rare X-linked recessive disorder characterized by mental retardation, spasticity resembling cerebral palsy, choreo-athetosis, self-mutilation and hyperuricemia. Self-mutilative behavior is a hallmark of the disease. Hyperuricemia leads to hyperuricuria and uric acid nephrolithiasis. The underlying defect is a deficiency of hypoxanthine-guanine-phosphoribosyl transferase. We report on a 7-year-old boy with Lesch-Nyhan syndrome, lacking self-mutilative behavior, who was erroneously diagnosed as having athetotic cerebral palsy. He also had no renal stones; hyperecho

Diabetes, stone composition, and renal function in a large cohort of patients with nephrolithiasis.

BACKGROUND: Nephrolithiasis is linked to chronic kidney disease (CKD), and diabetic patients may be at higher risk for renal decline. However, the interaction of diabetes, stone composition, and estimated glomerular filtration rate (eGFR) trajectory in patients with nephrolithiasis remains underexplored, particularly in community-based populations. METHODS: In this retrospective cohort study of adult stone formers with a urinary stone composition analysis on file, diabetes status and stone composition were extracted from electronic records of a large health maintenance organization. Nephropleural Fistula as a Rare Complication of Nephrolithiasis: A Case Report and Literature Review.

Nephropleural fistula (NPF) is an exceptionally rare yet clinically significant complication that typically arises in the context of severe urinary tract infections or following percutaneous nephrolithotomy. We describe a case of NPF secondary to a complicated urinary tract infection, outlining its clinical course, diagnostic challenges, and therapeutic management. This report highlights the importance of early recognition and appropriate interventions to prevent the morbidity associated with this unusual entity.

Unravelling new mechanisms of occurrence and recurrence in calcium nephrolithiasis: the role of epigenetics.

INTRODUCTION: Calcium kidney stone disease, a common urinary tract disorder characterized by a high recurrence rate, remains poorly understood in terms of its recurrent etiology. Existing preventive measures are inadequate, limiting their clinical utility. Recent evidence suggests a significant role for epigenetic modifications in stone formation. METHODS: Based on expert-conducted PubMed literature searches, appropriate articles focusing on the investigation of epigenetic mechanisms in calcium kidney stone disease were carefully selected for inclusion in the study. RESULTS: The current unders

SGLT2 Inhibitors in Nephrolithiasis: Emerging Evidence and What Urologists Need to Know.

BACKGROUND: Nephrolithiasis frequently coexists with type 2 diabetes, obesity, gout, chronic kidney disease, and other cardiometabolic disorders. Sodium-glucose cotransporter 2 (SGLT2) inhibitors may influence stone formation through effects on glucosuria, natriuresis, urine volume, urinary citrate, pH, urate handling, and relative supersaturation. This review aimed to examine the current mechanistic, biochemical, and clinical evidence linking SGLT2 inhibition to nephrolithiasis, with particular attention to its relevance for urological practice. METHODS: A clinically oriented narrative revie

Association of nephrolithiasis history with adverse outcomes in cardiovascular-kidney-metabolic syndrome.

Cardiovascular-kidney-metabolic (CKM) syndrome significantly impacts a large segment of the general population. The risk factors associated with progression of CKM syndrome as well as all-cause mortality warrant further investigations. In this study, we aimed to assess whether nephrolithiasis history would help to identify the high-risk populations among CKM patients. We conducted a multi-center study involving patients with CKM syndrome at stages 2-3 from 28 urban centers across China. We employed multivariable Cox proportional hazards regression analysis to estimate hazard ratios (HRs) along
