Personal experience with abortion influences how we practice as obstetrician-gynecologists.

PURPOSE: To evaluate whether a personal history of having an abortion, or knowing someone close who did, has an impact on how a physician practices. METHODS: A web-based survey was sent to Ob/Gyn residents asking about personal abortion experience, demographics, religious background, political views, residency program metrics, and intent to provide abortion. Pearson Chi-square and Mann-Whitney U tests were performed to assess whether or not personal history of abortion (PHA) was significantly associated with personal characteristics or training patterns. RESULTS: Ob/Gyn residents with a pers

[Obstetrician-gynecologists and urologists perspective on the treatment of cystitis: the need for a multidisciplinary approach].

Cystitis in women is one of the most common urological diseases and requires a multidisciplinary approach. Depending on the stage of a womans life, her gynecological status, including hormonal levels, the condition of the vaginal microflora, and anatomical features, has a significant impact on the course of cystitis and its treatment. Underestimation of gynecological factors is sometimes a cause of treatment failure. The clinical features of cystitis in women of reproductive age and in perimenopause are discussed in the article. The need to assess estrogen status and vaginal flora are substant

An ethical issue in the prenatal and postnatal management of trisomy 18: a survey of obstetricians.

BACKGROUND: This study aimed to determine how obstetricians' knowledge of T18 influences their attitudes toward the antepartum, intrapartum, and postpartum management of T18 cases and to compare these attitudes with their approaches to trisomy 21 (T21) cases. These attitudes were analysed within the framework of the medical ethics principles of beneficence and autonomy. METHODS: This cross-sectional descriptive survey study was conducted between 1 February and 31 July 2021. Data from 194 of 388 participants who completed all questionnaire items were analysed. RESULTS: The study included spec

Pregnancy complications and loss: an observational survey comparing anesthesiologists and obstetrician-gynecologists.

OBJECTIVE: While there is increasing information regarding the occupational risks to pregnant physicians, there is inconsistent and limited subspecialty data. Physicians may be at increased risk for pregnancy complications due to occupational exposure, long work hours, nightshifts, and physical/mental demands. Additionally, little is known regarding the education physicians receive pertaining to pregnancy risks respective to their specialties as well as departmental/institutional support for pregnancy loss or complication. Therefore, a survey was developed and distributed across multiple acade

Perinatal Management of Pregnancies with Fetal Congenital Anomalies: A Guide to Obstetricians and Pediatricians.

BACKGROUND: Congenital anomalies are responsible for approximately 20% of all neonatal deaths worldwide. Improvements in antenatal screening and diagnosis have significantly improved the prenatal detection of birth defects; however, these improvements have not translated into the improved neonatal prognosis of babies born with congenital anomalies. OBJECTIVES: An attempt has been made to summarise the prenatal interventions, if available, the optimal route, mode and time of delivery and discuss the minimum delivery room preparations that should be made if expecting to deliver a fetus with a c

Mifepristone Use Among Obstetrician-Gynecologists in Massachusetts: Prevalence and Predictors of Use.

OBJECTIVES: We estimated the prevalence of mifepristone use for evidence-based indications among obstetrician-gynecologists in independent practice in Massachusetts and explored the demographic and practice-related factors associated with use. METHODS: We used data from a cross-sectional survey administered to Massachusetts obstetrician-gynecologists identified from the American Medical Association Physician Masterfile. We measured the prevalence of mifepristone use for four clinical scenarios: early pregnancy loss, medication abortion, cervical preparation before dilation and evacuation proc

Differences in obstetrical care and outcomes associated with the proportion of the obstetrician's shift completed.

BACKGROUND: Understanding and improving obstetrical quality and safety is an important goal of professional societies, and many interventions such as checklists, safety bundles, educational interventions, or other culture changes have been implemented to improve the quality of care provided to obstetrical patients. Although many factors contribute to delivery decisions, a reduced workload has addressed how provider issues such as fatigue or behaviors surrounding impending shift changes may influence the delivery mode and outcomes. OBJECTIVE: The objective was to assess whether intrapartum obs

ACOG Committee Opinion No. 772: Immunization Implementation Strategies for Obstetrician-Gynecologists.

Immunization against vaccine-preventable diseases is an essential component of women's primary and preventive health care. Many studies have shown that a recommendation from an obstetrician-gynecologist or other health care provider for a vaccine is one of the strongest influences on patient acceptance. Obstetrician-gynecologists and other health care providers should develop a standard process for assessing and documenting the vaccination status of patients and for recommending and administering vaccines. If allowed by state law, obstetrician-gynecologists and other health care providers are

Patterns of point-of-care test use among obstetricians and gynaecologists in the US.

UNLABELLED: Background Point-of-care tests (POCTs) for reproductive health conditions have existed for decades. Newer POCTs for syphilis, HIV and trichomonas are currently available and easy to use. We surveyed practicing obstetricians and gynaecologists to determine current POCT use and perceived obstacles to use. METHODS: Between June and August 2016, 1000 members of the American College of Obstetricians and Gynecologists were randomly selected and invited to complete a Qualtrics (222 West river Park Drive, Provo, Utah 84604, USA) survey; 600 of these were members of the Collaborative Ambul

Threats to reproductive health care: time for obstetrician-gynecologists to get involved.

To be healthy, support their families, and be productive members of their communities, women must have access to comprehensive reproductive health services including treatment of miscarriage and ectopic pregnancy and access to abortion, sterilization, and other contraceptive methods. However, in the United States, hospitals and legislative bodies are erecting barriers and limiting access to these basic health care services. These barriers are caused by factors such as hospital mergers (specifically those that are religiously affiliated); federal, state, and local legislation; hospital policies

von Hippel-Lindau disease and pregnancy: what an obstetrician should know.

BACKGROUND: von Hippel-Lindau disease is an autosomal dominant genetic disorder that presents with a variable constellation of symptoms and signs due to formation of hemangioblastomas and cysts. We address the management of pregnancies associated with this disorder from an obstetric perspective. CASE: A 26-year-old known to have type 2A von Hippel-Lindau disease with a significant surgical history secondary to disease manifestations was managed from early pregnancy to delivery. A survey of U.S. obstetrician-gynecologists' clinical and epidemiological knowledge of cryptosporidiosis in pregnancy.

Although cryptosporidiosis is frequently diagnosed in the U.S., there has been very little assessment of obstetrician-gynaecologist knowledge about this disease. In 2010, we surveyed U.S. obstetricians about the diagnosis, treatment and epidemiology of cryptosporidiosis. Data were examined through univariable analysis and multivariable regression models. Of 1000 obstetrician-gynaecologists surveyed, 431 (43.1%) responded. Only 44.4% of respondents correctly identified that prolonged, intermittent diarrhoea would lead them to consider cryptosporidiosis in a differential diagnosis. Routine ova a

Routine bimanual pelvic examinations: practices and beliefs of US obstetrician-gynecologists.

OBJECTIVE: Less-than-annual cervical cancer screening is now recommended for most US women, raising questions about the need for routine annual bimanual pelvic examinations. Little is known about clinicians' bimanual pelvic examination practices, their beliefs about its importance, or the reasoning underlying its performance in asymptomatic women. STUDY DESIGN: We conducted a nationwide survey of US obstetrician-gynecologists. Respondents (n = 521) reported their examination practices and beliefs based on vignettes for asymptomatic women across the lifespan. RESULTS: Nearly all obstetrician-

Obstetrician-gynecologists, religious institutions, and conflicts regarding patient-care policies.

OBJECTIVE: The purpose of this study was to assess how common it is for obstetrician-gynecologists who work in religiously affiliated hospitals or practices to experience conflict with those institutions over religiously based policies for patient care and to identify the proportion of obstetrician-gynecologists who report that their hospitals restrict their options for the treatment of ectopic pregnancy.
