[Atlanto-axial dislocation--instability index and indications for surgery (author's transl)].

Thirty cases of atlanto-axial dislocation were investigated and the results are reported. The may be divided into 2 groups; (a) 15 cases of abnormalities of the odontoid process, i. e. 7 cases of os odontoideum, 7 cases of fractured odontoid, 1 cases of deformed odontoid, and (b) 15 cases with no abnormalities of the odontoid process. Neurological signs were noted in 14 cases which may be divided into the following three types, i.e. acute, recurrent, and chronic-progressive. The spinal canal diameter at the atlanto-axial level was measured in the position of flexion and extension using cervica

The Endoscopic Endonasal Approach to Craniovertebral Junction Pathologies: Surgical Skills and Anatomical Study.

INTRODUCTION: Surgical anterior decompression is the treatment of choice for symptomatic irreducible ventral craniovertebral junction (CVJ) compression. Along with the classic transoral approach, the endoscopic endonasal approach has evolved and is gaining growing success. MATERIALS AND METHODS: In this work we discuss the surgical technique, give a complete step-by-step description of dissection of the craniovertebral junction and report a specific case of endoscopic endonasal odontoidectomy with use of a high-definition (HD) three-dimensional (3D) endoscope. DISCUSSION: The extended endona

Crowned dens syndrome presenting as pyrexia of unknown origin (PUO).

BACKGROUND: Deposition of calcium pyrophosphate crystals in the cervical spine around the odontoid process may lead to neck pain and fever. This condition is called crowned dens syndrome (CDS). CASE REPORT: An 89-year-old female presented with complaints of fever for one-month duration and recent onset neck pain. During her admission, she developed right knee pain with evidence of chondrocalcinosis on X-ray. Considering her clinical presentation in setting of pseudogout, she had a CT scan of her neck that revealed erosion of the dens and hyperdense soft tissue surrounding the odontoid process

Delayed subarachnoid hemorrhage following failed odontoid screw fixation.

Iatrogenic vascular injury is a rare but potentially devastating complication of cervical spine instrumentation. The authors report on a patient who developed an anterior spinal artery pseudoaneurysm associated with delayed subarachnoid hemorrhage after undergoing odontoid screw placement 14 months earlier. This 86-year-old man presented with spontaneous subarachnoid hemorrhage (Fisher Grade 4) and full motor strength on neurological examination. Imaging demonstrated pseudarthrosis of the odontoid process, extension of the odontoid screw beyond the posterior cortex of the dens, and a pseudoane

Pathological cervical fracture after spinal manipulation in a pregnant patient.

OBJECTIVE: To present the rare case of a displaced odontoid fracture after manipulative treatment. CLINICAL FEATURES: A 37-year-old, 15-week pregnant patient was referred with acute neck pain and a diffuse paravertebral swelling that started after cervical manipulation performed by her general medical practitioner 5 days before. Because of pregnancy, a cervical spine radiographic series was not obtained before treatment. Magnetic resonance imaging revealed a displaced odontoid fracture associated with a pathological process in the vertebral body of C2 and a paravertebral hematoma on the left

Severe traumatic atlantoaxial dislocation and type III odontoid fracture treated with occipitocervical fixation: a case report.

The combination of atlantoaxial joint dislocation accompanied by an odontoid process fracture is exceptionally rare, with only a few cases reported. The estimated frequency of these cases is < 2% of all upper cervical spine injuries. In this report, the authors describe an unusual case of traumatic atlantoaxial dislocation with a type III odontoid fracture in a 44-year-old male patient. Before the diagnosis, the patient had a history of seeking a masseur for a neck massage. Subsequently, the patient underwent occipitocervical stabilization to address the underlying condition. This procedure ai

Report of four cases of crowned dens syndrome: Clinical presentation, CT findings and treatment.

The clinical manifestations of crowned dens syndrome (CDS) include acute neck pain and neck stiffness accompanied by restricted cervical range of motion. CDS is frequently misdiagnosed as meningitis, epidural abscess, rheumatoid arthritis, rheumatoid polymyalgia, giant cell arteritis, cervical spondylosis or metastatic bone tumor, and the incidence of CDS appears to be underestimated. The present study reported on four cases of CDS diagnosed by CT. They included one male and three females, aged from 67 to 78 years, and their major symptoms were acute neck pain and restricted cervical range of

Transnasal and Transoral Approaches to Atlantoaxial Synovial Cysts: Report of 3 Cases and Review of the Literature.

BACKGROUND: Synovial cysts are cystic masses lined with pseudostratified columnar cells and containing clear or xanthochromic fluid. Although they are commonly encountered in the lumbar spine, synovial cysts infrequently occur in the cervical spine and rarely involve the odontoid process. The causes of synovial cysts of the odontoid process are unknown, but growth of synovial rests, proliferation of multipotent mesenchymal cells, atlantoaxial instability, and trauma are thought to play a role. CASE DESCRIPTION: We present 3 cases of atlantoaxial cysts with the associated radiographic features

Locally aggressive monostotic fibrous dysplasia of the cervical spine mimicking malignancy: a case report and literature review.

We report the case of a 30-year-old woman with histologically proven monostotic fibrous dysplasia of C2 revealed by a pathological fracture of the odontoid process. Radiological investigations showed a ground-glass mineralization of the vertebral body, a centimetric lytic area with poorly defined margins involving the inferior part of the vertebral body and inferior endplate and a fracture through an osteolytic area in the base of the odontoid process. Owing to the vertebral instability, a surgical procedure combining C0-C5 fixation and posterior bone grafting was performed. The surgical biops

Primary amyloidoma of the axis and acute spinal cord compression: a case report.

BACKGROUND: Localized primary amyloidosis is a disease characterized by a single tumor and localized amyloid deposit (amyloidoma) with no evidence of generalized amyloidosis. The occurrence of an amyloidoma in the spine is rare and only three cases affecting the axis have been previously reported. We describe the case of a 79-year-old woman presenting with a mass involving the odontoid process, responsible for an acute tetraparesia. Diagnosis of local primary amyloidosis was made after surgical excision. RESULTS: Despite the critical presentation, outcome was excellent after total excision of

[Atlantoaxial dislocation in neurofibromatosis.--Report of three cases--].

Atlantoaxial dislocation has received little attention in many studies of spinal deformity in neurofibromatosis. The only four cases of atlantoaxial dislocation associated with neurofibromatosis has been previously reported in the literature. We reported three rare cases of atlantoaxial dislocation associated with neurofibromatosis. These characteristics in roentgenogram were as follows; (1) marked narrowing of sagittal diameter at C1 level without instability (instability index 0%) (2) association with other mesodermal dysplasia, such as posterior vertebral body scalloping, vertebral body dys

Paget's disease and the nervous system.

Paget's disease usually is found in patients past the age of 40. Early presenting symptoms include headache, deafness, tinnitus, and pain due to radicular compression. The diagnosis is confirmed by radiographic features and elevated levels of serum alkaline phosphatase and urinary hydroxyproline. Bony overgrowth results in pressure on nearby soft tissues such as the brain, spinal cord, and certain peripheral nerves. The abnormally soft quality of the calvarial bone permits distortion by the weight of the brain. Dorsal inclination of the plane of the foramen magnum and the projection of the odo

Computer-assisted navigation of anterior odontoid screw fixation for type II odontoid fracture: case report and practical positioning technique.

BACKGROUND: Odontoid process fractures, particularly type 2 fractures, pose significant treatment challenges due to their high rates of nonunion. Anterior odontoid screw fixation (AOSF) is traditionally performed using percutaneous methods with biplanar fluoroscopy. Computer-assisted navigation has emerged as a promising tool to enhance surgical precision, but its application in AOSF is rarely utilized. Cervical spine stability during AOSF is crucial for optimal outcomes and navigational accuracy. CASE DESCRIPTION: A 64-year-old male presents with a displaced type 2 odontoid fracture followin

[Influence of Osseous and Ligamentous Injuries on the Stability of the Atlantoaxial Complex].

Background The odontoid process and the transverse ligament are the most important structures stabilising the atlantoaxial complex. It has not been fully elucidated how injuries of these structures contribute towards instability and a potential narrowing of the spinal canal. Therefore, our investigation aimed to perform a biomechanical analysis of spinal width and dislocation of the odontoid process depending on injuries of the aforementioned structures. Methods In 10 fresh human cadaveric specimens, physiologic flexion and extension were simulated under fluoroscopy in intact specimens and aft

[3D Fluoroscopy-Navigated Magerl Fusion of the 1st and 2nd Cervical Vertebra].

Background: Treatment of type II fractures of the odontoid process is still controversial. Besides conservative treatment, there are surgical options, including anterior screw fixation, as first described by Böhler and Magerl, or procedures using a dorsal approach. Many authors prefer dorsal fusion of C1 and 2, due to the reliable, biomechanical stability. In this context, pedicle screw fixation, as described by Harms, and transarticular screw fixation of the atlantoaxial joints, as described by Magerl, have to be mentioned. With the use of intraoperative fluoroscopic navigation, the risk of n

Novel Approach to Retro-Odontoid Pseudotumor Resection without Atlantoaxial Dislocation.

BACKGROUND Retro-odontoid pseudotumor is a fibrous or pannus mass behind the odontoid process, mainly caused by atlantoaxial instability, degeneration, or abnormal deposits behind the dens. Most scholars agree immobilization of the upper cervical spine can solve this problem; the mass can absorb spontaneously. However, retro-odontoid pseudotumor cases without atlantoaxial instability are limited, and it remains controversial whether the mass should be removed. CASE REPORT A 67-year-old man had intermittent neck pain for 10 years and numbness and weakness of the extremities for 1 month. Primary
