Hook plate fixation and tension band wiring in the treatment of Mayo type II olecranon fractures.

BACKGROUND: Olecranon fractures account for 8 ∼ 10% of all elbow fractures and usually require surgical intervention. Tension band wiring (TBW) is considered as the standard treatment while it is associated with high re-operation rates. OBJECTIVE: This study aims to compare the functional outcomes, complications and re-operations of hook plate fixation (HPF) versus TBW in treating Mayo Type II olecranon fractures. METHODS: A retrospective cohort study was conduct at Beijing Jishuitan Hospital. Medical records from May 2020 to April 2021 were reviewed. Functional outcomes, complications, and

Major trauma affecting the spine, chest wall and arm survived by a 9th/10th century CE individual from Rižinice Croatia.

This study investigates a series of antemortem injuries in the skeletal remains of a 36-45-year-old male from the medieval site of Rižinice. It presents the injuries comprising a fractured sternum, spinal hyperflexion, multiple rib fractures, and a non-union fracture to the olecranon process of the left ulna and a patella cubiti, a rare anomaly, which until now has not been reported in a paleopathological context. The research aims to uncover the causes, timing, and effects of these traumata, providing insight into the challenges of life in the mid-9th to mid-10th century.

Infantile dislocation of the elbow complicating obstetric palsy.

This is a report of three cases of complete dislocation of both the radius and ulna at the elbow in infants born with obstetric palsy. All three patients had painless elbow-flexion contractures. The pathologic anatomy included (a) medial and posterior displacement of the radius and ulna on the humerus, (b) laxity of the lateral capsule and collateral ligaments, (c) contracted medial capsule and collateral ligaments, (d) contracted biceps and triceps muscles, and (e) hypoplasia of the capitellum, trochlea, coronoid process, and olecranon fossa. Open reduction was attempted in all three of these

Multicentric reticulohistiocytosis.

We describe a 42-year-old man with a five-year history of arthritis mutilans-like destructive joint changes and with a one-year history of nodules on the fingers, ears, oral mucosa, pharynx, larynx, vocal cords, some being ulcerated and haemorrhagic. He was diagnosed as having rheumatoid arthritis; however, biopsies from the nodules on the oral mucosa and ear revealed multicentric reticulohistiocytosis. The large nodule over the olecranon process, simulating a rheumatoid nodule but diagnosed as multicentric reticulohistiocytosis with biopsy; ulcerated and haemorrhagic nodules on the oral mucos

Clinical Results of Treatment of Comminuted Olecranon Fracture With Free Articular Fragments Using the Embedded Subchondral K-Wire Fixation Along With Plate Fixation.

PURPOSE: Comminuted olecranon fractures with free articular fragments pose considerable surgical challenges because of anatomical complexity and instability of the fracture fragments. This study evaluated the functional and radiological outcomes of embedded subchondral K-wire fixation combined with plate fixation. METHODS: A retrospective analysis of 14 patients (mean age ± SD: 43 ± 17 years; eight men and six women) treated between 2013 and 2022 was conducted. Inclusion criteria comprised patients diagnosed with comminuted olecranon fractures requiring surgical intervention using embedded su

Coronoid fractures and traumatic elbow instability.

The coronoid process is key to concentric elbow alignment. Malalignment can contribute to post-traumatic osteoarthritis. The aim of treatment is to keep the joint aligned while the collateral ligaments and fractures heal. The injury pattern is apparent in the shape and size of the coronoid fracture fragments: (1) coronoid tip fractures associated with terrible triad (TT) injuries; (2) anteromedial facet fractures with posteromedial varus rotational type injuries; and (3) large coronoid base fractures with anterior (trans-) or posterior olecranon fracture dislocations. Each injury pattern is as

Difficult elbow fractures: pearls and pitfalls.

Complex elbow fractures are exceedingly challenging to treat. Treatment of severe distal humeral fractures fails because of either displacement or nonunion at the supracondylar level or stiffness resulting from prolonged immobilization. Coronal shear fractures of the capitellum and trochlea are difficult to repair and may require extensile exposure. Olecranon fracture-dislocations are complex fractures of the olecranon associated with subluxation or dislocation of the radial head and/or the coronoid process. The radioulnar relationship usually is preserved in anterior but disrupted in posterio

Validation of minimally invasive articular cartilage sparing technique for olecranon osteotomy.

BACKGROUND: The authors present a cadaveric validation of a minimally invasive articular cartilage preserving olecranon osteotomy technique for use in the operative management of distal humeral fractures. METHODS: Twenty-four elbows in six male and six female formaldehyde embalmed cadavers were dissected. With the cadaver placed in a lateral decubitus position, a posterior sub-periosteal dissection was performed to the medial and lateral aspects of the olecranon at the level of the joint and Mini Hohmann retractors were inserted into each side of the ulnohumeral joint. The medial (M) and the

Editorial Commentary: Olecranon Bone Autografting Combined With an Arthroscopic Approach for the Treatment Scaphoid Nonunion-An Important Technique for Wrist Surgeons.

Effective treatment of scaphoid pseudoarthrosis is critical to reduce the risk of progression to the potentially debilitating scaphoid nonunion advanced collapse, including complications of persistent wrist joint instability, degenerative arthritis, decreased range of motion, chronic pain, and functional impairment. Both anatomic and fracture-related pathophysiology predispose patients to scaphoid nonunion, including limited retrograde blood flow, fracture location, and delay of appropriate treatment. Recent studies have demonstrated successful outcomes in treatment of scaphoid nonunions, with

The Ulnar Olecranon-Coronoid Notch Angle Affects Terminal Elbow Extension in Children and Adolescents.

BACKGROUND: Limitations to terminal elbow extension (TEE) in pediatric populations have been commonly associated with the degree of ligamentous laxity and not bony factors. Ligamentous laxity, quantified through the Beighton score, is criticized for unreliably assessing joint mobility. This study aims to show that the olecranon-coronoid notch angle (OCNA) affects TEE in healthy children and adolescents. METHODS: A retrospective study of 711 pediatric patients treated for upper extremity and shoulder injuries was cross-sectionally studied at 2 tertiary centers from 2014 to 2021. Radiographs we

Tension-band wiring of displaced stable olecranon fractures with Eyelet-pins in the elderly: A series of 17 cases.

BACKGROUND: Tension-band wiring (TBW) and plate fixation are commonly used to fix displaced olecranon fractures. However, the high incidence of complications in the elderly, such as wound breakdown, infection, and loss of reduction, remain a concern for orthopaedic surgeons. Furthermore, patients frequently suffer from removal of the hardware. Even so, the operation seems to be indicated for independent elderly patients to return to their former activities of daily living. HYPOTHESIS: TBW of displaced stable olecranon fractures with Eyelet-pins in the independent elderly reduce the incidence

Triceps Tenotomy as an Alternative Exposure for Fixation of OTA/AO 13-C Fractures: A Technical Trick.

Intra-articular fractures of the distal humerus present challenges to treating physician and patient alike. The olecranon osteotomy is accepted as the standard exposure for intra-articular distal humerus fractures; nevertheless, complications such as nonunion and implant prominence are common. In this article, we describe the clinical outcomes and anatomic features of the triceps tenotomy as an alternative method of exposure for internal fixation of intra-articular distal humerus fractures. The olecranon osteotomy approach affords greater exposure of the distal humerus articular surface; howev

Triceps-Sparing Total Elbow Arthroplasty for Distal Humerus Fracture: The Lateral Paraolecranon Approach.

Distal humerus fractures in the elderly are a difficult problem to treat. Open reduction internal fixation has a high rate of complications, particularly because osteopenia compromises what is already tenuous fixation in a metaphyseal fracture. Total elbow arthroplasty is a more predictable outcome and easier recovery for these patients. However, most surgeons perform a low volume of total elbow arthroplasty. In addition, traditional exposure requires detachment of the triceps tendon. The lateral paraolecranon approach maintains the central tendon attachment to the olecranon while still facili

Compressive Ulnar Neuropathy Caused by Olecranon Bursitis and Concomitant Epidermal Cyst: A Case Report.

Epidermal cyst is a dermal or subcutaneous epithelial cyst that contains keratin and is lined by true epidermis. Although extremely rare, it can cause pathology including nerve compression syndrome. We report a rare case of ulnar nerve compression in the elbow that was caused by olecranon bursitis and concomitant epidermal cyst in a 67-year-old man. The ulnar nerve was immediately adjacent to the olecranon bursa and was significantly compressed. There was no connection between the tumor and the ulnar nerve. Pain, numbness, and weakness in his ring and little fingers disappeared after resection

Malpositioned olecranon fracture tension-band wiring results in proximal radioulnar synostosis.

BACKGROUND: Tension-band wiring (TBW) is a well-established fixation technique for two-part, transverse fracture types of the olecranon. However, complication rates up to 80 % are reported. By reporting on the enormous impact on the patient if failed the aim of the present report was to emphasize the importance of correct K wire positioning in TBW. CASE PRESENTATION: We present the case of a 49-year-old woman who suffered from a radioulnar synostosis of the forearm due to malpositioned K wires after TBW treatment. The patient was treated by heterotopic bone resection supported by ossification
