SPLIT rectus abdominis myocutaneous double free flap for extremity reconstruction.

A Mathes and Nahai type III muscle, such as the rectus abdominis muscle, can be utilized to cover two separate wounds simultaneously utilizing its dual blood supply thereby minimizing donor site morbidity and operative time. We report a case for treatment of bilateral Gustillo type IIIB lower extremity injuries treated with a single rectus abdominis muscle split into two free flaps, with one based on the deep inferior epigastric vessels and one on the superior epigastric vessels to cover the contralateral wound. In our patient, both lower extremity wounds were covered with muscle flaps from th

[Patient blood management (part 2). Practice: the 3 pillars].

Patient blood management (PBM) is a patient-specific multidisciplinary, multimodal, evidence-based concept to appropriately conserve and manage a patient's own blood as a vital resource. PBM is based on 3 pillars: the first is the optimization of the patient's endogenous red cell mass, the second is the minimization of bleeding and blood loss and the third involves harnessing and optimizing the patient-specific physiological tolerance of anemia, including adopting more restrictive transfusion thresholds. PBM primarily identifies patients at risk of transfusion and provides a management plan ai

Indications for and complications of transfusion and the management of gynecologic malignancies.

Anemia, which is highly prevalent in oncology patients, is one of the most established negative prognostic factors for several gynecologic malignancies. Multiple factors can cause or contribute to the development of anemia in patients with gynecologic cancers; these factors include blood loss (during surgery or directly from the tumor), renal impairment (caused by platinum-based chemotherapy), and marrow dysfunction (from metastases, chemotherapy, and/or radiation therapy). Several peri- and intra-operative strategies can be used to optimize patient management and minimize blood loss related t

Outcomes of Spinal Deformity Treatment in Asphyxiating Thoracic Dystrophy.

BACKGROUND: Asphyxiating thoracic dystrophy (ATD), or Jeune syndrome, is a rare skeletal dysplasia marked by a severe hypoplastic chest wall and restrictive lung disease, leading to an often-lethal thoracic insufficiency syndrome. Early-onset scoliosis (EOS) can complicate ATD, but details of operative management remain limited. This study aims to analyze the peri- and postoperative course of children with ATD who underwent posterior spinal fusion (PSF). METHODS: A retrospective review was conducted for all patients with ATD treated for EOS who underwent PSF at our institution. Preoperative s

Intraoperative blood salvage may shorten the lifespan of red blood cells within 3 days postoperatively: A pilot study.

BACKGROUND: Intraoperative blood salvage (IBS) recovers most lost blood, and is widely used in the clinic. It is unclear why IBS does not reduce long-term postoperative requirements for red blood cells (RBCs), and 1 possibility is that IBS affects RBC lifespan. METHODS: Prospectively enrolled patients who underwent spine, pelvic, or femur surgery not involving allogeneic RBC transfusion were grouped based on whether they received IBS or not. Volumes of blood lost and of RBCs salvaged during surgery were recorded. Total blood cell counts, levels of plasma-free hemoglobin, and CD235a-positive g

Comparison of results of autologous versus homologous blood transfusion in open-heart surgery.

BACKGROUND: The aim of this study was to determine a method to decrease the use of homologous blood during openheart surgery using a simple blood-conservation protocol. We removed autologous blood from the patient before bypass and used isovolumetric substitution. We present the results of this protocol on morbidity and mortality of surgery patients from two distinct time periods. METHODS: Patients from the two surgical phases were enrolled in this retrospective study in order to compare the outcomes using autologous or homologous blood in open-heart surgery. A total of 323 patients were incl

Benchmarking Surgeon Performance in Lower Extremity Revascularization Using a Composite Outcome Metric.

BACKGROUND: Peripheral artery disease (PAD) programs rely on consistent surgical outcomes for high-level limb-salvage performance. Despite the known influence of procedural volume, reliable surgeon-level benchmarking methods remain limited. The Limb Evaluation and Amputation Prevention Program (LEAPP) is a multidisciplinary limb salvage initiative at a large tertiary center. This study aimed to analyze surgeon performance and volume-outcome patterns using a composite quality assessment framework. METHODS: In this single-center study, 147 PAD patients (Rutherford 0-6) undergoing lower extremit

Societal costs of inappropriate emergency department thoracotomy.

BACKGROUND: Emergency department (ED) thoracotomy can be lifesaving. It can also lead to resource waste and exposure to blood-borne infections. We investigated the frequency with which ED thoracotomy was performed for inappropriate indications and the resulting societal costs. STUDY DESIGN: This retrospective cohort study examined all trauma patients admitted directly from the scene of injury from 1992 to 2009 who underwent ED thoracotomy. The main outcomes included inappropriate ED thoracotomy. Secondary outcomes included resource use and societal costs for performing ED thoracotomy for impr

Critical reassessment of second-look exploratory laparotomy for epithelial ovarian carcinoma. Minimal diagnostic and therapeutic value in patients with persistent cancer.

From 1979 to 1984, 88 women with epithelial ovarian cancer were treated with surgery and chemotherapy, achieved a clinical complete response, and then had "second-look" exploratory laparotomy to assess the pathologic status of their disease. Persistent cancer was found in 50 (57%) patients: 34 of 50 (68%) had gross tumor, which was larger than 2 cm in 12 (24%) and smaller than 2 cm in 22 (44%), and 16 (32%) had microscopic disease. Salvage therapy was as follows for these patients: whole abdominal irradiation, 29 (58%); chemotherapy, 17 (34%); intraperitoneal chromic phosphate, 1 (2%); and no

Primary K-pouch creation versus J-to-K pouch conversion: An IDEAL stage 3 report.

AIM: Conversion of a failed ileoanal J-pouch to a continent ileostomy (K-pouch) is an uncommon salvage option that may preserve continence and small-bowel length. Outcomes after J-to-K pouch conversion compared with primary K-pouch creation remain poorly defined. This IDEAL stage 3 study compared short- and long-term outcomes between these two approaches. METHOD: We performed a retrospective review of patients undergoing primary K-pouch creation or J-to-K pouch conversion at a tertiary referral centre between 2013 and 2025. Demographic, clinical and operative characteristics, 30-day postopera

Direct internal jugular vein incision for salvage of 'stuck' port catheters in pediatric leukemia: a single-center experience.

BACKGROUND: Totally implantable venous access ports (TIVAPs) are essential for long-term venous access in pediatric acute lymphoblastic leukemia (ALL). However, catheter-related calcified fibroblastic sheaths often make port removal difficult, and conventional traction may fail. Evidence regarding safe salvage surgery for calcified sheath-related stuck ports in children remains limited. This study aimed to investigate the etiology of difficult port removal and to evaluate the safety and feasibility of direct internal jugular vein (IJV) incision for fibroblastic sheath dissection and port extra

Posterior Lumbar Element Enforcement by Decompression Alone with Interspinous Fixation without Interbody Fusion for the Surgical Management of Lumbar Spondylolisthesis.

OBJECTIVE: In degenerative lumbar spondylolisthesis, interbody fusion surgery (IFS) has long been recommended as the gold standard of surgical management. However, IFS is less recommended for high-risk patients such as the elderly because it involves extensive surgery, with a long operation time and high volumes of blood loss, which lead to marked perioperative morbidity. We report an alternative primary and salvage treatment technique for high-risk lumbar spondylolisthesis through posterior lumbar element reinforcement using interspinous fixation and decompression alone without interbody fusi

[Concurrent vascular injuries in fractures in childhood].

Ten-year-analysis of blood vessel lesions present in eleven children with bone fractures revealed two major localizations: 1. In the lower extremity we observed vascular lesions in both open and closed fractures of femur and tibia. In cases with (complete) ischemia emergency operation had to be performed. 2. In the upper extremity the prevalent fracture localization was the supracondylar fracture of the elbow. Incomplete ischemia present in fractures with severe dislocation (Felsenreich III) could be salvaged by closed reposition and transcutaneous fixation by means of Kirschner wires. In thos
