Endoscopic hematoma removal of supratentorial intracerebral hemorrhage under local anesthesia reduces operative time compared to craniotomy.

The surgical efficacy for supratentorial intracerebral hemorrhage (ICH) remains unknown. We compared the advantages of the widely practiced endoscopic hematoma removal under local anesthesia with that of craniotomy under general anesthesia for ICH. We also focused on our novel operative concept of intentional hematoma leaving technique to avoid further damage to the brain. We retrospectively analyzed 134 consecutive patients (66 endoscopies and 68 craniotomies) who were surgically treated for supratentorial ICH. The characteristics of the 134 patients were as follows: The median (interquartile

Minimally invasive parathyroidectomy for primary hyperparathyroidism: decreasing operative time and potential complications while improving cosmetic results.

Primarily because of the lack of highly accurate preoperative localizing studies, the standard operation for primary hyperparathyroidism (HPTH) has consisted of bilateral neck exploration with examination of all four parathyroid glands. Recent experience with the technetium-99m-Sestamibi scan at our hospital has suggested that this single test was now accurate enough to allow unilateral neck exploration. This study was designed to examine the efficacy and safety of minimally invasive parathyroid-ectomy in select patients with a single adenoma demonstrated on a preoperative sestamibi. Eighteen

Analysis of savings in operative time for primary hyperparathyroidism using localization with technetium 99m sestamibi scan.

BACKGROUND: The cost effectiveness of preoperative localization in cases of primary hyperpara-thyroidism has not been established. We analyzed the potential savings in operative time after localization with technetium 99m (99mTc) sestamibi scan. METHODS: Thirty-three patients had localization of a solitary adenoma with 99mTc-sestamibi. Measurement was made of the time required for adenomectomy, unilateral neck exploration (UNE), unilateral neck exploration and confirmation of one contralateral parathyroid gland (UNEC), or bilateral neck exploration (BNE). RESULTS: The total operative time in

Fully parallel correction algorithm for real-time distortion-free TCSPC operation via direct dead time measurement.

Time-correlated single-photon counting (TCSPC) is a pivotal technique for time-resolved photonic applications requiring exceptional temporal resolution. Unfortunately, conventional TCSPC has long been confined to a low-speed regime due to pile-up distortion, commonly regarded as a statistical bias favoring the detection of early-arriving photons within each excitation cycle at high count rates. A recently introduced measurement strategy theoretically extends distortion-free TCSPC operation into the multi-photon regime by leveraging an auxiliary histogram alongside the conventional photon arriv

Effect of an additional neurosurgical resident on procedure length, operating room time, estimated blood loss, and post-operative length-of-stay.

Introduction: Neurosurgical residency training is costly, with expenses largely borne by the academic institutions that train residents. One expense is increased operative duration, which leads to poorer patient outcomes. Although other studies have assessed the effect of one resident assisting, none have investigated two residents; thus, we sought to investigate if two residents versus one scrubbed-in impacted operative time, estimated blood loss (EBL), and length-of-stay (LOS).Methods: In this retrospective review of patients who underwent a neurosurgical procedure involving one or two resid

Lightweight real-time detectors of apple-leaf diseases operating on embedded devices.

Agricultural leaf disease detection is crucial for early intervention and yield protection in precision agriculture. Among representative economic crops, such as apples, leaf lesions are typically small and appear in complex backgrounds, making accurate detection performed on resource-constrained embedded devices challenging. To address this, we propose a lightweight small-object detection models, namely the dynamic Differential Compensation Lightweight-YOLO (DCL-YOLO) model and its pruned version (DCL-YOLO-P), based on YOLO11n. A novel Dual-Aspect Feature Complementary Mapping (DAFCM) module

Multimodal neural operators for real-time biomechanical modelling of traumatic brain injury.

BACKGROUND: Traumatic brain injury (TBI) remains a major public health concern, with over 69 million cases annually worldwide. Accurate patient-specific biomechanical modeling is critical for injury risk assessment, but it requires integrating heterogeneous data sources such as volumetric neuroimaging, scalar demographic parameters, and acquisition metadata. Conventional finite element solvers can perform this modeling, yet they remain too computationally expensive for time-sensitive clinical settings. Neural operators have emerged as a promising alternative by learning resolution-invariant ma

Laparoscopic Heller Myotomy with Dor Fundoplication: An Operation that has Withstood the Test of Time.

BACKGROUND: Achalasia is a primary esophageal motility disorder characterized by aperistalsis and defective relaxation of the lower esophageal sphincter in response to swallowing. Patients' symptoms include dysphagia, regurgitation, weight loss, chest pain and aspiration. The disease is idiopathic, and the goal of treatment is to eliminate the resistance determined by the abnormal lower esophageal sphincter, therefore allowing passage of the ingested food from the esophagus into the stomach. Three effective treatment modalities are available today-pneumatic dilatation, peroral endoscopic myoto
