Photoreceptor layer elevation due to subretinal fluid: Impact on visual acuity measurements and simulation from biometrics.

PURPOSE: Visual acuity (VA) is a primary outcome measure that defines the success of clinical interventions for retinal diseases such as age-related macular degeneration (AMD) or diabetic macular oedema (DME). These conditions can lead to the presence of subretinal fluid, causing substantial photoreceptor layer elevation. Hyperopic defocus then occurs, affecting the VA measurements. In this study, we simulated the induced hyperopic shift for real-world values of photoreceptor layer elevation and measured the effect on VA measurements. METHODS: To simulate the hyperopic shift, we used a four-s

Measurement of in vivo lens shapes using IOLMaster 700 B-scan images: Comparison with phakometry.

PURPOSE: This study compared in vivo crystalline lens shape measurements using B-scan images from the IOLMaster 700 with phakometry. METHODS: Twenty-four young adult participants underwent IOLMaster 700 and phakometry measurements under cycloplegia (1% cyclopentolate). The IOLMaster 700 generated B-scan images along six meridians in 30° increments, which were analysed using custom MATLAB software to determine lens surface radii of curvature. Phakometry measurements were obtained using Purkinje images reflected from the lens surfaces. RESULTS: The IOLMaster 700 image analysis method yielded a

Development and evaluation of a deep neural network model for orthokeratology lens fitting.

PURPOSE: To optimise the precision and efficacy of orthokeratology, this investigation evaluated a deep neural network (DNN) model for lens fitting. The objective was to refine the standardisation of fitting procedures and curtail subjective evaluations, thereby augmenting patient safety in the context of increasing global myopia. METHODS: A retrospective study of successful orthokeratology treatment was conducted on 266 patients, with 449 eyes being analysed. A DNN model with an 80%-20% training-validation split predicted lens parameters (curvature, power and diameter) using corneal topograp

Estimating the astigmatic power of the crystalline lens and eye from ocular biometry.

PURPOSE: To estimate the astigmatic power of the crystalline lens and the whole eye without phakometry using a set of linear equations and to provide estimates for the astigmatic powers of the crystalline lens surfaces. METHODS: Linear optics expresses astigmatic powers in the form of matrices and uses paraxial optics and a 4 × 4 ray transfer matrix to generalise Bennett's method comprehensively to include astigmatic elements. Once this is established, the method is expanded to estimate the contributions of the front and back lens surfaces. The method is illustrated using two examples. The fi

Assessment of large-diameter and small-diameter SoftK specialty contact lenses for early-stage keratoconus.

INTRODUCTION: Soft contact lenses may be a good alternative for early-stage keratoconus (KC) patients who do not tolerate rigid gas permeable (RGP) lenses due to ocular discomfort or complications. This prospective study compared outcomes obtained after 2 weeks of wearing two types of soft silicone hydrogel contact lenses for keratoconus that varied in their diameter and central thickness (cc). METHODS: Patients with Amsler-Krumeich grades I or II KC were fitted with small-diameter (14.2 or 14.8 mm) SoftK (SD-SoftK, cc = 0.48 mm) and large-diameter (17 mm) SoftK (LD-SoftK, cc = 0.60 mm) lense

Perimetric visual field testing reveals deficits in contrast sensitivity in workers exposed to occupational levels of pesticides.

PURPOSE: This case-control study investigated whether defects in visual field contrast sensitivity are associated with exposure to occupational levels of pesticides in agricultural workers. METHODS: Twenty-seven individuals exposed to occupational levels of pesticides (exposed group) from 14 agricultural systems and 27 non-exposed individuals (non-exposed group) were measured using standard automated perimetry (30-2 full threshold). Visual sector analysis, targeting regions of the optic nerve head, was performed to examine for potential region-specific sensitivity differences. Participants al

Non-orthogonal spectacle correction for irregular astigmatism.

PURPOSE: To investigate the potential improvement in visual acuity and subjective perception of image quality in patients with keratoconus using non-orthogonal correction (NOC) cylinder trial lenses where the steep and flat power meridians are set at angles less or greater than 90°. METHODS: A set of NOC plano/cylindrical trial lenses, where the axes between the power meridians were set at a range of non-orthogonal angles, were used to refract 18 participants with keratoconus in whom 23 eyes were used for testing. Corneal elevation data were processed by bespoke MATLAB code from Pentacam Sche

Prevalence of interocular symmetry in corneal astigmatism and the possible influence of age, sex and refractive error.

PURPOSE: The aim of this study was to investigate, using a power vector approach, whether corneal astigmatism follows a mirror symmetry pattern considering both the magnitude and axis, and whether age, sex and spherical equivalent refractive error can influence the pattern. METHODS: The IOLMaster 700 optical biometer was used to measure the radii of curvature of the anterior corneal surface. Refractive error was determined by non-cycloplegic subjective refraction. Descriptive statistical analyses and inferential logistic regression were applied over the dichotomous variable of mirror symmetry

Can we really distinguish 'responders' from 'non-responders' to myopia control interventions?

PURPOSE: It is common to hear talk of 'responders' and 'non-responders' with respect to myopia control interventions. We consider the reality of distinguishing these sub-groups using data from the first year of the Low-concentration Atropine for Myopia Progression (LAMP) study. METHODS: The first year of the LAMP study was a robustly designed, placebo-controlled trial of three different low concentrations of atropine using a large sample size (N > 100 randomised to each group). The authors subsequently published mean axial elongation and myopia progression rates by age group. We used these da

Neural consequences of symptomatic convergence insufficiency: A small sample study.

INTRODUCTION: Convergence insufficiency (CI) is an oculomotor abnormality characterised by exophoria and inadequate convergence when focusing on nearby objects. CI has been shown to cause symptoms when reading. However, the downstream consequences on brain structure have yet to be investigated. Here, we investigated the neural consequences of symptomatic CI, focusing on the left arcuate fasciculus, a bundle of white matter fibres which supports reading ability and has been associated with reading deficits. METHODS: We compared the arcuate fasciculus microstructure of participants with symptom

Predicting perimetric defects from en face maps of retinal nerve fibre layer reflectance.

PURPOSE: To develop criteria to predict visual hemifields with deep perimetric defects based on retinal nerve fibre layer (RNFL) reflectance, in a transparent process whose components can be assessed by independent laboratories analysing data from their own small groups. METHODS: The analysis was carried out in four stages, using three independent groups of patients-30, 33 and 62 participants-with glaucoma and age-similar controls. The first stage used Group 1 to develop a criterion for RNFL reflectance images at 24, 36 or 48 μm below the inner limiting membrane (ILM). The second stage evalua

The Maddox rod: Revisiting the optics.

The Maddox rod is a staple in the assessment of strabismus. However, its optics mechanism is puzzling. How is it that an orthogonal line focus is formed? Surprisingly, few texts offer an explanation and those that do either provide insufficient detail, contain misleading information or are difficult to follow. There is a prevalent misconception that multiple cylinders are required for the orthogonal line focus to appear. However, this is untrue; Maddox himself only used a single cylinder when he first described the phenomenon. Herein, we review the optics explanations provided in a sample of t
