Intracranial hypotension and hypertension: reversible Chiari malformation due to dynamic cerebrospinal fluid abnormalities in Gorham-Stout disease. Case report.

Gorham-Stout disease (GSD) is an intractable disease characterized by massive osteolysis caused by abnormal lymphangiogenesis in bone. In rare cases of GSD, CSF abnormalities develop. The authors present the case of a 19-year-old woman with GSD presenting with orthostatic headache due to intracranial hypotension (5 cm H2O). The clinical course of this case was very unusual. Orthostatic headache was associated with a CSF leak from the thigh after pathological fractures of the femur and pelvis. The chronic CSF leak led to acquired Chiari malformation (CM) with syringomyelia. After an epidural bl

A New Case and Review of Chylothorax in Generalized Lymphatic Anomaly and Gorham-Stout Disease.

Generalized lymphatic anomaly (GLA) and Gorham-Stout disease (GSD) are related diseases involving the lymphatic vasculature. Patients with these diseases frequently develop chylothorax, which can cause respiratory distress, failure, and death. Unfortunately, the optimum treatment for GLA and GSD patients with chylothorax remains unknown. Here we review 64 previously reported cases of chylothorax in GLA and GSD and describe a GLA patient with bilateral chylothorax that was treated with a pleurovenous shunt after multiple other treatments failed. Unfortunately, this shunt was not able to control

Gorham disease of the lumbar spine with an abdominal aortic aneurysm: a case report.

BACKGROUND CONTEXT: Reports of Gorham disease of the lumbar spine complicated by abdominal aortic aneurysms are rare. PURPOSE: We herein report the case of a patient with Gorham disease of the lumber spine involving an abdominal aortic aneurysm (AAA). STUDY DESIGN: Case report. METHODS: A 49-year-old man had a 1-month history of right leg pain and severe low back pain. Plain lumbar radiography revealed an osteolytic lesion in the L4 vertebral body. Computed tomography images demonstrated the presence of an extensive osteolytic lesion in the L4 vertebral body and an AAA in front of the L4 ve

Progressive, massive osteolysis of mandible (Gorham's disease): report of a case.

A rare case of progressive osteolysis of mandible is discussed with clinicoradiological presentation. The purpose of the case report is to make the medical community aware of this rare and fascinating disorder in the maxillofacial region. The disease is characterised by spontaneous progressive osteolysis of one or more skeletal bones. Occurrence in maxillofacial region is rare with fewer than 35 reported cases affecting the facial and jaw bones. The present case report is about the mystery of bilaterally missing ascending rami. An attempt is made to draw attention of the medical fraternity to

PTEN hamartoma tumor syndrome and Gorham-Stout phenomenon.

PTEN: hamartoma tumor syndrome (PHTS) is a group of syndromes caused by mutations in PTEN. Gorham-Stout phenomenon (GSP) is a rare condition characterized by proliferation of vascular structures in bones, resulting in progressive osteolysis. Here we present a 1-year-old boy with PHTS and GSP. The lesion that later proved to be GSP was evident from the age of 4 months, and became symptomatic at the age of 1 year. Eventually, he developed a fatal chylothorax. Mutation analysis revealed a germline heterozygous mutation c.517 C>T (p.Arg173Cys) in exon 6 of PTEN. Analysis of the lymphatic malformat

Gorham disease in the maxilla.

Gorham disease is a rare condition that is characterized by the proliferation of thin-walled vascular channels associated with regional osteolysis. The exact etiology of Gorham disease is unknown. The diagnosis of Gorham disease is based on clinical, radiological, and histological features after excluding osteolysis, which is secondary to other pathologic processes. Those pathologic processes include congenital, metabolic, neoplastic, and immunologic etiologies and infections. The appearance of the disease in the craniofacial region often involves the mandible. In the reported literature (Engl

Idiopathic multicentric osteolysis: upper extremity manifestations and surgical considerations during childhood.

PURPOSE: Idiopathic multicentric osteolysis (IMO) is an uncommon disease presenting during childhood with resorption of the carpus and tarsus with nephropathy. The few case reports and literature reviews do not focus on the upper extremity disease manifestations or surgical treatment options. We review our experience with the upper extremity in IMO. METHODS: We evaluated 8 affected children, specifically assessing early disease manifestations, misdiagnoses, radiographic progression, and surgical treatments rendered. RESULTS: Wrist pain and swelling are typically the first manifestations of I

Hemangiomatosis associated with osteolysis of the mandible in a dog resembling Gorham-Stout disease in humans.

A 6-month-old female German Shepherd Dog died as a result of profuse oral bleeding. At postmortem examination, the oral cavity showed visible roots of the right mandibular fourth premolar and first molar teeth and, in addition, they were very mobile and compressible. Radiographs showed a generalized radiolucency in the body of the right mandible, with evidence of resorption of the affected alveolar bone. Histologically, the lesion of the right mandible was characterized by the lysis of bony structures and a non-malignant proliferation of blood-filled vascular spaces lined by a single layer of

Gorham's disease of the chest wall: CT and MR characteristics.

We report a case of Gorham's disease of the chest wall in a 29-year-old man. A chest radiograph showed osteolysis of the left first, second, and third ribs. CT and MR images revealed a soft tissue mass around the affected ribs. The mass presented as having heterogeneous high signal intensity on T2-weighted MR images, and it showed minimal enhancement on contrast-enhanced MR images. A histologic specimen obtained by incisional biopsy from the left third rib revealed proliferation of lymphangiomatous tissue in the bone marrow. A diagnosis of Gorham's disease was made based on these clinical, rad

Total osteolysis of the mandibular condyle in progressive systemic sclerosis.

This report calls attention to the complete resorption of the mandibular condyle in progressive systemic sclerosis (scleroderma), a previously unreported finding. This was associated with osteolysis of the ipsilateral coronoid process, both mandibular angles, and autoamputation of the fingertips. The Panorex provides a simple, effective method for studying the mandible in systemic sclerosis. Similar mandibular osteolysis with vinyl chloride exposure is noted.

Surgical Management of Gorham-Stout Disease in Cervical Compression Fracture with Cervicothoracic Fusion: Case Report and Review of Literature.

BACKGROUND: Gorham-Stout disease (GSD) or "vanishing bone" disease is characterized by progressive osteolysis with intraosseous lymphangiomatosis (hemangiomatosis). Given its rarity, with about 300 reported cases, its pathophysiology, etiology, and treatment guidelines are not established yet. CASE DESCRIPTION: A 22-year-old man was admitted to Severance Hospital with the chief complaint of neck pain from an injury due to falling. Initial cervical radiography showed a C4 burst fracture, and cervical magnetic resonance imaging revealed diffuse osteolytic lesions with coarse trabeculation with

18F-NaF and 18F-FDG PET/CT in Gorham-Stout Disease.

Gorham-Stout disease (GSD) is an extremely rare skeletal disorder of unknown etiology characterized by benign proliferation of vascular or lymphatic channels, leading to progressive bone resorption. We report on a patient diagnosed with GSD affecting the right scapula and the right ribs, who underwent PET/CT scans using F-FDG and F-NaF. The remnant upper portion of the affected scapula did not show F-FDG uptake but demonstrated markedly increased F-NaF activity. Furthermore, intense F-NaF activity was seen on the right posterior ribs, which were actively being resorbed, suggesting the potentia

Intraosseous CSF fistula in a patient with Gorham disease resulting in intracranial hypotension.

A 7-year-old girl with a history of headaches and Gorham disease was surgically treated in infancy for Chiari I malformation. Subsequent investigation revealed that her cerebellar tonsillar ectopia was due to a long-standing spinal CSF-lymphatic fistula causing intracranial hypotension. Percutaneous fistula closure was performed several times, resulting in transient symptomatic improvement.

Treatment of Gorham's disease with zoledronic acid.

Gorham's disease (GD) is a rare disorder characterized by spontaneous and progressive osteolysis of one or more bones and thought to belong to lymphangiomatoses spectrum of diseases. Surgical, radiation and medical therapies have been performed with variable and often discouraging outcomes and currently there is no recognized effective treatment. In this paper we describe a 24-year-old girl with GD localized to mandible who was effectively managed with zoledronic acid, a nitrogen-containing high-potency bisphosphonate.
