A patient who received treatment for acute myelogenous leukemia (chemotherapy, irradiation, corticosteroid therapy) developed cerebral radionecrosis with hemiplegia (treated by corticosteroids) then multiple foci of osteonecrosis on the nonparalyzed side. The mechanisms which may have prevented osteonecrosis from occurring on the side of the hemiplegia are discussed.

Osteonecrosis occurred in nine of thirty-six children following renal transplantation. The distal femoral condyle was the most common location but the femoral head was the most symptomatic and required total hip replacement in three of five patients. Total steroid dosage did not correlate with the development of osteonecrosis. There were no cases of osteonecrosis in patients under the skeletal age of ten years or in patients who did not have rejection reactions. In three children non-progressive focal lesions developed, similar to those of osteochondritis dissecans.

The authors examined a 55-year-old female who had received eight intravenous infusions of a potent bisphosphonate (BP) for metastatic bone disease. A mandibular extraction was subsequently performed. At presentation, infection with suppuration and sequestrum formation were observed in the area of extraction. Both the oncologist and dental practitioner must be made aware that when BPs are going to be administered, dental care should be performed in a timely fashion. Prophylactic measures must be taken by the dentist if dental surgery is required for a patient who has received BP.

BACKGROUND: A case of bisphosphonate related osteonecrosis of calvarial bone has not been reported.
METHODS: The clinical, imaging, and histopathological findings of the patient are presented. A 72-year-old female treated with bisphosphonates for osteoporosis presented with an open wound of her scalp. CT, MRI and bone scan suggested lytic and sclerotic changes with inflammation.
RESULTS: Surgical debridement was performed and final pathology was consistent with osteonecrosis and osteomyelitis.
CONCLUSIONS: This case illustrates that bisphosphonates can induce osteonecrosis of calvarial bone

UNLABELLED: This paper describes a previously unpublished side-effect of nicotine replacement chewing gum and presents useful information about the effects of nicotine on the oral environment. It is important to consider this when diagnosing an area of ulceration.
CLINICAL RELEVANCE: Patients who use nicotine chewing gum should be advised that prolonged contact with the alveolar mucosa should be avoided. They should be told that regular chewing helps dispersal of the nicotine.

Osteonecrosis (avascular necrosis, AVN, aseptic necrosis) is the consequence of the disturbance of blood supply to specific bone area. AVN occurs as a result of many different conditions such as fracture, thrombosis, mechanical vessel injury, fat embolism and many others. The most frequent location is the femoral head, but aseptic necrosis can also affect epiphysis of long bones and small bones of upper and lower limb. We reviewed the clinical records of 6 patients with osteonecrosis and human immunodeficiency virus (HIV) infection. We conducted a literature search for the putative mechanism by which this association may occur. None of these 6 patients had other known conditions associated with a predisposition to osteonecrosis. Osteonecrosis is usually considered to be a minor and rare late sequelae of treatment for Hodgkin's Disease (HD). However, over ten years six patients out of a total of 53 patients treated for HD at the haematology department of a district general hospital developed osteonecrosis, an incidence of 11.3%. Routine isotope bone scanning of patients produced two further asymptomatic cases. These results suggest a higher incidence than previously suspected of a problem which caused our patients considerable morbidity. Skeletal radiological surveys in 161 renal transplant recipients identified 36 patients with 115 lesions of osteonecrosis.
