Pitfalls of diagnostic laparoscopy in abdominal tuberculosis.

BACKGROUND: Diagnostic laparoscopy currently is emerging as an important tool in the diagnostic armamentarium for abdominal tuberculosis (TB). However, the laparoscopic view may be deceiving even to the most experienced eyes, and it is not uncommon for TB to be diagnosed erroneously before the final histologic confirmation is received. METHODS: A retrospective review of 20 diagnostic laparoscopies was conducted. The cases erroneously diagnosed at laparoscopy were collected and analyzed. RESULTS: Five cases (20%) were identified. For two patients, the laparoscopic findings were thought to be

Not only fat: omental infarction and its mimics in children. Clinical and ultrasound findings: a pictorial review.

Acute abdominal pain in children is the most common cause of emergency department admissions. Omental infarction is a rare cause of acute abdominal pain in this age group, accounting for approximately 15% of cases in children and 0.024-0.1% of cases of surgery for suspected appendicitis at the same age. Its clinical presentation may mimic similar diseases such as acute appendicitis, epiploic appendagitis, and mesenteric panniculitis. Ultrasound is the modality of choice for the initial evaluation of acute abdominal pain in pediatric patients and it can be used with confidence in the diagnosis

[Sclerosing mesenteritis diagnosed using computed tomography- and ultrasound-guided needle biopsy -utility of the coaxial technique].

Here, we report the case of an 82-year-old woman with sclerosing mesenteritis diagnosed using needle biopsy under the guidance of computed tomography (CT) and ultrasound (US). The patient manifested appetite loss, weight loss, and epigastric pain. CT of the abdomen and pelvis revealed increased density of the mesentery adjacent to the small bowel along with enlarged lymph nodes. Hence, we suspected sclerosing mesenteritis but also considered malignancies, such as lymphoma. We then performed CT- and US-guided needle biopsy with coaxial technique. We inserted an introducer needle by verifying it

18F-FDG PET/CT Findings in a Patient With IgG4-Related Sclerosing Mesenteritis.

A large mass in the abdomen was revealed by ultrasonography on a 68-year-old woman presenting with abdominal pain and weight loss. To exclude the malignancy, an F-FDG PET/CT was performed, which showed a solitary soft tissue mass with heterogeneous F-FDG avidity at the root of small bowel mesentery. Because of the elevation of inflammatory markers and serum IgG4 level, IgG4-related sclerosing mesenteritis was suspected, which was proved by the subsequent biopsy. The mass gradually shrunk and ultimately disappeared in 6 months after a low-dose steroid therapy.

Relationship between mesenteric abnormalities on computed tomography and malignancy: clinical findings and outcomes of 359 patients.

BACKGROUND: Mesenteric abnormalities are detected on abdominal computed tomography (CT) performed for various indications. GOALS: Determine the risk of malignancy on follow-up of patients with these abnormalities without a preexisting malignancy. STUDY: Data were collected on all patients at NorthShore University HealthSystem with abdominal CT scan reports of mesenteric abnormalities labeled as "panniculitis" from January 2005 to April 2010. RESULTS: Three hundred fifty-nine patients were identified, 81 (22.6%) had a known malignancy at the time of the index abdominal CT scan. Nineteen (6.8

Ultrasound aspect of mesenteric panniculitis. Case report.

Mesenteric panniculitis represents a cronic inflammation of the adipose tissue of the mesentery which can present sometimes with the imagistic aspect of one or multiples pseudotumors. We present the case of female patient in which mesenteric panniculitis was found accidentally during a routine abdominal ultrasound examination leading to differential diagnostics problems. Color Doppler and contrast enhancement ultrasonography were helpful for diagnosis evidencing a benign circulatory signal.

Sclerosing mesenteritis.

Sclerosing mesenteritis is a rare benign process involving mesenteric fat that has non-specific histology and imaging characteristics, which are critical to the diagnosis. It has two different pathological variants: mesenteric panniculitis (acute or subacute) and retractile mesenteritis (chronic). This case study illustrates the radiological findings of sclerosing mesenteritis, describes certain signs that suggest the diagnosis and reviews recent published literature.

[Retractile mesenteritis after resection of Meckel's diverticulum].

HISTORY AND ADMISSION FINDINGS: A 22-year-old man who had never been seriously ill previously was admitted because of epigastric pain and vomiting of bile. INVESTIGATIONS: He had slight pain on pressure over the epigatric region and decreased intestinal peristaltic sounds. There was evidence of ileus of the small intestine both by ultrasound and radiologically. TREATMENT AND COURSE: As the patient's condition deteriorated on conservative treatment, an exploratory laparotomy was performed. It revealed an invaginated Meckel's diverticulum, Ileus of the small intestine recurred postoperatively,

Sclerosing mesenteritis: imaging findings in 17 patients.

OBJECTIVE: The purpose of this study was to evaluate the radiologic findings of sclerosing mesenteritis and to describe certain signs that suggest the diagnosis. CONCLUSION: Sclerosing mesenteritis represents a single disease with two radiologically different variants. In an appropriate clinical setting, radiologic features can suggest the diagnosis, delineate the extent of the process, and prescribe or limit surgical procedures. The "fat ring" sign and the presence of a tumoral pseudocapsule are important findings for the diagnosis of the disease.

[X-ray computed tomographic aspects of mesenteric panniculitis].

Mesenteric panniculitis also known as lipodystrophy is an inflammatory condition of adipose tissue. A case of a 55 year old, female who presented a weight loss and a tender palpable mass located in the left flank is reported. CT patterns were not specific but might suggest the diagnosis: fatty mass, with a higher density than subcutaneous fat, located in the root of the mesentery and surrounding mesenteric vessels without distoting them. Intestinal loops were only pulled in periphery. This signs are not always present and even in this typical pattern mesenteric panniculitis could not be differ

Mesenteric panniculitis resulting in bowel obstruction: response to steroids.

Two patients with mesenteric panniculitis are presented. The first developed obstruction of the sigmoid colon requiring a decompressing proximal sigmoid end-colostomy; the second presented with an incomplete small bowel obstruction. A convincing, immediate, symptomatic response to steroids was noted in both patients. The first patients responded to such an extent that closure of the colostomy was possible some 10 months later. It would appear that active subacute mesenteric panniculitis, as evidenced by continuing fever, high erythrocyte sedimentation rate and predominance of inflammatory cell

Recognizing the Ultrasound Patterns of Mesenteric Panniculitis.

Mesenteric panniculitis (MP) is a rare, benign, and nonspecific chronic fibrosing inflammation of the mesenteric adipose tissue. Abdominal pain is the most frequent presentation. The objective was to recognize ultrasound (US) features in MP. A retrospective 9-year review of computed tomography- (CT) or US-diagnosed cases of MP recorded in the radiology department was performed. The inclusion criteria included an US recorded in PACS imaging data base within 10 days before CT, to compare and evaluate the correlation of MP diagnosis. The prevalence of MP on CT was 2.8%. The 120 selected patients ha

The Use of Glucocorticoids for Better Control of Diabetes Mellitus: The Paradox of Sclerosing Mesenteritis (The Rare Could Become Common).

BACKGROUND Sclerosing mesenteritis is a rare disease characterized by chronic inflammation of mesenteric adipose tissue. To our knowledge, this is the first case report that presents the effects of glucocorticoid therapy on metabolic control in diabetes mellitus, aggravated by sclerosing mesenteritis. We want to show the significance of this rare disease, which could be underestimated as a cause of decompensation of diabetes mellitus. CASE REPORT A 57-year-old man with diabetes type 2 was admitted to the hospital to obtain better metabolic control of this disease. In addition, he reported pers

Sclerosing Mesenteritis and Disturbance of Glucose Metabolism: A New Relationship? A Case Series.

BACKGROUND: Sclerosing mesenteritis is an idiopathic inflammatory and fibrotic disease that affects the mesentery. It is a rare disease, with the total number of reported cases in the literature ranging from 122 to 300. It mainly affects men in the sixth decade of life, and its etiology remains unknown. Clinical presentation is variable, but it is frequently asymptomatic. Diagnosis is often made by computed tomography (CT) scan, although biopsy may be needed for confirmation. An association between other diseases (e.g., neoplasms) and sclerosing mesenteritis has been described, but the relatio

Extensive mesenteric lipodystrophy of the left colon: case report and brief review of the literature.

Mesenteric lipodystrophy is a rare inflammatory process that predominantly affects mesenteric adipose tissue of the small bowell. Several mechanisms have been suggested as responsible for this entity although the precise etiolog remains unknown. The diagnosis is based on CT or MRI imaging and generally confirmed by surgical biopsies. Treatment is individualized and empiric and depends on disease stage and symptoms. We report a case of a 35-year-old male who was admitted to our hospital with a history of abdominal pain, constipation and a palpable mass in the left lower quadrant. Abdominal CT s
