BACKGROUND: We report a case of an elderly patient who was diagnosed with lateral atlantoaxial subluxation with type II odontoid fracture, an extremely uncommon upper cervical spine injury that has not been previously reported in the literature to the knowledge of the authors.
METHODS: An 87-year-old male reported to the emergency room following a syncopal episode after sustaining a fall. He complained of dizziness, fatigue, disruption of vision and audition, and worsening neck pain. Computed tomographic scans were positive of partial dislocation of the C1 relative to C2 and chronic fracture

High body mass index (BMI) patients undergoing autologous breast reconstruction with the deep inferior epigastric perforator (DIEP) flap face the increased risk of wound healing complications, fat necrosis, and donor-site morbidity. While postoperative outcomes are well documented, the intraoperative technical challenges of flap harvest in this population are underreported. The heavy abdominal pannus complicates exposure, often necessitating experienced assistance and prolonging operative times. We describe a reproducible, single-operator DIEP flap harvest technique designed to overcome these

OBJECTIVE: One of the pathologic hallmarks of juvenile rheumatoid arthritis (JRA) is a tumor-like expansion of inflamed synovial tissue, or pannus, which causes much of the joint damage in this disease. The expansion of pannus is supported by extensive formation of new blood vessels. We have previously shown that revascularization of minced JRA synovial tissues engrafted into SCID mice correlated with the intensity of inflammatory activity in the tissues and with interleukin-15 (IL-15) expression. Since synovial vascular endothelial cells (VECs) expressed IL-15 receptors, the present study was

PURPOSE: Molecular pathogenesis underlying persistent ocular surface inflammation in chronic Stevens-Johnson syndrome (SJS) still remains largely unexplored. The present study investigates the expression of matrix metalloproteinase 2 (MMP2), MMP3, MMP9, MMP11 and TIMP1 (tissue inhibittor of matrix metalloproteinase 1) in pannus tissues of chronic ocular SJS undergoing cultivated oral mucosal epithelial transplantation (COMET) and their prognostic relevance.
METHODS: In this prospective study, 45 eyes with chronic SJS underwent COMET for visual and anatomical rehabilitation. Preoperative and p

Chondrogenic dwarfism in Morquio disease (mucopolysaccharidosis IV) has been suggested to be strongly linked to the abnormal lysosomal storage of cartilaginous extracellular matrix waste products within chondrocytes and fibroblasts. The specific genetic defects of enzymes of the keratan sulfate and chondroitin-6-sulfate metabolism have been detected at the molecular level and importantly contributed to the current knowledge on the phenotype of this rare metabolic disorder. However, the pathogenesis of this epiphyseal centered progressive skeletal disease does not seem to be fully explained by

To determine whether magnetic resonance (MR) imaging can improve assessment of the extensor tendons of the wrist and their propensity to rupture, the dorsal tendons in 23 wrists in 20 patients with rheumatoid arthritis were clinically assessed and then examined with MR imaging. Clinical and MR imaging evaluations were performed blindly and independently. The six extensor tendon compartments were assessed with MR imaging for presence of pannus, synovial effusion, and tendon involvement or rupture. Clinical scores of systemic disease, dorsal tendon sheath swelling, and risk of tendon rupture ran

We present a case of cardioembolic stroke in a patient with a history of mechanical aortic valve who was compliant with anticoagulation medication. Cardiac computed tomography was used as an alternative, noninvasive means of evaluation for the cardioembolic source of stroke and identified subvalvular mobile pannus of the mechanical aortic valve.

The use of immunosuppressive therapies for rheumatoid arthritis (RA) is associated with substantial adverse effects. In addition, existing diagnostic reagents are limited in their ability to detect local joint damage. Here, we address both these needs using a collagen-hybridizing peptide (CHP) that selectively binds denatured collagen present in inflamed joints in RA models. Light-sheet scanning and immunohistology of the inflamed ankles in RA mice pre-injected with fluorescently-labeled-CHP revealed specific CHP accumulation within matrices of the synovial pannus and cartilage that express ma

AIM: Assessing the activity of synovitis, which is characterized by an increase in cell metabolism, is important for the prediction of future articular destruction in clinical and preclinical studies. To evaluate the correlation between ¹⁸F-FDG accumulation and arthritis pathology during its establishment, we used microPET to evaluted ¹⁸F-FDG accumulation in vivo during rat Mycobacterium wall-induced knee arthritis.
METHODS: ¹⁸F-FDG PET images of arthritic rats were acquired on days 1, 2, 3 and 7 after arthritis induction. The subjects (n=2/time) were subsequently subjected to macro-autoradio

We report a case of simplified surgical conversion from a prior mechanical composite valved conduit to a bioprosthetic aortic valve using a leaflet fracture technique. A 69-year-old man presented with progressive heart failure 7 years after aortic root replacement for bicuspid aortic stenosis and root aneurysm. Imaging revealed severe prosthetic valve stenosis and suspected pannus formation. Given significant comorbidities, a simplified approach was chosen to avoid full root re-replacement. Following redo sternotomy and graft incision, the mechanical valve leaflets were fractured and removed.

Synovial tissue in the hand and wrist is frequently involved by inflammatory, degenerative, infectious, traumatic, metabolic, or neoplastic lesions. Although normal synovium is not shown on MR images, the morphology and signal characteristics of abnormal synovium is generally well demonstrated. Special MR techniques for accurate quantification of pannus are under investigation.

OBJECTIVE: To demonstrate a technique for performing panniculectomy and skin graft for a patient with adult buried penis, highlighting a number of technical modifications that allow for the procedure to be performed as an outpatient.
METHODS: Pannus is mobilized with a modified trapezoid incision, leaving superior attachments intact for skin graft harvest. Diseased penile skin is removed, defect is measured, and an appropriately sized split thickness skin graft is harvested from the pannus in 2-inch sections at a depth of 18/1000. The unmeshed graft is applied to the penis and covered with a

Intraarticular steroid therapy in juvenile chronic arthritis (JCA) is performed because of high local efficacy with few side effects. Imaging is used for initial evaluation and for monitoring of treatment response. The aim of this study was to compare imaging findings in diseased hips and knees before and after therapy. A prospective study was performed on 10 patients (15 joints) scheduled for intraarticular therapy. Pretherapeutic assessment included clinical work-up, radiographs, ultrasound (US), and magnetic resonance imaging (MRI) of affected joints. Following therapy, clinical and sonogra
