Transcatheter coil embolization of the inferior epigastric artery in a huge abdominal wall hematoma caused by paracentesis in a patient with liver cirrhosis.

Therapeutic paracentesis is considered to be a relatively safe procedure and is performed commonly for the control of massive ascites in patients with liver cirrhosis. The commonest puncture site, approximately 4 or 5 cm medial of left anterior superior iliac spine, can be located across the route of the inferior epigastric artery, which is one of the sites of potential massive bleeding. In a 46-year-old woman with liver cirrhosis and refractory ascites, a huge abdominal wall hematoma developed after therapeutic paracentesis. The patient was not stabilized by conservative treatment, and inferi

Low-Dose vs. Standard Care Iv Human Albumin During Large-Volume Paracentesis in Patients With Liver Cirrhosis: A Systematic Review.

Cirrhosis is a global health concern, with ascites being the most prevalent decompensating event. Intravenous (IV) human albumin (HA) is administered during large-volume paracentesis (LVP) to prevent complications. The standard (6-8 g HA/L ascites) is empirically chosen, potentially leading to overtreatment and added costs. We reviewed evidence comparing the efficacy and safety of reduced versus standard-care HA dosing during LVP. We searched PubMed, EMBASE, Cochrane Library, CINAHL, and Scopus (up to January 31, 2025). We included RCTs and cohort studies in adults with cirrhosis undergoing LV

B-type natriuretic peptide (BNP) predicts 90-day mortality and need for paracentesis in cirrhotic patients without systolic heart failure.

Fluid overload is a common complication in patients with cirrhosis. B-type natriuretic peptide (BNP) is a marker of increased blood volume, commonly used in heart failure, that has been shown to be elevated in patients with liver disease. This study examined if BNP levels can be used to determine prognosis and predict worsening of ascites in patients with cirrhosis without concomitant heart disease. A retrospective study was performed at a large urban hospital in Chicago, Illinois and included 430 patients with cirrhosis who had BNP levels ordered during their hospital stay. Patients with clin

Abdominal paracentesis drainage attenuates intestinal inflammation in rats with severe acute pancreatitis by inhibiting the HMGB1-mediated TLR4 signaling pathway.

BACKGROUND: Our previous studies confirmed that abdominal paracentesis drainage (APD) attenuates intestinal mucosal injury in rats with severe acute pancreatitis (SAP), and improves administration of enteral nutrition in patients with acute pancreatitis (AP). However, the underlying mechanisms of the beneficial effects of APD remain poorly understood. AIM: To evaluate the effect of APD on intestinal inflammation and accompanying apoptosis induced by SAP in rats, and its potential mechanisms. METHODS: SAP was induced in male adult Sprague-Dawley rats by 5% sodium taurocholate. Mild AP was ind

A retrospective study on the safety of small and large volume paracentesis and risk factors for adverse events in patients with gastrointestinal cancer with malignant ascites.

BACKGROUND: The optimal drainage volume for paracentesis in malignant ascites (MA) has not been established, and identifying risk factors for paracentesis may serve as a guideline for clinical decisions regarding drainage volume in MA patients. Therefore, the aim of this study is to compare the safety of small-volume drainage (SVD) versus large-volume drainage (LVD) in MA patients and to identify risk factors for adverse events (AEs). METHODS: This single-center retrospective study analyzed gastrointestinal cancer patients who underwent paracentesis between 2014 and 2023. Patients were catego

Ascitic fluid filtration and intravenous infusion versus total-volume paracentesis with infusion of plasma expander in cirrhosis with tense or intractable ascites.

BACKGROUND: Patients with cirrhotic ascites have low serum albumin levels, and paracentesis of ascitic fluid could compromise them further. AIM: We compared the therapeutic efficacy of ascitic fluid filtration and concentrate infusion (AFI) versus total-volume paracentesis (TVP) with colloid infusion in control of tense or intractable cirrhotic ascites. METHODS: Ten patients underwent AFI; their ascitic fluid was filtered repeatedly through hollow-fiber hemodialyzer, and the concentrate reinfused intravenously. In ten patients TVP was done with simultaneous intravenous colloid infusion. Foll

Timely Ultrasound-Guided Thoracentesis and Paracentesis for Refractory Dyspnea and Abdominal Distension in Terminally Ill Cancer Patients Receiving Home Hospice Care.

BackgroundPleural effusion and ascites frequently cause refractory dyspnea and abdominal distension in patients with advanced cancer. Although ultrasound-guided thoracentesis and paracentesis are established procedures, accessing hospital- or outpatient-based drainage may impose substantial burdens on patients near the end of life. We evaluated the efficacy and safety of timely home-based ultrasound-guided thoracentesis and paracentesis for symptom relief in terminally ill patients with cancer receiving home hospice care.MethodsWe retrospectively analyzed prospectively collected clinical data

Delayed Diagnostic Paracentesis Is Associated with Increased Preventable Healthcare Utilization in Disadvantaged Patient Populations with Advanced Liver Disease and Elevated INR.

BACKGROUND: Patients hospitalized with cirrhosis, ascites, and elevated INR often experience delays in timely diagnostic paracentesis. AIMS: Identify whether delays in diagnostic paracentesis were associated with adverse outcomes in a hospital system serving a large disadvantaged population. METHODS: Retrospective cohort analysis of patients admitted from January 2017 to October 2019 with cirrhosis, ascites, and INR ≥ 1.5 across a multi-hospital health system in central Texas. We examined demographic and clinical characteristics of patients with diagnostic paracentesis (1) ≤ 24 h; (2) > 24 h

Randomized comparative multicenter study of hydroxyethyl starch versus albumin as a plasma expander in cirrhotic patients with tense ascites treated with paracentesis.

OBJECTIVE: Large-volume paracentesis associated with plasma volume expansion with albumin is an effective, safe, but costly therapy for ascites in patients with cirrhosis. The aim of this study was to compare the use of a synthetic plasma expander, hydroxyethyl starch (HES), with that of albumin. DESIGN: Sixty cirrhotic patients with ascites were studied. Patients were randomly assigned to be infused with either albumin (8 g/l of ascites removed, n = 33) or HES (200 ml/l of ascites removed, n = 27). None of the patients was treated with diuretics or had renal impairment or hyponatremia at ent

Medicines in solution or given by aerosol therapy led sometimes to surprising outcomes when dealing with chronic stubborn middle ear infections withstanding conventional treatments. Besides, the flow of air, as well as radio-opaque drugs, introduced this way, aid both the examination and the inventory of the actual state of the tub

Patients who underwent TIPS face increases in hernia incarceration rates compared to patients managed with paracentesis: An analysis of prevalence and risk factors.

TIPS is commonly used to treat complications of portal hypertension. Hernia incarceration is a complication of the TIPS procedure that is poorly understood and not well documented. We examine this complication in a large, single-center cohort of TIPS patients and compared it to a cohort of patients with refractory ascites managed with serial large-volume paracentesis alone to determine whether TIPS itself increases incarceration risk. We identified 259 adults who underwent TIPS and 644 adults with refractory ascites managed with serial large-volume paracentesis at the University of California,
