Silence in interview: psychiatry and the Argent conditions.

A man charged with serious sexual offences was advised by his solicitor not to answer the questions which were put to him during the police interview and in the absence of an appropriate adult whose assistance he had declined. At his trial, counsel for the prosecution wanted the jury to be allowed to draw an adverse inference from his silence. Psychiatric evidence was admitted to the effect that he had a paranoid personality disorder. On the basis of the ruling in R v. Argent 1997, in which grounds were set out for not allowing a jury to draw an adverse inference, it was submitted that the jur

Antisocial Personality Traits as a Risk Factor of Violence between Individuals with Mental Disorders.

BACKGROUND: Mental disorder can increase the likelihood of taking violent acts of some individuals, but only a small percentage of violence in societies could be attributed to patients with mental health problems. For the past several years numerous studies related to forensic psychiatry has confirmed a close causal relationship between violent offenders and psychiatric comorbidity. Several studies have provided strong evidence that antisocial personality disorders (APD) represent a significant clinical risk for violence. AIM: This study aims to show the relationship between antisocial person

[Liaison psychiatry in a general hospital: seven paradigmatic cases].

Through the use of case reports, this article reviews frequent causes that origin the need for psychiatric intervention in patients hospitalized in medical and surgical wards. Particular diagnosis aspects are focused, so is the necessity of integration of the biological, psychological and social dimensions of the patient. The integrated approach by the various members of the medical staff is also emphasised. The cases presented were observed in the Liaison Psychiatry Consult of the Psychiatry Service of Hospital Santa Maria, in Lisbon, between November 2007 and January 2008. Seven cases were s

Diethylpropion and psychosis.

Five female patients who developed psychosis while taking diethylpropion hydrochloride are described, four with paranoid psychosis and one with manic psychosis. In all but one patient, these drugs were medically prescribed. Although relatively few cases of psychosis and psychosis-like illness were reported to the Committee for Safety of Medicines from 1963 until 1986, further cases may be anticipated. Patients on the drug should be carefully supervised and it should not be prescribed to those with personality disorder, previous evidence of amphetamine, alcohol, or other drug abuse, or those wi

A follow-up study of post partum illness, 1946-1978.

Eighty-two patients, who were treated for post partum illness between 1946 and 1971, were identified and followed up. Diagnostically, the sample comprised unipolar depression (52%), bipolar disorder (18%), schizophrenia (16%), abnormal personality with depression (8%), organic disorder (2%), and obsessional state with depression and paranoid disorder (1% each). The overall prognosis was good, except for schizophrenia, in which more than 50% of patients had chronic disability. Further childbirth intensified, and caused deterioration of, the underlying schizophrenia process. Following an initial

Moral injury in psychiatric patients with personality and other clinical disorders: development, psychometric properties, and validity of the Moral Injury Events Scale-Civilian Version.

Background: Moral injury emerges when someone perpetrates, fails to prevent, or witnesses acts that violate their own moral or ethical code. Nash et al. [(2013). Psychometric evaluation of the moral injury events scale. Military Medicine, 178(6), 646-652] developed a short measure, the Moral Injury Events Scale (MIES) to facilitate the empirical study of moral injury in the military. Our study aimed to develop a civilian version of the measure (MIES-CV) and examine its psychometric properties in a sample of psychiatric inpatients .Methods: In this cross-sectional study, the sample comprised 24

Conduct disorder and adult psychiatric diagnoses: associations and gender differences in the U.S. adult population.

The authors' objective was to examine the presence of Axis I and II psychiatric disorders among adult males and females with a history in childhood and/or adolescence of conduct disorder (CD). Data were derived from a large national sample of the U.S. population. Face-to-face interviews of more than 34,000 adults ages 18 years and older were conducted during 2004-2005 using the Alcohol Use Disorder and Associated Disabilities Interview Schedule-DSM-IV Version. After adjusting for sociodemographic characteristics and psychiatric comorbidity, CD was associated with all Axis I and II disorders, p

Childhood maltreatment and personality disorders in patients with a major depressive disorder: A comparative study between France and Togo.

Few studies have examined the association between childhood maltreatment (CM) and personality disorders (PDs) in adulthood in two different cultural contexts, including sub-Saharan Africa. The aims of this study were to compare the frequency of CM between patients in treatment in France and Togo for a major depressive disorder (MDD), to explore the link between CM and PDs, and to examine the mediating effect of personality dimensions in the pathway from CM to PDs in 150 participants (75 in each country). The 28-item Childhood Trauma Questionnaire, the International Personality Item Pool, and t

Delusional misidentification syndromes in obsessive-compulsive disorder.

Delusional misidentification syndromes (DMS) have been rarely reported in patients with conditions other than schizophrenia-related disorders, diffuse brain disease (dementia) and focal neurological illness. In this report, we describe DMS (i.e. Capgras and Fregoli syndromes) in two patients with severe and treatment resistant obsessive-compulsive disorder (OCD), one with paranoid personality disorder (PPD) and the other with a pervasive developmental disorder (PDD) not otherwise specified. While our findings highlight an interesting phenomenon (the occurrence of DMS in OCD), it is presently u

The SCID-II and DIB-R interviews: diagnostic association with poor outcome risk factors in Borderline Personality Disorder.

This study assesses whether patients diagnosed with Borderline Personality Disorder (BPD) according to the Structured Clinical Interview for DSM-IV Axis II Disorders (SCID-II) or the Revised Diagnostic Interview for Borderlines (DIB-R) present differences in factors associated with risk of poor outcome. Three hundred fifty-two patients were evaluated with SCID-II and DIB-R. Patients diagnosed as BPD according to one or both instruments were compared in BPD poor outcome risk factors. The analysis was conducted on the participants who were assigned to SCID-II (n = 135) and SCID-II/DIB-R (n = 126

Antisocial personality disorder with childhood- vs. adolescence-onset conduct disorder: results from the National Epidemiologic Survey on Alcohol and Related Conditions.

This study, based on a nationally representative, epidemiologic sample (N = 43,093, response rate 81%), compared sociodemographic and family history correlates, antisocial personality disorder (ASPD) symptom patterns, and Axis I and Axis II comorbidity, among adults with DSM-IV ASPD who reported onset of conduct disorder (CD) in childhood (<age 10) versus adolescence (> or =age 10). Prevalence of each ASPD diagnostic criterion and comorbid lifetime disorder was estimated. Logistic regression was used to examine associations of childhood-onset CD with ASPD symptom patterns and comorbid disorder

Three types of paranoid processes.

In classical descriptive psychiatry the term 'paranoid' is often used ambiguously -- referring to a variety of clinical processes which should be more clearly differentiated. Specifically, in this paper, we have differentiated three distinct sets of clinical phenomena all usually lumped together as 'paranoid': 1. Paranoid from a Sense of Guilt, 2. Paranoid from a sense of Low Self-Esteem, and 3. Paranoid from a Sense of Persecution. These three processes are distinct descriptively, dynamically and genetically. Further, this differentiation is most significant pragmatically as the treatment is

Screening for personality disorders among adults seeking speech treatment for stuttering.

UNLABELLED: Stuttering is frequently associated with negative consequences which typically begin in early childhood. Despite this, no previous studies have investigated the presence of personality disorders among adults who stutter. Therefore, the aims of the present study were to screen for personality disorders among adults who stutter, and to compare these screening estimates with matched controls from a national population sample. Using a matched case-control design, participants were 94 adults seeking treatment for stuttering, 92 of whom completed the International Personality Disorders E

Refining personality assessments by combining MCMI high point profiles and MCMI codes. Part III. MMPI code 24/42.

In this study it has been shown that the addition of MCMI high point code data to the MMPI 24/42 code type serves to clarify contradictory MMPI descriptors and produces three distinct clusters. These MMPI/MCMI clusters comprise an interpersonally acting-out group, an interpersonally acting-in group, and an emotionally acting-out group. These groups were useful in determining different roles for the MMPI 2 and 4 scales insofar as they relate to both personality (Axis II) and its clinical consequences (Axis I). The results of this study also lend support to the utility of combining objective ass

A reappraisal of the possible seizures of Vincent van Gogh.

The tragic life of Vincent van Gogh is summarized, emphasizing his early departure from formal education, failure as a successful salesman in the art world, attempt at religious studies, difficulty with female and family relationships, return to the art world, and tendencies toward extremes of poor nutrition or near self-starvation and excessive drinking and smoking. In Paris he joined the Impressionists, but drank very heavily both absinthe and cognac. Southward he went to Arles and was joined by Paul Gauguin, with whom he had major personality problems, causing van Gogh to cut off part of hi
