[Hand infections].

HAND INFECTIONS. Hand and finger infections are very common. They result from the inoculation of a germ through the skin barrier. They can range from simple paronychia to extremely serious necrotizing fasciitis. Certain infections, such as those resulting from bites, have their own specific characteristics, which will determine how they are managed. While management can be medical in the early stages, it is important not to ignore the need for surgical treatment, otherwise serious complications may arise, leading to functional and aesthetic sequelae. Delays in treatment cannot be made up. Any

Sarcoptes scabiei infestation in a cat.

Sarcoptes scabiei infestation was diagnosed in a cat. Clinical signs included thick, crusty, exudative dermatitis on the feet, caudal aspect of the thighs, and tail. Paronychia and dystrophic nails also were observed. The course of the disease was chronic. Concomitant and potentially predisposing additional health problems were suspected, but were not confirmed.

Septic arthritis of the sternoclavicular joint.

An adult patient with isolated septic arthritis of the sternoclavicular joint (SCJ) caused by Staphylococcus aureus is described. The patient had no underlying diseases other than paronychia, which might have served as the portal of entry. Antibiotic therapy resulted in clinical cure but x-ray revealed progression of the destructive process.

Current Knowledge on Nail Involvement in Autoimmune Bullous Disorders.

BACKGROUND: There are few studies, mainly case reports, on the involvement of the nail unit in autoimmune bullous disorders. SUMMARY: Nail involvement in autoimmune bullous disorders is a significant clinical phenomenon, marked by a range of manifestations, most often not presenting with blisters like on the skin but rather with alterations of the nail unit such as paronychia, onychomadesis, or onycholysis. This involvement is particularly notable due to the unique immunological features of the nail unit, including the expression of various antigens and the presence of Langerhans cells. Condi

[Results of early intervention in acute finger infections].

In the examination of 66 patients treated for acute felons, we classified the infections of fingers as (1.) cutaneous/subcutaneous felons; (2.) paronychia/subungual felons, (3.) thecal whitlow; (4.) articular/bony felons. Primarily, the therapy was based on early aggressive surgical treatment, and secondly on antibiotics. With the management described we achieved a good to very good result in 76%.

Congenital asplenia and group B streptococcus sepsis in the adult: case report and review of the literature.

Asplenia is associated with an increased incidence of fatal and life-threatening sepsis caused by encapsulated pathogens. Isolated congenital asplenia is a very rare condition, with only 33 cases reported in the literature. The authors report another case of this condition complicated by overwhelming Group B streptococcus sepsis secondary to paronychia that was managed successfully.

Delirium in an adolescent patient during treatment with cephalexin.

Although cephalosporins are generally safe, they have been reported to cause neurotoxic effects in some subjects. We report on a 16-year-old boy with no previous history of psychiatric illness, who developed symptoms of delirium during treatment of paronychia infection with cephalexin. Possible reasons for our patient's particular vulnerability to adverse drug effects in the central nervous system include his young age, a developmental delay, and an earlier head injury.

Onychomycosis caused by Scytalidium dimidiatum.

We report a case of onychomycosis caused by Scytalidium dimidiatum (syn., Hendersonula toruloidea) in a patient who did not live in an endemic area. This nondermatophyte mold may produce disease indistinguishable from dermatophyte fungi, but it does not respond to current systemic antimycotic therapy. Distal subungual onychomycosis, lateral onycholysis followed by lateral nail plate invasion, and chronic paronychia are common nail presentations.

Nail polish removers: are they harmful?

An 11-year-old girl developed blisters on her fingers after wiping up spilled nail polish remover. This reaction and other adverse effects and hazards of polish removers are reviewed. In addition to blister formation, polish removers may cause irritant and allergic contact dermatitis, onycholysis, paronychia, and brittleness. They represent a fire hazard and may cause systemic toxicity following excessive inhalation.

Cutaneous Adverse Events in Children Receiving Mitogen-Activated Protein Kinase Pathway Inhibitors.

BACKGROUND/OBJECTIVE: Mitogen-activated protein kinase pathway inhibitors (MAPKi) are increasingly used as targeted therapies for paediatric central nervous system tumours and other malignancies. Cutaneous toxicities, including xerosis, dermatitis, follicular-based infections, acne, and paronychia, are common and may result in significant morbidity and treatment interruption. METHOD: In this 18-month ambispective observational cohort study, 24 consecutive children receiving MAPKi at a tertiary paediatric centre were included. Patients were followed prospectively, with retrospective data colle

Surgical interventions for ingrown toenail.

BACKGROUND: Paronychia is a prevalent clinical disease affecting the soft tissue surrounding the nails. Most cases of toenail paronychia are commonly associated with ingrown toenails. While conservative treatment is effective for mild cases of ingrown toenails, surgical intervention becomes necessary for moderate to severe cases, particularly when granulomas form. OBJECTIVE: To provide a systematic understanding of these classic and modified procedures for surgeons to select the appropriate surgical interventions for patients suffering from moderate to severe ingrown toenails and discuss this

Retronychia.

Retronychia is a recently described disorder caused by ingrowth of the proximal nail plate into the proximal nail fold. It is suspected when there is persistent paronychia, particularly in the setting of trauma. This disease is probably underdiagnosed due to limited knowledge among dermatologists and the presence of incomplete clinical forms. Nail plate avulsion is the diagnostic and curative procedure of choice, despite reports of relapse.

Nail disorders.

Nail lesions can be related to a number of disease states. These may be intrinsic to the nail, caused by infection, or reflect systemic disease. Careful history and examination often is necessary for accurate diagnosis. Anatomy and gross changes of the nail are reviewed. The diagnosis and treatment of disease process such as psoriasis, lichen planus, paronychia, onychomycosis, myxoid cysts, and pincer nails are reviewed.

Infections of the hand.

The unique anatomic arrangements in the hand make possible rapid extension of infections through synovial-lined spaces if abscesses are not drained and pressure is allowed to build up. Adequate knowledge of these anatomic features and of routes for surgical drainage is mandatory if one is to give adequate management of hand infections. Localized processes, such as paronychia, felon, and human bite infections also require special insight for adequate treatment.

Nail Changes in Sézary Syndrome: A Single-Center Study and Review of the Literature.

BACKGROUND: The onychodystrophies associated with Sézary syndrome (SzS) have rarely been described in the literature. We performed a retrospective analysis of SzS patients at a single institution and compared our data with previous publications. OBJECTIVES: The objectives of this study were to identify and describe the most frequent nail alterations in patients with SzS. METHODS: A retrospective analysis was performed with some prospective observations at the University of Pittsburgh from 1989 to 2017. RESULTS: We identified 54 patients with SzS out of 535 patients with cutaneous T-cell lym

[Cutaneous toxicities].

The cutaneous toxicities of epidermal growth factor receptor(EGFR)inhibitors including cetuximab, gefitinib and erlotinib, and multi-kinase inhibitors including imatinib mesylate, sorafenib, and sunitinib, are described. Acneiform eruption, paronychia and xerosis are common cutaneous toxicities in patients receiving EGFR inhibitors. Acneiform eruption usually consists of follicular papules and pustules without comedones and is observed in more than 50% patients. Paronychia occurs less frequently(10 approximately 15%)than acneiform eruption and involves multiple fingers and great toes. Xerosis

Parakeratosis pustulosa--a distinct but less familiar disease.

Parakeratosis pustulosa (PP) is a distinct but less commonly known skin disease, which is frequently seen, in young girls. We describe the clinical and histological features of PP in a 7 month old female baby. Further, it is stressed that unless carefully looked for, this entity may be easily misdiagnosed as chronic paronychia, acrodermatitis of Hallopeau, pustular psoriasis, atopic dermatitis, tinea pedis or dry fissured eczematoid dermatitis and mistreated subsequently.
