Revision total knee arthroplasty for patellar dislocation in patients with malrotated TKA components.

PURPOSE: Patellar dislocation is a serious complication leading to patient morbidity following total knee arthroplasty (TKA). The cause can be multifactorial. Extensor mechanism imbalance may be present and result from technical errors such as malrotation of the implants. We sought to understand the reasons for post-arthroplasty patellar dislocation and the clinical outcomes of patients in whom it occurs. METHODS: This is a retrospective cohort study assessing the outcomes of revision surgery for patellar dislocation in patients with component malrotation in both primary and revision TKAs. Pa

Open knee dislocation with a patellar tendon rupture: Result of staged surgical repair.

Knee dislocation with concomitant multiligament injury is a rare and devastating injury. We report the successful repair of a rare case of open knee dislocation with concomitant multiligament injury and patellar tendon rupture of an 18-year-old male due to a motorcycle accident. The patient presented with an open wound running parallel to the knee joint line and patellar tendon rupture with full exposure of the cartilage of the distal femur. Staged surgical management including the application of a ring-type external fixator with a hinged joint, lateral collateral ligament repair, medial colla

Scoring system for optimal management of acute traumatic patellar dislocation: A multicenter study.

BACKGROUND: In first-time acute traumatic patellar dislocations, numerous anatomic and clinical factors are complicatedly associated and should be comprehensively considered for the optimal management of conservative or surgical intervention. The purpose of this study was to establish a scoring system, using a concept that is not complicated and easily used in clinic. METHODS: Of 131 first-time acute traumatic patellar dislocations, 81 patients (51 females, 30 males) with an average age of 19.6 (95%CI, 17.5-21.6) years and an average follow-up duration of 17.1 (95% CI, 12.3-21.9) months, who

Unusual Horizontal Patellar Dislocation Reduced without General Anesthesia: A Report of 2 Cases.

CASE: Horizontal patellar dislocation is a rare clinical event, and there is no consensus on its management. We describe 2 relatively elderly patients with 2 different types of horizontal patellar dislocation who were successfully treated with closed reduction without general anesthesia. CONCLUSION: Appropriate and gently managed closed reduction without general anesthesia may be successful in cases of acute horizontal patellar dislocation, provided there are no osteophytes or an injury that requires surgical treatment. With closed reduction and subsequent rehabilitation, patients can regain

TBC1D7 mutations are associated with intellectual disability, macrocrania, patellar dislocation, and celiac disease.

TBC1D7 forms a complex with TSC1 and TSC2 that inhibits mTORC1 signaling and limits cell growth. Mutations in TBC1D7 were reported in a family with intellectual disability (ID) and macrocrania. Using exome sequencing, we identified two sisters homozygote for the novel c.17_20delAGAG, p.R7TfsX21 TBC1D7 truncating mutation. In addition to the already described macrocephaly and mild ID, they share osteoarticular defects, patella dislocation, behavioral abnormalities, psychosis, learning difficulties, celiac disease, prognathism, myopia, and astigmatism. Consistent with a loss-of-function of TBC1D

Transient lateral patellar dislocation: review of imaging findings, patellofemoral anatomy, and treatment options.

Transient patellar dislocation is a common sports-related injury in young adults. Although patients often present to the emergency department with acute knee pain and hemarthrosis, spontaneous reduction frequently occurs, and half of cases are unsuspected clinically. Characteristic magnetic resonance imaging (MRI) findings often lead to the diagnosis. The purpose of this review is to illustrate the MRI findings of lateral patellar dislocation and concomitant injuries, such as kissing contusions of the medial patella and lateral femoral condyle; osteochondral and avulsion fractures; and injurie

Incarcerated patellar tendon in Hoffa fracture: an unusual cause of irreducible knee dislocation.

Irreducible knee dislocation due to incarcerated soft tissue structures in associated fractures is extremely uncommon. We report a case of irreducible, closed dislocation of the knee secondary to incarcerated patellar tendon in a Hoffa fracture (unicondylar fracture of femur in the coronal plane) with associated communited fracture of patella and disruption of both cruciates. We treated this case initially with open reduction and internal fixation of fracture and repair of the patellar tendon. During the total followup period of eighteen months, the patient had restricted joint range of motion

Articular cartilage injury with acute patellar dislocation in adolescents. Arthroscopic and radiographic correlation.

Acute patellar dislocation and relocation produces major forces at the articular surfaces. Articular injury was assessed in 48 patients, 24 boys and 24 girls (mean age, 14 years), with acute, initial, noncontact patellar dislocations. Correlation of the findings of radiographic and arthroscopic examinations was done. Eleven of the 48 patients (23%) had radiologic diagnoses of articular injuries. Thirty-four of the 48 patients (71%) had arthroscopic evidence of articular damage, 32 osteochondral and 2 chondral injuries. Eight of the 28 (29%) arthroscopically identified osteochondral loose bodie

[Biomechanics and semiology of transient lateral patellar dislocation: reliability of magnetic resonance diagnosis].

Transient patellar dislocation is a rare finding. It is usually due to direct trauma in mediolateral direction or to indirect trauma related to violent quadriceps muscle contraction. The clinical diagnosis of transient forms is usually rather difficult because the associated signs (hemarthros and pain at the joint and/or at the vastus medialis insertion) are not specific. Conventional radiography and CT are very accurate in bone studies, but exhibit major limitations in the detection of capsular injuries with intramedullary and/or cartilaginous bone involvement. We investigated MR diagnostic r

Recurrent patellar dislocation: personalised therapy or operative treatment? The REPPORT randomised trial protocol.

INTRODUCTION: Recurrent patellar dislocation is a debilitating musculoskeletal condition, affecting mainly adolescents and adults under the age of 30. It can persist for many decades, causing pain and cartilage and soft-tissue damage, potentially leading to osteoarthritis. Recurrent patellar dislocation can be managed with physiotherapy or surgery. However, it is not known which treatment is most effective. METHODS AND ANALYSIS: Recurrent Patellar Dislocation: Personalised Therapy or Operative Treatment (REPPORT) is a pragmatic, multicentre, two-arm, superiority, randomised controlled trial.

Anatomical characteristics and potential gene mutation sites of a familial recurrent patellar dislocation.

BACKGROUND: Recurrent patellar dislocation is the result of anatomical alignment and imbalance of restraint of bone and soft tissue. We investigate the anatomical characteristics of the knee joint in a family of patients with recurrent patella dislocation, and to screen the possible pathogenic genes in this family by whole exome sequencing in 4 patients and 4 healthy subjects, so as to provide theoretical basis for the pathogenesis of this disease. METHODS: The data related to patella dislocation were measured by imaging data. The peripheral blood DNA of related family members was extracted f

Predictors of Recurrent Patellar Instability in Children and Adolescents After First-time Dislocation.

BACKGROUND: Patellar dislocations are one of the most common knee injuries in children and adolescents and are challenging to treat. Recurrence rates are relatively high and many patients have functional limitations, even in the absence of a recurrent instability episode. The purpose of this study was to examine the risk factors in patients with first-time patellofemoral dislocations to develop a prediction model of recurrence. METHODS: A single institution retrospective review of all patients with a first-time patellofemoral dislocation from 2002 to 2013 was performed. Demographic risk facto
