Head and neck cancer (HNC) trials have demonstrated pathological complete response (pCR) in patients receiving neoadjuvant treatment prior to surgery; however, systematic reviews have reported marked variation in pCR rates. This scoping review was designed to determine the definition, criteria, and assessment methodologies for pCR in interventional neoadjuvant HNC clinical trials. The review was conducted in accordance with the JBI Manual for Evidence Synthesis and the protocol prospectively registered. Medical databases, trial registries, and grey literature were systematically searched, and

BACKGROUND: Primary neuroendocrine carcinoma (small-cell carcinoma) of the breast is a rare tumor; most cases are reported in women. Small-cell carcinoma is a type of neuroendocrine tumor characterized by high-grade, poorly differentiated, and aggressive behavior, which are common features regardless of the tumor's location. Here, we present a rare case of primary small-cell carcinoma of the breast in a male patient treated successfully with neoadjuvant chemotherapy followed by surgery.

Hormone receptor-positive, HER2-negative breast cancer represents a significant subset of patients who often exhibit suboptimal responses to neoadjuvant chemotherapy (NACT). Identifying strategies to enhance chemosensitivity in this population is a clinical priority. In this study, we investigated the potential contribution of statin therapy to the efficacy of NACT. We retrospectively analyzed 60 patients treated between 2014 and 2025, comprising 22 statin users and 38 non-users. While the overall cohort analysis did not demonstrate a statistically significant difference in pCR rates, an explo

OBJECTIVE: This study aimed to evaluate early predictors of pathological complete response (pCR) to neoadjuvant chemotherapy (NAC) in breast cancer and analyze tumor regression factors by integrating clinicopathological and imaging features. METHODS: We retrospectively analyzed the clinical data from 149 breast cancer patients who received neoadjuvant chemotherapy (NAC) at our hospital between January 2021 and January 2023, dividing them into pCR (61) and non-pCR (88) groups. Student’s t-test, chi-square test, multivariate logistic regression, and ROC curve analysis assessed pCR-associated fac

A 39-year-old man patient was admitted to a nearby clinic with bloody stool. Digital rectal examination revealed a mass lesion in the lower rectum. The patient was referred to our hospital and finally diagnosed with cT4a, N1b, M0, and c-Stage Ⅲb locally advanced rectal cancer. Neoadjuvant chemotherapy(NAC)with CAPOX and bevacizumab was administered to facilitate R0 resection, permitting anal preservation. When the chemotherapeutic regimen was evaluated to have achieved a complete clinical response, the patient underwent laparoscopic low anterior resection. He was shortly discharged without com

IMPORTANCE: Use of modern neoadjuvant chemotherapy (NAC) regimens has markedly increased rates of pathologic complete response (pCR) in breast cancer, raising the question of whether surgical removal of the primary tumor is required for patients with pCR. For surgery to be omitted, one must be able to accurately predict pCR before surgery.
OBJECTIVE: To investigate if adding post-NAC core needle biopsy of the tumor bed to trimodality imaging in patients who have clinical complete response (cCR) will predict pCR (resolution of both invasive disease and ductal carcinoma in situ) in 90% or more

BACKGROUND: The I-SPY 2 trial is a national-wide, multi-institutional clinical trial designed to evaluate multiple new therapeutic drugs for high-risk breast cancer. Previous studies suggest that pathological complete response (pCR) is a viable indicator of long-term outcomes of neoadjuvant chemotherapy for high-risk breast cancer. While pCR can be assessed during surgery after the chemotherapy, early prediction of pCR before the completion of the chemotherapy may facilitate personalized treatment management to achieve an improved outcome. Notably, the acquisition of dynamic contrast-enhanced

A 56-year-old man was referred to our institution with bloody stools. Colonoscopy revealed a type 3 tumor in the lower rectum. Histological analysis revealed moderately differentiated adenocarcinoma. Magnetic resonance imaging indicated that the tumor was confined to the rectal wall; however, enlarged lymph nodes were observed. Computed tomography revealed no signs of metastasis. The patient was diagnosed with rectal cancer(cT3N1aM0, cStage Ⅲb). The patient received neoadjuvant chemotherapy(NAC)consisting of capecitabine and oxaliplatin(CAPOX). After 4 courses of CAPOX, the diagnosis was revis

BACKGROUND/AIM: Neoadjuvant radiochemotherapy followed by surgery is a standard of care in locally advanced rectal cancer (LARC). Only a subgroup of patients can obtain a pathological complete response (pCR) and achieve good local control. However, the role of pCR on patient survival is debated. The aim of the study was to evaluate the impact of pCR on clinical outcomes and toxicities in LARC patients treated with dose intensification and concomitant capecitabine treatment in a neoadjuvant radiochemotherapy schedule.

Pathological complete response (pCR) after neoadjuvant chemotherapy (NCT) is associated with good long-term prognosis in breast cancer (BC) patients. However, some patients still recur and eventually die from this disease. For years, clinical stage at diagnosis has been consistently linked to recurrence and survival in the pCR setting. Herein, we aimed to identify other potential predictors of recurrence and survival in patients that achieved pCR. We performed a retrospective analysis of patients diagnosed between 2011 and 2020 in our center. We calculated overall survival (OS), invasive disea

BACKGROUND: Management of the node-positive axilla after neoadjuvant chemotherapy is controversial. The aim of this study is to predict the group of patients who may require a less invasive approach for axillary management. One possible group are patients with pathological complete response of the primary after chemotherapy.
RESULTS: A unicentral retrospective cohort study including all breast cancer patients with axillary node metastases at presentation who received neoadjuvant chemotherapy resulting in pathological complete response. Pathological complete response in the axillary lymph node

Preoperative chemotherapy is efficacious in several cancers. However, it is not an established treatment for locally advanced colon cancer, particularly cases with microsatellite instability-high (MSI-H)/deficient mismatch repair. Herein, we report a case of pathological complete response of MSI-H clinical T4b ascending colon cancer to preoperative treatment with pembrolizumab. A 78-year-old man was diagnosed with ascending colon cancer that invaded into the iliacus muscle and enlarged regional lymph nodes. The tumor was classified as T4bN1bM0 stage IIIC according to the 8th Union for Internat

A 68-year-old woman was presented with anorexia. Upper gastrointestinal endoscopy revealed type 4 gastric cancer at corpus of the stomach. Peritoneal metastasis was detected by staging laparoscopy. After a diagnosis of cT4aN1M1, cStage ⅣB advanced gastric cancer, we performed chemotherapy(SOX regimen; S-1 100 mg/body on day 1-14, followed by 7 days of rest, oxaliplatin 130 mg/m2 on day 1). After the 3 courses of chemotherapy, the primary tumor had been reduced. Second staging laparoscopy revealed no peritoneal metastasis. Then, we performed total gastrectomy with D2 lymph node dissection. Hist

A small proportion of patients with malignant pleural mesothelioma (MPM) achieve pathological complete response (CR) following treatment with current practice induction chemotherapy. Our analysis of 58 patients with MPM treated with platinum-based chemotherapy showed 4 patients (7%) attained pathological CR at subsequent extrapleural pneumonectomy (EPP). Patient and tumour factors such as age, gender, smoking habit, histological subtype, and clinical stage were not found to be associated with pathological CR. Patients with pathological CR had longer disease-free survival (29.2 vs. 13.8 months;

BACKGROUND: Pancreatic cancer is among the top 5 most common cancers worldwide, but is particularly devastating due to its insidious nature. Complete surgical resection remains the only potential curative treatment, although only 20 % of patients present with a resectable tumor. Patients may alternatively present with borderline resectable pancreatic cancer or locally advanced pancreatic cancer and can be offered treatment with neoadjuvant intent. The effectiveness of these treatments is unclear and there is a paucity of data to suggest one optimal treatment approach.

Diffuse malignant peritoneal mesothelioma (DMPM) is a rare disease. Although most patients eligible for surgery undergo cytoreductive surgery in combination with hyperthermic intraperitoneal chemotherapy, the role of perioperative systemic chemotherapy still remains undefined. Here we report the case of a 52-year-old female patient with advanced sarcomatoid DMPM. After five cycles of systemic pemetrexed and cisplatin, along with two cycles of regional hyperthermia, tumor resection with histomorphological examination showed a complete pathological response. We therefore conclude that there is a

We report a 49-year-old Chinese male with locally advanced gastric adenocarcinoma achieving pathological complete response after neoadjuvant chemotherapy with trastuzumab-containing regimen. He underwent esophagogastroduodenoscopy in September 2009, which revealed a 2-cm gastric ulcer on the lesser curvature proximal to angularis. Biopsy of gastric ulcer showed moderately differentiated adenocarcinoma with overexpression of human epidermal growth factor receptor 2 (HER2) by immunohistochemistry and fluorescence in situ hybridization. Further workups with endoscopic ultrasound, computed tomogra

PURPOSE: The recently published results of the National Surgical Adjuvant Breast and Bowel Project B51 trial suggest that regional nodal irradiation may be safely omitted in patients with cT1-3N1 breast cancer treated with either lumpectomy or mastectomy, and achieve a pathologic complete response (pCR) in the regional nodes. Of note, almost 80% of patients on the trial demonstrated breast pCR. The goal of our study was to compare clinical outcomes between patients with breast pCR versus not.
METHODS AND MATERIALS: We included all patients treated at a single institution between 2010 and 2021

BACKGROUND: In rectal cancer patients with a clinical complete response managed nonoperatively, local regrowth occurs in up to 35%. Although prior studies suggest a higher metastatic risk after regrowth, most data are derived from conventional chemoradiotherapy cohorts. The risk in a total neoadjuvant therapy setting remains unclear.
OBJECTIVE: To assess whether local regrowth after clinical complete response in patients treated with total neoadjuvant therapy increases the risk of distant metastasis and to evaluate whether the risk could be reduced by upfront surgery performed after total neo

PURPOSE: We aimed to determine whether patients with cytologically proven node-positive disease who achieved axillary pathologic complete response (ypN0) after neoadjuvant chemotherapy (NAC) have similar survival outcomes to those with initially pathologically negative lymph nodes (pN0).
METHODS: Patients with cytologically proven node-positive breast cancer who achieved ypN0 after NAC and those with pN0 between June 2005 and March 2012 in a large cancer hospital were reviewed. The relapse-free survival (RFS), distant-disease-free survival (DDFS) and overall survival (OS) of the 2 groups were
