There is a vast body of literature supporting the use of stress myocardial perfusion imaging (MPI) in patients with known or suspected coronary artery disease. When applied in the appropriate clinical setting, MPI can aid, not only in diagnosis and risk stratification, but also in decision-making. In a case of a 58-year-old man with suspected coronary artery disease, we highlight how the appropriate use of stress MPI can leverage the diagnostic and prognostic utility of MPI in an effective, evidence-based decision-making aimed to achieve the best patient outcome.

BACKGROUND: Myocardial necrosis after myocardial infarction (MI) is common; extent and severity are however variable. The pattern is recognized by myocardial perfusion imaging (MPI) as fixed perfusion defects (FPD). The fate of such FPD is not well appraised. This study addressed this important issue in a large number of patients undergoing serial MPI in relation to type of intervening therapy.
METHODS: Patients with prior MI or MPI-evidence of myocardial necrosis undergoing serial MPI without intervening acute coronary syndromes were included. The fate of necrosis by MPI on per-patient and p

BACKGROUND: Myocardial bridging (MB) is a common, congenital coronary-abnormality that is found on average in one out of every three adults at autopsy (Moehlenkamp et al in Circulation, 106:2616-2622, 2002; Erbel et al in Circulation, 120:357-359, 2009). However, its clinical significance and impact on myocardial ischemia remains controversial and unclear. Myocardial perfusion imaging (MPI) is widely used to assess myocardial ischemia in patients with known or suspected coronary artery disease, and is frequently performed to evaluate the hemodynamic significance of MB. This study was undertake

To determine whether the distribution of pulmonary perfusion can be applied as a noninvasive means of evaluating patients with mitral valve replacement (MVR), computerized Tc-99m MAA pulmonary perfusion images (digital perfusion images: DPI) were obtained for 32 patients in the preoperative, early postoperative and late postoperative (18 months mean) periods. DPI consisted of isocount areas, and a 100-70% area was defined as a hyperperfusion area. The distribution of pulmonary perfusion was evaluated using patterns of hyperperfusion area in anterior DPI. In 32 patients above-mentioned had the

BACKGROUND: In patients with right dominant coronary circulation, the right ventricular (RV) myocardium and the inferior region of the left ventricular (LV) myocardium share a common source of blood flow. We hypothesized that stress/rest SPECT myocardial perfusion imaging (MPI) could detect reversible perfusion defects in the RV in some patients with LV inferior wall perfusion abnormalities.
MATERIAL AND METHODS: We identified 2 groups of patients with LV inferior wall perfusion defects (with or without defects in other regions of LV myocardium) from our database. Patients in group 1 (n = 17)

Machine perfusion is an emerging technique in organ preservation and transplantation, with broader potential for studying disease mechanisms and testing novel therapeutics. Magnetic resonance imaging (MRI) offers non-invasive, quantitative assessment of tissue perfusion and microstructure. This study demonstrates the feasibility of acquiring high-resolution MRI sequences from porcine livers undergoing ex vivo perfusion using a custom MRI-compatible rig. Seven porcine livers were retrieved from the abattoir and perfused with oxygenated autologous blood inside a 3 T clinical MRI scanner. MRI seq

BACKGROUND: Current spatiotemporal Non-Local Means (NLM) filters for dynamic medical images are computationally expensive. The aim of this study is to develop an efficient spatiotemporal denoising framework tailored for dynamic medical images.
METHODS: The proposed method utilizes a decoupled two-step architecture: a prior-guided spatial fast NLM (FNLM) filter followed by a temporal moving average filter (MAF). Dynamic computed tomography images were used for performance evaluation, and the results were compared with those of FNLM, NLM with a spatiotemporal search window (NLM-ST), partial tem

BACKGROUND AND PURPOSE: Cerebral perfusion imaging may help detect the presence of, and infer vessel occlusion location in, anterior circulation stroke, but its value in posterior circulation large vessel occlusion (PC-LVO) is less well defined. We evaluated the accuracy and inter-reader reliability of CT and MR perfusion imaging for occlusion classification and posterior circulation vessel localization.
METHODS: We performed a retrospective multicenter diagnostic accuracy study from 11 centers that underwent perfusion imaging for stroke evaluation. The cohort was enriched for posterior circu

Conventional selection criteria are not consistently able to discriminate between responders and non-responders to cardiac resynchronization therapy (CRT). The objective of this study was to evaluate the usefulness of quantitative gated single-photon emission computed tomography (SPECT) in predicting the response to CRT. This prospective cross-sectional study included 25 patients with advanced congestive heart failure who underwent quantitative gated SPECT before and after CRT implantation. Patients with the left ventricular (LV) lead positioned at the latest activation segment away from the s

PURPOSE: To propose respiratory motion-informed locally low-rank reconstruction (MI-LLR) for robust free-breathing single-bolus quantitative 3D myocardial perfusion CMR imaging. Simulation and in-vivo results are compared to locally low-rank (LLR) and compressed sensing reconstructions (CS) for reference.
METHODS: Data were acquired using a 3D Cartesian pseudo-spiral in-out k-t undersampling scheme (R = 10) and reconstructed using MI-LLR, which encompasses two stages. In the first stage, approximate displacement fields are derived from an initial LLR reconstruction to feed a motion-compensate
