Concentration-dependent effects of dimethyl sulfoxide on cochlear efferent function and outer hair cell motility.

Dimethyl sulfoxide (DMSO) is widely used as a solvent in auditory and vestibular pharmacology, yet its direct effects on cochlear function are not well defined. We tested whether DMSO in artificial perilymph, delivered through the posterior semicircular canal in mice, alters distortion product otoacoustic emissions (DPOAEs) and medial olivocochlear (MOC)-mediated cochlear responses in the absence of active drug. Artificial perilymph alone produced no measurable change in either MOC-mediated responses or baseline DPOAEs. In contrast, 5% DMSO progressively increased MOC-mediated suppression, 10%

Quantitative Proteomics of Vestibular Schwannoma Cerebrospinal Fluid: A Pilot Study.

This pilot study aimed to identify candidate proteins for future study that are differentially expressed in vestibular schwannoma (VS) cerebrospinal fluid (CSF) and to compare such proteins with those previously identified in perilymph and specimen secretions. CSF was collected intraoperatively prior to removal of untreated sporadic VS (3 translabyrinthine, 3 middle cranial fossa approaches) and compared with reference CSF samples. After proteolytic digestion and iTRAQ labeling, tandem mass spectrometry with ProteinPilot was used to identify candidate proteins. Of the 237 proteins detected, 13

Measles virus in otosclerosis and the specific immune response of the inner ear.

Histologic and immunohistochemical studies of otosclerotic lesions have shown that there is a chronic inflammatory reaction of the otic capsule with bone resorption resulting from vascular invasion accompanied by inflammatory cells. During the active lytic stage of otosclerosis, paramyxoviral structures have been identified by electron microscopy and measles virus antigen expression by immunohistochemistry. Recently, measles virus related sequences have been detected in tissue of otosclerotic lesions. Because the otosclerotic focus has a close relation to the perilymphatic space, the expressio

[Experimental defects of the round window membrane].

Previous and current studies of experimentally induced fistulas in the round window membrane (RWM) are reviewed. After puncture (or "micro-perforation") of the RWM leaking perilymph was not been observed and auditory evoked potentials have remained unaltered. In contrast, most publications have reported that gross incisions ("macro-perforations") of the RWM have resulted in the deterioration of auditory evoked potentials, with leaking perilymph and/or cerebrospinal fluid and/or with entrance of air bubbles into the scala tympani depending on the site of the perforation. Spontaneous healing was

Immunologic and electrophysiological response to cytomegaloviral inner ear infection in the guinea pig.

We investigated the development of labyrinthitis in animals inoculated with guinea pig cytomegalovirus (GPCMV). These animals all became deaf eight days after inoculation into the perilymphatic compartment and showed severe inflammatory changes within the inner ear. Animals that received inactivated virus maintained their hearing throughout the observation period and exhibited normal cochlear morphology. Neither group had serum or perilymph antibody to GPCMV eight days after viral inoculation. We then investigated the effect of systemic immunity to GPCMV on inner ear viral challenge in animals

High-quality imaging of endolymphatic hydrops acquired in 7 minutes using sensitive hT2W-3D-FLAIR reconstructed with magnitude and zero-filled interpolation.

BACKGROUND: It is still challenging to detect endolymphatic hydrops (EH) in patients with Meniere's disease (MD) using MRI. The aim of the present study was to optimize a sensitive technique generating strong contrast enhancement from minimum gadolinium-diethylenetriamine pentaacetic acid (Gd-DTPA) while reliably detecting EH in the inner ear, including the apex. MATERIALS AND METHODS: All imaging was performed using a 3.0 T MR system 24 h after intratympanic injection of low-dose Gd-DTPA. Heavily T2-weighted 3-dimensional fluid-attenuated inversion recovery reconstructed with magnitude and z

Endolymphatic hydrops evaluation on MRI: Practical considerations.

Four-hour delayed three-dimensional fluid-attenuated inversion recovery (3D-FLAIR) sequence after intravenous gadolinium-based contrast agent administration is an optimal magnetic resonance imaging technique to evaluate endolymphatic hydrops in patients with known or suspected Ménière's disease. Nonenhanced endolymphatic space surrounded by enhanced perilymphatic space is evaluated in the cochlea and vestibule separately. In cochlear hydrops, the scala media is enlarged, potentially obliterating the scala vestibuli. In vestibular hydrops, the size of the saccule becomes equal to or larger than

Effects of cerebrospinal fluid loss on hearing.

There is a direct communication between the cerebrospinal fluid (CSF) and the perilymph through the cochlear aqueduct (CA). Perilymph depletion due to perilymphatic fistulas has been demonstrated to influence inner ear function, with signs and symptoms similar in many ways to those of Meniere's disease. A leakage of CSF could also theoretically affect the inner ear. The present study was conducted to investigate whether hearing was affected in patients submitted to a leakage of CSF and, if so, to what extent, in which frequency range, which time course and incidence. The study comprised 126 pa

The active process is affected first by intense sound exposure.

Evidence exists to suggest that intense sound releases excess neurotransmitter from the inner hair cells. However, it has been previously reported that intense sound affects the cochlear micromechanics by altering the stereocilia. Therefore, we tested the hypothesis that intense sound affects structures involved in transduction before it affects the nerve endings. In order to test this hypothesis, we examined the interaction of intense sound with kynurenate which blocks the action of the neurotransmitter on the afferent nerve endings. Intracochlear perfusion of artificial perilymph containing

[The pathogenesis of the acute symptoms of Menière's disease (author's transl)].

The pathogenesis of vertigo attacks and hearing fluctuations in Menière's disease is discussed on the basis of new electronmicroscopic observations on the perilymph endolymph barrier. The zonulae occludentes (z.o.) of the epithelial cells lining the endolymphatic space are the main sealing elements which control the passive ion transport. They consist of parallel running intramembranous fibrils (cell membrane fusion lines). Recent investigations demonstrated conditions which partially destruct some of such fibrils (e.g. increased osmotic pressure, endocrine disorders). This also may be true in

One of seven cases of Pendred's syndrome had mixed hearing loss, bilateral congenital stapes foot-plate fixation, surgically proven unilateral perilymph fistula and delayed fluctuating hearing loss in the unoperated ear. A unifying hypothesis is proposed to link these diversified findings.

Diagnosis and Treatment of Perilymphatic Fistula.

Perilymphatic fistula (PLF) is defined as an abnormal communication between the fluid (perilymph)-filled space of the inner ear and the air-filled space of the middle ear and mastoid, or cranial spaces. PLF is located in the round or oval window, fractured bony labyrinth, microfissures, anomalous footplate, and can occur after head trauma or barotrauma, chronic inflammation, or in otic capsule dehiscence. This clinical entity was initially proposed more than a century ago, yet it has remained a topic of controversy for more than 50 years. The difficulty of making a definitive diagnosis of PLF

Osmotic stabilization prevents cochlear synaptopathy after blast trauma.

Traumatic noise causes hearing loss by damaging sensory hair cells and their auditory synapses. There are no treatments. Here, we investigated mice exposed to a blast wave approximating a roadside bomb. In vivo cochlear imaging revealed an increase in the volume of endolymph, the fluid within scala media, termed endolymphatic hydrops. Endolymphatic hydrops, hair cell loss, and cochlear synaptopathy were initiated by trauma to the mechanosensitive hair cell stereocilia and were K+-dependent. Increasing the osmolality of the adjacent perilymph treated endolymphatic hydrops and prevented synaptop

Adenylate cyclase modulation of ion permeability in the guinea pig cochlea: a possible mechanism for the formation of endolymphatic hydrops.

The pathophysiological mechanisms leading to endolymphatic hydrops in Meniere's disease are unknown. Changes in ionic permeability of the cellular membranes between the endolymph and the perilymph, which alter the composition and osmolarity of the inner ear fluid, may be a major factor in the etiology of endolymphatic hydrops. To determine the possible involvement of adenylate cyclase in the formation of endolymphatic hydrops, we measured the endolymphatic K+, Na+, Cl- activities (AK, ANa, ACl) and the endocochlear potential (EP) by means of ion-selective microelectrodes while inner ear adenyl

Possible role of peptides derived from otosclerotic bone in the mechanism of sensorineural hearing loss.

Otosclerotic stapes footplates, superstructures and temporal cortical bones were extracted with 0.25 M guanidine X HCl 0.5 M EDTA (pH 7.4) solution. The extracted non-collagenous peptides/proteins were separated chromatographically on a Sephadex G-25 microcolumn. The peptide composition of the bone samples were compared by capillary analytical isotachophoresis (ITP) in the molecular mass range 0.3-5 kD. The otosclerotic stapes footplate contained 13 ITP subfractions, while the stapes superstructures and cortical bone contained only 9 and 10, respectively. An otosclerosis-specific ITP subfracti

Postural responses to electrical stimulation of the vestibular end organs in human subjects.

A multichannel vestibular prosthesis that delivers electrical stimulation to the perilymph of individual semicircular canals is a potential new treatment modality for patients with vestibular deficiencies. Most research in this field has evaluated the efficacy of this approach by its ability to reproduce eye movements in response to head rotations. Our group has developed such a device and implanted it in four human subjects with intractable unilateral Meniere's disease. This allows us to evaluate individual semicircular canal contribution to the control of balance and posture in human subject

Electrochemical composition of the cochlear fluids in the early experimental hydrops. Preliminary results.

The composition of endolymph and perilymph was studied in the guinea pig cochlea after 2 and 6 weeks of blockage of the vestibular aqueduct in an experimental model of hydrops. Compound action potential was monitored several times in the observation period. The endocochlear potential was measured and the endolymph was sampled at the first and third turns of the scala media. The Na, K, and Cl concentrations were determined in nanolitre aliquots of endolymph and of perilymph, the latter sampled from the basal scala vestibuli. After 2 weeks, no change in endolymphatic electrochemical composition

Unusual middle-ear mischief: trans-tympanic trauma from a hair grip resulting in ossicular, facial nerve and oval window disruption.

A case of piercing of the tympanic membrane, resulting in unusual consequences, is described. This is the first reported case of the long process of a dislocated incus resulting in trauma to the horizontal portion of a dehiscent facial nerve. Simultaneous depression of the stapes footplate resulted in a perilymph leak, but with delayed presentation.
