Germline Genetic Mutations in a Multi-center Contemporary Cohort of 550 Phyllodes Tumors: An Opportunity for Expanded Multi-gene Panel Testing.

BACKGROUND: A paucity of data exists regarding inherited mutations associated with phyllodes tumors (PT); however, some are reported (TP53, BRCA1, and RB1). A PT diagnosis does not meet NCCN criteria for testing, including within Li-Fraumeni Syndrome (TP53). We sought to determine the prevalence of mutations associated with PT. METHODS: We performed an 11-institution review of contemporary (2007-2017) PT practice. We recorded multigenerational family history and personal history of genetic testing. We identified patients meeting NCCN criteria for genetic evaluation. Logistic regression estima

Marked Unilateral Breast Enlargement Caused by a Fibroepithelial Lesion Suspicious for Phyllodes Tumor in the Modern Screening Era: A Case Report and Literature Review.

Markedly enlarged fibroepithelial breast lesions have become increasingly uncommon in the modern era of routine breast imaging and earlier diagnostic evaluation. We report the case of a 52-year-old woman who presented with progressive unilateral right breast enlargement and pain after delaying evaluation despite previously abnormal breast imaging. Clinical examination demonstrated marked right breast enlargement without overt inflammatory or infectious changes. Ultrasound-guided core needle biopsy revealed a fibroepithelial lesion with benign phyllodes tumor included in the differential diagno

Are both distinct epithelial and stromal cells molecular analysis from phyllodes tumors versus fibroadenoma components affected in breast fibroepithelial progression?

PURPOSE: To determine molecular events involved in the tumorigenesis of phyllodes tumors (PT) and the role of each stromal (SC) and epithelial (EC) cell. METHODS: Frozen breast samples enriched with epithelial and stromal cells from three fibroadenomas and 14 PT were retrieved and laser microdissected. Sanger and polymerase chain reaction-based sequencing of exon 2 MED12 and TERT promoter hotspot mutations were performed; 44K microarray platform was used to analyze gene expression. RESULTS: All three fibroadenomas (FAs) presented mutations in MED12, but not in TERT, whose mutation was observ

Synchronous bilateral phyllodes tumors with discordant grades and false-positive axillary 18F-FDG PET-CT uptake: a case report and literature review.

RATIONALE: Phyllodes tumors are uncommon fibroepithelial breast neoplasms with variable biologic behavior. Synchronous bilateral phyllodes tumors are exceptionally rare, and the interpretation of 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET-CT) may be confounded by tumor necrosis and inflammation, leading to false-positive nodal staging. PATIENT CONCERNS: A 44-year-old Korean woman presented with a rapidly enlarging, painful, ulcerated left breast mass for 3 months. She had no nipple discharge, fever, or weight loss. A right breast lesion was clinically

Retrospective study of malignant phyllodes tumors of the breast: Younger age, prior fibroadenoma surgery, malignant heterologous elements and surgical margins may predict recurrence.

PURPOSE: The potential recurrence rate of malignant phyllodes tumors (MPTs) of the breast is high, and the prognostic factors are still unclear. We therefore aim to study the factors affecting the outcome of MPTs. METHODS: A retrospective review of MPT patients treated from 2006 to 2020 at our institution was conducted. Univariate and multivariate Cox proportional hazard models were used to examine the influence of different variables on RFS. Moreover, significant prognostic factors were combined to construct the nomogram to predict the probability of relapse occurring in MPT patients. The 5-

Postoperative radiation therapy in localized breast malignant phyllodes tumors: RPA-derived risk model identifies high-benefit subgroups with local control improvement.

INTRODUCTION: Breast malignant phyllodes tumors (MPTs) are rare, aggressive neoplasms with high locoregional recurrence rates despite wide-margin resection. Though NCCN recommends postoperative radiation therapy (PORT), its efficacy remains controversial. This study aims to evaluate PORT's survival benefits in non-metastatic MPT and refine individualized radiation therapy strategies in clinical practice. METHODS: We retrospectively analyzed 108 non-metastatic MPT patients treated with R0 resection. Comparative analyses employed Kaplan-Meier methods, multivariable Cox models, and recursive par

Case Report: Coexistence of a giant borderline phyllodes tumor of the breast and a contralateral fibroadenoma: a challenging case with clinical manifestations and PET-CT imaging mimicking advanced cancer.

BACKGROUND: Phyllodes Tumor (PT) is a rare fibroepithelial tumor, which is classified into three categories: benign, borderline, and malignant. The three types show a gradually increasing trend in terms of local recurrence and distant metastasis. Giant PT usually refers to a mass with a diameter larger than 10 cm, accounting for approximately 20% of PT cases. Borderline PT (BPT) has overlapping clinical manifestations with both benign fibroadenomas and malignant breast tumors. Especially when it presents with rapid growth and ulceration, it is easily misdiagnosed as advanced breast cancer. CA

Case Report: Giant Malignant Phyllodes Tumor of the Breast: Immediate Reconstruction with a Pedicled Latissimus Dorsi Myocutaneous Flap Following Total Mastectomy-A Case Report and Review of Reconstructive Considerations.

Phyllodes tumor of the breast is a rare fibroepithelial tumor, constituting less than 1% of all breast neoplasms. These tumors are characterized by rapid stromal growth and can reach significant sizes, particularly in malignant cases, necessitating aggressive surgical management due to their high recurrence risk. In this case report, we present a 40-year-old Tunisian woman diagnosed with a giant phyllodes sarcoma. The patient experienced considerable physical and psychological impacts, including impaired mobility, body image disruption, and professional limitations. Clinical examination and im

Diagnostic value of a multiparametric magnetic resonance imaging-based nomogram incorporating apparent diffusion coefficient in preoperative differentiation of phyllodes tumors from breast cancer: a retrospective study.

BACKGROUND: Preoperative differentiation between phyllodes tumors (PTs) and breast cancers (BCs) remains a clinical challenge due to their overlapping imaging characteristics. The aim of this study was to evaluate the diagnostic performance of multiparametric magnetic resonance imaging (MRI), incorporating morphological, hemodynamic, and apparent diffusion coefficient (ADC) parameters, to develop a nomogram model for differentiating PTs from BCs, and to preliminarily explore the association between ADC parameters and the histologic grade of PTs. METHODS: A total of 92 patients (mean age, 46.0

Benign and low-grade fibroepithelial neoplasms of the breast have low recurrence rate after positive surgical margins.

Breast phyllodes tumors are uncommon fibroepithelial neoplasms with a range of histologic features. Surgical excision is the primary management, but the need for excision to negative margins in benign and borderline phyllodes tumors is unclear. Here, we review the surgical management patterns and outcomes of 90 patients with benign and low-grade fibroepithelial lesions of the breast treated at our institution, including 19 borderline phyllodes tumors, 52 benign phyllodes tumors, and 19 representative neoplasms with overlapping features of fibroadenoma and benign phyllodes tumors, which were cl

Occurrence of two rare malignant neoplasms (breast and ovarian) in an adolescent female.

BACKGROUND: Phyllodes breast tumors, particularly malignant ones, are rare in adolescents. Tumor behavior does not correlate with histopathology. This case of an adolescent with a malignant phyllodes tumor, followed by the occurrence of a borderline ovarian tumor, also uncommon in adolescence, may shed light on our understanding of phyllodes and uncommon gynecologic malignancies in the young. CASE: A-14 year-old Caucasian female underwent wide local excision of a 4-cm malignant phyllodes breast tumor. At 17 years of age she had excision of a serous borderline tumor of the ovary. Cystosarcoma phyllodes with muscular and lymph node metastasis. Our experience and review of the literature.

BACKGROUND: Phyllodes tumors are biphasic fibroepithelial neoplasms of the breast. While the surgical management of these relatively uncommon tumors has been addressed in the literature, few reports have commented on the surgical approach to tumors greater than ten centimeters in diameter - the giant phyllodes tumor. CASE REPORT: We report a case of a 45-year-old woman who presented with a large lump in her right breast, involvement of multiple ipsilateral axillary lymph nodes and pectoralis major muscle. Clinical findings and cytologic examination (fine-needle aspiration) were suggestive of

Disseminated malignant phyllodes: Presentation after a decade.

Phyllodes tumors are rare biphasic fibroepithelial lesions of the breast and account for 0.3%-0.5% of primary breast tumors. Malignant phyllodes tumor has a 10%-26% risk of distant metastasis. The most common site of metastasis is lungs followed by bone and soft tissue. This is a rare case of a 42-year-old female with a previous history of malignant phyllodes tumor breast. She presented after 10 years with metastases to multiple sites including lung, abdominal wall, retroperitoneum, bone, and brain. These tumors have a poor overall survival. A five-gene reverse transcription-PCR assay for pre-operative classification of breast fibroepithelial lesions.

BACKGROUND: Breast fibroepithelial lesions are biphasic tumors and include fibroadenomas and phyllodes tumors. Preoperative distinction between fibroadenomas and phyllodes tumors is pivotal to clinical management. Fibroadenomas are clinically benign while phyllodes tumors are more unpredictable in biological behavior, with potential for recurrence. Differentiating the tumors may be challenging when they have overlapping clinical and histological features especially on core biopsies. Current molecular and immunohistochemical techniques have a limited role in the diagnosis of breast fibroepithel

Distinguishing Fibroepithelial Lesions Requires Clinical, Imaging, and Pathology Correlation.

Phyllodes tumor (PT) is a rare tumor that can present as benign, borderline, or malignant. These tumors arise from the breast stroma, similar to fibroadenomas. Phyllodes tumors and fibroadenomas often have overlapping features in both radiological imaging and pathologic analysis. As a result, these two lesions are often difficult to differentiate and require the correlation of multiple modalities, including clinical context, radiologic imaging, and histological evaluation. This article presents a case of a borderline phyllodes tumor in a 51-year-old female, with the inclusion of its radiologic

Fibroepithelial lesions; The WHO spectrum.

Fibroepithelial lesions of the breast comprise a morphologically and biologically heterogeneous group of biphasic tumors with epithelial and stromal components that demonstrate widely variable clinical behavior. Fibroadenomas are common benign tumors with a number of histologic variants, most of which pose no diagnostic challenge. Cellular and juvenile fibroadenomas can have overlapping features with phyllodes tumors and should be recognized. Phyllodes tumors constitute a spectrum of lesions with varying clinical behavior and are graded as benign, borderline or malignant based on a set of hist

Conservative management of breast fibroadenomas.

Is it conservative or radical management to excise all fibroadenomas of the breast, especially in women less than 30 years old? Once a definite diagnosis is established by physical examination, fine-needle aspiration cytologic testing, and mammography, is it prudent to monitor women with small fibroadenomas (less than 4 cm in diameter)? We reviewed 498 cases of biopsy-proved fibroadenomas and 17 cases of phyllodes tumors (by biopsy) seen at Los Angeles County/University of Southern California Medical Center from 1986 to 1989. Analysis of patient age and measured tumor size in 203 fibroadenomas

Periductal stromal sarcoma of the breast: A case report and review of the literature.

Periductal stromal sarcoma (PSS), spindle and epithelioid types, is a rare subtype of malignant fibroepithelial tumor. The morphological characteristics of this neoplasm are different from phyllodes tumor and stromal sarcoma. PSS exhibits biphasic histology with benign ductal elements and a sarcomatous stroma composed of spindle cells and lacking phyllodes tumor architecture. The therapeutic management of PSS is based on wide surgery with free margins, and adjuvant therapies are not required. To the best of our knowledge, the recurrence of PSS in ≤5 months has not been reported in the literatu
