Pulmonary vein (PV) isolation (PVI) is the most important component of catheter ablation of atrial fibrillation (AF) and can be achieved by radiofrequency or cryoballoon ablation (CBA). The CBA system has shown excellent efficacy and safety in a number of clinical trials and is independent of the PV anatomy. However, pneumonectomy can significantly alter the anatomy posing a challenge to CBA. Few cases of PVI accomplished by CBA have been described in patients with lobectomy, but none in the pneumonectomy population. We describe a case of successful CBA for paroxysmal AF in a patient with a le

INTRODUCTION AND IMPORTANCE: Primary adenoid cystic carcinoma (ACC) of the lung is extremely rare. This tumor can be asymptomatic or have non characteristics symptoms, and the diagnosis is often late. The treatment of choice is surgery when it's possible.
CASE PRESENTATION: We herein report the case of a young patient with ACC of the left main bronchus. He had dyspnea and chest pain for 6 months. Complete atelectasis of the left lung was found on the chest x-ray. Bronchoscopy showed a tumor obstructing the LMB and invading the carina. The CT scan revealed a 5 cm tumor obstructing the left mai

We report a 6-year-old with single ventricle physiology secondary to tricuspid atresia who had cardiorespiratory failure who was not a candidate for further single ventricle palliation. The patient underwent planned staged left pneumonectomy for recurrent pneumonias secondary to bronchomalacia followed by orthotopic heart transplantation. This aggressive approach improved the patient candidacy for heart transplantation by removing the source of recurrent infection and respiratory failure (left lung).

BACKGROUND: Despite advances in surgical technique and perioperative management, pneumonectomy remains associated with significant morbidity and mortality. The purpose of this study was to examine the impact of annual institutional volume of anatomic lung resections on outcomes after elective pneumonectomy.
METHODS: We evaluated all patients who underwent elective pneumonectomy from 2005 to 2014 in the National Inpatient Sample. Patients less than 18 years of age, or with trauma-related diagnoses, mesothelioma, or a nonelective admission were excluded. Hospitals were divided into volume quart

INTRODUCTION: Bronchiectasis is chronic infectious and inflammatory disease that results in irreversible thickening and dilatation of bronchi, and significant lung function decline in children. Prompt early diagnosis and multidisciplinary intervention is crucial to control recurrent exacerbation and preserve lung function.
PRESENTATION OF CASE: We present a case of pediatric bronchiectasis in a 10-year-old female who presented with a complaint of intermittent wet cough of 5 weeks' duration associated with low grade intermittent fever, shortness of breath, easy fatigability and loss of appetit

Background Patients who survive long after pneumonectomy may develop heart valve disease. The consecutive operations can be complex because of the challenging anatomical conditions and the limited physiologic reserves of the patient. Case Presentation The subject is a 78-year-old patient who underwent a left-sided pneumonectomy for a metastasized testicular tumor 50 years ago. At 32 and 37 years postpneumonectomy, mitral regurgitation was manifested, and valve repair was performed. Bioprosthetic mitral valve replacement and tricuspid valve reconstruction became necessary 44 years postpneumon

BACKGROUND/OBJECTIVES: We evaluated postoperative mortality and complications after extrapleural pneumonectomy (EPP) and pleurectomy decortication (P/D) to better understand their effectiveness in malignant pleural mesothelioma (MPM).
METHODS: A meta-analysis was done to evaluate 30-day mortality and postoperative complications. In addition, in-patients data of 500 eligible patients with MPM who underwent EPP or P/D was extracted from the New York Statewide Planning and Research Cooperative System (SPARCS). Multivariate analyses and propensity matching were used to compare in-hospital mortali

INTRODUCTION: Pleural empyema in a post-pneumonectomy cavity (PEC) occurs with a frequency of 2% -15% and a mortality of more than 10%. It can occur with or without bronchopleural fistula (BPF). The treatment of empyema in the PEC requires a strict algorithm: drainage, bronchoscopy, closure of the fistula, thorough cleaning of the PEC, filling the cavity, thoracoplasty.
METHODS: 39 cases with an empyema in the PEC were analysed retrospectively (men: n = 38; women: n = 1; mean age: 60.3 ± 7.6 years). In 32 (82.1%) of the patients, a BPF was detected (right: n = 26, left: n = 6). The average le

Pneumonectomy for benign disease is rare but may generate more postoperative morbimortality than when performed for lung cancer. We questioned this assessment and retrospectively reviewed 1436 pneumonectomies and 54 completions of which 82 and 10 performed for benign disease (5.7% and 18.5%, respectively): left n=65 and right n=27. Indications were: post-tuberculosis destroyed lung (n=37), aspergilloma (n=18), bronchiectasis (n=19), infection (n=5), congenital malformations (n=5), inflammatory pseudotumor (n=3), trauma (n=2), post-radiation (n=2) and mucormycosis (n=1). INTRODUCTION: There are limited treatment options for patients with prior pneumonectomy and a new lung malignancy. The safety and efficacy of stereotactic body radiotherapy in this subpopulation has not been well defined.
METHODS: Postpneumonectomy patients treated with lung SBRT were identified from a prospective single institution database. Treatment toxicity was recorded prospectively using the Common Terminology Criteria for Adverse Events version 3.0. Disease recurrences were categorized as local, regional, or distant metastatic disease. Overall survival was calculated using the Kaplan-M

A 44-year-old man underwent radical thymectomy for malignant thymoma 5 years ago. He subsequently underwent right extrapleural pneumonectomy because a right pleural metastatic lesion had developed. The operation was completed uneventfully. Immediately after arrival at the intensive care unit, the patient appeared restless and in pain. His heart rate increased to 140 bpm and then abruptly decreased to 20-30 bpm concomitant with profound systolic hypotension of 30-40 mmHg. Chest X-ray showed that the heart was shifted into the right thorax. Emergent re-thoracotomy was performed and the heart was

A 39 year-old Japanese man who had been treated for pleuritis developed a tumor in the left lung and was admitted to our hospital. Although evidence of tuberculous pleurisy was not proved medically, he had been treated successfully with antituberculous drugs for 9 months. Two months after completion of antituberculous therapy, his X-ray showed a tumor shadow close to his left hilum. A computed tomography (CT) scan showed a left lung tumor with a diameter of 3.3cm, atelectasis in an ajacent field, and no effusion. Tumor markers targeted for lung cancer remained within normal range. Bronchofiber

A pedicled flap obtained by mobilizing the right lobe of the thymus was used to protect bronchial sutures in 29 consecutive patients undergoing a right pneumonectomy and in 4 additional patients. Fourteen patients had received preoperative chemotherapy with or without radiotherapy. The flap procedure was, in general, easy to do, required an average time of 20.4 minutes, and did not cause added operative morbidity. Postoperative magnetic resonance imaging, performed in 21 of the 29 patients who had pneumonectomy, showed a viable flap in every instance. One bronchopleural fistula occurred in a p

We report a case of carcinoma following 42 years of chronic empyema in a patient who underwent surgery for a hydatid cyst at the age of 3. At the time of diagnosis, an esophageal fistula was observed and treated with cyanoacrylate. We hypothesize that chronic inflammation of the pleura, caused by decades of empyema, associated with the presence of heterotopic squamous epithelium due to a long-standing esophago-pleural fistula, led to neoplastic transformation.

Authors present their results with experimental closure of bronchial stump after pneumonectomy with pedicled flaps of the greater omentum in sheep. In the experimental group left thoracotomy and laparotomy was performed. The left lung was resected and the bronchial stump was closed with the pedicled flap of the greater omentum. In controls standard left pneumonectomy was performed. After different periods of time (1-12 weeks) the animals were sacrificed, the bronchial stumps were excised and submitted to pathological studies. In none case bronchial fistula occurred. Pathological studies showed

Between 1972 and 1993, 19 patients (15 males and 4 females) with bronchopleural fistulae and pleural empyema after pneumonectomy were treated with transsternal transpericardial operations and closure of the fistula. The underlying malignant disease was a non-small cell carcinoma in 12, a malignant epithelial mesothelioma in two, and an atypical carcinoid tumor in one case. One patient each presented with tuberculosis, chest trauma, and lung destroyed by bronchiectasis. Fistulas affected the right bronchial stump in 17, and the left in 2, cases after pneumonectomy. The time between pneumonectom

Incompetence of the main bronchus within the first 3 postoperative weeks (early incompetence) is proposed as a criterion of efficacy of the main bronchus closure in pneumonectomy. The authors have performed 495 pneumo- and pleuropneumonectomies according to 5 original techniques. No cases of the main bronchus incompetence were registered. Indications to each of the bronchus treatment techniques are detailed.

Although most primary cancers of the left main bronchus extending to the carina are inoperable, some patients with such a tumour may benefit from an extended, curative surgical procedure. This type of resection presents specific problems in reconstruction and physiologic management during operation. Between 1977 and 1983, five such patients were managed by left pneumonectomy followed by resection of the carina. They ranged in age from 49 to 65 years. None were irradiated before the procedure. In all cases, an end-to-end anastomosis was made between the right main bronchus and the mediastinal t
