Osteolytic changes after polyglycolide pin fixation in chevron osteotomy.

Absorbable polyglycolide pins were used for fixation of 94 chevron osteotomies in 70 patients at the Department of Orthopaedics and Traumatology, Helsinki University Central Hospital, between 1986 and 1992. Postoperative osteolytic changes around the degrading pin occurred in 21 of 94 (22%) metatarsal heads. In 17 of 21 metatarsal heads, polydioxanone-coated polyglycolide pins were used. This type of pin has not been used since 1988. At follow-up, 16 of 21 osteolytic changes resolved completely and four partially resolved. In the remaining one, the osteolytic area remained visible after 6 year

PDO screw threads versus fractional CO2 laser, after subcision, for atrophic acne scar remodelling: a randomized group-based comparative study.

Atrophic acne scars present a significant therapeutic challenge. While subcision with various adjunctive treatments, including fractional CO₂ lasers and polydioxanone (PDO) threads, has been employed for scar remodeling, comparative evidence on their efficacy remains limited. This study aims to compare the clinical efficacy and patient satisfaction between subcision with fractional CO₂ laser and subcision with PDO screw threads in managing atrophic acne scars. A randomized, controlled study was conducted on 40 patients with atrophic acne scars. Patients were assigned to two groups: Group A und

Modification of U-Stitch Laparoscopic Gastrostomy Technique to Minimize Suture Knot Abscess Formation.

Aim U-stitch laparoscopic gastrostomy is a commonly used technique for placement of balloon gastrostomy for pediatric patients. The U-stitch method was modified by others whereby the stay sutures are placed in a subcutaneous tissue. Although this modification has been reported to be superior, it has led to suture knot abscess formation which was not reported in the original method. We developed further modification whereby the stay-suture knots are positioned within the gastrostomy tract instead of the subcutaneous tissue which minimizes suture knot abscess formation. Methods Modified U-stitch

An objective comparison of holding, slippage, and pull-out tensions for eight suspension sutures in the malar fat pads of fresh-frozen human cadavers.

BACKGROUND: In recent years, a number of designed suspension sutures have been rapidly introduced into clinical practice. OBJECTIVE: This study compares the holding tension, slippage tension, and pull-out tension for each of 8 suspension suture systems randomly positioned in the midface soft tissue in 4 fresh-frozen cadaver heads. METHODS: For each suture suspension system, a suture was positioned and set within the soft tissue of the cheek adjacent to the nasolabial line and attached to a tensiometer. A reference point was marked on the skin at the point where the suture first engaged the t

[Reabsorbable banding. Our initial experience].

Since May 1989 we have been using a reabsorbable 3 mm, polidioxanona band for pulmonary banding instead of the dacron one. This procedure was performed in 5 patients with ages between 33 to 230 days. The diagnoses were atrioventricular canal in one case and ventricular septal defect in the other 4 cases. One of them also had vascular ring with right aortic arch, left patent ductus arterious and aberrant subclavian artery. All the patients were in a poor condition. Subsequent reoperations performed in short, medium, and long term were uncomplicated, and the technique had the added benefit of av

Epidermal Inclusion Cyst Formation After Barbed Thread Lifting.

Barbed thread lifting is a widely used method because it is easier to learn, faster, and the lower frequency of complications than the invasive lifting technique. The number of reported cases about complication requiring an invasive procedure is very small, and most cases are limited to minor complications. The authors introduce a chronic, recurrent epidermal inclusion cyst that occurs after Barbed thread lifting. A 61-year-old woman visited the hospital with recurrent inflammation and scar accompanied by discharge in the temporal area lasting 2 years. She had barbed thread lifting in the area

Craniosynostosis: posterior two-third cranial vault reconstruction using bioresorbable plates and a PDS suture lattice in sagittal and lambdoid synostosis.

INTRODUCTION: Beginning in 2004, we modified our surgical technique for a cranial vault remodeling in sagittal and lambdoid synostosis. Beginning in the early 1990s, we started using a calvarial vault remodeling technique in sagittal and lambdoid synostosis that involves removing the posterior two thirds of the skull, extending from the coronal suture to below the lambdoid suture to within 1-1.5 cm of the foramen magnum. Up until 2004, the bone fixation evolved from wire fixation, then micro-metallic fixation plates and resorbable sutures. DISCUSSION: Over the last 9 years, we have used a nov

Clinical evaluation of the efficacy of materials used for primary reconstruction of orbital floor defects: Meta-analysis.

BACKGROUND: The aim of this network meta-analysis (NMA) was to compare the clinical results obtained after primary reconstruction of orbital floor fractures (OFF) using different materials. METHODS: PubMed, Cochrane, and Google Scholar databases were screened from 1989 to 2019. For a study to be eligible, it had to evaluate two or more materials and report the following clinical parameters: diplopia and/or enophthalmos and/or other complications. RESULTS: Nine studies involving 946 patients presenting with an OFF were included. After the surgical procedure, 105 patients (11%) had diplopia, w

Tensile strength of surgical knots in abdominal wound closure.

Abdominal wound dehiscence is a surgical catastrophe that can be attributed to patients or technical factors. The technical properties of the monofilament sutures and knots that are commonly used in abdominal closure are poorly understood. The aim of this study was to compare the tensile strength of monofilament sutures tied with conventional knots. To do this, the knot-holding capacity of four types of knots (square, surgeons', Aberdeen and loop) were tested using three types of gauge 1 monofilament suture, namely nylon, polyglyconate and polydioxanone, using a tensiometer. We found that the

A Narrative Review of Evolving Techniques in Nonsurgical Rhinoplasty: Risk Profile and Clinical Outcomes of Fillers.

Nonsurgical rhinoplasty (NSR) is a minimally invasive alternative to surgical rhinoplasty that uses dermal fillers to enhance nasal aesthetics. When performed by trained injectors, NSR offers rapid correction with relatively low downtime. This narrative review explores the historical evolution, anatomical considerations, filler materials, patient selection, procedural techniques, risk mitigation strategies, and clinical outcomes associated with NSR. Hyaluronic acid (HA)-based fillers remain the gold standard due to their reversibility, biocompatibility, and favorable safety profile. Recent inn

Reducing the incidence of incisional hernias following open gastric bypass surgery.

BACKGROUND: Abdominal wall hernias are a common complication following open gastric bypass (RYGBP) surgery. In an effort to reduce the incidence of incisional hernias with our form of open RYGBP, progressively smaller, upper midline epigastric incisions have been utilized along with permanent sutures. The purpose of this study is to analyze whether incision location, size and choice of suture material affect the incidence of incisional hernias following open RYGBP. METHODS: Clinically evident incisional hernias and other wound complications and non-wound related complications were analyzed fo

A pen in the liver.

A 24-year-old woman with anxiety, depression, and emotionally unstable personality disorder was referred to a tertiary center 2 weeks after ingesting multiple foreign bodies. She had undergone a laparoscopic cholecystectomy and a laparotomy for extraction of ingested foreign bodies several years ago. A sagittal CT scan view showed a ballpen and a hair clip in the stomach. A coronal view demonstrated that a second ballpen had penetrated the duodenal wall to enter the liver parenchyma. There was no free intraperitoneal air or fluid or evidence of abscess formation. At laparotomy, a toothbrush, a

Parotid Duct Ectasia in a Dog.

A 3yr old spayed female Brittany was referred with a cylindrical mass on the right side of her face. Three months earlier, a small stone embedded in the right parotid duct was noted and removed through a ductal incision by the referring veterinarian. At referral, the dog's general physical condition was normal except for a cylindrical mass on the right cheek. Skull radiographs showed a possibly retained sialolith or dystrophic mineralization within the previous surgical site. Aspiration of the mass yielded a thick, yellow/tan, mucopurulent fluid. Cytology of the fluid demonstrated degenerative

Results of dorsal midline plication in children with penile curvature and hypospadias.

OBJECTIVES: To present our initial results using dorsal midline (12-o'clock position) plication in children with penile curvature and hypospadias. METHODS: Twenty-five children with hypospadias and ventral curvature confirmed after artificial erection underwent dorsal midline plication and hypospadias repair. Ventral curvature was corrected by making a 5 to 10-mm-long vertical incision through the tunica albuginea at the dorsal midline and approximating the outer edges of the incision with a monofilament polydioxanone stitch. RESULTS: One plication suture was needed in 15 patients. In 8 pati

Biliopancreatic diversion with transitory gastroplasty preserving duodenal bulb: 3 years experience.

BACKGROUND: The authors have performed 521 bariatric surgery operations (319 restrictive procedures and 202 malabsorptive procedures). METHODS: During the last few years we have introduced an evolution of biliopancreatic diversion (BPD): BPD with transitory gastroplasty, preserving the duodenal bulb (53 cases). From a technical point of view, the operation consists of a BPD, coupled with a gastroplasty which is transitory due to the use of a polydioxanone (PDS) band. In the last few cases, instead of a VBG (with PDS band) in order to make the operation completely reversible without any suture

Asymptomatic Uterine Rupture in a Pregnancy Following B-Lynch Suture Application in a Previous Delivery: A Case Report.

The B-Lynch suture, designed to control severe postpartum hemorrhage by compressing the uterus, is effective in reducing maternal mortality rates. However, it can lead to complications such as infection and uterine necrosis. This report presents a rare case of asymptomatic uterine rupture in a subsequent pregnancy following a B-Lynch suture. A woman in her mid-30s, gravida 3, para 2, presented at 32 weeks gestation in labor and underwent a repeat caesarean section. During delivery and visualization of the uterus, a 3-cm uterine rent was incidentally discovered near the site of the previous B-L

Use of jejunal serosal patch and pyloric exclusion in the management of complex duodenal injury.

BACKGROUND: Duodenal injuries are relatively rare but remain a management challenge with a high incidence of postoperative complications. Guidelines from the World Society of Emergency Surgery and American Association for the Surgery of Trauma favour a primary repair for less-complex injuries, but the management of more complex duodenal trauma remains controversial with varying techniques supported, including pyloric exclusion, omental or jejunal patch closure, gastrojejunostomy and pancreatoduodenectomy. We describe the techniques used in one case of complex duodenal trauma. TECHNIQUE: The d
