[Chronic instability of the proximal tibio-fibular articulation: hemi-long biceps ligamentoplasty by the Weinert and Giachino technique. Apropos of 3 cases].

PURPOSE OF THE STUDY: Recurrent dislocations of the superior tibiofibular joint are rare. We report three cases of Weinert and Giachino ligament reconstruction. MATERIALS: From 1989 to 1994, two soccer players and one young girl (10 years old) presented recurrent dislocation of the superior TFJ. The mechanism was a direct trauma, and the type of dislocation was antero-lateral. These patients were free of tibial fracture, major ligament lesion of the knee, and neurologic symptoms (peroneal nerve injury). Pain was over the fibular head, mechanical and increased by jumping. A lateral "cracking"

Return to sport activities after medial displacement calcaneal osteotomy and flexor digitorum longus transfer.

PURPOSE: Medial displacement calcaneal osteotomy with flexor digitorum longus transfer is a common treatment for the management of the adult flatfoot associated with posterior tibial tendon dysfunction. In the literature, there is a paucity of information regarding the ability of patients to return to sport and recreational activities after this surgical procedure. The purpose of this retrospective clinical study was to assess the rate and type of athletic activities that patients participated in before and after medial displacement calcaneal osteotomy with flexor digitorum longus transfer. Surgical Management of Stage 2 Adult and Pediatric Acquired Flatfoot Without Tendon Transfer or Arthrodesis: A Retrospective Study.

Posterior tibial tendon dysfunction (PTTD) is a common pathology of the foot and ankle. Numerous techniques are available to reconstruct the flatfoot. Fusions and tendon transfers are popular methods; however, a wide range of complications have been associated with these procedures. The objective of the present study was to demonstrate the outcomes of reconstructive surgery for flatfoot correction without tendon transfer or arthrodesis. We performed a retrospective study of 43 patients and 56 feet who had undergone flatfoot reconstructive surgery from November 2011 to June 2016, with a mean fo

Adult-acquired flatfoot deformity.

Originally known as posterior tibial tendon dysfunction or insufficiency, adult-acquired flatfoot deformity encompasses a wide range of deformities. These deformities vary in location, severity, and rate of progression. Establishing a diagnosis as early as possible is one of the most important factors in treatment. Prompt early, aggressive nonsurgical management is important. A patient in whom such treatment fails should strongly consider surgical correction to avoid worsening of the deformity. In all four stages of deformity, the goal of surgery is to achieve proper alignment and maintain as

Outcomes for surgical correction for stages 2 and 3 tibialis posterior dysfunction.

Between the years 1991 and 1996, 13 patients with stage 2 or 3 tibialis posterior dysfunction were evaluated following surgical reconstruction. Those patients with posterior tibial tendon tendinitis with a progressive flatfoot were categorized as having a stage 2 deformity according to Mueller's developmental stages of tibialis posterior dysfunction. Those patients with increasing severity of symptoms including a forefoot abductus component were classified as stage 3. There were five patients in stage 2, ranging in age from 53 to 80 years old; and there were eight patients in stage 3, ranging

The mechanics behind the image: foot and ankle pathology associated with gastrocnemius contracture.

Contracture of the gastrocnemius musculature is a prevalent finding in the setting of foot and ankle pathology. Tightness of the posterior musculotendinous structures in the leg limits ankle range of motion and affects an equinus posture of the foot. Increased contact pressures are generated in the plantar foot with weightbearing. The resultant overload of the ligaments and the intrinsic muscles of the midfoot and forefoot is manifest in a variety of pathologic processes. The altered mechanics contributes to, among other conditions, ankle impingement, plantar fasciitis, midfoot arthritis, post

The relationship between the supination resistance test, lower limb biomechanics and the effects of foot orthoses on foot and ankle biomechanics in individuals with posterior tibialis tendon dysfunction during gait.

BACKGROUND: Posterior tibialis tendon dysfunction (PTTD) is a debilitating condition that leads to biomechanical changes, for which foot orthoses are often prescribed to attenuate. There is a need to improve the ability to predict these biomechanical alterations, determine the biomechanical effectiveness of foot orthoses, and anticipate their effects on individuals with PTTD during gait. RESEARCH QUESTION: Is the supination resistance test (SRT) reliable, and capable of predicting foot and ankle biomechanics, as well as the biomechanical effects of foot orthoses in individuals with PTTD durin

Single-incision medial approach for double arthrodesis of hindfoot in posterior tibialis tendon dysfunction.

BACKGROUND: Triple arthrodesis through a 2-incision approach has been the primary salvage procedure for rigid hindfoot malalignment resulting from posterior tibialis dysfunction. However, complications like calcaneocuboid joint nonunion, lateral wound dehiscence, and adjacent joint arthritis have been reported after triple arthrodesis. Hence we adopted single-incision medial approach arthrodesis of subtalar and talonavicular joints, sparing the uninvolved calcaneocuboid joint and lateral skin. METHOD: We report the results of a series of 18 feet with posterior tibialis dysfunction that had co

Multisegmental foot modeling: a review.

Over the past two decades, a number of multisegmental foot models have been developed in order to characterize foot kinematics. This paper reviews methods of multisegmental foot modeling, technical elements of the models, select clinical applications of the models, and future directions in this area of research. Technical areas discussed include angular derotation mechanisms and capture technology. Models discussed address two-, three-, four-, five-, and nine-segment approaches. Additional models which address foot segments using other definitions, are also discussed. Clinical applications of

Double calcaneal osteotomy: realignment considerations in eight patients.

Posterior calcaneal displacement and anterior calcaneal distraction osteotomies have been recommended for surgical management of flexible flatfoot deformity. We combined these procedures for surgical management of adolescent flatfoot and late stage II posterior tibial tendon dysfunction in the adult. Lateral column distraction has been shown to reduce rearfoot valgus and forefoot abduction by stabilizing the midtarsal joint. Posterior calcaneal displacement osteotomy, which results in medial translation of the tuberosity, converts the posterior muscle group from an everter to an inverter. A su

Association between Elastic Modulus of Foot Soft Tissues and Gait Characteristics in Young Individuals with Flatfoot.

Flatfoot is a common foot deformity, causing foot pain, osteoarthritis of the midfoot, and even knee and hip dysfunction. The elastic modulus of foot soft tissues and its association with gait biomechanics still remain unclear. For this study, we recruited 20 young individuals with flatfoot and 22 age-matched individuals with normal foot arches. The elastic modulus of foot soft tissues (posterior tibial tendon, flexor digitorum brevis, plantar fascia, heel fat pad) was obtained via ultrasound elastography. Gait data were acquired using an optical motion capture system. The association between

Arthroscopic Technique for Isolated Posterolateral Rotational Instability of the Knee.

Isolated posterolateral corner injury is an uncommon injury that could be a source of unexplained knee pain and dysfunction. Most of these patients present instability caused by concomitant ligament injuries. Numerous studies have shown open approach posterolateral repair or reconstruction surgeries to restore posterolateral rotational stability. Still, there is a certain population of patients who present posterolateral rotation instability without significant injury to the fibular collateral ligament, popliteal fibular ligament, and popliteus tendon. The posterolateral capsular ligament is c

Flatfoot: New diagnostic modalities.

Adult acquired flatfoot deformity, recently renamed Progressive Collapsing Foot Deformity (PCFD), is challenging to diagnose and treat due to the still poorly understood nature of its pathogenesis, which involves a complex interaction between soft tissues and bony structures. Long regarded as being primarily linked to posterior tibial tendon dysfunction, PCFD is now considered a multifactorial deformity (osseous dysplasia, joint malposition, tendon muscle imbalance, etc.), with many aspects yet to be explored. This study aims to provide an update on this pathology by addressing the following fiv

Assessment of Bony Subfibular Impingement in Flatfoot Patients Using Weight-Bearing CT Scans.

BACKGROUND:: Lateral hindfoot pain in patients with flatfoot deformity is frequently attributed to subfibular impingement. It remains unclear whether this is primarily due to bony or soft-tissue impingement. No studies have used weight-bearing CT scans to evaluate subfibular impingement. METHODS:: Patients with posterior tibial tendonitis were retrospectively searched and reviewed. Subjects had documented flatfoot deformity, posterior tibial tenderness, weight-bearing plain radiographs, and a weight-bearing CT scan. CT scans were evaluated for calcaneofibular impingement on the coronal view a

Preliminary radiographic findings and sizing implications on patients undergoing bioabsorbable subtalar arthroereisis.

Metallic subtalar arthroereisis implants can require removal. Similarly configured bioabsorbable "interference screws" placed alternatively to metal implants may obviate removal. Radiographic imaging may show the location and sizing of the implant, and evaluate for implant degradation. Patients undergoing subtalar arthroereisis were evaluated with magnetic resonance imaging (computed tomography in one patient) to measure the size of the tarsal canal. The tarsal canal length, along with medial height and lateral height, were assessed. The patient's actual implant size was also noted as well as

Diagnosis and treatment of chronic ankle pain.

The differential diagnosis for chronic ankle pain is quite broad. Ankle pain can be caused by intra-articular or extra-articular pathology and may be a result of a traumatic or nontraumatic event. A detailed patient history and physical examination, coupled with judicious selection of the appropriate imaging modalities, are vital in making an accurate diagnosis and providing effective treatment. Chronic ankle pain can affect all age groups, ranging from young athletes to elderly patients with degenerative joint and soft-tissue disorders. It has been estimated that 23,000 ankle sprains occur ea
